Early-onset severe voriconazole-induced liver injury with progressive cholestasis in pulmonary aspergillosis: A case report.

BACKGROUND: Voriconazole is the recommended first-line treatment for invasive pulmonary Aspergillosis. However, its nonlinear pharmacokinetics and marked interindividual variability increase the risk of drug-induced liver injury, particularly in older patients with multiple comorbidities. CASE PRESENTATION: A 76-year-old Asian man with chronic obstructive pulmonary disease, type 2 diabetes mellitus, hypertension, polycythemia, and glucocorticoid-induced adrenal insufficiency was admitted with an infective exacerbation of COPD. Sputum culture identified Aspergillus fumigatus, and oral voricona

Pulmonary and ocular aspergillosis with reduced susceptibility to amphotericin B: A Malaysian case series.

BACKGROUND: Elevated amphotericin B (AmB) minimum inhibitory concentrations (MICs) in Aspergillus spp. may compromise empirical therapy. We describe three aspergillosis cases caused by distinct species with AmB MICs >16 µg/mL. CASE PRESENTATIONS: The first case involved postoperative invasive pulmonary aspergillosis due to A. fumigatus. Conventional AmB was used initially because renal impairment and bowel ischaemia limited voriconazole use; the patient later died with multiorgan failure, invasive aspergillosis and nosocomial infections. The second case involved traumatic A. flavus keratitis

A cornea-specific role for the Aspergillus fumigatus carbon catabolite repressor, CreA, in tissue penetration and infection establishment.

PURPOSE: Elucidate the influence of glucose metabolic pathways on A. fumigatus lung and corneal infection. METHODS: The A. fumigatus acuF and creA genes were deleted in an mcherry -expressing strain. The mutants were tested for alterations in radial growth, cell wall composition by fluorescence staining assays, and antifungal sensitivity through broth microdilution assays. Hyphal penetration of the strains through explanted porcine corneas was tracked by confocal microscopy using the mCherry signal. Virulence was evaluated in established models of invasive pulmonary aspergillosis (IPA) and fu

Bronchoalveolar Lavage Fluid Cellularity Reflects Peripheral Cytopenias and Fungal Burden but Not Short-Term Outcomes in Invasive Pulmonary Aspergillosis.

We characterized leukocyte profiles in bronchoalveolar lavage fluid (BALF) of 63 patients with hematologic malignancy and culture-documented invasive pulmonary aspergillosis and evaluated associations with peripheral blood leukocyte counts, Aspergillus burden, respiratory coinfections, and clinical outcomes. BALF cellularity mirrored peripheral cytopenias yet increased with higher Aspergillus burden and respiratory coinfections, consistent with both systemic immune availability and local inflammatory stimuli. However, BALF cellularity was not independently associated with 42-day mortality, whi

Refractory Invasive Pulmonary Aspergillosis Following Treatment of Acute Rejection in a Kidney Transplant Recipient: A Case Report.

Invasive pulmonary aspergillosis (IPA) is an uncommon but highly morbid complication after solid organ transplantation, particularly following treatment for acute rejection. We report a case of refractory IPA caused by Aspergillus terreus in a 70-year-old kidney transplant recipient, diagnosed 2 weeks after receiving thymoglobulin and pulse corticosteroids for acute T cell-mediated rejection. The patient presented with acute hypoxic respiratory failure requiring intubation, and a chest computed tomography scan demonstrated a large, necrotic, mass-like cavitary opacity in the left upper lobe an

Invasive pulmonary aspergillosis in the ICU: the corticosteroid link.

Invasive pulmonary aspergillosis (IPA) is a life-threatening fungal infection traditionally associated with severely immunocompromised hosts, particularly those with hematologic malignancies. However, its epidemiological profile has shifted in recent years, with a rising incidence among critically ill patients in intensive care units (ICUs), many of whom lack classical risk factors. This change is driven by increased use of corticosteroids and immunomodulatory therapies, the growing prevalence of chronic lung disease, and severe viral pneumonias such as influenza and COVID-19. In these patient

Preventing invasive aspergillosis in lung transplant recipients: A cost-utility analysis.

BACKGROUND: Invasive aspergillosis (IA) is a major cause of morbidity and mortality in lung transplant recipients. While most centers use universal prophylaxis, others opt for pre-emptive treatment. The optimal prevention strategy remains uncertain. OBJECTIVE: To evaluate the cost-utility of universal antifungal prophylaxis versus pre-emptive therapy for preventing IA during the first post-transplant year, from an Ontario healthcare payer perspective. METHODS: A state transition model with a lifetime horizon was developed. Transition probabilities, utilities, and costs were sourced from the

Invasive pulmonary aspergillosis with no abnormal immunity overlapped with allergic bronchopulmonary aspergillosis: A case report.

RATIONALE: Invasive pulmonary aspergillosis (IPA) overlapping with allergic bronchopulmonary aspergillosis (ABPA) is an exceedingly rare disease called Aspergillus overlap syndrome. Fewer than 30 cases have been reported in the literature since 1936. Furthermore, early diagnosis and treatment improve the prognosis of patients, especially those without underlying diseases. PATIENT CONCERNS: A 45-year-old male patient with no underlying disease who was admitted to the hospital with cough, expectoration, fever, and shortness of breath. The patient was diagnosed with pulmonary aspergillosis overl

Clinical spectrum of ICU-acquired invasive pulmonary aspergillosis according to SARS-CoV2 infection: a multicenter prospective cohort study.

BACKGROUND: Invasive pulmonary aspergillosis (IPA) is a common cause of fungal infection acquired in intensive care unit (ICU) whose clinical landscape may differ according to SARS-CoV2 infection status. METHOD: We included all mechanically ventilated patients hospitalized (≥ 48 h) participating in ICU of the REA-REZO network during a 6-year period. Among IPA patients, initial characteristics and outcomes were compared according to COVID-19. Additionally, to account for potential confounders, we performed an inverse probability of treatment weighting (IPTW). Rates of IPA were also compared ac

Tracheobronchial ulcers caused by Aspergillus infection is a potential cause of retrosternal pain: A case report and literature review.

Retrosternal pain is a common symptom encountered in clinical practice. Although it is most frequently attributed to cardiac or esophageal disease, it is rarely considered an airway-related condition. Herein, we report the case of a patient who presented with fever, cough, retrosternal pain, dyspnea, and nausea accompanied by vomiting. The retrosternal pain was persistent and nonradiating and was aggravated by coughing. Based on the combination of retrosternal pain, fever, and dyspnea, viral myocarditis was strongly suspected. However, bronchoscopy performed after hospital admission revealed d

Immunocompromise and early-onset invasive pulmonary aspergillosis in viral pneumonia: a retrospective cohort study.

BACKGROUND: In the context of viral pneumonia, immunocompromised status represents a recognized risk factor for invasive pulmonary aspergillosis (IPA), its association with the timing of IPA diagnosis remains unclear. METHODS: In the present study, 261 patients hospitalized with viral pneumonia were consecutively enrolled and categorized as immunocompromised hosts (ICHs) or non-ICHs. Baseline characteristics, outcomes, and time to IPA diagnosis were compared. Cox regression was used to evaluate the association between immunocompromised status and adverse outcomes. Patients diagnosed with IPA

Synergistic impact of Gram-negative bacterial co-infection on invasive pulmonary aspergillosis in non-neutropenic patients: a machine learning-based risk stratification approach.

BACKGROUND: Invasive pulmonary aspergillosis (IPA) is increasingly recognized in non-neutropenic patients, where coexisting bacterial infections, particularly with Gram-negative pathogens, may impair susceptibility. However, validated tools for early risk stratification in this population remain unavailable. METHODS: We retrospectively analyzed 437 non-neutropenic adults with bacterial co-infection (derivation N.=331; validation N.=106) admitted between 2019 and 2024. Independent predictors of IPA were identified through multivariable logistic regression and incorporated into both a weighted

A machine learning-based risk prediction tool for early identification of invasive pulmonary aspergillosis in immunocompetent patients: a systematic review and meta-analysis.

BACKGROUND: Invasive pulmonary aspergillosis (IPA) increasingly affects non-neutropenic patients, posing diagnostic challenges due to nonspecific clinical features and limited sensitivity of conventional tests. Early identification is critical for improving outcomes. This study aimed to develop and validate a machine learning-based tool for early risk stratification of IPA in this population. METHODS: A systematic meta-analysis identified independent IPA risk factors in non-neutropenic patients. A retrospective cohort of non-neutropenic patients was analyzed.
