[Disseminated aspergillosis in acute leukemia probably chemically-induced].

One case of invasive aspergillosis in patient with myeloblastic leukemia probably induced by chimiotherapy is reported. Aspergillosis is a severe infection in immunodepressed host with 70 to 80% of mortality. Early diagnosis can be often carried by broncho-alveolar lavage, route of entry is always pulmonary one. Rapidly established antimycotic treatment can avoid death. Laminar air flow rooms and air spores numeration allow prevention of this infection in the immunodepressed host.

Cutaneous and mucosal manifestations of the deep mycotic infections.

The deep mycoses are increasing in importance both as opportunistic infections and from exposure in geographically defined areas. Diagnosis may be difficult in both groups. Mucosal involvement may be non-specific (e.g., in disseminated candidiasis) or highly predictive of disseminated disease (e.g., histoplasmosis, blastomycosis and paracoccidioidomycosis). Skin involvement is generally uncommon in disseminated aspergillosis, mucormycosis and cryptococcosis but is more common in candidemia and coccidioidomycosis. Manifestations of mucosal and cutaneous lesions of the deep mycoses are reviewed

Disseminated aspergillosis in dogs.

Clinical and pathological findings are reported from a series of 12 cases of disseminated aspergillosis (A. terreus) in 11 German Shepherd dogs and one Dalmatian referred to Murdoch University Veterinary Hospital (MUVH) over the period 1980 to 1984. A preliminary study of humoral and cell mediated immune components and complement levels revealed no consistent abnormality in 9 dogs tested apart from raised IgG levels. Serum IgA levels were depressed in 30% of cases. Serial data from one extensively monitored case is presented. The unusual epidemiological and pathogenetic features of the disease

Chronic granulomatous disease in a 23-year-old female.

A 23-year-old girl with clinical and laboratory findings characteristic of chronic granulomatous disease is described. The patient was admitted to hospital because of severe disseminated aspergillosis. Studies of neutrophils showed normal phagocytosis but impaired microbicidal killing and a failure of the respiratory burst activity as measured by NBT-reduction, superoxide generation, chemiluminescence and antibody-dependent cell mediated cytotoxicity. Patient's neutrophils had normal chemotactic responsiveness but a marked impairment in the level of serum chemotactic activity was observed. Neu

Aspergillus deflectus infection in four dogs.

Four cases of disseminated aspergillosis caused by Aspergillus deflectus in German Shepherds are presented. Three of the cases, which involved multiple organs, terminated in euthanasia. One case, with bony involvement of the limbs and skull, lived. The unique morphological characteristic of the conidial head resembling a briar pipe led to the identification of A. deflectus. To the authors' knowledge these are the first reported cases of infections caused by A. deflectus in man or animal.

Detection of fungal antigen in body fluids for diagnosis of invasive aspergillosis.

In clinical trials, the diagnostic utility of a radioimmunoassay (RIA) to detect Aspergillus antigen was demonstrated in sera obtained from patients with invasive pulmonary, endothelial, and disseminated aspergillosis, in cerebrospinal fluids from patients with Aspergillus meningoencephalitis, and in bronchoalveolar lavage fluids from patients with invasive pulmonary aspergillosis. The RIA was further evaluated in three blinded, controlled clinical trials. In one, sera were collected prospectively from patients with acute leukemia. Antigenemia was detected in four patients with invasive pulmon

Aleukemic leukemia cutis.

A 46-year-old man presented with nodular skin lesions, a biopsy specimen of which demonstrated a poorly differentiated malignancy. Touch preparations with histochemical staining and electron microscopy confirmed leukemia cutis. Results from a bone marrow aspirate disclosed focal areas of increased myeloblasts, and cytogenetic analysis revealed an abnormal karyotype as follows 46,XY, t(1;2) (q44p13). Antileukemic therapy resulted in prompt disappearance of the skin nodules, and a return of the patient's bone marrow to normal was noted, but after six months of intensive chemotherapy leukemia cut

[Aspergillosis with obstruction of the central airways].

Two cases are reported of obstruction of central airways by plugs containing Aspergillus fumigatus. In one case the obstruction was located in the right stem bronchus and, despite treatment with amphotericin B, the immunocompromised patient died from invasive disseminated aspergillosis. In the other case the obstruction was in the trachea and, since the immune function of the patient was normal, she responded favorably to treatment by amphotericin B.

[Right endocarditis in disseminated aspergillosis].

A 53 year old man with an anaplastic bronchial carcinoma was hospitalised for septic shock and acute respiratory distress after a cutaneous, probably staphylococcal infection, and died in spite of anti-staphylococcal antibiotherapy. The autopsy showed pulmonary, cardiac, cerebral and renal aspergillosis. A right heart aspergillous endocarditis, very rare in this pathology, was also discovered but there were no cardiac valves lesions. The patient was in an "immunodepressed" state as usually observed in pulmonary aspergillosis. The endocardial localisation of aspergillosis and the "pseudo-miliar

[Aspergillosis with involvement of the central nervous system].

Clinical and pathological findings in a patient with central nervous system lesions due to a disseminated aspergillosis are reported. Symptoms and signs were suggestive of a meningoencephalitis with onset two years after nephrectomy for cancer and four months after a corticotherapy was started for post-radiation digestive disorders. A review of the literature shows that C.N.S. is rarely involved in this disease, that the diagnosis is difficult in immunodepressed patients and that the prognosis is poor in the absence of a specific treatment.

Primary cutaneous aspergillosis in six leukemic children.

We report a cluster of primary cutaneous aspergillosis in six children with hematologic malignancy. When first seen, they had hemorrhagic bullae caused by Aspergillus flavus, Aspergillus fumigatus, and Aspergillus niger at the sites of insertion of intravenous cannulas or where arm boards had been taped to the extremities. Rapid diagnosis of cutaneous aspergillosis was made by direct examination of the blister roof with potassium hydroxide before it progressed to a necrotic ulcer. Intravenous amphotericin was instituted promptly in five of six patients, and none died of disseminated aspergillo

Disseminated aspergillosis and pacemaker endocarditis.

Clinical and pathologic findings in a 65-year old woman with fever of unknown origin are described in this report. Generalized aspergillosis with endocarditis was demonstrated at autopsy. The patient had no recognized risk factors for the development of fungal infection. A functional transvenous pacemaker lead, inserted 2 years previously, was completely encased in a large infected thrombus and may have been the initial site of infection. Septicemia and endocarditis are rare but well-described complications of cardiac pacing, and should be considered in the differential diagnosis of fever of u

Disseminated aspergillosis following radiation therapy for medulloblastoma.

A 16 year old girl who underwent surgery and radiotherapy for medulloblastoma is described. Shortly following the completion of treatment symptoms developed suggestive of tumour recurrence. At post mortem the cause was identified as disseminated aspergillosis with extensive cerebral involvement. The importance of this unusual complication is discussed.

The effect of ketoconazole on amphotericin B in a model of disseminated aspergillosis.

The potential of ketoconazole prophylaxis to antagonize the activity of amphotericin B against aspergilli was investigated in vitro and in neutropenic mice. Exposure of Aspergillus fumigatus (six strains) and of Aspergillus flavus or Aspergillus niger to ketoconazole resulted in a uniform increase of the minimal fungicidal activity of amphotericin B, from 0.15-0.63 mg/liter to greater than 2.5 mg/liter in a microwell assay. To test the relevance of this antagonism in vivo, we challenged neutropenic mice iv with a lethal dose of conidia from two strains of A. fumigatus and then treated the mice

Comparison of serial diagnostic tests in a patient with invasive aspergillosis.

Studies on a liver transplant recipient with fatal disseminated aspergillosis are described. The concentration of IgG antibodies to Aspergillus rose sharply with the onset of fever and changes in the chest X-ray, reaching a peak on day 10. Thereafter, antibody concentrations fell and were within the normal range by day 21, when Aspergillus was first isolated from an endotracheal aspirate and one day before death. The fall in antibodies preceded a rise in circulating immune complexes but Aspergillus antigens were not detected in the serum. Serial quantitative assay for antibodies to Aspergillus

[Toxic epidermal necrolysis (Lyell's syndrome) caused by piroxicam, with fatal outcome from disseminated aspergillosis].

A case of Lyell Syndrome provoked by ingestion of an antirheumatic drug (Piroxicam) is commented. The evolution under treatment with steroids and broad spectrum antibiotics was excellent but once recovered from this, the patient died subsequently to a systemic infectious process caused by an Aspergillosis with pulmonary involvement and secondary affectation of the kidneys, brain liver, and heart. The hepatic involvement during the acute stage is discussed and evaluated (increase of transaminases), amylasemia, amylasuria, anemia and thrombopenia which persisted throughout the process and presum
