Case Report: Beyond commensal: Staphylococcus epidermidis as a novel cause of NARDS.

Staphylococcus epidermidis (S. epidermidis), usually a harmless skin bacterium, can become an opportunistic pathogen in newborns, particularly those with risk factors like premature membrane rupture. Although it commonly causes late-onset sepsis, its association with neonatal acute respiratory distress syndrome (NARDS) is rare. This report describes a unique case of NARDS in a full-term newborn caused by S. epidermidis. The infant, born via cesarean at 40 2/7 weeks with a 30.5-hour membrane rupture, developed severe respiratory failure shortly after birth, necessitating mechanical ventilation.

Nitric oxide in Extracorporeal Membrane Oxygenation Sweep Gas Promotes Oxygenator Longevity: A Technical Case Report.

In the accompanying article in this issue, we reported the survival of a 4 month old infant with severe necrotizing pneumonia due to respiratory syncytial virus (RSV) and methicillin-resistant Staphylococcus aureus (MRSA), resulting in cardiogenic shock and respiratory failure, who required a total of 233 days of extracorporeal membrane oxygenator (ECMO) support. In this article, we report the function of the ECMO membrane (oxygenator). To preserve oxygenator function, nitric oxide was added to the ECMO sweep gas (sNO). Despite prolonged ECMO support, a single membrane lung functioned effectiv

Methicillin Resistant Staphylococcus Aureus Colonization, The Tip of the Iceberg for Congenital Pulmonary Airway Malformation. A Case Report.

BACKGROUND: Congenital Pulmonary Airway Malformation (CPAM) is a rare congenital anomaly of the lung that is infrequently diagnosed in the neonatal period, especially in preterm infants without prenatal detection. Methicillin-Resistant Staphylococcus aureus (MRSA) colonization and infection in preterm neonates can further complicate diagnosis and clinical progression. AIM: To describe a rare presentation of CPAM diagnosed in a preterm neonate following late-onset MRSA sepsis, emphasizing the diagnostic challenge and the interplay between infection and congenital malformation. METHODS: We pre

Case Report: Novel variants in the MYD88 gene (c.104T>C, c.141G>C) in a patient with recurrent abscesses as a cause of type 68 immunodeficiency.

BACKGROUND: Primary immunodeficiencies (PIDs) comprise a heterogeneous group of disorders characterized by defects in the immune system, predisposing patients to recurrent and severe infections. Type 68 immunodeficiency, caused by biallelic pathogenic variants in MYD88, is rare; to date, at least 26 affected individuals have been reported in the literature, several of whom belong to the same families. This condition typically presents in early childhood with recurrent severe bacterial infections (SBIs), often accompanied by an absent or attenuated inflammatory response. CASE PRESENTATION: We

Unravelling the mystery of a rare infection: a challenging case of pulmonary sequestration with Mycobacterium avium complex and the importance of a thorough microbiological investigation.

Pulmonary sequestration is a rare congenital condition. It is a dysplastic lung tissue with a separate systemic blood supply and without a bronchial tree connection. The emergence of a superimposed infection can lead to its diagnosis, such as Staphylococcus aureus, Pseudomonas aeruginosa, Nocardia asteroids and Aspergillus sp pneumonia. Mycobacterium avium complex (MAC) superimposed disease is exceedingly rare. We report a case of a man in his third decade without known medical disorders presenting with a persistent cough. After an extensive microbiological workup, an MAC infection was diagnos

Lentiviral Gene Therapy of Chronic Granulomatous Disease: Functional Assessment of Universal and Tissue-Specific Promoters.

Chronic granulomatous disease (CGD) is a rare congenital immunodeficiency characterized by a defect in nicotinamide adenine dinucleotide phosphate oxidase required for phagocytosis. Hematopoietic stem cell (HSC) transplantation is currently the only curative treatment, but it is ladened with morbidities and mortality. Gene therapy is a promising treatment for CGD. However, if not properly designed, the gene therapy approach may not be successful. We engineered lentiviral vectors (LVs) carrying a universal promoter (EF1a) and two myeloid-specific promoters (miR223 and CD68) to drive the express

Congenital pulmonary airway malformation (CPAM) with initial presentation in an adult: a rare presentation of a rare disease.

Congenital pulmonary airway malformation (CPAM) is a rare congenital abnormality with unknown exact aetiology or clear genetic association. It is characterised by a failure of bronchial development and localised glandular overgrowth. Typically, it is diagnosed on prenatal ultrasound, only infrequently in children, and even less commonly in adults. We present a case of a 25-year-old man, with no previous lung diseases who presented with right-sided chest pain, fever and cough suggestive of pulmonary infection. Chest imaging, including CT scan, showed a large focal cystic mass within the right l
