Understanding the implementation of specialist maternity services for pregnant women with FGM/C in Germany: a situation analysis applying normalization process theory.

BACKGROUND: Female Genital Mutilation/Cutting (FGM/C) comprises all procedures involving the partial or total removal of the external female genitalia or any other injury to the female genital organs for non-medical reasons. There are about 600.000 women in Europe estimated to live with the consequences of FGM/C, of whom about 104.000 are residing in Germany. Less is known about maternity service provision for affected women living in Germany. An exploratory case study was conducted to examine the implementation of specialist services for women with FGM/C in a German Maternity Unit. This publi

Safety and efficacy of the chitosan covered tamponade for the management of lower genital tract trauma during childbirth.

OBJECTIVE: This retrospective analysis evaluates the safety and efficacy of the vaginal use of the chitosan covered gauze ("Celox™ PPH") in managing lower genital tract trauma with high blood loss, where conventional repair failed. METHODS: Data from patients receiving chitosan covered gauze intravaginally or locally at the vulva due to substantial blood loss because of birth injury were examined retrospectively, using data from a university hospital 2017-2024. Parameters included blood loss, anesthesia, transfusions, length of hospitalization, infection signs, need for intensive care, and ta

Inadvertent Intrauterine Instillation of Trichloroacetic Acid During Sonohysterography.

Trichloroacetic acid (TCAA) solution is used to treat vulvar condyloma and cervical intraepithelial neoplasia. Saline infusion sonohysterography (SIS) detects intrauterine pathology and fallopian tube patency in infertile patients. A 33-year-old Caucasian nulliparous fertility patient presented to the emergency department with extensive burns following accidental intrauterine instillation of TCAA during SIS with tubal perfusion. On presentation, the patient was hemodynamically stable. There was extensive erythema and tenderness in a spill/burn pattern on both thighs as well as the groin, perin

Frequency and Characteristics of Postoperative Neuropathy in Individuals on Gender-Affirming Hormone Therapy Undergoing Gender Affirmation Surgery: A Retrospective Cohort Study.

Introduction Gender affirmation surgery includes procedures of the face, larynx, chest, reproductive system, external genitalia, and adipose tissue performed to ameliorate incongruence between gender identity and phenotype. The annual number of gender affirmation surgeries performed in the United States has increased significantly. There have been no investigations into the frequency of peripheral neuropathy after gender affirmation surgery, which is an important topic, given transgender individuals are at increased risk for delaying necessary medical care. After appreciating a number of cases

Risk of obstetric anal sphincter tear among primiparous women with a history of female genital mutilation, giving birth in Sweden.

BACKGROUND: Female genital mutilation (FGM) includes a range of procedures involving partial or total removal of the external female genitalia. It is a harmful procedure that violates human rights of girls and women. FGM has been associated with obstetric anal sphincter injury (OASI), among other adverse obstetric complications. However, the obstetric outcomes in high-income countries are not clear. The aim of this study was to compare the risk of OASI among primiparous women, with and without a history of FGM, giving birth in Sweden. METHOD: A population-based cohort-study based on data from

Location of obstetric anal sphincter injury scars on translabial tomographic ultrasound.

OBJECTIVE: Obstetric anal sphincter injury (OASI) is a common preventable cause of anal incontinence. Both diagnosis and primary repair of OASI are often suboptimal, partly owing to the absence of effective clinical audit. The aim of this study was to evaluate the location of scars or defects of the external anal sphincter (EAS), diagnosed by translabial ultrasound (TLUS), following primary OASI repair. METHODS: This was a retrospective analysis of 309 women who were seen at a tertiary obstetric unit after primary repair of OASI between June 2012 and May 2019. All women underwent a standardiz

Female genital mutilation and contraceptive use: findings from the 2014 Egypt demographic health survey.

OBJECTIVES: Female genital mutilation (FGM) includes procedures that remove partial or total external female genitalia, or causes other injury to female genital organs with no medical reason. Physical and psychological trauma associated with FGM may interfere with a woman's ability and intent to utilize contraception. Our study examines the association between FGM and utilization of contraception methods among sexually active reproductive-aged women in Egypt. METHODS: Data from the 2014 Egypt Demographic and Health Survey were analyzed (n = 20,055). Multinomial logistic regression was used to

Neovaginal Canal Dissection in Gender-Affirming Vaginoplasty.

PURPOSE: Gender-affirming vaginoplasty creates the vulva and vaginal canal for individuals assigned male sex at birth who have gender dysphoria. Dissection of the neovaginal space can be particularly challenging, with risk of injury to the anal and urethral sphincters, urethra and rectum. We present an anatomically based technique for vaginal canal dissection. MATERIALS AND METHODS: We retrospectively analyzed a cohort of patients who underwent gender-affirming vaginoplasty by a single surgeon between May 2016 and July 2019. We describe our technique for dissection and report relevant outcome

It is the first birth that does the damage: a cross-sectional study 20 years after delivery.

INTRODUCTION AND HYPOTHESIS: Levator ani muscle (LAM) and anal sphincter tears are common after vaginal birth and are associated with female pelvic organ prolapse and anal incontinence. The impact of subsequent births on LAM and external anal sphincter (EAS) integrity is less well defined. The objective of this study was to determine the prevalence of LAM and EAS trauma in primiparous (VP1) and multiparous (VP2+) women who had delivered vaginally to assess if there were differences between the two groups. The null hypothesis was: there is no significant difference in the prevalence of LAM and

ACOG Practice Bulletin No. 198 Summary: Prevention and Management of Obstetric Lacerations at Vaginal Delivery.

Lacerations are common after vaginal birth. Trauma can occur on the cervix, vagina, and vulva, including the labial, periclitoral, and periurethral regions, and the perineum. Most of these lacerations do not result in adverse functional outcomes. Severe perineal lacerations, extending into or through the anal sphincter complex, although less frequent, are more commonly associated with increased risk of pelvic floor injury, fecal and urinary incontinence, pain, and sexual dysfunction with symptoms that may persist or be present many years after giving birth. The purpose of this document is to p

A survey of Australian midwives' knowledge, experience, and training needs in relation to female genital mutilation.

BACKGROUND: Female genital mutilation (FGM) involves partial or total removal of the external female genitalia or any other injury for non-medical reasons. Due to international migration patterns, health professionals in high income countries are increasingly caring for women with FGM. Few studies explored the knowledge and skills of midwives in high income countries. AIM: To explore the knowledge, experience and needs of midwives in relation to the care of women with FGM. METHODS: An online self-administered descriptive survey was designed and advertised through the Australian College of Mi

Exploring Genitoanal Injury and HIV Risk Among Women: Menstrual Phase, Hormonal Birth Control, and Injury Frequency and Prevalence.

BACKGROUND: Genital, anal, and oral injuries sustained from sexual intercourse may explain HIV transmission among women. We determined the variability in genitoanal injury frequency and prevalence in women after consensual sexual intercourse, exploring the role of menstrual phase and hormonal birth control. METHODS: We used a longitudinal observational design with a convenience sample of 393 women aged 21 years and older. Participants had a baseline interview with gynecological examination, followed by consensual sexual intercourse with a male partner and a second gynecological examination. W
