[Invasive pulmonary aspergillosis].

Infections caused by fungi of the genus Aspergillus have become a major and dangerous problem in the modern treatments of blood diseases using cytotoxic drugs. Invasive pulmonary aspergillosis, generally caused by A. fumigans and A. flavius, is the usual clinical manifestation of the disease, but subacute forms have been described in patients with little immunodeficiency. The clinical signs, not very suggestive, are those of an infective pneumonia, and localized radiological forms are initially more frequent in most cases. Diagnosis rests on pathological data, but in many cases the only possib

[Invasive pulmonary aspergillosis during the treatment of acute leukaemia (3 case report)].

The authors describe 3 patients with confirmed diagnosis of invasive pulmonary aspergillosis (IPA) during the treatment of acute leukaemia in the period July-September 1987. High statistic significance of previous incidence of the disease, high fungal contamination and epidemiologic relationship between cases point at the epidemic. All patients were treated with amphotericin B; two patients died and the third was cured with intensive 6-month--treatment of amphotericin B in the total dose of 4 grams. It is necessery to introduce amphotericin B in therapy of every febrile patient in prolonged gr

Bronchoalveolar lavage in an immunosuppressed rabbit model of invasive pulmonary aspergillosis.

A rabbit model of invasive aspergillosis has been used to investigate the pathogenesis of Aspergillus infection in the immunosuppressed host. The animals received hydrocortisone daily and a single dose of cyclophosphamide 2 days prior to intratracheal instillation of conidia from Aspergillus fumigatus. Bronchoalveolar lavage (BAL) was performed in 3 infected and 2 control saline treated animals sacrificed on days 1, 2, 4, 7 and 10 following inoculation. Infective load within the lung was quantified using an assay for chitin which is an important component of fungal cell walls (in particular th

Invasive pulmonary aspergillosis: evaluation with MR imaging.

Eleven patients with suspected invasive pulmonary aspergillosis underwent magnetic resonance (MR) imaging. Images were obtained with standard spin-echo sequences and electrocardiographic triggering before and after intravenous administration of gadolinium diethylenetriaminepentaacetic acid. Thirty-seven of 48 lesions seen on MR images were nodular infiltrates; 34 of these had a targetlike appearance, with hypointense centers and iso- or hyperintense rims. Twenty-three of 37 nodular lesions contained areas of hyperintensity on T1-weighted images. All 37 had enhanced rims on postcontrast MR imag

Computed tomography in invasive pulmonary aspergillosis.

Early recognition of invasive pulmonary aspergillosis and of other similar angiotrophic fungal pneumonias has been claimed in previous reports to be possible with computed tomography (CT) and may improve survival of immunocompromised hosts. Chest CT was performed, in the course of fungal pneumonia, in 11 leukemia patients with chemotherapy-induced neutropenia either with (n = 8) or without (n = 5) contrast enhancement. Early (n = 5), before the 7th day from the beginning of the clinical setting) chest CT always demonstrated one or more nodules or mass-like infiltrates surrounded by a halo of l

[Mycetoma-like manifestations in invasive pulmonary aspergillosis].

Invasive pulmonary aspergillosis is characterized by radiological signs allowing a correct diagnosis, including differentiation from pulmonary candidiasis, when they are associated with appropriate clinical symptoms (neutropenia and fever persisting despite broad-spectrum antibiotics). In particular the formation of a pulmonary mycetoma in a previously normal lung is one of these signs. Unlike a simple fungus ball (the saprophytic form of aspergillosis), the rounded density of invasive pulmonary aspergillosis consists of sequestrum of devitalized lung tissue owing to blood vessel invasion by A

[Aspergillus infections of the lung].

In this article the three different manifestations of pulmonary aspergillosis are compared with each other: bronchopulmonary aspergillosis, aspergilloma and invasive pulmonary aspergillosis, and their different clinical and radiological manifestations. In particular, invasive pulmonary aspergillosis--being a severe complication in immunosuppressed patients whose defenses are weak--often presents diagnostic problems due to its initially mostly unspecific clinical and radiological signs. The late signs of a pulmonary aspergillus infection, such as formation of a cavern or the manifestation of th

Invasive pulmonary aspergillosis of horses: an association with acute enteritis.

Invasive pulmonary aspergillosis was identified in 19 horses. In 16 cases mycotic lesions were associated with enterocolitis; 14 cases appeared to result from Salmonella infection. Pulmonary lesions included multifocal areas of acute necrosis, leukocytoclastic vasculitis, and thrombosis with fibrinosuppurative inflammation surrounding mycelial masses. Thromboemboli with fungi were in the brain and kidneys of three cases. Factors which appeared to predispose to pulmonary aspergillosis included corticosteroid therapy, disseminated neoplasms, hepatitis, pleuritis, and peritonitis. This study sugg

[The computed tomographic spectrum of pulmonary aspergillosis].

The authors describe the CT findings in 10 cases of pulmonary aspergillosis which include allergic bronchopulmonary aspergillosis, aspergilloma and invasive pulmonary aspergillosis. CT reveals a more distinct radiologic-pathologic correlation than plain films and shows suggestive features of pulmonary aspergillosis earlier and in a more detailed fashion. Thus the use of CT in the appropriate clinical setting should allow earlier diagnosis than conventional radiography.

Aspergillus terreus as a cause of invasive pulmonary aspergillosis.

A 70-year-old woman with nodular, poorly differentiated lymphocytic lymphoma is the third reported patient with invasive pulmonary aspergillosis caused by Aspergillus terreus. This case differs from the two previously reported in that neither neutropenia nor broad spectrum antibiotics preceded the infection. A terreus should not be dismissed as a laboratory contaminant in pulmonary specimens, especially those from immunosuppressed patients.

Aerosol amphotericin B is effective for prophylaxis and therapy in a rat model of pulmonary aspergillosis.

Invasive pulmonary aspergillosis is a major life-threatening complication among transplant recipients and patients receiving cancer chemotherapy. In a rat model of progressive pulmonary aspergillosis that is characterized by hyphal bronchopneumonia, aerosol amphotericin B (aero-AmB; 1.6 mg/kg given 2 days before infection) significantly delayed mortality in rats compared with animals in a control group. The first death in the aero-AmB-treated group occurred on day 11, by which time seven of the eight control animals had died. The same dose of aero-AmB given as treatment (1.6 mg/kg given 24 h a

[Invasive pulmonary aspergillosis].

Invasive pulmonary aspergillosis is an opportunistic pneumonia which is particularly common in immuno-depressed subjects. The most frequent risk factors are the intensity and duration of leucopaenia, cytotoxic drugs and steroid therapy. In the respiratory tract, the proliferation of Aspergillus fumigatus becomes possible when there is a failure of the reticulo-endothelial system and of the polymorphonuclear leucocytes. There is no typical clinical picture: one sees an acute febrile pneumonia resistant to antibiotics. However, the radiological signs are more suggestive with nodules and bilatera

An outbreak of invasive aspergillosis in a haematologic unit.

Within a period of 15 months 8 cases of invasive pulmonary aspergillosis were seen in a haematologic unit; 7 of them between January and October 1984. No previous cases of invasive aspergillosis had been encountered during the existence of the unit since 1979. Environmental studies did not prove a single likely source for the fungal spores. Previous window renovation with concomitant fiber deposits on ventilation grids, poor sealing of air filter fittings in patient rooms, occasional ventilation through windows in the ward, and low speed of booster fans in the ventilation system may have creat
