Neonatal Kidney Function, Injury and Drug Dosing: A Contemporary Review.

In neonates, estimation of the glomerular filtration rate is problematic, and assessment of renal impairment is challenging. Serum creatinine is a widely used marker, and urine output is an important vital parameter monitored in intensive care settings, particularly in unwell neonates. However, the rapid changes after birth with adaptation to the extrauterine environment is a unique situation in which absolute serum creatinine is not a reliable indicator of renal function. A rise in serum creatinine from the previous value during the neonatal period can be a result of worsening renal function

Remote screening protocol for functioning and contextual factors (e-Followkids) in Brazilian children with biological risk in the first 2 years: A longitudinal prospective study.

Children with biological risk may experience developmental delays, even in the absence of brain injury. Identifying warning signs and knowledge of contextual factors are vital to favor intervention. Telehealth can aid in screening by providing access for populations that face physical and social barriers. As such, the aim of this study was to describe the e-Followkids screening protocol and how its feasibility will be assessed. A multicentric longitudinal prospective study with 80 caregivers of children born with some form of biological risk: preterm, low birth weight or perinatal complication

In-depth single-cell transcriptomic exploration of the regenerative dynamics in stony coral.

Coral reef ecosystems face escalating threats from anthropogenic global climate challenges, leading to frequent bleaching events. A key issue in coral transplantation is the inability of fragments to rapidly grow to sizes that can resist environmental pressures. The observation of accelerated growth during the early stages of coral regeneration provides new insights for addressing this challenge. To investigate the underlying molecular mechanisms, we study the fast-growing stony coral Acropora muricata. Using single-cell RNA sequencing, bulk RNA sequencing, and high-resolution micro-computed t

Statistical learning to identify and characterise neurodevelopmental outcomes at 2 years in babies born preterm: model development and validation using population-level data from England and Wales.

BACKGROUND: Children born preterm face elevated risks of neurodevelopmental impairments across domains. Prior studies have relied on expert-imposed typologies within single domains. This study applies statistical learning to a national database to identify transdomain clusters and their maternal and neonatal predictors. METHODS: Latent class analysis (LCA) was used to derive transdomain clusters from parent-reported visual, auditory, neuromotor, and communication impairments in preterm-born children at two years corrected age using the UK National Neonatal Research Database data (N = 27,261).

Neurological Outcomes in Late Preterm Infants: An Updated Review of Recent Research and Clinical Insights.

Research on late preterm infants is limited compared with extremely low birth weight infants, despite their vulnerability to brain injury. Early intervention is crucial, as these infants often face higher risks of cerebral palsy and developmental delays. This review examines methods to predict neurological outcomes and evaluates standard care protocols for neurologically affected late preterm infants. It also explores the potential for developing a comprehensive care bundle that integrates family involvement and delineates the responsibilities for continuous developmental monitoring. A total o

Acute Kidney Injury in the Neonatal Period: Retrospective Data and Implications for Clinical Practice.

Background: Neonates, particularly those born prematurely or with low birth weight, face an elevated risk of developing Acute Kidney Injury (AKI) due to various factors. Perinatal and maternal considerations, often linked to preterm delivery, contribute to this heightened risk. Methods: A retrospective study of neonates admitted to the intensive care unit at a single Israeli Hospital who were diagnosed as having AKI. The study includes follow-up data on these children. Results: During the study period, 971 neonates were admitted to the Pediatric Intensive Care Unit (PICU), and 47 cases had a d

High-throughput sequencing unravels placental vascular dysfunction and oxidative stress as mechanistic drivers of advanced maternal age-associated pregnancy.

INTRODUCTION: Women with advanced maternal age (AMA) face a higher risk of pregnancy complications including preeclampsia, fetal growth restriction, and preterm birth. While placental dysfunction is implicated, the underlying mechanisms remain unclear. This study employs high-throughput sequencing-based transcriptomics to investigate AMA-associated dysregulation in placental angiogenesis, exploring links to redox imbalance. Our goal is to establish mechanistic and functional links between altered gene expression and perinatal complications. METHODS: Placental pathology from 129 cases was anal

Understanding the differences in occupational injuries due to accidents among native-born and immigrant workers in Sweden: a repeated cross-sectional register-based study.

BACKGROUND: Immigrants continue to face challenges after entering the labor market and remain overrepresented in '3-D jobs' (dirty, difficult, degrading). This study aims to investigate the differences in occupational injury due to accidents (OIA) among immigrants compared to native-born workers in Sweden, and to examine the role of migrant-specific and work factors in these differences. METHODS: This repeated cross-sectional study used nationwide registers including all gainfully employed individuals in 2004-2020 (average annual sample 4.5 million individuals). OIA was treated as a binary ou

Growth Standards for Children With Smith-Magenis Syndrome (SMS).

Smith-Magenis syndrome (SMS, OMIM 182290) is a complex syndromic diagnosis marked by neurobehavioral differences and distinct facial dysmorphisms, caused by haploinsufficiency of the retinoic acid-1 (RAI1) gene either by a pathogenic sequence variant or deletion at chromosome 17p11.2 involving a portion or all of this gene. Dysmorphisms may include a broad square face and brachycephaly, heavy eyebrows, a full mouth with an everted upper lip, and early micrognathia evolving to prognathism after excessive relative mandibular growth. All patients with SMS have variable global cognitive impairment

Urologic Manifestations of Uterine Leiomyomata in Women with Spina Bifida.

OBJECTIVE: To examine our experience with presentation and management of uterine leiomyomata (fibroids) in women with spina bifida (SB). Advances in health care have dramatically improved the life expectancy for individuals living with SB, but they face unique health challenges surrounding reproductive health. Uterine leiomyomata have not previously been described in this population. We hypothesize that their course is likely complicated by SB-specific disabilities and prior surgeries. METHODS: We queried our institutional database for adults with SB and a diagnosis of uterine leiomyomata and

The Myths, Perils, and Pitfalls of Redo Pediatric Cardiac Surgery: The New Normal in Developing Countries Such as India.

Pediatric patients undergoing reoperative cardiac surgery after a previous sternotomy face a higher degree of surgical complexity compared to those undergoing initial procedures. They have higher intraoperative and postoperative risks. The increased risk of surgery is due to preoperative patient factors and intraoperative technical challenges. Redo-pediatric cardiac surgery is a common event in almost every pediatric cardiac surgeon's professional life. Redo-surgery is almost inevitable in patients who have multi-stage repair of congenital heart surgeries and biological valves at a young age,
