Characteristics of individual cyp51A SNPs and combinations thereof impacting the azole phenotype in TR34-mediated resistance genotypes of Aspergillus fumigatus.

The World Health Organization has flagged the rise of drug resistance in Aspergillus fumigatus as a critical concern. Elevated mutation rates in this pathogen contribute to the rapid development of resistance, complicating treatment efforts. We conducted a study on the prevalence of azole resistance among clinical A. fumigatus isolates in the Netherlands from 1994 to 2022 and identified 34 cyp51A variants. To investigate the impact of individual single-nucleotide polymorphisms (SNPs) and combinations thereof on the azole phenotype in TR34-mediated resistance genotypes, we focused on novel, rec

[Allergic bronchopulmonary aspergillosis-essential knowledge for radiology].

CLINICAL/METHODICAL ISSUE: Allergic bronchopulmonary aspergillosis (ABPA) is a rare but potentially severe hypersensitivity reaction to Aspergillus fumigatus, primarily affecting patients with asthma or cystic fibrosis. Early diagnosis is crucial to prevent irreversible lung damage. STANDARD RADIOLOGICAL METHODS: High-resolution computed tomography (HRCT) is the standard imaging method for detecting bronchiectasis and other typical findings. Common features include central, saccular bronchiectasis, mucus impaction, and centrilobular micronodules. Radiological assessment must be closely integr

Assessment of the performances of blood tests for the antemortem diagnosis of aspergillosis in wild or captive aquatic birds.

The antemortem diagnosis of aspergillosis in birds remains a complex challenge. A variety of diagnostic methods are currently available, including direct detection of Aspergillus components, antibody-based assays, and nonspecific markers such as serum protein electrophoresis (SPE), but their diagnostic performances remain limited. The objective of this study was to assess and compare the performances of several diagnostic approaches, specifically, galactomannan index measurement, beta-D-glucan assay, 3-hydroxybutyrate quantification, SPE, and mannoprotein detection tests. A secondary objective

Characterization of a bZIP Transcription Factor ZipD in Aspergillus flavus.

Aspergillus flavus produces aflatoxin B1, a Group 1 carcinogen that causes severe economic and agricultural losses and can lead to invasive aspergillosis in humans. During its growth, A. flavus develops hyphae that form a continuous mycelium, which is associated with host colonization and toxin production. Transcription factors are considered regulators of fungal development and pathogenicity. Among these families, bZIP proteins are associated with growth, metabolism, and stress tolerance. In A. fumigatus, the bZIP transcription factor ZipD regulates calcium signaling pathways and cell wall in

Changes in the fungal ecology in the era of CFTR modulators: Results from a French multicentre study focused on cystic fibrosis airways.

The introduction of CFTR modulator therapies (CFTRmt) has changed cystic fibrosis (CF) management. By improving airway rheology and function in people with CF (pwCF), CFTRmt are expected to modify cyto-microbiological features. This French multicentre study aimed to assess changes in airway fungal ecology before and during the CFTRmt era. Data from pwCF followed at CF reference centres in Besançon, Bordeaux, Limoges, and Rennes were collected before CFTRmt use (2014) and after their widespread implementation (2022), including elexacaftor/tezacaftor/ivacaftor (ETI) as well as other CFTR modulat

[Invasive aspergillosis of the sphenoid sinus as a differential diagnosis in lesions of the sellar region].

We present a case of a 60-year-old woman with a history of poorly controlled type 2 diabetes who came to the emergency room with severe headache, nausea, and vomiting that had been ongoing for one week. Her vision was progressing with decreased visual acuity and diplopia. A MRI of the brain and sellar region was ordered, which revealed a lesion occupying the sphenoid sinus. Biochemical parameters included leukocytosis, hyperglycemia, central hypothyroidism and hypogonadism. Considering these findings, an interdisciplinary approach was undertaken to initiate a diagnostic and therapeutic algorit

Primary Therapy for Invasive Aspergillosis With Triazoles or L-AmB: A Multicenter Retrospective Study.

OBJECTIVE: To compare clinical outcomes of patients treated with liposomal amphotericin B (L-AmB) versus mold-active triazoles as primary treatment for invasive aspergillosis (IA). METHODS: Retrospective study of adult patients treated with either L-AmB or triazoles for proven or probable IA at 2 academic hospitals over a 10-year period. The primary endpoint was all-cause 90-day mortality from IA diagnosis. Landmark trial emulation at day 7 postdiagnosis was used to compare initial triazole versus L-AmB for IA. Confounding by indication was addressed using inverse probability of treatment wei

Time to re-evaluate combination antifungal therapy for invasive aspergillosis and other invasive mycoses.

Mortality from invasive aspergillosis remains unacceptably high, and the currently recommended treatment of triazoles is further challenged by rising resistance to these first-line agents. A previous randomised controlled trial of combination therapy with voriconazole plus anidulafungin versus voriconazole alone showed that the combination was safe and associated with a clinically significant reduction in all-cause mortality that, however, failed narrowly to meet conventional levels of significance. In this Personal View, we argue against a binary, negative interpretation of these results, and

A clinically significant interaction between voriconazole and rifapentine: a case report and review of evidence.

We present a case of concurrent pulmonary aspergillosis and tuberculosis in a 53-year-old male, treated with voriconazole and rifapentine. In this case, co-administration with rifapentine resulted in a markedly lower voriconazole trough concentration (0.4 μg/mL on day 7) compared to that without it (4.3 μg/mL on day 25), reflecting a 90.7% reduction. After rifapentine was discontinued and the voriconazole dose was increased to 300 mg q12h intravenously (day 8), the trough concentration remained at 0.4 μg/mL two days later (day 10). Subsequently, it increased to 3.0 μg/mL by day 14 (6 days post

Subclavian mycotic aneurysm caused by Aspergillus flavus in a child with acute lymphoblastic leukemia: a case report.

Invasive aspergillosis is a severe opportunistic infection in immunocompromised children, particularly those receiving chemotherapy for hematologic malignancies. We report the case of a 2-year-old girl with acute lymphoblastic leukemia who developed massive hemoptysis during consolidation chemotherapy. Thoracic computed tomography revealed a saccular pseudoaneurysm of the proximal left subclavian artery. Surgical resection and autologous vein graft replacement were performed, and Aspergillus flavus DNA was detected in the resected tissue using Aspergillus-specific polymerase chain reaction. Th

A neutrophil membrane-biomimetic drug delivery system enhances the antifungal and anti-inflammatory efficacy of natamycin for fungal keratitis.

Fungal keratitis (FK) is a potentially blinding infectious corneal disease. The invasion of fungi into corneal tissue and the resulting inflammatory response primarily drive the rapid corneal damage. Although natamycin (NAT) is the first-line treatment for FK, its clinical efficacy faces challenges due to several drawbacks, including poor anti-inflammatory effects, low water solubility, frequent dosing requirements, and considerable ocular surface irritation. In our study, we aimed to develop a novel neutrophil membrane-biomimetic drug delivery system to solve these challenges of NAT and and e

Genome-wide discovery and phenotyping of non-coding transcripts in A. fumigatus reveals lncRNAs with a role in antifungal drug sensitivity.

Recent data suggests one fungus, Aspergillus fumigatus, causes more deaths annually than HIV or malaria combined. Coupled with rapid emergence of antifungal drug resistance, the limited range of effective treatments, and mortality rates of >50%, aspergillosis represents a major challenge in infectious diseases. Recent studies have identified long-noncoding RNAs (lncRNAs) involved in drug resistance and virulence in pathogenic yeasts such as Candida spp. However, there is very limited knowledge of lncRNAs in human pathogenic moulds, including A. fumigatus. Here we exploit transcriptomics data o

Virulence of Aspergillus flavus and relatives using the Galleria mellonella model.

Aspergillus section Flavi encompasses multiple species, with A. flavus being significant for human health because of its dual role as a major aflatoxin producer and opportunist. However, the mechanisms underlying its virulence remain incompletely understood. This study evaluates the pathogenic potential of A. flavus and its relatives using the Galleria mellonella infection model. Twenty-six A. flavus isolates (clinical and environmental) and 17 relatives/domesticated species were tested in G. mellonella, with larval survival monitored over 7 d. Histology, direct microscopy, and culture were us

How Should IgG Seroconversion to Aspergillus fumigatus be Interpreted in Children With Cystic Fibrosis.

BACKGROUND: Cystic fibrosis (CF) is one of the most common autosomal recessive diseases in the world population. Allergic bronchopulmonary aspergillosis (ABPA) is a severe complication of CF, the diagnosis of which is based on symptoms and blood IgE levels. However, many techniques of specific IgG levels' measures are used, whose clinical significance is still unclear. We evaluated the clinical evolution of CF in children who presented a first A. fumigatus IgG seroconversion. METHODS: Monocentric pediatric case-control study led in Rouen, France. Every patient with a first A. fumigatus IgG se
