Extracorporeal Membrane Oxygenation-Supported Aspiration Thrombectomy and Catheter-Directed Thrombolysis for Massive Pulmonary Artery and Inferior Vena Cava Thrombosis in a Neonate.

Massive pulmonary artery thrombosis in neonates is rare and frequently fatal when associated with right ventricular failure and hemodynamic collapse. We report a premature neonate with extensive inferior vena cava, right ventricular outflow tract, and bilateral pulmonary artery thrombosis resulting in respiratory failure requiring venoarterial extracorporeal membrane oxygenation. Progressive thrombus burden despite systemic anticoagulation prompted catheter-directed mechanical aspiration thrombectomy using escalating aspiration systems with adjunctive localized thrombolysis. Significant resolu

Nine years' experience of trial of labor after two previous cesarean sections at a tertiary hospital - A retrospective cohort study.

INTRODUCTION: The global cesarean section (CS) rate has increased to 21.1% between 1990 and 2018. In Singapore, the annual CS rate has increased by ~ 0.6%, reaching an overall rate of 37.4%. Almost one-third of all cesarean deliveries are performed for at least one previous CS. Women with two CSs are often denied a "trial of vaginal delivery" due to the increased risk of uterine scar rupture. This study aims to examine maternal and neonatal outcomes of women undergoing trials of labor following two CSs (TOLAC-2). MATERIAL AND METHODS: We conducted a retrospective observational study of women

Azithromycin therapy for prevention of chronic lung disease of prematurity (AZTEC): a randomised placebo-controlled trial.

BACKGROUND: Systematic reviews have reported conflicting evidence to confirm if macrolides reduce rates of chronic lung disease of prematurity in at-risk preterm-born infants, including in those colonised with pulmonary Ureaplasma spp. Since an adequately powered trial has been lacking, we conducted a double-blind, randomised, placebo-controlled trial to assess if the macrolide azithromycin improved survival without the development of physiologically defined moderate or severe chronic lung disease of prematurity in infants born at < 30 weeks' gestation. METHODS: Infants recruited from 30 neon

A Case of Severe Neonatal Neutropenia Secondary to Late-Onset Sepsis From E. coli Omphalitis.

Severe neonatal neutropenia, defined as an absolute neutrophil count (ANC) of <500/µL, is a critical hematologic finding that signals either a severe consumptive process or a primary bone marrow disorder. We present the case of an eight-day-old, full-term female infant who presented with fever and irritability, later diagnosed with severe neutropenia (ANC: 400/µL) and elevated C-reactive protein (CRP) (150 mg/L). Omphalitis was identified as the source, with pus culture growing pan-sensitive Escherichia coli, confirming late-onset neonatal sepsis (LONS). The patient was managed with targeted i

A Neonatal Transfer from Birth Center to Hospital with a Postpartum Hemorrhage During Transport.

There is increasing interest from women and birthing people for midwifery-led care with community birth options in birth centers and for home birth. Birth centers are a high-value and high-quality option for place of birth and also address the increasing need to provide access to perinatal care for women in maternity care deserts. This case report reviews the transfer of a postpartum patient and her newborn from an accredited, freestanding birth center to the designated hospital. The reasons for transfer were neonatal respiratory distress with hypoxemia and an emergent postpartum hemorrhage th

Intrauterine interventions for severe fetal anemia due to parvovirus B19 national outbreak.

OBJECTIVE: Parvovirus B19 infection during pregnancy can cause severe fetal anemia, hydrops fetalis, and fetal death. This study describes our experience managing ten cases of severe fetal anemia due to parvovirus B19 infection during the 2023-2024 national outbreak in Israel. METHODS: This single-center descriptive cohort study included ten singleton pregnancies with fetal anemia (MCA-PSV >1.5 MoM) requiring intrauterine transfusion. Parvovirus B19 infection was confirmed by maternal IgM serology and/or amniotic fluid PCR. For pregnancies <20 weeks of gestation, we implemented a modified int

Multidrug-Resistant and Carbapenemase-Producing Gram-Negative Bacteria among Intensive Care Unit Patients in Amhara National Regional State, Ethiopia.

Antimicrobials are extensively used in patients admitted to intensive care units (ICUs). As a result, the ICU has become a hot spot for multidrug-resistant (MDR) and carbapenemase-producing (CP) Gram-negative bacteria (GNB) infection in hospitals. Klebsiella pneumoniae, Escherichia coli, Pseudomonas aeruginosa, and Acinetobacter baumannii are critical priority GNB causing serious infections in ICUs. Therefore, this study aimed to assess the prevalence, antibiotic resistance profile, and associated factors of critical GNB among ICU patients in Amhara National Regional State, Ethiopia. A hospita

Prospective Comparative Study on the Relationship Between Adverse Maternal and Fetal Outcomes and the Total Number of Antenatal Visits: Evaluating the Focused Antenatal Care Model and Assessment of Prenatal Care Utilization Index.

OBJECTIVES: Antenatal care (ANC) visits during pregnancy aim to identify and address factors associated with unsafe pregnancy while educating pregnant women on the nuances of pregnancy and motherhood. The recommended number of visits varies across different countries. We compared the adverse maternal and fetal outcomes in participants with adequate ANC visits to the Focused ANC (FANC) model by the World Health Organization with those in the adequate and adequate plus categories of the Adequacy of the Prenatal Care Utilization (APNCU) index, to evaluate if the number of visits had any influence

Proinflammatory monocyte-derived granulocyte-macrophage colony-stimulating factor fuels airway inflammation in bronchopulmonary dysplasia.

BACKGROUND: Bronchopulmonary dysplasia (BPD) remains a leading cause of morbidity and long-term respiratory complications in preterm neonates. Effective strategies for moderate-severe BPD are currently lacking. METHODS: Tracheal aspirate (TA) samples were collected from 62 preterm neonates, and the immune cell composition was analyzed using multicolor flow cytometry. Subsequently, CD14+ monocytes were isolated and purified from the TA samples for transcriptomic sequencing analysis. Cytokine arrays and enzyme-linked immunosorbent assays were utilized in the cytokine level detection from TA sam

Dose-dependent effects of Alveofact with and without steroids on neutrophil extracellular traps in neonatal respiratory distress syndrome.

BACKGROUND: Neutrophil extracellular traps (NETs), peptidyl arginine deiminase 4 (PAD4), and oxidative stress play central roles in neonatal respiratory distress syndrome (RDS). This study investigated whether exogenous surfactant (Alveofact) combined with intratracheal budesonide modulates these pathways. METHODS: Sixty preterm neonates with RDS were randomized into four groups to receive Alveofact 50 mg/kg or 100 mg/kg, with or without budesonide 0.25 mg/kg. Tracheal aspirates (TA) and blood samples were obtained at baseline and 24 h post-treatment, and an additional blood sample was obtain

Baseline immune status associates respiratory viral infection in preterm infants: A prospective cohort study.

Preterm infants (PI) are particularly vulnerable to respiratory viral infections (RVI), which may affect their lung development and immune responses, increasing the risk of recurrent wheezing (RW) and asthma. This study aimed to evaluate the cytokine immune profile in PI admitted to neonatal intensive care units (NICUs) and to explore its association with RVI and subsequent respiratory outcomes during the first year of life. A prospective cohort of 118 PI (<32 weeks gestational age) was studied in 2 NICUs. Nasopharyngeal aspirates were collected at birth and discharge for virological and immun

Implementation of a Prospective Birth Cohort for Newborn Screening and Early Linkage to Comprehensive Sickle Cell Disease Care in a Low-Resource Setting.

In sub-Saharan Africa, where approximately 75% of newborns with sickle cell disease (SCD) are born, under-five mortality remains high, partly due to the absence of newborn screening (NBS) and delayed linkage to comprehensive care. We conducted a prospective, quasi-experimental study involving two sequential newborn screening cohorts at Aminu Kano Teaching Hospital (AKTH), Kano, Nigeria (December 2022-December 2025), to evaluate the feasibility of integrating newborn screening (NBS) with early comprehensive SCD care and to identify barriers to enrollment before 3 months of age. Following an ini

Renal Immaturity, Comorbidities and Outcomes in Neonatal Acute Kidney Injury.

The kidneys of newborns are morphologically and physiologically different from those of children and adults. Embryonic kidney development begins at the 3rd week after conception and ends at 32-35 weeks prenatally, while in premature infants it continues for some time after birth. Neonatal kidney function is "immature", due to high vascular resistance and low renal blood flow. Therefore, neonatal kidneys are particularly vulnerable to the effects of hypoperfusion. Predisposing factors for the occurrence of acute kidney injury (AKI) in newborns are certain clinical conditions such as prematurity
