High-altitude sleep disruption is a complex syndrome that many lowlanders experience when they reach plateaus above 2,500 meters, characterized by frequent awakenings, reduced slow-wave sleep, nocturnal hypoxemia, and central sleep apnea related to unstable ventilation. A prevailing assumption in high-altitude sleep medicine holds that stabilising ventilation, chiefly via acetazolamide, is sufficient to restore sleep quality. This assumption is challenged by a randomised controlled trial demonstrating that acetazolamide significantly reduced the apnoea-hypopnoea index (AHI) and improved oxygen

Neonatal respiratory distress syndrome shows high clinical severity and economic burden for the Brazilian Unified National Health System. This study estimated the direct costs of hospitalizations from 2018 to 2021 and described their distribution by diagnosis, region, and period. A cost-of-disease study with a top-down approach and authorizations for hospitalization of newborns aged from 0 to 27 days was carried out. Values were updated to December 2025 and standardized per 1,000 live births. The period included 1,449,694 neonatal hospitalizations, of which 227,130 (15.7%) were due to respirat

BACKGROUND: Neonatal acute respiratory distress syndrome (NARDS) can cause respiratory failure in newborns and even pose a threat to their lives, causing a heavy financial burden on families.
AIM: This study aims to explore the potential role of miR-155 as a diagnostic marker and prognostic predictor for NARDS.
METHODS: This study included 100 non-NARDS newborns and 100 NARDS newborns. The plasma miR-155 expression was measured by RT-qPCR. The ROC curve was used to analyze the diagnostic efficacy of miR-155 alone and the combined diagnostic efficacy of PT, APTT, FIB, and CRP. The correlation

Primary care clinicians who provide medical homes for neonatal intensive care unit (NICU) graduates may benefit from an enhanced understanding of new breathing support strategies available in NICUs. Understanding the severity of a patient's respiratory course in the NICU may help anticipate the patient's tolerance of future viral illnesses. Primary care clinicians may also serve as the leaders of neonatal resuscitation teams in referring hospitals. Understanding new equipment options in the delivery room setting and the rationale for these changes can help resuscitation teams adopt new practic

BACKGROUND: Narcolepsy (NT) and obstructive sleep apnea (OSA) frequently coexist, yet the sleep electroencephalographic (EEG) spectral profile of narcolepsy with comorbid OSA (NT-OSA) remains unclear. We compared spectral power among NT-OSA, NT, and OSA across polysomnography (PSG) and Multiple Sleep Latency Test (MSLT).
METHODS: We retrospectively analyzed 224 treatment-naïve adults with excessive daytime sleepiness (37 NT-OSA, 77 NT, and 110 OSA); 39 subjectively sleepy controls (SSC) were included in a secondary analysis. Relative EEG spectral power was calculated in central and occipital

Sleep apnea (SA) diagnosis remains challenging due to the cost and limited accessibility of polysomnography. Most automated screening approaches estimate the Apnea-Hypopnea Index (AHI) from oximetry-derived features and do not provide temporally resolved characterization of respiratory events. This study investigates the feasibility of dense temporal respiratory event profiling and clinically meaningful SA severity estimation from single-channel photoplethysmography (PPG).
Approach: A two-stage framework was developed using single-channel PPG. A fully convolutional neural network operating

Sleep-disordered breathing (SDB), particularly obstructive sleep apnea (OSA), is a highly prevalent group of disorders associated with disrupted sleep, intermittent hypoxemia, excessive daytime sleepiness, neurocognitive impairment, and cardiometabolic morbidity. Although laboratory polysomnography remains central to diagnosis, conventional sleep testing can be costly, labor-intensive, and difficult to scale. Recent advances in data mining, machine learning, deep learning, wearable sensing, mobile health, multimodal analytics, and large language models are creating new opportunities for more a

The earliest clinical sign of pulmonary vascular disease in neonates is persistent pulmonary hypertension of the newborn (PPHN), traditionally classified as World Symposium on Pulmonary Hypertension (WSPH) Group 1. Typical PPHN is defined by the delayed transition from intra- to extra- uterine life with failure of pulmonary artery pressures to fall after delivery. This results in sustained elevation of pulmonary vascular resistance and contributes to hypoxemic respiratory failure (HRF). However, early HRF can also arise from atypical PPHN phenotypes that may persist beyond the expected resolut

This study aimed to identify key biomarkers for obstructive sleep apnoea (OSA) by analysing RNA-seq (GSE75097: 6 control and 42 OSA) and single-cell RNA-seq (GSE214865: 11 control and 11 OSA) datasets. Differential expression analysis revealed 575 DEGs between OSA and control groups. Using weighted gene co-expression network analysis (WGCNA) combined with machine learning approaches (Random Forest, SVM-RFE and LASSO), we identified four hub genes (C17ORF91, HBEGF, PGBD2 and ZNF616) as candidate biomarkers, showing exploratory discriminatory performance (AUC > 0.7). Gene set enrichment analysis

Transient tachypnea of the newborn (TTN) commonly affects near- and full-term infants and often requires respiratory support. The use of nebulized salbutamol to improve respiratory outcomes has been of interest, though its clinical efficacy remains controversial. This systematic review and meta-analysis aimed to evaluate whether nebulized salbutamol improves clinical outcomes in affected infants. We searched PubMed, Scopus, Embase, and the Cochrane Library for randomized controlled trials (RCTs) evaluating the efficacy of nebulized salbutamol in infants with TTN. We used a random-effects model

Massive pulmonary artery thrombosis in neonates is rare and frequently fatal when associated with right ventricular failure and hemodynamic collapse. We report a premature neonate with extensive inferior vena cava, right ventricular outflow tract, and bilateral pulmonary artery thrombosis resulting in respiratory failure requiring venoarterial extracorporeal membrane oxygenation. Progressive thrombus burden despite systemic anticoagulation prompted catheter-directed mechanical aspiration thrombectomy using escalating aspiration systems with adjunctive localized thrombolysis. Persistent pulmonary hypertension of the newborn (PPHN) is a critical condition marked by elevated pulmonary vascular resistance (PVR) and persistent hypoxemia, due to the failure of transition from foetal to neonatal circulation. Management focuses on reducing PVR, enhancing pulmonary vasodilation, and ensuring adequate oxygen delivery. Inhaled nitric oxide (iNO), an FDA-approved therapy, effectively lowers PVR and improves oxygenation in term neonates. However, in resource-limited settings, its use is constrained by high costs, the need for specialized equipment, and limited availability of

UNLABELLED: Neonatal acute respiratory distress syndrome (NARDS) carries high morbidity and mortality. Preterm infants at risk need to be identified early so that treatment is not delayed. We aimed to identify factors associated with NARDS in preterm infants. Clinical data of 548 preterm infants admitted to the NICU between 2019 and 2023 were retrospectively analyzed. Preterm infants were categorized into the NARDS group (n = 275) and the control group (n = 273) according to the Montreux definition. Univariate analysis and multivariable logistic regression were used to identify factors indepen
