Case Report: Lung squamous cell carcinoma mimicking recurrent aspergillosis in systemic lupus erythematosus.

Distinguishing between life-threatening infection and malignancy in immunocompromised patients remains a major clinical challenge. Individuals with systemic lupus erythematosus (SLE) are at heightened risk for both invasive pulmonary aspergillosis (IPA) and lung cancer, and their coexistence can create a profound diagnostic dilemma. We report the case of a 66-year-old man with a history of SLE and prior IPA who presented with fever, headache, and progressive cognitive decline. Imaging revealed a cavitary lung lesion and multiple brain nodules, initially suggestive of intracranial IPA dissemina

Incidence of Voriconazole-induced periostitis in a tertiary medical center: a single-center retrospective analysis.

OBJECTIVE: Voriconazole is a commonly used antifungal medication, particularly for treating invasive pulmonary aspergillosis and as prophylaxis in hematologic malignancies. However, it can cause adverse effects, including periostitis—a painful inflammation of the periosteum. Reported incidence rates of voriconazole-induced periostitis vary widely. This study aimed to determine the incidence and characteristics of voriconazole-induced periostitis in a large cohort of patients with hematologic malignancies at a tertiary medical center, using various imaging modalities. METHODS: A retrospective c

Case Report: Rare Multidrug-Resistant Enterobacter Cloacae Complicated by Invasive Pulmonary Aspergillosis in an Elderly Patient with Advanced Lung Adenocarcinoma Treated with Osimertinib.

Osimertinib, a third-generation epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI), is the standard treatment for EGFR-mutated advanced non-small cell lung cancer (NSCLC). We report a rare case of multidrug-resistant (MDR) Enterobacter cloacae pneumonia complicated by invasive pulmonary aspergillosis (IPA) in a 75-year-old male with lung adenocarcinoma who had been receiving osimertinib for 8 months. Prompt susceptibility-guided therapy with polymyxin B plus tigecycline (for MDR bacteria) and voriconazole (for aspergillosis) achieved complete resolution, highlighting the imp

Thermal adaptation crosstalk with azole response through lncRNA in Aspergillus fumigatus.

Fungal pathogens are adapting to increased temperatures altering host-pathogen interactions, disease patterns, and response to the antimicrobial drugs. Here, we show that thermal adaptation to 42°C leads to reversible changes in fungal colony size and azole drug response in the human pathogenic fungus Aspergillus fumigatus. Importantly, this adaptation is mediated by the lncRNA, afu-182, whose RNA levels negatively correlate with temperature. Growth at a lower temperature or ectopic upregulation of afu-182 RNA levels reverses the temperature adaptation. Previously, we have shown that Δafu-182

Clinical utility of non-induced sputum galactomannan testing for suspected invasive pulmonary aspergillosis: a real-world retrospective cohort study.

PURPOSE: Non-induced sputum galactomannan (GM) testing is increasingly used in routine evaluation of suspected invasive pulmonary aspergillosis (IPA), yet its real-world performance and the clinical implications of negative results remain incompletely characterised. We aimed to evaluate its diagnostic performance relative to bronchoscopic mycological findings and to describe the short-term evolution of GM results in routine practice. METHODS: We retrospectively analysed adult patients who underwent non-induced sputum GM testing between 2017 and 2025. Diagnostic performance was assessed in pat

Invasive Necrotizing Tracheobronchial Aspergillosis in Children: A Case Series and Literature Review.

BACKGROUND Invasive tracheobronchial aspergillosis is rare in children. Here, we describe 3 cases in which mucosal necrosis and erosion were observed on bronchoscopy, with pseudomembrane-like attachments on the mucosal surface. CASE REPORT Case 1: An 8-year-old girl with leukemia was admitted because of recurrent fever, persistent cough (>1 month), and 1 day of hemoptysis. Chest computed tomography (CT) revealed progression of patchy opacities and consolidation in the right upper and middle lobes compared with prior imaging. Pulmonary Aspergillosis Developing After the First Chemotherapy Session in a 15-Year-Old Boy: A Case Report.

A 15-year-old boy diagnosed with acute myeloid leukemia (AML) was readmitted to the hospital with a fever and an atypical lung infection just one day after being discharged following his initial chemotherapy session. The symptoms continued and worsened with time even though multiple levels of antibiotic treatment were tried. It was the typical radiological findings aided in diagnosing the cause and demonstrated to be invasive pulmonary aspergillosis. Furthermore a dramatic response to the standard treatment and positive serum GM test confirmed the diagnosis.

Invasive Pulmonary Aspergillosis as a Consequence of Influenza A Infection and Refractory Septic Shock: A Case Report.

Glucocorticoids are commonly administered in refractory septic shock because of their potential hemodynamic benefits. However, their immunosuppressive effects carry a recognized risk of opportunistic infections. In the presence of concomitant viral infections, such as influenza A, which themselves induce local immune dysregulation, this risk may be amplified. The combined immunological impairment can create a permissive environment for opportunistic pathogens, such as aspergillosis, potentially worsening clinical outcomes. We present the case of a 59‑year‑old man with type 2 diabetes mellitus,

Nitroxoline exerts potent anti-Aspergillus fumigatus activity by disrupting copper homeostasis and inducing oxidative stress.

Invasive pulmonary aspergillosis (IPA) caused by Aspergillus fumigatus remains a clinical challenge due to limited therapies and rising antifungal resistance. Here, we evaluated nitroxoline (NTX), an FDA-approved 8-hydroxyquinoline derivative, for its antifungal potential and mechanism of action. NTX exhibited potent in vitro activity against 64 clinical isolates and the AF293 reference strain, delaying conidial maturation and significantly improving survival, reducing pulmonary fungal burden, and alleviating lung inflammation and hyphal invasion in an IPA mouse model. Transcriptomic analysis

Invasive Pulmonary Aspergillosis in an Immunocompetent Patient Following Occupational Exposure to Iron Oxide Dust: A Long-Term Follow-Up Case Report.

PURPOSE: Invasive pulmonary aspergillosis (IPA) is a life-threatening fungal infection that predominantly occurs in immunocompromised hosts. However, IPA may also develop in immunocompetent individuals following substantial environmental or occupational exposure, though such cases are rare and often underrecognized. To our knowledge, there are limited reports of IPA associated with iron oxide dust exposure in otherwise healthy individuals, and long-term follow-up data are scarce. CASE PRESENTATION: The patient was a 59-year-old male with no history of immunosuppression, who presented with cou

Invasive chronic pulmonary aspergillosis masquerading as lung malignancy in a patient with asthma and ABPA overlap.

Chronic pulmonary aspergillosis (CPA) is a slowly progressive fungal infection that commonly complicates underlying structural lung disease and is frequently misdiagnosed in tuberculosis-endemic regions. Invasive forms of CPA can closely mimic lung malignancy.A man in his late 30s with a history of repeated empirical antitubercular therapy presented with progressive dyspnoea, chronic cough with mucoid sputum, seasonal variation of symptoms and recurrent haemoptysis. Imaging revealed a large right upper lobe mass with mediastinal extension, vascular encasement and complete bronchial obstruction

Invasive pulmonary aspergillosis among ICU patients with COVID-19, influenza, and preexisting host factors: a nationwide cohort study.

BACKGROUND: Previous studies on the association between coronavirus disease 2019 (COVID-19) and invasive pulmonary aspergillosis (IPA) were limited by their single-center nature and small patient population. Whether aspergillosis risk is related to COVID-19 or other factors remains unclear. This study aimed to determine whether COVID-19 among intensive care unit (ICU) patients is associated with an increased incidence of IPA compared with that among individuals with influenza or preexisting host factors (control). METHODS: This was a nationwide study performed using the Korean National Health

Nebulized Liposomal Amphotericin B as an Alternative Therapy for an Aspergillus Tracheal Plaque.

Invasive pulmonary aspergillosis (IPA) typically affects immunocompromised individuals; however, increasing cases in patients without traditional risk factors suggest evolving host susceptibility and predisposing airway pathology. We present a case of an Aspergillus tracheal plaque in a 68-year-old immunocompetent woman with atypical presentation. Due to adverse effects associated with systemic antifungal therapy, treatment was transitioned to nebulized liposomal amphotericin B (LAmB), resulting in complete plaque resolution without complications. This case highlights the potential role of neb

Bronchoalveolar Lavage Fluid Collection in Invasive Pulmonary Aspergillosis Murine Model for Downstream Proteomic Analysis.

The immunocompromised populations are susceptible to invasive fungal infections, such as invasive pulmonary aspergillosis (IPA). Aspergillus spp. produce airborne spores, called conidia. The small size of conidia allows them to reach the most proximal airways, such as the alveoli. The bronchoalveolar lavage fluid represents an important source of proteins, either from the host or the fungal pathogen, that are associated with the pathobiology of Aspergillus fumigatus and, thus, serve as crucial factors to elucidate virulence factors and/or promising novel antifungal drug targets. This chapter d

Aspergillus fumigatus Cell Wall: Current Advances in Structural Elucidation Methods.

Being an extracellular organelle, the fungal cell wall (CW) plays a crucial role in fungal biology by providing mechanical strength and protecting from its hostile environment. In pathogenic fungi, their CW is explored with two main objectives: (a) to develop novel antifungal molecules, as the disruption of CW leads to fungal lysis due to osmotic imbalance, and (b) to decipher fungal pathogenesis, as CW is the first fungal structure to interact with the host immune system. Aspergillus fumigatus is an airborne filamentous fungal pathogen, which causes a spectrum of infections ranging from aller

Effects of Antifungal Treatment in COVID-19-Associated Pulmonary Aspergillosis: A European Multicenter Cohort Study.

BACKGROUND: The management of COVID-19-associated invasive aspergillosis (CAPA) is still debated while cases continue to occur, and more and more frequently in vulnerable populations, stressing the importance of obtaining data on the treatment of this disease. Only small cohort studies and case reports have yet discussed this essential issue, and the data obtained are insufficient to make conclusions with confidence. RESEARCH QUESTION: Is antifungal treatment associated with lower 60-day mortality in patients with probable or proven CAPA? STUDY DESIGN AND METHODS: This study assessed the ass
