Early enzyme replacement therapy in late-onset Pompe disease diagnosed by newborn screening.

OBJECTIVE: Newborn screening (NBS) now allows for early detection and clinical monitoring in infants and children with late-onset Pompe disease (LOPD), potentially identifying those needing early enzyme replacement therapy (ERT). Early symptom onset has been reported in LOPD, but with a paucity of information on when to initiate ERT and on outcomes with early treatment. We report our experience at Duke University Medical Center with seven symptomatic children diagnosed with LOPD by NBS, compound heterozygous for the common IVS1 variant c.-32-13 T > G and a second pathogenic or likely pathogeni

Recessive loss of DIAPH1 function causes a progressive neurodevelopmental syndrome with variable immunological involvement.

PURPOSE: Biallelic DIAPH1 pathogenic variants cause a neurodevelopmental syndrome occasionally associated with immunodeficiency. This study aims to define the clinical and immunological spectrum of DIAPH1-related neuroimmunological syndrome and explore the gene's developmental role using vertebrate models. METHODS: A total of 53 individuals with biallelic DIAPH1 variants, including 33 previously unreported patients, were studied. Clinical features were analyzed, and functional studies were conducted using knockout models in Danio rerio and Xenopus tropicalis. RESULTS: Clinical features inclu

Immunization with mRNA-LNP Elicits De Novo IgG Responses in the Presence of Maternal Antibody.

BACKGROUND/OBJECTIVES: Maternal antibodies can inhibit vaccine-specific humoral responses in early life, leaving infants at increased risk for severe disease for vaccine-preventable infections. In the case of SARS-CoV-2, infants under the age of 3 months represented most child hospitalizations, yet there is no approved vaccine for children under the age of 6 months. There is a clear need for effective immunization in early life to prevent infant morbidity and mortality. Here, we established a mouse model to define how maternally derived antibodies shape early-life responses to mRNA vaccination

Fulminant Unilateral Pseudomonas aeruginosa Endogenous Endophthalmitis Secondary to Peripheral Venous Catheter-Related Bacteremia in a Full-Term Neonate: A Case Report.

Endogenous endophthalmitis is a rare but devastating complication of neonatal sepsis, predominantly reported in premature infants with very low birth weight. We report fulminant unilateral Pseudomonas aeruginosa endogenous endophthalmitis in a seven-day-old full-term male neonate (gestational age 39 weeks, birth weight 3,200 g) following delivery complicated by maternal chorioamnionitis and acute fetal distress. The infection originated from a peripheral intravenous catheter-related bloodstream infection, confirmed by triple-concordant cultures of blood, catheter tip, and vitreous aspirate. Op

Clinical characteristics, management strategies, and outcomes of pediatric immune thrombocytopenic purpura: a five-year single-center experience.

OBJECTIVE: Immune thrombocytopenia (ITP), which is caused by immune-mediated platelet loss, is the most common cause of acquired thrombocytopenia in pediatric patients. In addition to evaluating treatment approaches and outcomes within a single-center cohort, this study aimed to evaluate the clinical and laboratory characteristics of juvenile ITP. MATERIALS AND METHODS: Out of 296 patients with thrombocytopenia, 135 children (ages 0-18) with acute ITP were included in this retrospective analysis. We reviewed clinical observations, laboratory results, therapeutic modalities, length of hospital

Fulminant Hepatitis Due to Enterovirus E25 Systemic Infection in a Pediatric Patient.

Pediatric acute liver failure (PALF) is a rare but life-threatening condition characterized by rapid clinical deterioration and high mortality. Viral infections represent a major etiology of PALF, although the causative agent remains unidentified in a substantial proportion of cases. Human Enteroviruses (EVs) are typically associated with self-limiting illnesses; however, they may rarely cause severe systemic disease, including fulminant hepatitis, particularly in neonates and young children. We describe the case of a 4-year-old previously healthy male who presented with acute fulminant hepati

In silico insights into shared molecular mechanisms and drug repurposing for non‑small cell lung cancer and tuberculosis.

UNLABELLED: Universal trauma-informed screening is increasingly recommended across pediatric settings, yet it remains unclear which forms of adversity most accurately and equitably identify youth at risk for psychopathology. In response, the current study tested whether defining maltreatment by cumulative quality (i.e., number of subtypes) versus cumulative frequency (i.e., number of experiences regardless of subtypes), as well as potential differences within each subtype, yielded more accurate and fair predictions of trauma-related psychological distress. Participants included 839 adolescents

Procalcitonin for Predicting Sepsis in Infants in the Emergency Department: A Retrospective Study.

BACKGROUND: Among children younger than 5 years, infectious conditions account for the majority of pediatric emergency department visits. Due to nonspecific presentation, early identification of Sepsis and serious bacterial infection is challenging. Procalcitonin (PCT) level has been examined as a marker of sepsis. It has been shown to have advantages over other inflammatory markers. AIM: This study aimed to evaluate the association between high PCT levels and positive blood, urine, or cerebrospinal fluid (CSF) cultures in all infant patients suspected of having sepsis/Serious bacterial infec

Misdiagnosis of bronchiolitis and misuse of antibiotics - experience from the largest paediatric hospital of Bangladesh.

BACKGROUND: Bronchiolitis remains the commonest acute lower respiratory tract infection in younger children globally. Bronchiolitis is often misdiagnosed as World Health Organization (WHO)-classified pneumonia due to closely related clinical manifestations. Our primary objective was to assess the prevalence of viral bronchiolitis among WHO-classified pneumonia, and the secondary objective was to evaluate their management. METHODS: This was a prospective observational study. Infants (aged <2 years) suffering from WHO-classified pneumonia attending the country's largest academic paediatric hosp

Neonatal Suppurative Sialadenitis With Bilateral Gland Involvement in a Preterm Infant: A Rare Complication of Bacteraemia.

INTRODUCTION: Neonatal suppurative sialadenitis (NSS) is an uncommon but serious complication of salivary gland infection, with few cases reported in literature, most often affecting preterm infants. Typical cases involve the parotid gland and are caused by Staphylococcus aureus infection. Contributing factors include prematurity, dehydration, prolonged hospitalisation, and immature immunity. Sequential bilateral gland involvement through haematogenous spread is rare, and presents significant management challenges in prematurity. PRESENTATION: We report the case of a preterm infant, born at 3

DNA injection through coleoptile; a choice to overcome species barrier and bring about rapid expansion of gene pool in rice.

INTRODUCTION: Femoral neck fractures are among the most common injuries in the elderly, and total hip arthroplasty is increasingly preferred due to superior functional outcomes and lower reoperation rates; however, instability and dislocation remain major concerns with conventional fixed-bearing total hip arthroplasty. Dual mobility total hip arthroplasty has been introduced to reduce these complications. METHODS: This retrospective analytical study included 335 patients aged over 55 years with AO/OTA type 31B or 31C femoral neck fractures who underwent total hip arthroplasty between January

SUCCESSFUL PREGNANCY AND TERM DELIVERY AFTER RADICAL SURGERY FOR COLON CANCER: A CASE REPORT.

BACKGROUND: Colorectal cancer (CRC) is one of the most common malignancies worldwide, but pregnancy following curative resection for CRC is rarely reported. Concerns about cancer recurrence, hormonal influences, and obstetrical risks often lead to hesitation in counseling young cancer survivors who desire pregnancy. CASE PRESENTATION: We report a 40-year-old woman who presented with obstructive colon cancer at age 37 while 7 weeks pregnant with triplets. Due to the patient's critical condition, left hemicolectomy was performed on July 10, 2022, followed by vacuum aspiration of the uterine cav

States, humanitarian advocacy, and crisis: persuasion, mobilization, and denunciation.

BACKGROUND: Allergic, especially anaphylactic, reactions during immunoglobulin replacement therapy are rare, but their pathophysiology and classification remain ambiguous. Recent findings show positive results of skin tests with commercially available immunoglobulins, but target antigens and responsible compounds of the tested immunoglobulins have not been strictly identified. CASE DESCRIPTION AND FINDINGS: Four adult patients with recently diagnosed common variable immunodeficiency qualified for standard subcutaneous immunoglobulin replacement therapy regimen. They had no history of receivin

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A Neonate Presenting With Large Abdominal Mass: A Rare Case of Congenital Leukemia.

Leukemia symptoms occurring in the first 4 weeks of infancy are known as congenital leukemia. We present a case of congenital leukemia in a full-term neonate manifesting at birth with a grossly distended abdomen due to a large abdominal mass. Ultrasonography of the abdomen showed a large abdominal mass originating from the liver. Congenital leukemia was suspected based on the very high total leukocyte counts and the presence of blast cells in the peripheral blood smear, which was confirmed by bone marrow aspiration. The bone marrow aspirate smear showed 42% blast cells. Due to overwhelming sep

Methylmalonic acidemia with recurrent hemophagocytic lymphohistiocytosis: a case report and review of the literature.

BACKGROUND: Methylmalonic acidemia is a rare autosomal recessive disorder of propionate catabolism characterized by the accumulation of propionic acid and methylmalonic acid caused by methylmalonyl-CoA mutase deficiency. Clinical presentations range from acute deterioration in the neonatal period to later onset with a heterogeneous clinical course. Metabolite accumulation results in systemic involvement, affecting the nervous, gastrointestinal, and renal system functions and causing cardiomyopathy. Bone marrow dysfunction manifesting as neutropenia and anemia is a common hematological finding.

Bronchopulmonary dysplasia: signatures of monocyte-macrophage reactivity and tolerance define novel placenta-lung endotypes.

BACKGROUND: Bronchopulmonary dysplasia (BPD) is a complex disease involving aberrant immune responses across the lifespan, but these mechanisms are challenging to follow in human infants. Leveraging novel Signatures of Macrophage Reactivity and Tolerance (SMaRT), we hypothesized that distinct profiles of immune cell polarization in blood and lung are associated with BPD. METHODS: Published transcriptomic datasets of cord blood-derived monocytes (CB-MNC), peripheral blood monocytes (PBMC) and tracheal aspirate-derived lung macrophages were linked to placental inflammatory (PID) and vascular (P

Comparison of gastric volumes using ultrasound in term pregnant women with and without gestational diabetes before elective cesarean delivery: an observational cohort study.

OBJECTIVE: This study aimed to evaluate the impact of gestational diabetes mellitus (GDM) on gastric volume and content using gastric ultrasonography in term pregnant women undergoing elective cesarean delivery. Given the increased aspiration risk in pregnant patients due to physiological changes, GDM-related gastric motility alterations may further complicate anesthetic management. METHODS: A prospective observational cohort study included 60 pregnant women scheduled for elective cesarean delivery: 30 diagnosed with GDM and 30 without GDM. Gastric ultrasonography was performed in both the su
