Bronchoalveolar Lavage Fluid Cellularity Reflects Peripheral Cytopenias and Fungal Burden but Not Short-Term Outcomes in Invasive Pulmonary Aspergillosis.

We characterized leukocyte profiles in bronchoalveolar lavage fluid (BALF) of 63 patients with hematologic malignancy and culture-documented invasive pulmonary aspergillosis and evaluated associations with peripheral blood leukocyte counts, Aspergillus burden, respiratory coinfections, and clinical outcomes. BALF cellularity mirrored peripheral cytopenias yet increased with higher Aspergillus burden and respiratory coinfections, consistent with both systemic immune availability and local inflammatory stimuli. However, BALF cellularity was not independently associated with 42-day mortality, whi

Aspergillus tubingensis pneumonia with pulmonary oxalosis in a virologically suppressed person with HIV: A case report.

Opportunistic aspergillosis has occasionally been reported in individuals with advanced HIV infection but remains rare overall and is particularly uncommon under well-controlled conditions. Here, we present a case of Aspergillus tubingensis pneumonia in a virologically suppressed person with HIV (PWH) in his late sixties. He was receiving bictegravir-based antiretroviral therapy, with a CD4 cell count of 368/μL and undetectable HIV RNA two months before symptom onset. His symptoms began with a cough, followed 20 days later by a fever and dyspnea, which required hospitalization. Computed tomogr

Cerebral Aspergillosis in a Patient on Ibrutinib: A Case of Diagnostic Ambiguity and Multidisciplinary Care.

This case report describes the challenging diagnosis and management of cerebral aspergillosis in a patient with chronic lymphocytic leukaemia undergoing treatment with a BTK inhibitor. The initial presentation was deceptively unspecific, comprising cognitive changes, receptive aphasia, gait instability and mild neurological deficits without fever or significant systemic inflammation. The combination of a muted and non-indicative clinical picture and a broad differential diagnosis reflects the evolving complexity of CNS infections in patients treated with targeted haematological therapies. Economic burden of chronic pulmonary aspergillosis in Uganda: a cost-of-illness analysis.

OBJECTIVES: Chronic pulmonary aspergillosis (CPA) is a complication of tuberculosis that is under-recorgnizedin resource-limited settings. We quantified the economic burden of CPA in Uganda from societal, healthcare system, and household perspectives. METHODS: We used a prevalence-based cost-of-illness approach with an estimated 26,765 annual CPA cases, stratified by clinical subtype. Direct medical costs were obtained from local health facilities and Joint Medical Stores pricing. Undiagnosed cases were modeled through a tuberculosis misdiagnosis pathway with 20% annual mortality. Bortezomib in combination with standard-of-care for late active antibody-mediated rejection with de novo donor-specific antibodies after kidney transplantation: a multicenter randomized trial.

Bortezomib has been proposed as a treatment for antibody-mediated rejection (AMR) after kidney transplantation; however, its efficacy remains uncertain. We conducted a multicenter, randomized, open-label trial in France between February 2015 and July 2019 in adult kidney transplant recipients with late active AMR associated with de novo donor-specific antibodies (dnDSA). Sixty of the planned 100 patients were randomized 1:1 to receive standard-of-care consisting of corticosteroids, plasmapheresis, and intravenous immunoglobulin, either alone or with bortezomib administered as two cycles of fou

Voriconazole-associated peripheral polyneuropathy: A case report.

Invasive fungal infections, especially aspergillosis, severely affect immunocompromised patients. The use of azoles, particularly voriconazole, has been considered an effective antifungal therapy for the treatment of these infections and prevention. However, cases of peripheral neuropathy have been reported in patients treated with this drug. We present two clinical cases of patients with immunocompromise (acute myeloblastic leukemia and primary immunodeficiency) who, during treatment with voriconazole, developed peripheral sensory motor axonal polyneuropathy, which completely resolved after d

Refractory Invasive Pulmonary Aspergillosis Following Treatment of Acute Rejection in a Kidney Transplant Recipient: A Case Report.

Invasive pulmonary aspergillosis (IPA) is an uncommon but highly morbid complication after solid organ transplantation, particularly following treatment for acute rejection. We report a case of refractory IPA caused by Aspergillus terreus in a 70-year-old kidney transplant recipient, diagnosed 2 weeks after receiving thymoglobulin and pulse corticosteroids for acute T cell-mediated rejection. The patient presented with acute hypoxic respiratory failure requiring intubation, and a chest computed tomography scan demonstrated a large, necrotic, mass-like cavitary opacity in the left upper lobe an

Normothermic regional perfusion (NRP) use in controlled donation after circulatory determination of death (cDCDD): results of the European Society for Organ Transplantation Bucharest consensus conference.

Normothermic Regional Perfusion (NRP) is emerging as a game-changer in enhancing outcomes for Donation after Circulatory Determination of Death (DCDD). NRP maintains physiological conditions through perfusion with oxygenated blood, outperforming conventional super-rapid recovery techniques significantly improving outcomes and organ utilization. Despite its clinical benefits, widespread adoption of NRP is impeded by heterogeneous organizational, legal, and ethical frameworks. At the ESOT Bucharest Consensus Conference, leading experts in transplantation achieved consensus on 130 relevant NRP-re

Post-traumatic Aspergillus niger empyema in an immunocompetent military patient: a case report.

BACKGROUND: Fungal empyema is an uncommon clinical entity, and empyema attributable to Aspergillus species is exceedingly rare. The vast majority of reported cases occur in immunocompromised hosts. Empyema caused specifically by Aspergillus niger has been described only in exceptional circumstances and is virtually absent from the medical literature. We report the first documented case of post-traumatic Aspergillus niger empyema in a young, immunocompetent individual. CASE PRESENTATION: A 22-year-old male active-duty special forces soldier sustained a penetrating gunshot wound to the left tho

GM-CSF signaling on monocytes and monocyte-derived dendritic cells is required for effective pulmonary immunity to Aspergillus fumigatus.

UNLABELLED: Aspergillus fumigatus is the most common cause of invasive aspergillosis (IA), an opportunistic pulmonary infection of immunocompromised patients that can be fatal, despite treatment with modern antifungal drugs. During infection, myeloid cells, such as neutrophils, monocytes, and monocyte-derived dendritic cells (Mo-DCs) are recruited to the lung and are critical for phagocytosing and killing A. fumigatus spores. Recent evidence has shown that intracellular crosstalk between these professional immune cells and other resident pulmonary cell types is required for an effective immune

Efficacy and safety of tirabrutinib monotherapy for relapsed/refractory primary CNS lymphoma: PROSPECT phase 2 trial.

Primary central nervous system lymphoma (PCNSL) is an aggressive subtype of non-Hodgkin lymphoma with a poor prognosis, high rate of refractory or relapsed (R/R) disease, and no established standard of care for R/R PCNSL. This phase 2 study evaluated the efficacy and safety of tirabrutinib, a potent, highly selective, second-generation Bruton tyrosine kinase inhibitor, in patients with R/R PCNSL in the US. In the open-label PROSPECT study, adults with R/R PCNSL whose disease progressed on or after ≥1 high-dose methotrexate-based therapy received once-daily oral tirabrutinib 480 mg. The primary

DETECTX: a novel deep learning approach for detecting pneumonia, tuberculosis, and aspergillosis from chest X-rays.

BACKGROUND: Aspergillosis, pneumonia, and tuberculosis are distinct respiratory illnesses, but they share similar symptoms, including cough, fever, and shortness of breath. This overlap can make accurate diagnosis challenging. Delayed or incorrect identification, particularly of aspergillosis, can lead to worsening patient outcomes and significant lung damage. METHODS: To address this diagnostic challenge, we developed a deep learning based system designed to differentiate between aspergillosis, pneumonia, tuberculosis, and healthy lungs using chest X-ray images. We implemented three models:

Comparing the real-world effectiveness of oral corticosteroids and antifungals for the treatment of allergic bronchopulmonary aspergillosis in cystic fibrosis.

BACKGROUND: Allergic bronchopulmonary aspergillosis (ABPA) is a complication of cystic fibrosis (CF) that often requires treatment. Oral corticosteroids (OCS) are the first-line therapy; however, antifungals alone or in combination with OCS are commonly used despite limited data supporting their role. RESEARCH QUESTION: What factors are associated with antifungal treatment initiation for ABPA in people with CF? Do antifungals, with or without OCS, improve respiratory outcomes compared to OCS alone? STUDY DESIGN AND METHODS: We conducted a retrospective cohort study of people with CF with fir

Rare clinical presentation of invasive pulmonary aspergillosis in patient with early-stage lung cancer: a case report.

Invasive pulmonary aspergillosis (IPA) presents life-threatening complication mostly seen in patients with severe neutropenia or in patients with solid organ cancers after applied anticancer therapy. The aim of this manuscript is to identify clinical manifestations of IPA in lung cancer patient. A 65-year-old male with a history of severe chronic obstructive lung disease and prior laryngeal carcinoma was admitted to hospital due to fever. Five months before he was operated because of early-stage squamous cell lung carcinoma. Postoperative course was complicated with subcutaneous emphysema and

Methodological quality and risk of bias in non-randomised comparative effectiveness studies of invasive mould infections: a systematic review.

Many treatment recommendations for invasive mould infections, particularly those caused by rare moulds, rely on non-randomised comparative effectiveness research (CER), which is vulnerable to design and analytical limitations that can bias effect estimates. We systematically reviewed 117 non-randomised studies evaluating interventions for aspergillosis, mucormycosis, fusariosis, scedosporiosis, lomentosporiosis, or phaeohyphomycosis published from 2003 to 2024. We assessed structural susceptibility to bias from misalignment of eligibility, treatment assignment, and start of follow-up, and appr

A synergistic antifungal agent from Ligusticum chuanxiong: Total synthesis, biological evaluation, and mechanism of ligustrumdiol A.

Invasive fungal infections threaten immunocompromised patients, and the toxicity, resistance, and narrow spectrum of current therapies demand new agents. A chemical investigation of the medicinal plant Ligusticum chuanxiong led to the isolation of a rare pentyl-substituted cyclohexene derivative, ligustrumdiol A (LDA). Its structure was fully elucidated by spectroscopic analysis, and an enantioselective total synthesis was accomplished in 12 steps from d-quinic acid with an overall yield of 11.2%. Although LDA alone showed only weak antifungal activity against Aspergillus fumigatus (MIC = 400

Reconstructing host-pathogen interactions: analysis of the intercellular network of macrophages and other immune cells within an aspergillosis-on-chip model.

The human-pathogenic fungus Aspergillus fumigatus can cause life-threatening infections, particularly in immunosuppressed or neutropenic patients with hematological malignancies. The human antifungal immune response primarily relies on neutrophils with contribution from macrophages, while the roles of unconventional lymphocytes such as Natural Killer (NK) cells and mucosal-associated invariant T (MAIT) cells remain less understood. Therefore, using an advanced microfluidic "invasive aspergillosis-on-chip" (IAC) model, we investigated the coordinated antifungal response of various immune cell t
