Disseminated Penicillium marneffei infection in human immunodeficiency virus-infected children.

Disseminated infection with the fungus Penicillium marneffei is one of the most common opportunistic infections in human immunodeficiency virus (HIV) disease in northern Thailand. We report the clinical, laboratory and therapeutic features of 21 human immunodeficiency virus-infected children with disseminated P. marneffei who were prospectively followed. Significant clinical and laboratory features included generalized lymphadenopathy (90%), hepatomegaly (90%), body temperature > 38.5 degrees C (81%), papular skin lesions with central umbilication (67%), splenomegaly (67%), failure to thrive (

Heart transplantation in neonates and in children.

Between May 1988 and July 1991, 28 neonates and children underwent orthotopic heart transplantation at Children's Memorial Hospital in Chicago. Indications for heart transplantation were hypoplastic left heart syndrome (10), dilated cardiomyopathy (13), aortic stenosis with endocardial fibroelastosis (1), complex D-transposition of the great arteries after Senning repair (1), L-transposition of the great arteries with single ventricle after shunt (1), cor biloculare, pulmonary atresia, and situs inversus after Fontan (1), and chronic rejection after heart transplantation for hypoplastic left h

Heart-lung transplantation in 1990--indications, surgical technique, postoperative complications and outcome.

Using cyclosporine A (CyA), long-term results after heart-lung transplantation became possible. Since 1981, 761 clinical heart lung transplantations have been registered at the International Society for Heart Transplantation. Candidates for this operation reveal signs of irreversible heart and lung diseases which are caused by cardiac lesions (e.g. valvular disease, Eisenmenger reaction due to congenital malformation) or pulmonary disorders (e.g. primary pulmonary hypertension, emphysema, fibrosis). The standard surgical procedure necessitates three anastomoses which combine donor and recipien

Human immunodeficiency virus infection in childhood.

Acquired immunodeficiency syndrome is associated with considerable morbidity in infants and children. It is caused by human immunodeficiency virus (HIV) which can be transmitted vertically from mother to infant early in pregnancy. Transmission might also occur via breast milk. Although the exact transmission rate of HIV from mother to infant is not known, HIV can become a major threat to child survival. This threat is already present in Africa where high seroprevalences have been reported among infants and young children. Transmission via blood products is decreasing due to reliable methods of

Cytomegalovirus pneumonia after bone marrow transplantation successfully treated with the combination of ganciclovir and high-dose intravenous immune globulin.

STUDY OBJECTIVE: To assess the efficacy of the combination of the antiviral agent ganciclovir (9-1,3 dihydroxy-2-propoxymethylguanine) and high-dose intravenous immune globulin for treating cytomegalovirus interstitial pneumonitis after allogeneic bone marrow transplantation. DESIGN: Nonrandomized prospective trial of combined treatment with two drugs; findings in these patients were compared with those in control patients treated with either of the two drugs alone. SETTING: Medical, pediatric, and intensive care units of a tertiary-care cancer treatment center. PATIENTS: Consecutive cases

Human immunodeficiency virus infection in children: nature of immunodeficiency, clinical spectrum and management.

The causative agent of acquired immunodeficiency syndrome is a retrovirus, human T lymphotropic virus type III/lymphadenopathy-associated virus, now known as human immunodeficiency virus (HIV). Infection of children with HIV results in a wide spectrum of clinical manifestations, ranging from asymptomatic to symptomatic, with the severest disease forms including neurologic deterioration, opportunistic infections and malignancy. This virus infects preferentially T cells bearing the CD4 receptors and also seems to exhibit preference for the central nervous system. The predominant route of infecti

Cytomegalovirus infection in pregnancy.

Cytomegalovirus (CMV) is the most common agent of prenatal (peri- and early postnatal) infection of the newborn with an incidence of 0.2-2.0% (5-10%) depending on the socio-economic status. Only one out of 20 congenitally CMV-infected newborns shows serious symptoms. Another two may reveal mental retardation and other significant handicaps in later age. Perinatal CMV infection occasionally causes atypical pneumonia, mostly combined with Pneumocystis carinii infections similar to reports of AIDS cases. The risk of vertical infection has been quantified. About 2-4% of pregnant seronegative women

[Cyclocytidine therapy for the herpes-group virus infection in children. A preliminary study].

Cyclocytidine (Cyclo-C) was evaluated as an antiviral chemotherapeutic agent in 10 children with herpes-group virus infections. The diagnoses of the patients were severe cytomegalovirus (CMV) syndrome with pneumonitis in acute leukemia (2), congenital or infantile CMV infections (3), herpes zoster encephalitis in acute leukemia (1), severe nosocomial varicella in compromised patients (2), varicella pneumonia (1) and acute encephalitis due to herpes simplex type-1 (1). Cyclo-C was effective in all the cases with acute infections, although it had no benefit on the course of chronic CMV infection

Immunization: Evaluation of some currently available and prospective vaccines.

Acute respiratory and diarrheal diseases cannot be expected to be effectively controlled by vaccines for a variety of reasons but largely because of the great multiplicity of causative agents. Measles could be eradicated by appropriate strategies. Paralytic poliomyelitis caused by polioviruses can be controlled by oral poliovirus vaccine in both developed and undeveloped countries but by different strategies. Vaccines cannot be expected to have a beneficial effect on recurrent genital, ocular, and facial herpes. Since naturally acquired immunity does not prevent subsequent fetal intrauterine i

Acute pneumonia in the newborn: changing picture.

Pneumonia is one of the most serious infections in the neonate and is responsible for a large percentage of neonatal mortality. Pneumonia in a premature or term infant who is debilitated by an underlying problem such as hyaline membrane disease carries an extremely high morbidity and mortality. Since most of the bacterial pneumonias are treatable, early recognition and diagnosis and vigorous treatment are essential. X-ray findings, though helpful, serve only as a guideline. Prognosis is adversely affected if pneumonia results in generalized sepsis, leading to meningitis, disseminated intravasc
