Coptis chinensis Franch. Suppresses Invasive Pulmonary Aspergillosis by Augmenting NADPH-Dependent Neutrophil Extracellular Traps via Dual Modulation of Complement Activation and Gut Microbiota.

Invasive pulmonary aspergillosis (IPA) poses a serious threat to immunocompromised hosts, with limited therapeutic options highlighting the need for novel strategies. Coptis chinensis Franch. (CCF), a traditional Chinese herb containing antimicrobial alkaloids like berberine, was investigated for its therapeutic efficacy and immunological effects in a murine IPA model. Immunosuppressed female KM mice infected with Aspergillus fumigatus AF293 were treated with CCF or amphotericin B (AmB). CCF significantly improved survival, reduced fungal burden, and alleviated lung pathology, without inducing

Unveiling mechanistic insights of inactivated Aspergillus niger spore by fishery-waste-derived chitosan via synchrotron radiation tomography.

Aspergillus niger (A. niger) is a pathogenic fungus responsible for severe and potentially fatal invasive pulmonary aspergillosis. For the first time, this study elucidates the antifungal mechanism of fishery-waste-derived chitosan (CTS) against A. niger using synchrotron-based X-ray microscopy integrated with conventional assays, enabling 3D visualization of the inactivation process. CTS (2.0 g L-1) exhibited potent efficacy, achieving a 6.05-log reduction within 6 h. Crucially, transmission X-ray microscopy revealed a transition from spherical spores to centrally collapsed structures, while

A silent invader: Invasive pulmonary aspergillosis in an immunocompetent host.

Invasive pulmonary aspergillosis (IPA) is a fulminant fungal infection that predominantly occurs in immunocompromised patients; however, it may occasionally be encountered in immunocompetent individuals, often leading to diagnostic delay and unfavorable outcomes. We describe a case of invasive pulmonary aspergillosis (IPA) in an immunocompetent woman in her mid-20s who presented with productive cough, hemoptysis, low-grade fever, and pleuritic chest pain, closely resembling pulmonary tuberculosis at initial evaluation. While the chest x-ray was normal, high-resolution computed tomography of th

Confronting the "lethal duo" in the ICU: early identification of Aspergillus-Mucorales co-infection using a clinical-immuno-inflammatory signature.

BACKGROUND: Co-infection with Aspergillus and Mucorales in the intensive care unit (ICU) represents a devastating syndrome with high mortality that is frequently clinically occult. Clinically distinguishing this co-infection from invasive pulmonary aspergillosis (IPA) is challenging but critical for tailoring precise antifungal strategies. METHODS: We conducted a single-center, retrospective observational study involving 93 critically ill patients (75 with Aspergillus infection and 18 with co-infection) admitted between 2017 and 2025. We compared clinical characteristics, inflammatory markers

Structure and biosynthetic mechanisms of galactomannans in filamentous fungi.

The cell wall of filamentous fungi is crucial for the physical protection of the cell, the sensing environmental cues, and the expression of virulence. The fungal cell wall consists of a rigid yet flexible three-dimensional network in which structural polysaccharides, such as β-(1 → 3)-glucan and chitin, are intricately intertwined with α-(1 → 3)-glucan, galactosaminogalactan, and galactomannan. Galactomannan is a glycan containing mannose and galactofuranose, the latter of which is a sugar residue absent in humans, and it serves as an important indicator in the clinical diagnosis of invasive

Comparative clinical features and outcomes of invasive pulmonary aspergillosis following influenza versus COVID-19: a retrospective cohort study.

BACKGROUND AND AIM: Patients with influenza or Coronavirus disease 2019 (COVID-19) are at risk of developing invasive pulmonary aspergillosis (IPA). Whether the clinical profiles and outcomes differ between influenza-associated pulmonary aspergillosis (IAPA) and COVID-19-associated pulmonary aspergillosis (CAPA) remains unclear, hindering efforts to optimize management. This study aimed to compare the demographic, clinical, laboratory characteristics, and outcomes of patients with IPA following influenza A/B versus COVID-19 during the same period. METHODS: We conducted a single-center retrosp

Fatal AA-like bone marrow failure and invasive pulmonary aspergillosis after long-term pembrolizumab in squamous NSCLC: a case report.

Immune checkpoint inhibitors (ICIs) such as pembrolizumab have substantially improved outcomes in advanced non-small cell lung cancer (NSCLC), including squamous histology, but prolonged exposure may be complicated by immune-related adverse events (irAEs) and opportunistic infections. We report a 58-year-old man with advanced squamous NSCLC who achieved durable tumor control after six cycles of pembrolizumab plus platinum-based chemotherapy, followed by pembrolizumab maintenance monotherapy (18 cycles). During the later course, he developed severe bacterial pneumonia, invasive pulmonary asperg

A 56-Year-Old Male Farmer From China With Severe Fever With Thrombocytopenia Syndrome and Pulmonary Aspergillosis: A Case Report and Review of Literature.

BACKGROUND Severe fever with thrombocytopenia syndrome (SFTS) is an emerging tick-borne infectious disease caused by the Dabie bandavirus (commonly known as SFTS virus, or SFTSV). SFTSV-induced immunosuppression during infection renders patients highly susceptible to invasive pulmonary aspergillosis. SFTS-associated pulmonary aspergillosis (SAPA) presents major therapeutic challenges and is linked to drastically worsened outcomes, including high mortality. This report aims to highlight the diagnostic and therapeutic challenges of SAPA and emphasize the value of early diagnosis using metagenomi

Insights into Aspergillus fumigatus morphogenesis and pathogenesis through the putative lipid transporter ArvA.

UNLABELLED: Aspergillus fumigatus poses a significant threat to human well-being, in part due to the increasing emergence of strains resistant to frontline antifungal therapy. In this study, we observed that the gene arvA is required for A. fumigatus morphogenesis, antifungal drug susceptibility, and cell wall homeostasis. Intriguingly, our study reveals novel morphological and growth aberrations in the absence of arvA. Loss of arvA results in hyper-swollen conidia that give rise to stunted, polarity-deficient hyphae in numerous environmental conditions, indicating a pivotal role for arvA in A

Aspergillus niger, an Unusual Case With Fatal Massive Hemoptysis: A Case Report.

Aspergillus Niger is considered a low-virulence fungus. Primary and secondary immunocompromise represent the principal risk factors for developing this disease. There are only a few cases of invasive infection that have been documented, and only a few fatal cases of secondary invasive pulmonary aspergillosis as a cavitation by this agent have been published. We present the case of a 45-year-old man with immunocompromise who developed massive hemoptysis, which, despite treatment, led to his death. Pathological findings documented invasive pulmonary aspergillosis due to A. Niger and a co-infecti

Delayed decrease in voriconazole concentration after resolution of inflammation in a patient with invasive pulmonary aspergillosis: a case report.

BACKGROUND: Voriconazole (VRCZ) is an effective treatment for pulmonary aspergillosis. Active infectious disease leads to a reduction in cytochrome P450 activity, resulting in increased VRCZ concentrations. However, the impact of VRCZ dose reduction during the severe inflammatory state on VRCZ concentrations after the resolution of inflammation is not yet fully understood. Here, we report the time-course changes in VRCZ blood concentrations after inflammation in a patient with invasive pulmonary aspergillosis. CASE PRESENTATION: A man in his 60s receiving oral VRCZ tablets at 400 mg/day for i

Pulmonary Fungal Infections in Immunocompetent Patients.

Fungi are a group of microorganisms encompassing thousands of species, with only a minority causing disease in humans. Fungal pathogens and infections are increasing global public health problems. They are an important cause of opportunistic infections in immunocompromised patients; however, they can also affect immunocompetent individuals. This paper reviews the incidence, clinical features, radiographic manifestations, and pathological findings of major fungal infections in non-immunocompromised patients. Only a restricted subset of fungi, specifically the endemic dimorphic fungi, selected A

Pulmonary aspergillosis complicated by recurrent pneumothorax in a person living with HIV: A case report.

Pulmonary aspergillosis is uncommon in people living with HIV (PLHIV), and pneumothorax complicating invasive pulmonary aspergillosis (IPA) is exceptionally rare, with few reported cases and generally poor outcomes. We report the case of a 30-year-old man newly diagnosed with HIV infection who developed recurrent right-sided pneumothorax secondary to IPA. The diagnosis was supported by chest CT findings, bronchoalveolar lavage galactomannan positivity, and compatible clinical features. The patient received voriconazole and chest drainage, resulting in a favorable clinical course. This case und
