Invasive pulmonary aspergillosis (IPA) presents life-threatening complication mostly seen in patients with severe neutropenia or in patients with solid organ cancers after applied anticancer therapy. The aim of this manuscript is to identify clinical manifestations of IPA in lung cancer patient. A 65-year-old male with a history of severe chronic obstructive lung disease and prior laryngeal carcinoma was admitted to hospital due to fever. Five months before he was operated because of early-stage squamous cell lung carcinoma. Postoperative course was complicated with subcutaneous emphysema and

Many treatment recommendations for invasive mould infections, particularly those caused by rare moulds, rely on non-randomised comparative effectiveness research (CER), which is vulnerable to design and analytical limitations that can bias effect estimates. We systematically reviewed 117 non-randomised studies evaluating interventions for aspergillosis, mucormycosis, fusariosis, scedosporiosis, lomentosporiosis, or phaeohyphomycosis published from 2003 to 2024. We assessed structural susceptibility to bias from misalignment of eligibility, treatment assignment, and start of follow-up, and appr

Invasive fungal infections threaten immunocompromised patients, and the toxicity, resistance, and narrow spectrum of current therapies demand new agents. A chemical investigation of the medicinal plant Ligusticum chuanxiong led to the isolation of a rare pentyl-substituted cyclohexene derivative, ligustrumdiol A (LDA). Its structure was fully elucidated by spectroscopic analysis, and an enantioselective total synthesis was accomplished in 12 steps from d-quinic acid with an overall yield of 11.2%. Although LDA alone showed only weak antifungal activity against Aspergillus fumigatus (MIC = 400

The human-pathogenic fungus Aspergillus fumigatus can cause life-threatening infections, particularly in immunosuppressed or neutropenic patients with hematological malignancies. The human antifungal immune response primarily relies on neutrophils with contribution from macrophages, while the roles of unconventional lymphocytes such as Natural Killer (NK) cells and mucosal-associated invariant T (MAIT) cells remain less understood. Therefore, using an advanced microfluidic "invasive aspergillosis-on-chip" (IAC) model, we investigated the coordinated antifungal response of various immune cell t

INTRODUCTION: Bruton Tyrosine kinase inhibitors (BTKi) impair anti-aspergillus immune responses and increase the risk of invasive fungal infections (IFI). More selective second generation BTKi have fewer off-target effects thus a better tolerance than ibrutinib. Though clinical data suggest the incidence of IFI is lower with second generation BTKi, whether they have less impact on anti-fungal immunity remain unclear.
METHODS: We studied the activation phenotype and behavior of human neutrophils in vitro with fungal extracts or living Aspergillus fumigatus in the presence of either ibrutinib o

Mixed pulmonary infection by Aspergillus and Mucorales is a rare condition but highly fatal in immunocompromised hosts. We report a 61-year-old diabetic man with diabetic kidney disease who presented with fever and cough. Bronchoscopy-guided lung biopsy and lavage MUCOR 28S PCR assay confirmed dual invasive Aspergillus and Mucorales coinfection. He developed massive hemoptysis, septic shock, and eventually succumbed despite treatment with antibiotics, liposomal amphotericin B and posaconazole. This case underscores the importance of early diagnosis, antifungal therapy and vigilant monitoring i

BackgroundSymptomatic small-sized lumbar disc herniation (small LDH) presents a significant challenge in elite athletes, where rapid return to competition is paramount. This study aimed to characterize the clinical and imaging features of small LDH in this population and to evaluate the efficacy of transforaminal endoscopic lumbar discectomy (TELD) under local anesthesia.MethodsWe retrospectively reviewed the medical records of 11 elite athletes (12 discs) who underwent TELD for small LDH, defined as a herniation not exceeding 25% of the spinal canal's anteroposterior diameter on magnetic reso

Allergic bronchopulmonary aspergillosis (ABPA) is a complex hypersensitivity lung disease driven predominantly by type 2 immune responses. However, the classical immunological paradigm fails to adequately explain the persistent airway structural damage and frequent relapses observed even after inflammatory control is achieved. This review proposes a fibroblast-centered mechanistic framework that synthesizes evidence from ABPA, fungal-associated airway diseases, and pulmonary fibrosis to systematically delineate the mechanisms linking fibroblasts to chronic inflammation and airway remodeling. I

The case study presented involves a paediatric patient treated with voriconazole for fungal endocarditis caused by Aspergillus flavus Therapeutic plasma concentrations of voriconazole could not be achieved with the established dosing regimen and administration frequency. In response to this situation, and after successive dose increases and a pharmacogenetic analysis that confirmed the patient was not a CYP2C19 or CYP2C9 rapid or ultra-rapid metaboliser, a new administration frequency of every 8 hours was proposed, with limited evidence, but which allowed achieving target concentrations and pa

RATIONALE: Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitivity disease triggered by inhaled Aspergillus -fumigatus (Af) spores (i.e., conidia), in persons with cystic fibrosis (CF). High iron levels have been commonly described in CF respiratory secretions. Af proteases cause a severe allergic immune response, leading to pulmonary exacerbations and progressive lung function decline. The stimuli that promote Af proteases in ABPA are poorly understood.
OBJECTIVES: Determine if airway iron is a critical factor in CF-ABPA by regulating fungal protease production.
METHODS: We use

BACKGROUND: Influenza-associated pulmonary aspergillosis (IAPA) is a significant fungal complication in patients with influenza-induced acute-respiratory-distress-syndrome (ARDS). The impact of empirical antifungal treatment on IAPA incidence and outcomes remains unclear.
METHODS: In this observational multicenter study (9 treatment centers), we included all consecutive patients admitted to intensive care units (ICUs) with influenza-associated ARDS between 1 September 2016 and 1 March 2025. We compared patients receiving empirical antifungal treatment with those who did not, focusing on 30-da

Cystic fibrosis (CF) is caused by mutations in the CFTR gene resulting in alterations in the immune functions of the airway epithelium, including reduced biosynthesis of the specialized pro-resolving lipid mediators (SPMs). Aspergillus fumigatus is the most isolated filamentous fungus in patients with CF. This study explore whether specialized pro-resolving lipid mediators (SPMs) regulate the barrier function of airway epithelium in vitro during colonization by A. fumigatus. We have shown that mature A. fumigatus hyphae disrupt tight junctions in primary cultures of nasal epithelium from CF an

BACKGROUND: Chronic pulmonary aspergillosis (CPA) usually develops in patients with pre-existing lung damage. However, little is known about potential underlying immune defects that might predispose patients to developing this debilitating condition.
METHODS: We performed immunological analyses in 21 patients with CPA and 14 healthy controls. Polymorphonuclear granulocytes (PMNs) and peripheral blood mononuclear cells (PBMCs) were isolated from venous blood. We measured reactive oxygen species (ROS) production and Aspergillus fumigatus killing capacity in PMNs and cytokine production by PBMCs

Aspergillosis caused by Aspergillus fumigatus is a clinical issue of such severity that the World Health Organization has designated this organism as 1 of the 4 most critical fungi to study. Progress in A. fumigatus has been limited by the availability of genetic tools with which to study this filamentous fungus. Currently available means of altering the dosage of genes and gene products include construction of disruption mutants as well as regulated promoters. These are powerful techniques but somewhat limited for the analysis of essential genes. Here we describe a new method that permits reg

BACKGROUND: All triazoles decrease the metabolism of calcineurin inhibitors (CNIs) and mammalian target of rapamycin (mTOR) inhibitors through CYP3A4 and P-glycoprotein inhibition leading to increased exposure and the potential for serious adverse events (SAEs).
OBJECTIVES: We sought to describe triazoles and CNI and mTOR inhibitor use in solid organ transplantation (SOT) recipients hospitalized for invasive aspergillosis (IA).
PATIENTS AND METHODS: We included adults with ≥1 claim for an IA admission in a US claims database from October 2015 to November 2022 who received systemic antifungal

Primary cutaneous aspergillosis is a rare fungal infection in extremely preterm neonates. We report a male infant born at 25+5 weeks' gestation who developed necrotic lesions at the site of an umbilical catheter covered with an occlusive dressing. Culture confirmed Aspergillus fumigatus, and systemic liposomal amphotericin B combined with local enzymatic debridement and a non-adherent silicone wound contact layer was initiated after receipt of the culture result. The lesions resolved completely, leaving only a fine residual scar without functional impairment. This case emphasises the importance

BACKGROUND: Aspergillus candidus (A. candidus), while an uncommon clinical isolate, represents an emerging cause of severe invasive fungal disease, particularly among individuals with compromised immunity. Patients with hematologic or solid tumor malignancies-especially those with concomitant structural lung damage, such as resulting from long-term heavy smoking-are at heightened risk.
CASE PRESENTATION: We describe a case of chronic pulmonary aspergillosis caused by A. candidus in a 66-year-old man with untreated lung cancer. The patient had a 40-year history of heavy smoking, with known chr
