Safety and Efficacy of Therapeutic Agents in Viral Pneumonia in Immunosuppressed Patients.

INTRODUCTION/OBJECTIVE: Immunosuppressed Patients (ISP) - defined by congenital or acquired deficiencies in systemic defense mechanisms - are disproportionately susceptible to viral respiratory infections. Agents such as SARS-CoV-2, Respiratory Syncytial Virus (RSV), influenza A and B, and Cytomegalovirus (CMV) cause significant morbidity in both immunocompetent and immunosuppressed individuals. Clinical manifestations range from mild influenza-like illness to severe pneumonia, respiratory failure, multiorgan dysfunction, and concurrent bacterial infections. METHODS: A narrative synthesis of

Target Trial Emulation of Empiric Antibiotics on Clinical Outcomes in Moderately Immunocompromised Patients Hospitalized With Pneumonia.

BACKGROUND: Immunocompromised patients are often excluded from pneumonia trials, guidelines, and stewardship interventions. The objective of this study was to evaluate whether empiric broad-spectrum antibiotic (BSA) treatment impacts mortality and other clinical outcomes in moderately immunocompromised patients without risk factors for multidrug-resistant organisms (MDROs) hospitalized with community-acquired pneumonia. METHODS: This was a target trial emulation including moderately immunocompromised (asplenia, hematologic malignancies, solid organ malignancy receiving chemotherapy, kidney tr

Newborn Respiratory Distress: Evaluation and Management.

Newborn respiratory distress is one of the most common reasons for neonatal intensive care admission at birth. If not diagnosed and managed appropriately, newborn respiratory distress can progress to cardiopulmonary collapse and death. Presenting symptoms of respiratory distress consist of increased work of breathing (eg, grunting, nasal flaring, cyanosis, retractions, respiratory rate greater than 60 breaths/min [tachypnea]). The most common cause of respiratory distress at term is transient tachypnea of the newborn. Other causes include respiratory distress syndrome, meconium aspiration synd

Burden of Infections in Early Life and Risk of Infections and Systemic Antibiotics Use in Childhood.

IMPORTANCE: A high infection burden in early childhood is common and a risk factor for later disease development. However, longitudinal birth cohort studies investigating early-life infection burden and later risk of infection and antibiotic episodes are lacking. OBJECTIVE: To investigate whether early-life infection burden is associated with a later risk of infection and systemic antibiotic treatment episodes in childhood. DESIGN, SETTING, AND PARTICIPANTS: This longitudinal cohort study of children from birth to age 10 or 13 years included data from the Danish population-based Copenhagen P

Granulicatella adiacens infections in children: a single-center retrospective study.

PURPOSE: Granulicatella spp. although rare, are notable pathogens in various infections, particularly in bacteraemia and infective endocarditis. Because of their unique growth requirements and difficulties in antimicrobial testing, identifying and treating these microorganisms poses significant challenges. METHODS: This retrospective study was conducted at Dr. Sami Ulus Maternity and Children's Health and Diseases Research and Education Hospital, a tertiary care centre in Ankara, Turkey. Blood cultures were screened for Granulicatella spp. between January 2005 and January 2017. Clinical and l

A case of early-onset congenital syphilitic osteomyelitis of the calcaneus and literature review.

BACKGROUND: Congenital syphilis (CS) is a sexually transmitted disease caused by Treponema pallidum (TP). When the skeletal system is involved, it often results in multiple, symmetrical bone destruction at the epiphyses of long tubular bones such as the humerus and radius, rarely involving the calcaneus. This article reports a case of calcaneal osteomyelitis caused by TP in a child with no other bone damage and subtle clinical manifestations, No similar cases have been reported. Congenital brucellosis associated with subsequent Klebsiella pneumoniae co-infection in a premature neonate: A rare case report.

We report a case of congenital brucellosis subsequently associated with Klebsiella pneumoniae infection in a Saudi preterm neonate. A girl born with severe respiratory distress was admitted to a neonatal intensive care unit. Laboratory examinations revealed thrombocytopenia and slight leukocytosis. Her mother was a confirmed case of brucellosis. Initial blood culture confirmed the diagnosis of infection, and the baby was treated empirically with rifampicin, gentamicin, and ciprofloxacin. Follow-up revealed that her general condition was gradually improved. On day 27, the baby deteriorated, sho

Ureaplasma species and preterm birth: current perspectives.

Preterm birth is the leading cause of neonatal morbidity and mortality worldwide, and the human Ureaplasma species are most frequently isolated from the amniotic fluid and placenta in these cases. Ureaplasma colonisation is associated with infertility, stillbirth, histologic chorioamnionitis, and neonatal morbidities, including congenital pneumonia, bronchopulmonary dysplasia, meningitis and perinatal death. The human Ureaplasma spp. are separated into Ureaplasma urealyticum and Ureaplasma parvum with 14 known serotypes. The small genome has several genes, which code for surface proteins; most

Oral management of a patient with down syndrome and agammaglobulinemia: a case report.

BACKGROUND: Down syndrome is characterized by a variety of dysmorphic features and congenital malformations, such as congenital heart disease, gastrointestinal disease, and other conditions like leukemia and autoimmune disorders. Patients with Down syndrome are highly prone to respiratory tract infections, which might be fatal to them. However, there are only few available data on patients diagnosed with Down syndrome and agammaglobulinemia. In this report, we describe a case of successful prevention of post-dental treatment complications (e.g., pneumonia and other bacterial infections) in a p

Infant respiratory syncytial virus prophylaxis and nasopharyngeal microbiota until 6 years of life: a subanalysis of the MAKI randomised controlled trial.

BACKGROUND: Respiratory syncytial virus (RSV) infection during infancy is suggested to cause long-term wheeze. In turn, wheeze has been associated with bacterial dysbiosis of the respiratory tract. We investigated the effects of RSV prophylaxis with palivizumab in otherwise healthy preterm infants on respiratory microbiota composition at 1 year and 6 years of age. METHODS: In a multicentre, single-blind, randomised, placebo-controlled trial (the MAKI trial), infants born between 32-35 weeks of gestation, in one university and in 15 regional hospitals in the the Netherlands, were randomly assi

Off-Label Use of Sirolimus and Everolimus in a Pediatric Center: A Case Series and Review of the Literature.

BACKGROUND: It has been 15 years since sirolimus, an mTOR inhibitor, received Food and Drug Administration approval to prevent acute rejection in kidney transplantation, and 8 years since its analog everolimus acquired the same status. Since then, these drugs have become more and more utilized and their immunosuppressive and antiproliferative properties have been tested in a great variety of clinical conditions, often achieving excellent results. Despite such positive evidence, the on-label indications for these rapalogs are still very restrictive, especially in children. AIMS: The aims of th

Minimization of Surgical Site Infections in Patients With Delayed Sternal Closure After Pediatric Cardiac Surgery.

BACKGROUND: Delayed sternal closure (DSC) following pediatric cardiac surgery is commonly implemented at many centers. Infectious complications occur in 18.7% of these patients based on recent multicenter data. We aimed to describe our experience with DSC, hypothesizing that our practices surrounding the implementation and maintenance of the open sternum during DSC minimize the risk of infectious complications. METHODS: We reviewed patients less than 365 days who underwent DSC between 2012 and 2016 at our institution. Infectious complications as defined by the Society of Thoracic Surgeons Con

Diagnosis of Fraser syndrome missed out until the age of six months old in a low-resource setting: a case report.

BACKGROUND: Fraser syndrome is a rare genetic disorder that often presents with ocular, renal, genital and limb's congenital anomalies. The prognosis of this genetic disorder depends on the severity of the combination of congenital malformations, some of which may be fatal. The diagnosis of Fraser syndrome is based on established clinical criteria and genetic tests. The criteria enabling clinical diagnosis are visible dysmorphic features present at birth, hence, Fraser syndrome can easily diagnosed at birth, except when health professionals are inexperienced in clinical recognition. Herein, we
