Biomechanics of Brachial Plexus Injuries Due to Shoulder Dystocia.

Brachial plexus injuries during childbirth can be devastating injuries with lifelong consequences. Here we review the biomechanical literature related to this injury and integrate it with recent epidemiological and clinical literature to better understand how intrinsic and extrinsic factors contribute to this injury. Brachial plexus palsy is caused by excessive stretching, tearing, or avulsion of the nerve fibers of the brachial plexus and can lead to temporary or permanent injury to the motor and sensory functions of the upper extremity. Compared to other maternal and fetal factors, the highe

Multiplanar humeral shape changes from infancy to adulthood in patients with birth-related brachial plexus palsy: a cross-sectional analysis.

BACKGROUND: Birth-related brachial plexus palsy (BRBPP), occurring in up to 1.7% of live births, may have drastic effects on quality of life for those failing to recover shortly after birth. Because humeral shape changes correlate with function in older children with BRBPP, the purpose of this study is to evaluate the temporal patterns of humeral shape pathology resulting from unilateral BRBPP spanning the arc from infancy to adulthood, strengthening our knowledge as a basis for clinical intervention. METHODS: Three-dimensional (3D) axial magnetic resonance (MR) images of both arms were acqui

Clinical Classification Versus MRI Patterns of Injury in Brachial Plexus Birth Injury.

PURPOSE: The Narakas classification describes brachial plexus birth injury (BPBI) according to nerve root injury by the pattern of motor weakness on clinical examination. However, it is unknown whether the classification truly corresponds to the described nerve roots. The distribution of nerve root injuries on magnetic resonance imaging (MRI) in infants with BPBI was compared with the clinical classification. METHODS: Infants with BPBI were prospectively enrolled at three children's hospitals, and the Narakas group was determined by physical examination. Infants underwent MRI prior to age 16

Psychometric properties of Brachial Plexus outcome measure: COSMIN-based systematic review.

PURPOSE: Children with incomplete recovery from Brachial Plexus Birth Injury (BPBI) experience varying degrees of upper limb impairment, and 20-30% require interventions to optimize function. A psychometrically validated measure of upper limb activity capacity is essential to guide shared clinical decisions for surgical and rehabilitation interventions. MATERIALS AND METHODS: Following the Joanna Briggs Institute Manual for Evidence Synthesis, this systematic review included studies on the measurement properties of the Brachial Plexus Outcome Measure (BPOM) - Activity Scale, a performance-bas

Round table discussion. The management of complete brachial plexus birth injury.

Cases of complete brachial plexus birth injury are likely to result in severe limitation of upper limb function and are challenging to manage. There is little high-level evidence to guide management of these children. Surgical exploration and microsurgical repair of the plexus is often recommended when facilities are available. Three expert brachial plexus surgeons were asked to answer specific questions regarding outcomes of non-operative management, their surgical strategies, the indications for contralateral C7 nerve root transfer, outcomes of surgical reconstruction and evidence for operat

Erb's Palsy: Visual Diagnosis in Art before Medical History?

UNLABELLED: <p>Background: Close observation is fundamental to both art and medicine, although both disciplines use it in different ways. As doctors need to practice observation of their patients every day, visual arts are increasingly used for the development of observational skills within the medical curriculum. This is particularly useful for students and residents in surgery or in specializations with a surgical component such as gynecology and obstetrics, where close observation of anatomical structures and radiological imaging is essential in providing quality care. Conversely, artists -

Does age matter? The role of patient age in brain plasticity analyzed after a nerve transfer.

OBJECTIVE: Nerve transfers, which are increasingly applied not only for traumatic peripheral nerve injuries but also in oncologic reconstructions, spinal cord injuries, and a wide range of neurological disorders, rely heavily on the capacity of the central nervous system to reorganize and reassign motor control pathways. Among the multiple factors influencing postoperative recovery, age-dependent brain plasticity appears to play a critical role in determining the degree to which patients can regain functional control after reinnervation. This study aims to characterize this correlation in dept

Neonatal and Maternal Outcomes following Shoulder Dystocia Resolution Utilizing ≥ versus < 3 Maneuvers.

Most shoulder dystocia (SD) cases do not have associated adverse outcomes. The objective was to assess whether SD relieved with ≥3 maneuvers, compared with fewer, is associated with a higher likelihood of adverse outcomes. The secondary objective was to examine if postpartum hemorrhage is associated with SD managed with ≥3 maneuvers versus fewer. This was a secondary analysis of the assessment of perinatal excellence (APEX) study, an observational cohort of over 115,000 deliveries in 25 U.S. hospitals from 2008 to 2011. Transfer of Middle and Posterior Deltoid Extended by the Lateral Intermuscular Septum for Elbow Extension Reconstruction.

PURPOSE: Restoring elbow extension in tetraplegia and brachial plexus injuries is critical for improving hand function. In chronic lesions, the traditional method is based on the transfer of the posterior deltoid elongated by a tendon graft to the triceps. Tendon grafts need a prolonged immobilization period to avoid stretching at the muscle-tendon junction. To avoid the use of tendon grafts, we propose harvesting the middle and posterior deltoid muscle along with the entire length of the lateral intermuscular septum, facilitating direct suturing to the triceps tendon. METHODS: Before surgery

A preliminary report of lower trapezius muscle transfer for elbow extension in children with brachial plexus birth injury (BPBI).

BACKGROUND: Elbow extension is crucial for daily activities but is often neglected in the context of brachial plexus injuries. Triceps palsy in children with brachial plexus birth injury (BPBI) can lead to significant functional impairments, including difficulty with overhead activities, stabilizing the arm, and performing tasks that require elbow extension. METHODS: This cohort study included 12 children with BPBI who underwent lower trapezius to triceps transfer for elbow extension. Patients were followed for 6 months to 2 years postoperatively. Manual muscle testing using Lovett and Sunder

Arthroscopic subscapularis-complex release and conjoint tendon transfer in birth brachial plexus injuries: a case series with evidence-based insights.

PURPOSE: Shoulder deformities and impaired function in individuals with birth brachial plexus injury (BBPI) are often caused by internal rotator (IR) contractures and abductor weakness, which may progress to glenohumeral dysplasia. Although subscapularis-complex release and tendon transfer is a time-tested procedure, there are controversies regarding the appropriate management of shoulder contractures, especially in preschool children. Therefore, this study examines the efficiency of arthroscopically done progressive controlled release of IR contracture combined with tendon transfer. METHODS:

Surgical Technique: Spinal Accessory to Infraspinatus Nerve Transfer in Brachial Plexus Birth Injury.

Nerve transfers, nerve grafts, and tendon transfers have been used to restore shoulder active external rotation in patients with brachial plexus birth injuries. Traditionally used nerve surgery techniques are nerve transfer from the spinal accessory nerve to a suprascapular nerve (SSN) or nerve grafting from C5 to the SSN. However, results are often suboptimal. A more distal and more targeted transfer from the spinal accessory nerve directly to the infraspinatus branch of the SSN has previously been described and mid-term outcomes are encouraging. Herein, we describe a modification of this tec

Split Latissimus Dorsi Tendon Transfer for External Rotation in Obstetrical Brachial Plexus Injury.

The intramuscular neurovascular branching pattern of the latissimus dorsi muscle provides 2 parts with independent neurovascular supply. The medial and lateral branches arising from the thoracodorsal pedicle run parallel to the muscle fibers and, therefore, 2 independent motor flaps can be harvested. We propose a modification to the latissimus dorsi tendon transfer for external rotation based on these 2 muscular parts. The medial (transverse) part is transferred to the rotator cuff insertion to dynamically enhance external rotation, and the lateral part acts as a stabilizer of the humeral head

A Comparison of End-to-End Versus Reverse End-to-Side Nerve Transfer Techniques in Treating Brachial Plexus Birth Injury-Associated Glenohumeral Dysplasia.

BACKGROUND: Glenohumeral dysplasia (GHD) commonly occurs following brachial plexus birth injuries and, if not addressed appropriately, can result in shoulder dysfunction. Despite its severity, there are no clearly defined surgical criteria for correcting GHD. This study compares the outcomes of end-to-end and reverse end-to-side spinal accessory to suprascapular nerve transfers for correction of GHD. METHODS: All patients presented with shoulder functional limitations, as indicated by Active Movement Scale (AMS) scores in shoulder abduction, shoulder flexion, and external rotation, and eviden

Very Late Spinal Accessory Nerve-to-Infraspinatus Nerve Transfer to Restore Glenohumeral External Rotation in Brachial Plexus Birth Injury.

BACKGROUND: Poor recovery of active glenohumeral external rotation (aGHER) after brachial plexus birth injury (BPBI) is common. Late spinal accessory nerve-to-infraspinatus motor branch (SAN-IS) transfer has been reported as effective. The authors investigated its efficacy in children older than 4 years with BPBI. METHODS: This case series involved children with no aGHER undergoing SAN-IS transfer. Indications included absent aGHER in shoulder abduction and a positive external rotation drop test. Subscapularis release was done for passive GHER less than 30 degrees in patients without glenohum

Forearm Synostosis Using A Vascularized Pedicled Ulnar Periosteal Graft for Supination Deformity in Brachial Plexus Birth Injury.

BACKGROUND: Forearm supination posture is a common secondary deformity in brachial plexus birth injury (BPBI), leading to functional impairment of the upper limb, as pronation is required for most activities of daily living. The purpose of this report is to analyze a series of children with forearm supination deformity secondary to severe BPBI and complete pronation palsy, who were treated with forearm fusion in slight pronation through the creation of a radioulnar synostosis using a novel distal pedicled vascularized ulnar periosteal flap (VUPF). METHODS: Retrospective analysis. Inclusion cr

Pectoralis Minor Transfer for Internal Rotation Reconstruction in Brachial Plexus Birth Injuries.

PURPOSE: A subset of patients with brachial plexus birth injuries experiences a loss of internal rotation, resulting either from neurological paralysis, as a complication of surgical procedures intended to restore external rotation, or because of contractures of the external rotators. Although humeral internal rotation osteotomy can address this deficit, it often results in compromised external rotation. To mitigate this drawback, in supple shoulders, we investigated the pectoralis minor tendon transfer to the subscapularis footprint as an alternative. METHODS: Five patients with active inter
