Factors associated with the development of neonatal acute respiratory distress syndrome in preterm infants: a retrospective cohort study.

UNLABELLED: Neonatal acute respiratory distress syndrome (NARDS) carries high morbidity and mortality. Preterm infants at risk need to be identified early so that treatment is not delayed. We aimed to identify factors associated with NARDS in preterm infants. Clinical data of 548 preterm infants admitted to the NICU between 2019 and 2023 were retrospectively analyzed. Preterm infants were categorized into the NARDS group (n = 275) and the control group (n = 273) according to the Montreux definition. Univariate analysis and multivariable logistic regression were used to identify factors indepen

3-minute vs 10-minute Spontaneous Breathing Evaluations (SBE) for predicting extubation success in preterm very low birth weight infants: Retrospective observational study.

BackgroundExtubation in preterm very low birthweight (VLBW) infants is a critical step in respiratory management. Accurately predicting extubation success is challenging because of immature respiratory control, surfactant deficiency, and limited pulmonary reserve. The Spontaneous Breathing Evaluation (SBE) has emerged as an important bedside method to assess readiness for successful extubation, although the optimal duration of this evaluation remains uncertain.MethodsThis retrospective observational study included preterm neonates between 26 and 34 weeks of gestation with birthweight <1500 g w

Reducing mother-infant separation through early skin-to-skin care in a neonatal intensive care unit setting.

INTRODUCTION: In neonatal care, mother-infant separation is common even when infants are stable or require only short-term support. Early skin-to-skin (ESTS) care may reduce separation and improve stability, bonding and breastfeeding. We aimed at implementing the practice as a protective quality improvement project. METHODS: The study was conducted in a level-2 neonatal intensive care unit (NICU) in Denmark, embedded in routine practice. ESTS was offered whenever feasible to preterm infants, GA ≥ 30 weeks or term infants with low birth weight or respiratory symptoms. ESTS was initiated immedi

The effect of postnatal steroids on lung ultrasound scores and extubation readiness in very low birth weight infants.

OBJECTIVE: We assessed the utility of lung ultrasound scores (LUSs) to predict extubation readiness in VLBW infants and determined the effect of postnatal steroids on LUSs in babies who were chronically ventilated for > 30 days. STUDY DESIGN: We measured infants' LUS scores before planned extubations and determined the success of the subsequent extubation attempts. RESULTS: Overall, LUSs were lower in successfully extubated compared with unsuccessfully extubated infants in the entire population. Similar differences were seen in LUSs between successfully and unsuccessfully extubated chronical

A clinical study on the construction of a prenatal ultrasound-based predictive model for fetal lung maturity in late pregnancy using propensity score matching.

BACKGROUND: Fetal lung maturity (FLM) is closely associated with neonatal mortality, morbidity, and respiratory diseases such as neonatal respiratory distress syndrome (NRDS). At present, the assessment of FLM largely relies on invasive procedures, while the establishment of a noninvasive ultrasound-based method has important clinical significance. OBJECTIVE: To construct and validate a predictive model of FLM based on ultrasound parameters in late pregnancy, exploring a noninvasive approach for risk assessment in fetal lung development. METHODS: A total of 195 pregnant women (gestational ag

International Registry of NKX2-1-Related Disorders: Clinical, Genetic, and Imaging Perspectives.

BACKGROUND: NKX2-1-related disorders result from heterozygous variants in NKX2-1, a gene crucial for brain, lung, and thyroid development. Although movement disorders, hypothyroidism, and neonatal respiratory distress are recognized, the full phenotype and genotype-phenotype relationships remain incompletely defined. OBJECTIVES: To delineate neurological, respiratory, and endocrine features across ages, characterize movement disorder trajectories - particularly chorea - and explore genotype-phenotype associations with clinical relevance. METHODS: We conducted a multicenter, cross-sectional s

Utilization of integrated lung ultrasound and targeted neonatal echocardiography in preterm infant follow-up: is it feasible? Assessing value and practical challenges.

UNLABELLED: Lung ultrasound (LUS) and targeted neonatal echocardiography (TNE) are employed in some neonatal intensive care units throughout the hospital stay to monitor heart- and lung-related morbidities. However, preterm infants are at risk of increased rates of respiratory and cardiovascular morbidities across their lifespan. This commentary aims to define the utility of long-term TNE and LUS follow-up and to assess the challenges of incorporating these techniques into long-term practice. The cardiovascular phenotype of bronchopulmonary dysplasia (BPD) includes diverse cardiac and extracar

A review of pulmonary development and neonatal interstitial lung disease in dogs and cats from a pathology perspective.

Neonatal interstitial lung disease occurs within the first 3 wk of life and includes any disease process that affects alveolar septa and, in some species, interlobular septa. Postmortem diagnosis of neonatal interstitial lung disease is challenging because of our incomplete understanding of normal postnatal lung development, especially in altricial species such as dogs and cats, which are born with morphologically and physiologically immature lungs. Most altricial species are born with lungs in the saccular stage of development and continue development to the alveolar stage in the postnatal pe

Whole blood resuscitation is associated with decreased end-organ dysfunction in pediatric trauma patients.

INTRODUCTION: Low Titer Group O Whole Blood (WB) has emerged as a safe and efficient therapeutic agent for hemostatic resuscitation in injured children. The relationship between type of blood product transfused and incidence of organ dysfunction is not well elucidated. We hypothesize that WB transfusion is associated with decreased organ dysfunction compared with component therapy (CT). METHODS: This is a retrospective observational study of injured children ages 0 year to 17 years utilizing the TQIP database (2020-2022). Inclusion criteria were receipt of any blood product within 4 hours of

Breathe Easier: Optimizing Continuous Positive Airway Pressure in the Neonatal Intensive Care Unit.

BACKGROUND: Rates of bronchopulmonary dysplasia (BPD) continue to increase despite advances in healthcare. Various methods of delivering continuous positive airway pressure (CPAP) can be contributing factors that increase the risk of developing BPD. A recent increase in BPD rates at a local neonatal intensive care unit (NICU) prompted further investigation into potential preventative practices. PURPOSE: The project's goal was to reduce rates of BPD in the NICU by 20% after transitioning to FlexiTrunk CPAP, as well as through education to increase nursing confidence, which will improve complia

Dexmedetomidine for LISA procedure: a prospective observational single center experience.

BACKGROUND: Respiratory Distress Syndrome (RDS) is the most common respiratory problem of preterm newborns. The preferred way to manage RDS is with Less Invasive Surfactant Administration (LISA), which involves direct laryngoscopy in spontaneously breathing infants. Despite its widespread diffusion, the optimal sedation protocol for LISA remains unclear. Dexmedetomidine, an α2-adrenergic agonist, offers sedative and analgesic properties with minimal respiratory depression. This pilot study aims to evaluate dexmedetomidine for LISA procedure focusing on the frequency of adequate sedation (defin
