Epidemiology of nosocomial pneumonia in infants after cardiac surgery.

BACKGROUND: The pattern of nosocomial pneumonia (NP) in infants in a pediatric surgical ICU after cardiac surgery may differ from that seen in adult ICUs. STUDY OBJECTIVES: The primary aim of this study was to describe the epidemiology of NP in infants after cardiac surgery and, secondarily, to describe the changes of the distribution and antibiotic resistance of the pathogen during the last 3 years. METHODS: Data were collected between June 1999 and June 2002 from 311 consecutive infants who underwent open-heart surgery in our hospital. We retrospectively analyzed the distribution and antib

Postmortem findings in the Coffin-Lowry Syndrome.

The Coffin-Lowry Syndrome (CLS) is a congenital disorder that can be recognized by retarded growth and development, the characteristic appearance of the face and hands, and often by the typical deformities of the back and chest; there are many other anomalies. The history of the syndrome is reviewed, noting the x-linked semidominant pattern of inheritance, and two autopsies are presented and compared with the three autopsy reports that have been published previously. The five young patients died at ages between 18 to 28 years of advancing pneumonia, aspiration of food into the trachea, or post

Chronic traumatic and congenital diaphragmatic hernias: presentation and surgical management.

BACKGROUND: The diagnosis of chronic diaphragmatic hernias, whether due to congenital defects or trauma, may be difficult to make and may rely on clinical suspicion in the setting of persistent nondiagnostic radiographic findings. Repair is indicated to avoid catastrophic cardiopulmonary compromise and/or incarceration of abdominal organs. STUDY OBJECTIVES: To review the varied presentations and treatment of chronic diaphragmatic hernia. DESIGN: Retrospective review. SETTING: University of Washington and Harborview Medical Center, Seattle, Washington. PATIENTS: Between 1997 and 2001, nine

[Clinical relevance of the Streptococcus milleri group in head and neck infections].

Streptococcus constellatus, S. intermedius, and S. anginosus, the 3 species of the S. milleri group, form part of the normal flora commonly found in the mouth, throat, and gastrointestinal and genital tracts. This group has become known as an important pathogen in infections and abscesses, but data on the anatomical distribution of these species is lacking in relation to clinical significance. We obtained 275 strains of the S. milleri group from different departments at our hospital over the last 3 years, including 54 strains from dental surgery, 47 from internal medicine, 44 from otolaryngolo

[Aortic root abscess without involvement of the aortic valve: diagnosis and therapy in a 2.5-year-old child].

Although formation of an aortic root abscess is a frequent complication of aortic valve endocarditis in adults, this complication has been rarely observed in children. In the majority of cases it has been described in children without underlying congenital heart disease. Due to the rarity of this complication, diagnosis and treatment is frequently delayed in childhood. We report a 2 1/2 year old girl who developed pericardial effusion in the course of pneumonia. Echocardiographic examinations, which were performed because of the pericardial effusion, revealed after 6 days the development of a

A hospital outbreak of extended-spectrum beta-lactamase-producing Klebsiella pneumoniae investigated by RAPD typing and analysis of the genetics and mechanisms of resistance.

Between July and September 1997 a ceftazidime- and aminoglycoside-resistant strain of Klebsiella pneumoniae infected or colonized seven patients on three paediatric wards at Guy's Hospital in London. The patients were mostly neonates or infants recovering from cardiac surgery for congenital defects. The organism was probably introduced by an asymptomatic patient from Greece and the subsequent outbreak could largely be explained by person-to-person spread on individual wards and frequent transfers of patients between wards. The outbreak was controlled by patient isolation and attention to handw

Disseminated Penicillium marneffei infection in human immunodeficiency virus-infected children.

Disseminated infection with the fungus Penicillium marneffei is one of the most common opportunistic infections in human immunodeficiency virus (HIV) disease in northern Thailand. We report the clinical, laboratory and therapeutic features of 21 human immunodeficiency virus-infected children with disseminated P. marneffei who were prospectively followed. Significant clinical and laboratory features included generalized lymphadenopathy (90%), hepatomegaly (90%), body temperature > 38.5 degrees C (81%), papular skin lesions with central umbilication (67%), splenomegaly (67%), failure to thrive (

Recurrent pneumococcal meningitis. Search for occult skull fracture.

Skull fractures, particularly those affecting the base of the brain and extending to the sinuses and petrous pyramids, are the most common cause of recurrent bacterial meningitis. Detection of sinus fracture can be difficult in children. Thin-section computed tomographic scanning aids the diagnosis in patients with a remote history of head injury. Other causes of recurrent bacterial meningitis include congenital cranial and spinal cord defects, foci of infection, and immune system disorders. Streptococcus pneumoniae is often the infectious organism. Treatment includes surgical repair of any an

Invasive bacterial infections in children born to women infected with human immunodeficiency virus type 1.

To determine the rates and characteristics of invasive bacterial infections in children infected with the human immunodeficiency virus type 1 (HIV-1), we conducted a prospective, longitudinal, observational cohort study of infants born to HIV-1-infected mothers between Dec. 1, 1985, and Sept. 30, 1989. Of 104 subjects whose HIV-1 infection status could be definitively determined, 21 were infected with HIV-1 and 83 were not. In all, 11 (48%) of 23 invasive infections occurred among 10 HIV-1-infected patients and 12 (52%) of 23 occurred among 11 uninfected subjects. Infections with Streptococcus

Ureaplasma urealyticum intrauterine infection: role in prematurity and disease in newborns.

Ureaplasma urealyticum, a common commensal of the urogenital tract of sexually mature humans, is gaining recognition as an important opportunistic pathogen during pregnancy. While its etiologic significance in many aspects of adverse pregnancy remains controversial, recent evidence indicates that U. urealyticum in the absence of other organisms is a cause of chorioamnionitis. Furthermore, ureaplasmal infection of the chorioamnion is significantly associated with premature spontaneous labor and delivery. In at least some cases, it appears to be causal. Present evidence indicates that U. urealyt

Unique susceptibility of patients with antibody deficiency to mycoplasma infection.

Patients with congenital or acquired disorders of antibody production suffer from a wide variety of infections. They are most often bacterial and due to Haemophilus influenzae or Streptococcus pneumoniae. Chronic pulmonary disease accounts for most of the deaths. While non-urogenital tract infections due to Ureaplasma urealyticum or other mycoplasmas are unusual in individuals with normal resistance, patients with antibody deficiency demonstrate a unique susceptibility. With increasing frequency, patients with impaired humoral immunity have been shown to have a mycoplasmal infection that resul

The paper reports on a theme of important pediatric interest, pneumonia with Pneumocystis carinii (PC), a more widespread affection lately, due to the progress in pediatric reanimation, on the one hand (that allowed the survival for a long time of several biologically handicapped infants) and as a consequence of the more and more common use of immunosuppressing therapy, on the other hand (malignant diseases, transplant of organs, etc.); another important role in the increase of the PC pneumonia is played, recently, both in the adult and the child, by the infection with HIV (acquired immunodefi

Vaccine-preventable respiratory infections in childhood.

In the past decade, immunization rates among preschool-age children in the United States have decreased to levels lower than those in many developing countries. As a result, epidemics of vaccine-preventable diseases have occurred, especially in urban areas. Six of the infections prevented by immunization--those caused by Bordetella pertussis, Streptococcus pneumoniae, Haemophilus influenzae type B, Corynebacterium diphtheriae, measles virus, and influenza virus--frequently cause respiratory tract disease. Pneumonia in children may have subtle presentations and require special considerations de

Increased risk of neisserial infections in systemic lupus erythematosus.

Survival in systemic lupus erythamatosus (SLE) continues to improve because of better ancillary care, earlier diagnosis, and earlier treatment. However, infection remains a leading cause of morbidity and mortality in this disease. Although corticosteroids and immunosuppresives increase the risk of opportunistic infection, the SLE patient is still most at risk from common bacterial pathogens. As the prototypic immune-complex disease, patients with active SLE have low circulating complement as well as a reticuloendothelial system (RES) saturated with immune complexes. It seems intuitive that SLE

Systemic fungal infections in south Indian infants.

Systemic fungal infections were identified in 13 of 1468 necropsies of infants dying in the 1st year of life in a south Indian hospital. Candidiasis was present in 11 infants, most often as pneumonia or enteritis. Spread to other organs occurred in four cases. Intestinal candidiasis was associated with zygomycosis of the intestine in one infant and with aspergillosis of the lungs and intestine in another. One case of aspergillosis of the myocardium and one of gastric zygomycosis were also diagnosed. Septicaemia, broad spectrum antibiotic therapy, surgery and congenital abnormalities, including

Occurrence of neonatal and postnatal mortality in range beef cattle. II. Factors contributing to calf death.

Results are summarized of necropsy findings from 798 calves lost from birth to weaning over a 15-yr period. Autopsies determined cause of death and ascertained anatomical normalcy of the skeletal, musculature and organ systems and functinal status of the lungs. Of the 798 calves, 77.7% were anatomically normal and 22.3% were abnormal (P < 0.01) and 75.0% of the total deaths of abnormal calves occurred by Day 2 postpartum. Birth weights of normal calves averaged 4.2 kg heavier (P < 0.05) than that of abnormal calves. Internal hydrocephalus was identified and confirmed heritable as a lethal rece

Varicella zoster and herpes simplex virus pneumonias.

Varicella zoster (VZV) and herpes simplex (HSV) viruses commonly cause self-limited infection of the skin and mucous membranes. However, certain groups of subjects, including neonates, cancer patients, organ and bone marrow transplant recipients and those with congenital or acquired deficiencies of cell mediated immunity, are at increased risk for dissemination of either virus to the lungs and/or other viscera. The highest risk for VZV pneumonitis is in bone marrow transplant recipients, 44%, and in children with acute leukemia, 32%. The mortality from this complication of VZV infection in the

Prevention of central nervous system infections in patients at risk.

Infections of the central nervous system in patients at risk are relatively uncommon when compared with pneumonia, bacteremia, and soft tissue infection. However, they carry serious morbidity and are frequently fatal. Each of the diverse conditions that can place a patient at risk for central nervous system infection is associated with a fairly predictable spectrum of etiologic organisms. Various forms of trauma (including blunt and penetrating injuries and neurosurgery, especially when a cerebrospinal fluid shunt is implanted) predispose to infection with common pathogenic bacteria. Defects o

[Role of Mycoplasma infection in pediatric pathology].

The review presents data from the literature showing the variability of manifestations of mycoplasmal infection (M. pneumoniae, M. hominis, T-strain) in childhood. The etiological role of M. pneumoniae (as a monoantigen and in association with respiratory viruses) in acute respiratory infections and pneumonias has been established. M. pneumoniae also plays a certain role in the development of attacks of bronchial asthma and chronic recurrent pneumonias. M. hominis, being an urogenital mycoplasma, may also be the causative agent of acute respiratory infections and pneumonias. A high rate of inf

Generalized cytomegalic inclusion disease in neonates and infants.

Cytomegalic inclusion disease (C.M.I.) is caused by the salivary gland virus which is species-specific, and characterized by formation of inclusion-bearing cells in various organs. From the view point of spreading, it is divided into the localized and generalized type. We studied 18 generalized cases of infants in whom inclusion-bearing cells were detected in more than three organs. As to the portal of entry and the time of infection, cytomegalic inclusion disease can be classified into three subgroups in infants; congenital form (2 cases), early acquired form (8 cases) and acquired form with
