Pulmonary mycoses: a clinical perspective.

Pulmonary mycoses encompass a broad spectrum of pathogens, including filamentous moulds, dimorphic fungi and fungi with predominantly yeast-like growth forms. Susceptibility is driven by defects in T-helper (Th)1/Th17 immunity, neutrophil dysfunction and barrier disruption. Clinical presentation varies widely, ranging from asymptomatic colonisation to rapidly progressive, life-threatening disease, depending on host immune status and pathogen type. Radiological findings, particularly on high-resolution computed tomography, often provide critical diagnostic clues but are rarely pathognomonic. Al

Diagnostic Performance of Sōna GM LFA in Pulmonary Infections Caused by Different Aspergillus Species.

BACKGROUND: Although Aspergillus fumigatus remains the primary cause of invasive aspergillosis, other species-including emerging cryptic ones-are increasingly identified in clinical settings. These species often exhibit distinct antifungal susceptibility profiles, emphasising the importance of species-level identification for effective patient management. Galactomannan, once considered specific to A. fumigatus, is now widely used for aspergillosis diagnosis regardless of species. However, its performance in detecting non-fumigatus infections remains insufficiently characterised. OBJECTIVE: To

One-year clinical, radiological, and functional outcomes following antifungal therapy for aspergillus infection in lung transplant recipients.

BACKGROUND: Aspergillus is the most frequent fungal pathogen after lung transplantation, ranging from asymptomatic colonization to invasive disease. Treatment outcomes of aspergillosis in real-life settings remain insufficiently characterized because of the complexity and heterogeneity of its clinical presentations. METHODS: We conducted a retrospective study of all adult lung transplant recipients followed at Toulouse University Hospital from 2014 to 2024. Patients were stratified as Aspergillus-free, colonized, or treated for pulmonary aspergillosis (PA). Clinical, radiological, microbiolog

Primary Cutaneous Aspergillosis Due to Aspergillus flavus in an Immunocompetent Patient.

Invasive aspergillosis is a life-threatening infection caused by Aspergillus species, affecting the lungs, central nervous system, nasal and orbital regions, and skin. Primary cutaneous aspergillosis (PCA) occurs through direct skin inoculation via trauma, burns, or surgical wounds, with Aspergillus fumigatus, Aspergillus flavus, and Aspergillus niger as common causative species, and is rare in immunocompetent individuals. We report a case of PCA in a 56-year-old immunocompetent patient with facial and right ankle ulcers, persisting for two years. The patient had no history of diabetes, cortic

High-Volume Culture Improves Diagnostic Yield of Aspergillus Species From Sputum.

OBJECTIVES: Sputum is a commonly collected specimen for suspected respiratory fungal infections due to its non-invasive nature and diagnostic value. However, conventional culture at 29°C (CC29) often fails to detect filamentous moulds. To improve recovery, we applied a high-volume culture incubated at 37°C (HVC37) and compared its diagnostic performance with CC29, with particular focus on Aspergillus species and azole-resistant isolates. METHODS: Between October 2022 and June 2025, a total of 1546 sputum samples from 793 patients, including 219 samples from 67 patients with chronic pulmonary

Environmental and Phylogenetic Investigations of Aspergillus flavus Outbreak Linked to Contaminated Building Materials, Denmark, 2025.

An Aspergillus flavus outbreak occurred in a tertiary hospital in Denmark. We compared environmental sampling methods, investigated the outbreak through short tandem-repeat genotyping, STRAfla, and analyzed isolate phylogeny using whole-genome sequencing. Paired sampling revealed that air sampling underestimated A. flavus burden (8 CFU/81 air samples vs. 585 CFU/81 surface samples), and culturing at 37°C was superior to 25°C (risk ratio 1.77; p<0.001). STRAfla (n = 145) confirmed clonality of the outbreak isolates. Active growth was identified in a kitchen inside the affected ward. Genetically

The evolution of next-generation lateral flow assays for bacterial and fungal diagnostics.

Infectious diseases driven by increasingly resistant bacterial and fungal pathogens demand diagnostics that are faster and more accessible than conventional culture-based methods. This review traces the evolution of lateral flow assays (LFAs) from simple qualitative strips to sophisticated, molecularly enhanced diagnostic platforms. This review synthesizes literature mostly published between 2010 and 2025, identified through PubMed, Scopus, and Web of Science using search terms including 'lateral flow assay,' 'point-of-care diagnostics,' 'CRISPR diagnostics,' 'nanozyme biosensor,' 'antimicrobi

Pharmacist-Led Full-Course Pharmaceutical Care Model in Allergic Bronchopulmonary Aspergillosis: A Case Report-Based Approach to Optimizing Multidisciplinary Management.

INTRODUCTION: The management of allergic bronchopulmonary aspergillosis (ABPA) is complicated by prolonged, multi-drug regimens that carry a high risk of multi-system adverse drug reactions (ADRs). This complexity underscores the need for integrated, continuous pharmaceutical care to optimize safety and efficacy, yet standardized models for such care are lacking. CASE PRESENTATION: We report the case of a patient with ABPA and comorbid chronic hepatitis B, who had a history of failed ABPA therapy. During the current therapeutic course, the patient not only sequentially developed steroid-induc

Aspergillus deflectus-associated disseminated invasive aspergillosis in a German Shepherd dog with discospondylitis: first isolation in Europe.

Canine discospondylitis (CD) is an uncommon but serious condition. We report the first European case of disseminated Aspergillus deflectus infection causing thoracic discospondylitis in a 6-year-old female German Shepherd dog presenting with chronic spinal pain, lethargy and progressive neurological signs. Computer tomographic imaging revealed multiple thoracic intervertebral lesions and systemic abnormalities, involving the kidneys, spleen and femurs. Cytological and histochemical analysis confirmed the presence of fungal hyphae being A. deflectus molecularly identified in urine and post-mort

Synergistic antifungal activity of antiretrovirals with amphotericin B against Aspergillus species.

Aspergillosis is a life-threatening fungal infection that primarily affects the lungs of immunocompromised individuals, including those living with human immunodeficiency virus (HIV), and is associated with mortality rates exceeding 50%. The infection is predominantly caused by Aspergillus fumigatus, a pathogen that has become increasingly difficult to treat due to the emergence of azole-resistant strains. Although azoles have traditionally served as first-line antifungals, the rise in resistance has necessitated broader use of amphotericin B (AmB), a polyene agent whose clinical utility is li

α-1,3-Glucan-Driven Remodeling of the Conidial Cell Wall in an Aspergillus fumigatus Vaccine Strain Alters Innate Immune Recognition.

Aspergillus fumigatus is a major cause of invasive aspergillosis in immunocompromised patients, where current antifungal therapies are limited by toxicity, drug resistance, and lack of durable protection, and no vaccines are available. A mutant lacking the sterylglucosidase-encoding gene (sglA) has emerged as a candidate that induces protective immune responses, but the structural basis for this phenotype remains unclear. Here, we use cellular solid-state NMR spectroscopy to compare the organization of the conidial cell wall in ΔsglA and its wild-type counterpart. The ΔsglA conidial cell wall

Deciphering the toxicological mechanism of airway allergic diseases caused by dioctyl terephthalate: a composite study.

BACKGROUND: Dioctyl terephthalate (DOTP), a common plasticizer alternative, has unclear immunotoxic effects on respiratory allergic diseases such as allergic rhinitis (AR), asthma (AS), chronic rhinosinusitis (CRS), and allergic bronchopulmonary aspergillosis (ABPA). METHODS: We integrated network toxicology, target mining, bioinformatics, and machine learning to explore DOTP's potential role in allergic airway diseases. Shared targets were analyzed via PPI networks, enrichment analyses, immune profiling, single-cell data, and molecular docking. Key genes were validated in vitro using human a

A breakthrough invasive Aspergillus granulosus infection following azole antifungal therapy in a patient with primary myelofibrosis: case report.

Invasive aspergillosis (IA) primarily affects immunocompromised patients, with Aspergillus granulosus being a rare cause. Most cases occur in transplant recipients, and breakthrough infections may develop after azole therapy due to cryptic species and antifungal resistance. Here, we present the first non-transplant case of breakthrough A. granulosus infection following azole therapy with a fatal outcome. This case highlights the importance of early diagnosis, accurate species identification, and appropriate antifungal treatment for breakthrough invasive fungal infections.

Chronic pulmonary aspergillosis and pulmonary embolism/venous thromboembolism: a case series and systematic review of the literature.

Chronic pulmonary aspergillosis (CPA) often arises in the setting of structural lung damage and is associated with chronic inflammation, which increases the risk of venous thromboembolic disease (VTE), including pulmonary embolism (PE). However, the clinical context and pathophysiological mechanisms underlying CPA/VTE remain poorly defined. This study aims to highlight the clinical challenges through a case series and systematic review of existing literature regarding this rare co-occurrence. We present three complex cases from Kiruddu National Referral Hospital in Uganda: two males, one femal

Nosocomial Fungal Infections: Epidemiology, Control Strategies, and Prevention of Aspergillus and Other Molds.

Invasive mold infections are an important cause of hospital-acquired infections, particularly among immunocompromised patients and those exposed to contaminated environments. Conventional culture-based diagnostic methods often lack sensitivity, making advanced nonculture-based tests essential for accurate detection. Prevention and control strategies require a multifaceted approach, combining stringent infection control measures with targeted antifungal prophylaxis for individuals at highest risk. Early recognition and timely intervention are critical to reducing morbidity and mortality.

Incidence and outcomes of influenza-associated pulmonary aspergillosis and the role of antifungal prophylaxis: a structured literature review.

BACKGROUND: Influenza-associated pulmonary aspergillosis (IAPA) is a frequent and often fatal complication of severe influenza in critically ill patients. Despite increased recognition, its real global burden and optimal prevention strategies remain uncertain. METHODS: A structured literature review of studies published between January 2000 and June 2025 was conducted to summarize IAPA incidence, fatality rate and study heterogeneity. Continuous variables were summarized as trimmed medians (central 80%) with bootstrapped 95% confidence intervals. Between-study heterogeneity and methodological

Scedosporium apiospermum: An Emerging yet Overlooked Fungal Pathogen in Veterinary Medicine-A Case-Based Review.

Scedosporium apiospermum is an emerging filamentous fungus of increasing clinical relevance in human and veterinary medicine. Previously regarded as a ubiquitous soil saprophyte, it is now recognized as an opportunistic pathogen causing a wide spectrum of localized and systemic infections, particularly in immunocompromised hosts. Although infections in animals are considered rare, they are likely underdiagnosed or misidentified as aspergillosis or fusariosis due to overlapping clinical features and morphological similarities. The first confirmed animal isolate of S. apiospermum in the Western
