Smoking-Attributable Mortality in China - Epidemic Stage as a Temporal Dimension in Interpreting GBD Estimates.

Tobacco use remains a leading cause of preventable mortality worldwide. Estimates from the Global Burden of Disease, Injuries, and Risk Factors Study (GBD) are widely used to quantify smoking-attributable mortality. Over recent GBD cycles, substantial methodological refinements - including improved quantification of cumulative smoking exposure and updated relative risk estimation using the Burden of Proof (BOP) Framework - have strengthened the methodological robustness of smoking-attributable mortality estimates. Nevertheless, recent GBD estimates for China remain difficult to reconcile with

Children's Surgery: The Missing Link in the Global Child Survival Agenda.

Global child health is at an inflection point. Under-five mortality has fallen substantially over recent decades, yet progress is slowing and preventable deaths and disability remain concentrated in low- and middle-income countries. As mortality from vaccine-preventable diseases and other infections declines, birth defects and other surgically treatable conditions account for an increasing share of the residual burden. For many children, access to timely, safe and affordable surgical, anesthesia and perioperative care is the difference between survival, disability and death. Yet these services

Cervical lacerations are more frequently observed in nulliparas with obstetric anal sphincter injuries: evidence from a large cohort.

PURPOSE: Cervical lacerations, though infrequent, can lead to significant postpartum bleeding and maternal morbidity if undiagnosed. Obstetric anal sphincter injuries (OASIS) represent a more widely studied complication of vaginal delivery. However, the potential co-occurrence of OASIS and cervical lacerations has not been examined. METHODS: A retrospective cohort study was conducted at a single university-affiliated medical center from January 2018 to July 2024. The study included term singleton cephalic pregnancies that reached full cervical dilatation, including vaginal deliveries and intr

Maternal opioid prescriptions and subsequent risk of hospital admission in offspring in South Korea: A nationwide birth cohort study.

BACKGROUND: Prenatal opioid exposure may disrupt infant development and immune system function, potentially increasing long-term susceptibility to hospital admissions. Thus, we aimed to investigate the association between prenatal opioid exposure and the risk of hospital admission in children. METHODS AND FINDINGS: We established a population-based birth cohort in South Korea by linking mothers and children using National Health Insurance Service data and followed from January 1, 2009, to December 31, 2023. We constructed propensity score (PS)-matched and sibling comparison cohorts and perfor

A simple clinical model for predicting multiple liver histologic injuries in obese individuals undergoing bariatric surgery.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is common among obese individuals undergoing bariatric surgery, but liver biopsy is invasive and existing noninvasive tools generally assess single histologic features. We aimed to develop and externally validate a simple framework for predicting multiple liver histologic injuries. METHODS: We included 909 obese individuals undergoing sleeve gastrectomy with intraoperative liver biopsy in the training cohort and 186 temporally separated patients in the internal test cohort. Six separate outcome-specific binary classi

Incidence and prevalence of acquired brain injury in children and young people globally: A systematic review and meta-analysis protocol.

INTRODUCTION: Acquired brain injury refers to any injury or damage to the brain post-birth. It is considered a silent epidemic due to late or missed diagnosis, despite continued interaction with healthcare and longer-term impacts. There is a paucity of robust contemporary evidence that quantifies the population burden of childhood acquired brain injuries. This study aims to estimate the global burden of childhood acquired brain injury by pooling estimates of incidence and prevalence overall, by subtype, and by severity. We will assess variation by time, sociodemographic and clinical characteri

Vulvar Lichen Sclerosus as a Risk Factor for Birth-related Perineal Tear.

BACKGROUND: Vulvar lichen sclerosus is a chronic inflammatory skin disease that predominantly affects the anogenital region. While typically observed in postmenopausal women, it may also occur in women of reproductive age. The condition is diagnosed in approximately 1.7% of women; however, the actual prevalence is likely underestimated due to underdiagnosis and underreporting. Despite well-documented effects on vulvar anatomy and tissue integrity, evidence regarding its impact during pregnancy and childbirth remains limited. OBJECTIVE: To evaluate whether women diagnosed with vulvar lichen sc

Neonatal Gastric Perforation Temporally Associated with Nasogastric Tube Insertion: A Case Report.

BACKGROUND: Neonatal gastric perforation (NGP) is a rare but potentially life-threatening condition associated with substantial morbidity and mortality. Approximately half of reported cases are idiopathic or spontaneous, while the remainder are attributed to identifiable causes, including congenital gastric wall defects, hypoxic-ischemic injury, and iatrogenic trauma related to nasogastric tube (NGT) insertion, particularly in premature and low-birth-weight infants. Early diagnosis and prompt surgical intervention are critical for improving outcomes. We report a case of gastric perforation tem

The Mum-Alete: An Observational Survey-Based Study of Australian Athletes Returning to High-Performance Sport After Pregnancy.

BACKGROUND: As more female athletes extend their careers and resume high-performance sport following childbirth, there is an increasing need to understand factors influencing this transition. The return-to-high-performance sport process for postpartum athletes remains complex, with limited evidence available to guide sporting organisations and practitioners. This study aimed to identify the experiences, facilitators, challenges, and barriers for athletes returning to high-performance sport postpartum. DESIGN: An exploratory cross-sectional survey study. METHODS: Participants included postpar

Caregivers' reported views on firearm counseling.

BACKGROUND: Firearm related injuries are the leading cause of death in children older than one year of age. Safety counseling and prevention are cornerstones of injury prevention. This survey study sought to assess caregivers' recollection of firearm safety counseling and who provided that counseling in the past two years. It further sought to determine whom they entrusted to provide this information, and their preferred modalities for future counseling. Secondary objectives included assessing the relationship between parental perspectives on who provided this counseling and how this counselin

Assessment of Limb Dominance and Trumpeter Posture in Children With Brachial Plexus Birth Injury.

BACKGROUND: Children with brachial plexus birth injuries (BPBIs) may experience atypical posturing when performing activities of daily living, including eating. Clinical assessments often evaluate feeding ability through hand-to-mouth movements, using the unaffected arm as an internal control. This study examined differences between dominant and nondominant limbs and affected and unaffected BPBI limbs during feeding tasks and aimed to understand better the etiology of trumpeter posture in children with BPBI. METHODS: Joint angles were measured during feeding tasks using 3-dimensional motion c

Long-Term Outcomes of Spinal Accessory Nerve to Musculocutaneous Nerve Transfer in Brachial Plexus Birth Injuries.

PURPOSE: Brachial plexus birth injuries (BPBI) can be life-changing for both the patient and family. While most resolve spontaneously, surgical intervention is indicated for those who fail to improve. This can be performed via nerve grafting or nerve transfer. The spinal accessory nerve is a commonly used extraplexal donor in brachial plexus surgery. In cases of BPBI, it can be transferred to the musculocutaneous nerve to restore elbow flexion. While there are short-term reports in BPBI, few, if any, long-term outcome data are available. METHODS: A retrospective review of patients who underwe

A comparison in maternal and neonatal outcomes between mild fundal pressure and vacuum extraction for shortening the second stage of labor.

OBJECTIVE: Mild fundal pressure (MFP) and vacuum extraction (VE) are interventions used during the second stage of labor when clinical intervention is required. While VE is well established with standardized training and predictable risks, MFP remains controversial with limited comparative data. This study aimed to compare maternal and neonatal outcomes of MFP versus VE in women requiring intervention during the second stage of labor. METHODS: This is a retrospective cohort study of women with singleton term pregnancies requiring intervention during the second stage of labor at a tertiary med

Cardiovascular-kidney-metabolic syndrome staging in an internal medicine ward: an opportunity for risk stratification and intervention.

BACKGROUND: Effective rehabilitation of upper limb musculoskeletal disorders requires multimodal assessment to guide clinicians' decision-making. Furthermore, a comprehensive assessment must include reliable tests. Nevertheless, the interrelationship among various upper limb tests remains unclear. This study aimed to evaluate the reliability of easily applicable upper extremity assessments, including absolute values and asymmetries of muscle mechanical properties, pressure pain threshold, active range of motion, maximal isometric strength, and manual dexterity. A secondary aim was to explore c
