Fatal Fungal Peritonitis Caused by Aspergillus udagawae: An Autopsy Case Report.

A 56-year-old man with chronic progressive pulmonary aspergillosis due to multidrug-resistant Aspergillus udagawae presented with acute abdominal pain. Computed tomography revealed rupture of an intra-abdominal abscess with ascites. Despite emergency surgery for peritonitis, he died on day 5. Autopsy showed numerous fungal abscesses with direct invasion and hematogenous spread from the lungs to the peritoneum. A genetic analysis confirmed A. udagawae. This represents the first autopsy-proven case of fatal fungal peritonitis secondary to invasive pulmonary aspergillosis caused by A. udagawae, h

Ubiquinone-based gene mutation and protein compactness of CoQ5 may contribute to a novel caspofungin resistance mode in Aspergillus flavus from pulmonary aspergillosis.

Invasive pulmonary aspergillosis (IPA), primarily caused by Aspergillus fumigatus and A. flavus, is a life-threatening fungal infection with high mortality rates. This study investigated novel resistance mechanisms to caspofungin (CAS) in clinical Aspergillus isolates with particular focus on genetic alterations in the CoQ5 gene and their functional implications. One hundred and forty-eight Aspergillus isolates from IPA cases were undergone antifungal susceptibility testing, genotyping and phylogenetic analyses, mutations and gene expression studies of Fks1 and CoQ5 genes, and three-dimensiona

Invasive pulmonary aspergillosis in the post-COVID-19 era: diagnosis, treatment, and what lies ahead.

Invasive pulmonary aspergillosis (IPA) is a severe opportunistic fungal infection that predominantly affects immunocompromised individuals, including those with hematologic malignancies, organ transplants, and, more recently, patients with post-COVID-19 immune dysregulation. Despite advancements in medical mycology, IPA continues to pose significant diagnostic and therapeutic challenges, contributing to high global morbidity and mortality. Diagnostic accuracy remains limited due to nonspecific clinical manifestations and the suboptimal performance of conventional tools such as bronchoalveolar

The HosA histone deacetylase regulates stress resistance, host cell interactions, and virulence in Aspergillus fumigatus.

The capacity of Aspergillus fumigatus to cause invasive pulmonary aspergillosis depends on its ability to adapt to dynamic and stressful microenvironments within the host. Epigenetic regulation, including histone deacetylation, plays a critical role in fungal adaptation to stress. Here, we investigated the role of the class I histone deacetylase HosA in A. fumigatus stress resistance, host cell interactions, and virulence. A ΔhosA mutant had increased susceptibility to intracellular oxidant stress induced by menadione. It also had impaired capacity to invade and damage two pulmonary epithelial

Association of follow-up chest computed tomography findings at 6 and 12 weeks with survival in people with invasive pulmonary aspergillosis following lung or allogeneic hematopoietic stem cell transplantation.

The prognostic value of radiologic response in invasive pulmonary aspergillosis remains poorly defined. We evaluated the association between radiologic responses at 6 and 12 weeks after diagnosis and survival in transplant recipients. We conducted a single-center retrospective cohort study of lung and allogeneic hematopoietic stem cell transplant recipients diagnosed with invasive pulmonary aspergillosis between 2010 and 2020. Patients were included if they had a follow-up chest computed tomography 4 to 15 weeks after diagnosis. Radiologic responses at 6 ± 2 and 12 ± 3 weeks were classified us

Invasive Fungal Infections During Extracorporeal Membrane Oxygenation: A Case Series from Intensive Care Unit and Literature Review.

Background: Extracorporeal membrane oxygenation (ECMO) support is associated with potentially life-threatening complications, among which nosocomial infections play a significant role. The increasing incidence of fungi as causative agents of ECMO-associated infections is a growing concern. Methods: This case series includes all patients admitted to the Intensive Care Unit (ICU) of the "Renato Dulbecco" Teaching Hospital in Catanzaro who developed invasive fungal infections (IFIs) during ECMO support. Results: Of the 70 patients, 15.7% (N = 11) developed IFIs during ECMO. Among these, 91% (N =

Integrative diagnosis of invasive pulmonary aspergillosis in non-neutropenic patients using BALF-tNGS-derived Aspergillus load and host risk factors: a multicenter study.

BACKGROUND: Diagnosing invasive pulmonary aspergillosis (IPA) in non-neutropenic patients is challenging because of non-specific manifestations and limited diagnostic tools. Targeted next-generation sequencing (tNGS) of bronchoalveolar lavage fluid (BALF) enables rapid pathogen detection; however, its capacity for quantitative assessment of fungal load remains unclear. This study integrated BALF-tNGS fungal load with host risk factors to develop a diagnostic nomogram for IPA in non-neutropenic patients. METHODS: Non-neutropenic adults with suspected IPA were retrospectively enrolled at three

Invasive Pulmonary Aspergillosis in a Young Adult With Hyperimmunoglobulin E Syndrome and Hypogammaglobulinemia Following Rituximab Therapy.

Hyperimmunoglobulin E syndrome (HIES) is a rare primary immunodeficiency marked by elevated IgE levels, recurrent skin and pulmonary infections, and immune dysregulation. While typically diagnosed in childhood, adult presentations can occur, often complicated by structural lung disease, opportunistic infections, and malignancies. We report a 37-year-old female with signal transducer and activator of transcription 3 (STAT3)-deficient HIES, diffuse large B-cell lymphoma (DLBCL) in remission, rituximab-induced hypogammaglobulinemia, and recurrent infections, who presented with acute-on-chronic dy

Beyond the Norm: Atypical Manifestations of Aspergillosis.

Aspergillus fumigatus is a significant pathogen responsible for a wide spectrum of pulmonary diseases, ranging from allergic reactions to invasive pulmonary aspergillosis, particularly among immunocompromised individuals. We describe a 62-year-old female kidney transplant recipient who presented with chest pain, a right anterior chest wall mass, cough and anorexia of 2 months' duration. Laboratory evaluation revealed elevated inflammatory markers, prompting consideration of malignancy and infection. Bronchoalveolar lavage was positive for Aspergillus galactomannan antigen, and biopsy of the ch

How to safely discontinue antifungal treatment in invasive pulmonary aspergillosis?-clinical considerations in haematology.

BACKGROUND: Treatment duration in invasive pulmonary aspergillosis (IPA) in patients with haematological malignancies is not well defined through clinical trials and is therefore often driven more by experience than evidence, with implications for outcome, patient quality-of-life, drug toxicity, costs, and antifungal stewardship. OBJECTIVES: To review current evidence and provide expert considerations regarding cessation of antifungal therapy for IPA, and to propose pragmatic criteria and follow-up strategies to guide safe discontinuation and early detection of recurrence. SOURCES: Narrative

Amphotericin B promotes respiratory viral entry by enhancing late endosomal maturation and fusion via glucocerebrosidase-mediated ceramide remodeling.

Respiratory viral infections, such as influenza and COVID-19, pose significant global health challenges. For patients with invasive pulmonary aspergillosis, a subsequent viral infection can lead to markedly worse clinical outcomes. Although amphotericin B (AmB) remains a cornerstone antifungal therapy, our investigation demonstrates that it paradoxically enhances the entry of influenza A virus and SARS-CoV-2. Mechanistically, AmB directly binds to and activates glucocerebrosidase, leading to ceramide accumulation and RAB7 upregulation in the late endosomes, thereby enhancing late endosomal mat

Antibiotic-induced microbiota disruption impairs neutrophil-mediated immunity to respiratory Aspergillus fumigatus infection in mice.

Aspergillus fumigatus is an ubiquitous environmental mold that forms airborne conidia and can cause life-threatening infections in immunocompromised individuals. Invasive aspergillosis occurs in patients with quantitative or qualitative neutrophil defects who often receive systemic antibiotics to prevent or manage bacterial infections. Antibiotic-induced bacterial dysbiosis has been linked to impaired neutrophil bactericidal activity and to intestinal commensal bacteria escape during systemic candidiasis, though it remains unclear whether receipt of antibacterial antibiotics impairs neutrophil

Clinical classification of chronic obstructive pulmonary disease with invasive pulmonary aspergillosis: a retrospective study.

BACKGROUND: Patients with chronic obstructive pulmonary disease (COPD) complicated by invasive pulmonary aspergillosis (IPA) have high mortality and significant clinical heterogeneity. However, systematic classification criteria are currently lacking. To address this, our study proposes a clinical classification system specifically for COPD-IPA, based on its distinct clinical manifestations and pulmonary imaging features. METHODS: We performed a retrospective analysis of clinical and radiological features in patients diagnosed with COPD-IPA during hospitalization from 2008 to 2024 to develop

Diagnostic and prognostic value of serum miR-155 in chronic obstructive pulmonary disease.

We investigated serum miR-155 in chronic obstructive pulmonary disease (COPD) and its clinical utility. miR-155 in peripheral blood mononuclear cells (PBMCs) and serum cytokines (IL-1β, IL-6, IL-8, TNF-α) were quantified, and associations with disease occurrence, inflammation, severity, and prognosis were assessed over one year. A total of 117 participants were enrolled: 59 COPD patients (29 acute exacerbation [AECOPD], 30 stable), 31 heavy smokers, and 27 healthy controls. miR-155 was measured by RT-PCR and cytokines by ELISA. COPD patients were prospectively followed and categorized as frequ

Unraveling the complexities: A comprehensive review of severe fever with thrombocytopenia syndrome and its associated complications.

Severe fever with thrombocytopenia syndrome (SFTS), caused by Dabie bandavirus (DBV), is an emerging vector-borne infection with significant global health implications, particularly in East Asia. In recent years, its geographic footprint has expanded beyond traditional endemic regions, raising global concern due to high morbidity and variable case-fatality rates of up to 30%. Clinically, SFTS presents with an acute onset of high fever, thrombocytopenia, leukocytopenia, headache, and gastrointestinal disturbances. In severe cases, rapid progression may lead to life-threatening complications, in

Synergistic effects of S100 calcium-binding protein A12 combined with Pentraxin 3 in invasive pulmonary aspergillosis and their clinical application prospects.

Invasive pulmonary aspergillosis (IPA) is a severe deep-seated fungal infection caused by fungi of the genus Aspergillus. In recent years, its global incidence has shown a marked upward trend, posing a serious threat especially to immunocompromised patients, such as hematopoietic stem cell transplant recipients, cancer patients undergoing chemotherapy, and individuals on long-term glucocorticoid therapy. The core clinical dilemmas lie in the difficulty of early diagnosis and the narrow therapeutic window. Currently used clinical diagnostic indicators, including the galactomannan (GM) assay, (1
