Prematurity, temperament, physical wellbeing and maternal mental health in a low-income sample.

Preterm birth (i.e., before 37-week gestation) is a global health concern, with most prematurity occurring between 32 - 36.6 weeks gestation, yet research has primarily focused on extreme prematurity (i.e., before 32-weeks gestation). As such, less is known about the developmental and psychological outcomes associated with moderate-to-late preterm birth. This study examines how infant temperament and infant physical wellbeing are associated with maternal mental health, and whether moderate-late prematurity moderates these relationships in a low-income sample. Using a matched case-control desig

Ensemble learning for predicting birth trauma using high-dimensional data in the neonatal intensive care unit.

Birth trauma remains a significant global health challenge and contributes substantially to neonatal morbidity and mortality. Although machine learning (ML) approaches have shown promise in predicting birth trauma, persistent limitations related to class imbalance, model interpretability, and incomplete characterization of risk hinder their clinical utility. This study aimed to develop and evaluate an ensemble ML framework capable of leveraging high-dimensional clinical data to capture both established and latent risk factors while maintaining transparency for clinical interpretation. We imple

Pelvic floor symptoms and quality of life 1 year postpartum in Swedish primiparous women-A follow-up of a randomized controlled trial.

INTRODUCTION: Long-term pelvic floor symptoms after childbirth may impair women's quality of life. The aim of this study was to assess the prevalence of self-reported pelvic floor symptoms 1 year postpartum in primiparous women by degree of perineal tear, with a focus on minor and major second-degree tears, and their association with quality of life. MATERIAL AND METHODS: This prospective cohort study was based on data collected within a randomized controlled trial (the Oneplus trial). Women in the trial who had a vaginal birth and responded to a 1-year postpartum follow-up questionnaire were

Macrolides for the prevention of bronchopulmonary dysplasia in preterm neonates.

RATIONALE: Bronchopulmonary dysplasia (BPD) continues to be the most frequent complication of prematurity despite advances in neonatal care. It occurs from interactions between antenatal exposures, postnatal oxygen, ventilation-mediated injury as well as other postnatal injuries that induce a proinflammatory state in an immature developing lung. Macrolides, particularly azithromycin, have anti-inflammatory actions that may play a role in preventing BPD. A current review is required to investigate whether macrolide use in the highest-risk patient population, intubated and ventilated very preter

Editorial Commentary: Estrogen Influences Shoulder Health.

Estrogen influences bone mineral density, muscle quality and quantity, and collagen synthesis, rotator cuff tearing, worse outcomes after rotator cuff repair, hyperlaxity, and adhesive capsulitis. In this commentary, we discuss the potential explanations for the highly significant associations between hormonal systemic birth control and a variety of shoulder pathologies. Ultimately, recent literature suggests that future research should be performed to better understand the connection between sex hormones and musculoskeletal function.

Treatment guided by cerebral oximetry monitoring in extremely preterm infants: a Bayesian analysis of the SafeBoosC-III randomised clinical trial.

PURPOSE: The SafeBoosC-III trial compared treatment guided by cerebral oximetry monitoring for the first 72 h after birth with usual care in 1601 extremely preterm infants. Incidence of death or severe brain injury at 36 weeks of postmenstrual age did not differ between the cerebral oximetry and usual care group (relative risk with cerebral oximetry, 1.03; 95% CI 0.90 to 1.18). To assess the probability of clinically important benefit or harm, we conducted secondary analyses in a Bayesian framework. METHODS: Primary analyses used a weakly informative prior assuming a wide range of effects, to

Impact of episiotomy on anal incontinence following second- to fourth-degree perineal tears-A cohort study.

INTRODUCTION: Anal incontinence can have a significant negative impact on quality of life. The leading cause of anal incontinence among women of child-bearing age is obstetric-related perineal injury. The objective of this study was to examine the impact of episiotomy on the incidence of anal incontinence among primiparous women with second-, third-, or fourth-degree perineal tears at 8 weeks and 1 year postpartum and to assess change in incidence over time across varying degrees of perineal tears. MATERIAL AND METHODS: This Swedish cohort study included 12 658 primiparous women who sustained

Trends and urban-rural disparities in infant and cause-specific mortality in Shaanxi Province, China, 2014-2023: a population-based study.

BACKGROUND: China has achieved substantial reductions in infant mortality; however, pronounced subnational disparities persist, particularly in western and rural regions. In the context of demographic change and evolving perinatal risk profiles, region-specific evidence is essential to guide targeted interventions and support progress toward Sustainable Development Goal (SDG) 3.2. METHODS: This population-based observational study analyzed data from the Maternal and Child Health Surveillance System (MCHSS) in Shaanxi Province, China, from 2014 to 2023. A total of 320,259 live births and 954 i

The Burden of Lower Limb Deformities in Low- and Lower-Middle-Income Countries: A Scoping Review.

UNLABELLED: Musculoskeletal (MSK) conditions like lower limb deformities significantly contribute to the global burden of disease. Low-income countries (LICs) and lower-middle-income countries (LMICs) are disproportionately affected, yet specifics surrounding MSK conditions in these regions remain largely unquantified. This scoping review seeks to (1) assess the epidemiologic information currently available for lower limb deformities in LICs and LMICs, including etiology and treatment, as well as (2) examine the individual, societal, and socioeconomic impact of these conditions. A scoping revi

War, what is it good for? orthopedics the evolution of orthopedic surgery through armed conflict.

Orthopedic surgery has advanced through the demands of managing complex musculoskeletal trauma on the battlefield. The purpose of this review was to briefly summarize how successive conflicts have shaped principles and practice of orthopedic surgery-from wound management, fracture stabilization, limb salvage, prosthetics, and trauma systems-and to outline possible orthopedic input in future warfare and human exploration. From early empiricism in the Revolutionary War to organized military hospital systems in the Civil War, to the use of antibiotics and internal fixation in World War II, each a

The intersection of neuroscience, socioeconomic status, and preterm birth.

Preterm birth is a leading cause of brain dysmaturation and neurocognitive impairment in childhood. While both dysmaturation and adverse outcomes are often attributed to low gestational age and PTB-associated risk factors such as chorioamnionitis, immune dysregulation, suboptimal nutrition, and common co-morbidities, recent studies suggest socioeconomic inequalities also play an important role. In this review, we summarize current etiological evidence linking socioeconomic status with i) brain injury/dysmaturation defined using magnetic resonance imaging, and ii) cognitive and behavioral outco

A systematic review of nonpharmacological interventions to improve trauma symptoms and mother-infant bonding for women who experience birth trauma.

INTRODUCTION: Childbirth trauma affects a significant number of women and their families worldwide, with a broad range of prevalence estimations in Australia from 28% to 45%. Birth trauma has been found to influence the early mother-infant relationship. The role of occupational therapy within the perinatal context is emerging, although evidence of occupation-based interventions for women following birth trauma is sparse. This study aimed to (1) synthesise the literature on non-pharmacological interventions that address both childbirth-related trauma symptoms and the mother-infant relationship

Intrauterine Ulceration of the Lower Limbs: A Not-so-rare Complication Following Vesicoamniotic Shunting with Somatex.

INTRODUCTION: The recent use of Somatex shunts in the prenatal drainage of megacystis remains poorly documented. Here, we describe a number of complications affecting the cutaneous and muscular tissues that have not been reported to date. METHODS: The records of all male patients born alive after receiving a vesicoamniotic shunt were studied retrospectively. The primary end point was the presence of a cutaneous partially muscular lesion at birth. In addition, we analyzed the impact of shunt placement timing on the occurrence of these injuries. RESULTS: Thirty patients were included. Seven pa

Global and temporal trends in neonatal and under-five mortality due to birth asphyxia/trauma and prematurity (2000-2021) with projections up to 2040.

BACKGROUND: Despite substantial global progress in reducing neonatal mortality, the deceleration in neonatal mortality decline in many low- and middle-income countries threatens attainment of Sustainable Development Goal 3.2. Intrapartum-related events and complications of preterm birth remain the leading causes of neonatal deaths. Therefore, this study aimed to examine temporal trends in neonatal and under-five mortality due to birth asphyxia, trauma, and prematurity from 2000 to 2021, with projections to 2040. METHODS: This study used the United Nations Inter-agency Group for Child Mortalit
