Preterm birth is a major contributor to neonatal morbidity and mortality. This study evaluated the neonatal Sequential Organ Failure Assessment (nSOFA) score for predicting mortality in preterm neonates (< 34 weeks) with respiratory distress syndrome. Among 113 neonates, median (q1, q3) nSOFA scores at 0-6 h of life were higher in non-survivors than survivors [4 (4, 6) vs 0 (0, 2); P = 0.001], with an area under the curve (AUC) of 0.80 and a cutoff of 3. Predictive performance improved at 24 ± 3 h (AUC 0.93), with higher scores in non-survivors [8 (5, 11) vs 0 (0, 2); P < 0.001] and a cutoff o

Continuous positive airway pressure (CPAP) is the standard initial therapy for respiratory distress syndrome (RDS) and associated with failure rates of up to 50% in extremely preterm infants. Nasal intermittent positive pressure ventilation (NIPPV) has demonstrated consistent benefits over CPAP in reducing respiratory failure and intubation. Nasal high-frequency oscillatory ventilation (nHFOV) has also shown reduced treatment failure compared with CPAP, particularly in extremely preterm infants. However, direct head-to-head comparisons between nHFOV and NIPPV as primary support are lacking, an

BackgroundRespiratory distress is a frequent complication in preterm infants, often requiring respiratory support. Respiratory distress syndrome (RDS), caused by surfactant deficiency in the immature lung, is the predominant etiology in this population. Continuous positive airway pressure (CPAP) is a common intervention, but the optimal timing of CPAP initiation remains less clearly defined in the late preterm population (33-36 weeks gestation). This study aimed to determine the outcomes of preterm newborns with respiratory distress in relation to the timing of CPAP initiation.MethodsThis pros

Pulmonary hypertension (PH) in preterm infants is a complex and heterogeneous condition that significantly contributes to hypoxic respiratory failure (HRF), bronchopulmonary dysplasia (BPD), and mortality. Unlike term infants, PH in preterm neonates arises from multiple overlapping pathophysiological mechanisms and distinct hemodynamic phenotypes. These diverse phenotypes complicate diagnosis and limit the effectiveness of uniform treatment approaches. Despite increasing recognition of this heterogeneity, current literature and clinical guidelines provide limited clarity on the optimal diagnos

Background: Hypertriglyceridemia (HiTG) is a frequent metabolic complication in preterm neonates receiving parenteral nutrition (PN), with potential implications for respiratory outcomes. However, predisposing factors and the clinical impact of HiTG in this population remain poorly defined. Objectives: To identify factors predisposing preterm neonates to HiTG development and to assess the association between HiTG and neonatal respiratory morbidity. Methods: A retrospective matched cohort study was conducted, nested within a single-center cohort of neonates receiving routine parenteral nutritio

Neonatal respiratory diseases remain a leading cause of morbidity and mortality in intensive care settings, with management complicated by the limitations of current monitoring tools. Chest radiography provides only static snapshots and cannot capture the dynamic evolution of lung aeration, while lung ultrasound, though portable, remains operator-dependent and incapable of quantifying regional ventilation. Electrical Impedance Tomography (EIT) has emerged as a noninvasive, radiation free imaging modality that addresses these gaps by enabling real time, continuous visualization of regional lung

INTRODUCTION: Preterm birth (< 37 weeks) causes high neonatal mortality, with respiratory distress syndrome (RDS) as a major complication due to surfactant deficiency. Surfactant administered via several methods. This study compares effectiveness and complications of two different methods of surfactant therapy.
METHODS: This randomized clinical trial was conducted involving two groups of a total of 80 neonates aged 28-34 weeks who, based on clinical conditions, required surfactant administration. In the INSURE method, the infant is first intubated with an appropriate size endotracheal tube, s

Respiratory distress syndrome (RDS) remains a major cause of neonatal mortality following premature birth. Antenatal betamethasone is the standard of care to accelerate fetal lung maturation and improve neonatal outcomes; however, current dosing recommendations assume uniform maternal and fetal exposure across pregnancies. Gestational age (GA)-dependent physiological changes and maternal obesity may alter betamethasone disposition and fetal exposure. This study developed a maternal-fetal physiologically based pharmacokinetic (m-f-PBPK) model to quantify the effects of GA and maternal obesity o

UNLABELLED: Staphylococcus aureus carrying the Panton-Valentine leukocidin (PVL) genes, in both methicillin-susceptible (MSSA) and methicillin-resistant (MRSA) strains, causes severe infection in previously healthy children. We described critically ill children with PVL-positive infection and explored predictors of prolonged intensive care. In this multicenter, single-cohort retrospective study, we included 44 consecutive children aged 0-16 years with PVL-producing S. aureus infection admitted to a neonatal or pediatric intensive care unit (PICU) at 13 Italian centers between 2022 and 2025. Neonatal respiratory distress syndrome shows high clinical severity and economic burden for the Brazilian Unified National Health System. This study estimated the direct costs of hospitalizations from 2018 to 2021 and described their distribution by diagnosis, region, and period. A cost-of-disease study with a top-down approach and authorizations for hospitalization of newborns aged from 0 to 27 days was carried out. Values were updated to December 2025 and standardized per 1,000 live births. The period included 1,449,694 neonatal hospitalizations, of which 227,130 (15.7%) were due to respirat

Primary care clinicians who provide medical homes for neonatal intensive care unit (NICU) graduates may benefit from an enhanced understanding of new breathing support strategies available in NICUs. Understanding the severity of a patient's respiratory course in the NICU may help anticipate the patient's tolerance of future viral illnesses. Primary care clinicians may also serve as the leaders of neonatal resuscitation teams in referring hospitals. Understanding new equipment options in the delivery room setting and the rationale for these changes can help resuscitation teams adopt new practic

Transient tachypnea of the newborn (TTN) commonly affects near- and full-term infants and often requires respiratory support. The use of nebulized salbutamol to improve respiratory outcomes has been of interest, though its clinical efficacy remains controversial. This systematic review and meta-analysis aimed to evaluate whether nebulized salbutamol improves clinical outcomes in affected infants. We searched PubMed, Scopus, Embase, and the Cochrane Library for randomized controlled trials (RCTs) evaluating the efficacy of nebulized salbutamol in infants with TTN. We used a random-effects model
