Otogenic Aspergillus Brain Abscess in an SLE teen: Successful case management from Eastern India.

Neuroaspergillosis is a rare, high-mortality complication in Systemic Lupus Erythematosus (SLE). We report an otogenic Aspergillus fumigatus brain abscess in a 17-year-old female with SLE and seizure disorder, managed with immunosuppressants. She presented with altered sensorium and hemiparesis. Despite initial aspiration and combination antifungal therapy (voriconazole and liposomal amphotericin B), clinical worsening demanding radical excision. Culture and sequencing confirmed A. fumigatus, likely spread via chronic otitis media. The patient recovered and was discharged on oral posaconazole,

Knowledge and practices of care providers on fungal diseases in the Democratic Republic of the Congo: a cross-sectional study.

BACKGROUND: Fungal diseases are still not well known in many countries including the Democratic Republic of the Congo. In the Democratic Republic of the Congo, around 5.4 % of people suffer from a serious fungal disease each year. Their occurrence is higher in immunocompromised patients and those with respiratory diseases. A good level of knowledge and management practice of care providers contributes to reducing morbidity and mortality. METHODS: This cross-sectional multicenter study addressed knowledge and practice of care providers in many provinces of the Democratic Republic of the Congo

Clinical validation of a new multi-Aspergillus species reverse transcriptase quantitative PCR: a diagnostic case-control study.

OBJECTIVES: Invasive aspergillosis is a difficult-to-diagnose fungal disease, the diagnosis of which relies on many criteria including mycological criteria. Recently, the European Organisation for Research and Treatment of Cancer/Mycoses Study Group Education and Research Consortium promoted Aspergillus PCR as a specific marker given the significant progress made in optimizing and standardizing Aspergillus real-time quantitative PCR (qPCR) assays. However, there is no unique and optimal assay in the literature, particularly for optimally detecting aspergillosis due to the main Aspergillus spec

Exploring the roles of fungal extracellular vesicles in fungal pathogenesis and symbiosis: Current understandings and future perspectives.

Fungal extracellular vesicles (FEVs) have emerged as sophisticated emissaries in cross-kingdom communication, redefining our understanding of fungal secretome in pathogens like Cryptococcus neoformans and Candida species. Rather than mere cellular debris, these bilayered structures are actively exported across the fungal cell wall via complex biogenesis mechanisms, carrying a potent cargo of virulence factors, genetic material, proteins, lipids, and polysaccharides. In human hosts, FEVs facilitate myco-pathogenesis by interacting directly with immune cells to modulate the immune response, eith

A study on the antifungal susceptibility pattern of Aspergillus species.

PURPOSE: Increasing trends in invasive fungal infections including aspergillosis are observed worldwide. Varying drug resistance patterns are seen with different Aspergillus species. Selection of appropriate antifungal is pivotal in treatment of chronic and invasive aspergillosis. Hence understanding the local epidemiology and resistance patterns of Aspergillus species is of utmost importance. This study aims to analyse the antifungal susceptibility pattern of clinically relevant Aspergillus species isolates from clinical specimen. METHODS: This study was done on the Aspergillus species isola

Therapeutic drug monitoring-guided optimization of isavuconazole dose for ICU patients with refractory pulmonary aspergillosis: case report.

Isavuconazole (ISA) is recommended as the first-line therapy for pulmonary aspergillosis. In intensive care unit (ICU) patients, ISA exhibits enhanced clearance and a larger volume of distribution but lower plasma concentrations, indicating the need for plausible dose adjustment. However, current evidence regarding off-label ISA dosing regimens for ICU patients is restricted to pharmacokinetic prediction modeling, with no published clinical studies available to date. In this case, a prolonged ISA-loading dosing regimen was applied to address previous standard ISA treatment failures; thereby, p

PIM1 Inhibition Alleviates Aspergillus fumigatus Keratitis by Regulating DDX41-Mediated STING Signaling Pathway.

PURPOSE: Fungal keratitis (FK) is a severe, sight-threatening infection with limited therapeutic options. Although the proto-oncogene PIM1 has been implicated in various inflammatory diseases, its role and underlying mechanisms in FK remain unknown. METHODS: We investigated the expression and function of PIM1 in human corneal epithelial cells (HCECs) and a mouse model of Aspergillus fumigatus keratitis using genetic and pharmacologic approaches. Protein-protein interactions and phosphorylation events were analyzed by co-immunoprecipitation, GST pull-down, and in vitro kinase assays. Inflammat

Management and Immunotherapy for Fungal Allergy: Clinical Practice and Unmet Needs.

Fungal allergy is an increasingly recognized health issue, affecting a notable proportion of the general population and a large majority of asthmatics, with sensitization rates rising globally. It contributes to a spectrum of respiratory diseases such as allergic rhinitis, allergic asthma, allergic fungal rhinosinusitis, and allergic bronchopulmonary aspergillosis, often associated with severe symptoms and poor disease control. Current management of fungal allergy relies on environmental control, pharmacotherapy, and allergen immunotherapy (AIT). However, AIT for fungal allergy remains limited

Current Evidence on Voriconazole Exposure and Individualized Therapy for Aspergillosis.

With emerging global antifungal resistance, voriconazole is a preferred first-line antifungal agent for the treatment of aspergillosis, and therapeutic drug monitoring (TDM) is frequently utilized during therapy. This review examines evidence from international guidelines, clinical studies, case reports, and population pharmacokinetic analyses to investigate the multifactorial drivers influencing voriconazole therapy. Current guidelines demonstrate variability in recommended target trough concentrations, though a range of 1.0-5.5 mg/L appears as a frequently cited reference interval; however,

Antimicrobial effects and mechanism of action of carboxymethyl chitosan-loaded silver ion complexes against drug-resistant Aspergillus fumigatus.

Based on the escalating challenge of drug-resistant Aspergillus fumigatus infections, this study developed a silver ion-loaded carboxymethyl chitosan (CMCh-Ag⁺) nanocomposite as a potent antifungal agent. The composite was successfully synthesized and characterized, revealing distinct physicochemical properties, uniform dispersion, and confirmed coordination between CMCh and Ag⁺. In vitro evaluations, including minimum inhibitory concentration (MIC), minimum fungicidal concentration (MFC), growth curve, and plate spotting assays, demonstrated that CMCh-Ag⁺ exhibited significantly superior anti

MicroRNA-146a-5p Negatively Regulates Inflammation via the Dectin-1/Syk/NF-κB Signaling Pathway in Aspergillus fumigatus Keratitis.

This study investigated the role of microRNA-146a-5p (miR-146a-5p) in the pathogenesis of fungal keratitis (FK). In a murine model of Aspergillus fumigatus (A. fumigatus) infection, miR-146a-5p was significantly upregulated. Functionally, overexpression of miR-146a-5p attenuated clinical severity and neutrophil infiltration, whereas inhibition of miR-146a-5p exacerbated corneal damage. Mechanistic studies demonstrated that miR-146a-5p downregulated pro-inflammatory cytokines (IL-1β, IL-6, TNF-α) and suppressed the Dectin-1/Syk/NF-κB signaling pathway. These findings indicate that miR-146a-5p m

Does ABPA Contribute to Bronchiectasis? A Structured Evaluation of Competing Hypotheses.

Whether allergic bronchopulmonary aspergillosis (ABPA) causes bronchiectasis or merely represents Aspergillus fumigatus colonisation of damaged airways remains debated. Establishing causality is challenging because airway damage is predominantly driven by host immune responses rather than by direct fungal invasion. We systematically evaluate four competing hypotheses: (i) ABPA causes bronchiectasis, (ii) reverse causation, with bronchiectasis predisposing to ABPA, (iii) a shared underlying factor independently producing both conditions and (iv) a spurious association. We evaluate radiological,

Case Report: Pediatric isavuconazole supratherapeutic exposure: a case of long-term use, CYP3A5 poor metabolizer genotype and concomitant CYP3A4 medications.

Isavuconazole has high oral bioavailability and a predictable pharmacokinetic profile, and is a first-line agent for invasive fungal disease in adults. In children, its use is off-label. A 14-year-old boy received oral isavuconazole (200 mg daily) for pulmonary aspergillosis after allogeneic hematopoietic stem cell transplantation. After 2 months of therapy, the steady-state trough concentration increased from 2.2 mg/L to 15.0 mg/L, well above the suggested range (2.0-5.0 mg/L). Pharmacogenetic analysis revealed a CYP3A5∗3/∗3 (poor metabolizer) genotype. The drug accumulation likely reflected

Frequency of Aspergillus fumigatus sensitization and its impact on longitudinal asthma control: a 12-week prospective cohort study.

The role of fungi as the dominant allergen in asthma pathogenesis has been extensively explored. Most intriguing has been the relationship between asthma and Aspergillus fumigatus, given its ubiquitous presence and tendency to cause a wide spectrum of diseases in humans. Existing literature suggests that Aspergillus sensitization is associated with increased asthma severity, but prospective data comparing treatment outcomes between Aspergillus-sensitized and non-sensitized asthma patients remain limited. This study aimed to (i) determine the prevalence of Aspergillus fumigatus sensitization in

[Child with inflammatory tinea capitis after visiting a barbershop].

We report the case of a 9-year-old boy presenting with inflammatory tinea capitis (kerion celsi) after repeated visits to a barbershop. Trichophyton tonsurans was identified by fungal culture, while Staphylococcus aureus and Aspergillus spp. were detected as accompanying microorganisms. The infection was considered highly likely to be barbershop-associated, most probably resulting from inadequately reprocessed hair-cutting or shaving instruments. An initial misdiagnosis as dermatitis, followed by treatment with topical hydrocortisone, was associated with further clinical progression. This case
