Registry-Based Surveillance of Severe Acute Respiratory Infections in Norway During 2021-2024.

BACKGROUND: In 2021, the Norwegian Institute of Public Health established temporary registry-based surveillance of severe acute respiratory infections (SARI). We aimed to describe the surveillance system and evaluate selected attributes to inform the establishment of a permanent SARI surveillance system. METHODS: SARI cases were defined using ICD-10 discharge codes from national health and administrative registries, including codes for acute upper or lower respiratory infection (URI and LRI), COVID-19, acute respiratory distress syndrome, pertussis or otitis media. Data from polymerase chain

Renal Tubular Dysgenesis: Broadening the Discussion of the Etiological Spectrum.

Renal tubular dysgenesis (RTD) is a rare disorder characterized by impaired development of the renal tubules. It is often a fatal condition that should be considered in the differential diagnosis of neonatal kidney failure. RTD can be classified as primary (linked to deleterious variants in genes encoding renin-angiotensin system (RAS) proteins) or secondary to an underlying cause. In this case report, we present a late preterm female neonate born at 35 weeks by elective cesarean section due to oligohydramnios and fetal growth restriction. At birth, she exhibited hypotonia and features consist

Less Invasive Surfactant Administration Versus Intubate-Surfactant-Extubate: Associated With Reduced Mechanical Ventilation in Extremely Preterm Infants.

AIM: To compare less invasive surfactant administration (LISA) and intubation-surfactant-extubation (INSURE) in infants born before 28 weeks gestational age (GA) by the need for mechanical ventilation (MV). METHODS: LISA was introduced in 2017 in our unit without formal prior LISA-specific training as an equal alternative to INSURE. Medical records (July 2017 to July 2021) were reviewed for infants born < 28 weeks gestation treated with LISA or INSURE. The primary outcome was needing MV within 72 h, adjusted for potential confounders (including gestational age, birthweight, sex, mode of deliv

Update on nasal high frequency oscillatory ventilation: A living review.

Non-invasive high-frequency oscillatory ventilation (NHFOV) is the main non-conventional ventilatory mode used in newborn infants. NHFOV has been spreading, while knowledge about its physiology, mechanics and clinical application has increased overtime. This is to be considered as a living review, since we here update the knowledge that was originally summarized in a previously published review. We first focus on physiology of such a complex respiratory support technique with practical data on interfaces, patient-ventilator interaction and devices to provide NHFOV. We then move to the evidence

A Novel and Promising Method for Prediction of Early Surfactant Requirement in Preterm Infants With Respiratory Distress Syndrome: Pulmonary Near-Infrared Spectroscopy.

INTRODUCTION: Non-invasive monitoring techniques have been used more commonly in neonatal clinical practice. The aim of this study was to evaluate the role of lung near-infrared spectroscopy (NIRS) for determination of the severity of Respiratory Distress Syndrome (RDS) and the requirement of surfactant therapy in preterm infants. METHODS: A total of 60 preterm infants with RDS were evaluated in terms of the requirement of surfactant therapy, as surfactant group (SG) or no-surfactant group (CG). Lung NIRS (L-NIRS), including both basal (bL-rSO2) and apex (aL-rSO2) lung oxygenation monitoring,

Acute Respiratory Distress Syndrome in Children With Lower Respiratory Tract Infection Requiring Invasive Mechanical Ventilation: Post Hoc Analysis of the 2019-2020 Bronchiolitis and Codetection Cohort.

OBJECTIVES: Bronchiolitis and other lower respiratory tract infections (LRTIs) are the most common causes of pediatric respiratory failure. There is insufficient evidence characterizing pediatric acute respiratory distress syndrome (PARDS) in young children with LRTI to inform clinical management. We aimed to describe the prevalence and clinical characteristics of children intubated for LRTI and meeting PARDS criteria. DESIGN: We performed a post hoc analysis of data from the Bronchiolitis And COdetectioN (BACON) study, an international prospective observational study of critical bronchioliti

Advances in management of noninvasive ventilation for preterm infants.

Providing most appropriate noninvasive ventilation (NIV) for preterm infants remains priority in neonatal intensive care units. Several newer modes of ventilation have evolved including continuous positive airway pressure, nasal intermittent positive pressure ventilation and noninvasive high frequency ventilation to meet patients' demands. Advanced level of monitoring is performed while patients are receiving NIV for optimization of the respiratory support. Point of care lung ultrasound and electrical impedance tomography are the newer tools that give insights towards aeration of the lungs. Pu

Identification of distinct clinical profiles of sepsis risk in paediatric emergency department patients using Bayesian profile regression.

BACKGROUND: Sepsis affects 25 million children and neonates annually, causing significant mortality and morbidity. Early identification and treatment are crucial for improving outcomes. Identifying children at risk is challenging due to clinical heterogeneity and overlap with other conditions. Current evaluations of sepsis criteria adopt a variable-centred approach, evaluating each criterion independently. The objective of this study was to explore associations between patterns of sepsis screening criteria and sepsis risk in children screened in the emergency department (ED) to identify distin

The CFTR K464N variant in fetuses potential increases premature birth risk in Chinese families.

BACKGROUND: Global fertility decline has led to increased use of assisted reproductive technology (ART), raising concerns about genetic risks to offspring. This study aimed to investigate cystic fibrosis transmembrane conductance regulator (CFTR) variants in Chinese families and assess their association with pregnancy complications and neonatal outcomes. METHODS: This prospective cohort study included 446 Chinese families (148 natural conceptions, 298 ART conceptions) who underwent whole genome sequencing. We analyzed the frequency of pathogenic/likely pathogenic CFTR variants and their assoc

The Impact of the COVID-19 Pandemic on the Clinical and Epidemiological Profile of Severe Acute Respiratory Infection in Bahia, Brazil: A Comparative Analysis of Pre- and Post-Pandemic Trends.

In recent years, the incidence of Severe Acute Respiratory Infection (SARI) has increased due to the emergence of SARS-CoV-2. However, the impact of the COVID-19 pandemic extends beyond mortality rates. Recent analyses suggest that the introduction and spread of SARS-CoV-2 have significantly affected the epidemiology of other key respiratory viruses, such as influenza virus (FLUV), respiratory syncytial virus (RSV), and rhinovirus (RV). These changes raise new questions about the dynamics and incidence of post-COVID-19 respiratory infections, as well as potential alterations in symptom profile

Retinal detachment in a neonate with congenital chylothorax and purpura fulminans associated with the PAK2 genetic variant: A case report.

INTRODUCTION AND IMPORTANCE: A potential relationship between bilateral retinal detachment, chylothorax, and purpura fulminans in a female neonate with a PAK2 gene variant is not commonly reported. This emphasizes the significance of early ophthalmologic assessment in neonates with congenital chylothorax. CASE PRESENTATION: A full-term female infant weighing 2.775 kg was delivered by cesarean section due to breech presentation. Prenatal imaging revealed fetal bilateral pleural effusion, suggestive of chylothorax. The neonate developed respiratory distress and purpura fulminans after birth. Th

Intrauterine Adhesion Band Causing Fetal Leg Compression: A Case Report of a Rare Asherman Syndrome Complication in Pregnancy.

Asherman syndrome (AS), characterized by intrauterine adhesions, can lead to various pregnancy complications. We report a case of a 31-year-old woman who was admitted at 29+2 weeks due to preterm premature rupture of membranes at our university hospital. The woman had a history of AS following emergency curettage. Ultrasonography revealed an adhesion band in the lower uterine segment, closely positioned to the fetus's leg. Antibiotic therapy and antenatal corticosteroids were administered to prevent infection and neonatal respiratory distress syndrome. Two weeks later, the woman developed labo

Genetic Disorders of Surfactant Metabolism.

Genetic disorders affecting surfactant protein production and function can result in respiratory distress and severe respiratory failure in late-preterm and term neonates. Pathogenic variants in surfactant pulmonary-associated protein B (SFTPB) are typically loss-of-function and disrupt surfactant protein B (SP-B) production and surfactant function. Dominant variants in surfactant pulmonary-associated protein C (SFTPC) generally result in a toxic gain-of-function with disruption of surfactant protein C (SP-C) processing and trafficking in the alveolar epithelial type 2 cells. Adenosine triphos

Non-invasive respiratory support paired with minimally invasive surfactant therapy in preterm infants.

Non-invasive ventilation (NIV) commenced soon after birth is highly effective in providing mechanical respiratory support for preterm infants with respiratory distress syndrome (RDS). However, NIV alone frequently fails to provide adequate respiratory support for infants with more significant respiratory compromise due to RDS. Without an endotracheal tube as the conduit to administer exogenous surfactant in such cases, less invasive approaches to surfactant delivery have emerged, with those involving the use of a thin catheter (termed minimally invasive surfactant therapy, MIST) now in the asc
