Disruptions during perinatal fetal lung development can lead to postnatal chronic lung diseases such as bronchopulmonary dysplasia (BPD). Along with decreased alveolar and pulmonary vascular growth, abnormal airway growth occurs in BPD. Previous studies in rats have shown that antenatal endotoxin (AN-ETX) mimicking maternal chorioamnionitis causes dysanapsis during the neonatal period. Whether dysanaptic growth alters long-term lung function remains unknown. We hypothesized that antenatal endotoxin causes persistent differences between airway and distal lung growth with age in experimental BPD

Wilson-Mikity syndrome (WMS) is a rare condition characterized by various respiratory and pulmonary abnormalities in neonates and infants. However, the diagnosis is based on the findings of physiological tests, such as respiratory function tests. Reports describing the histopathological features of WMS are limited. The patient was a 22-year-old woman with a history of WMS. She had been on a ventilator for the first three months of life due to pulmonary hypertension after early delivery at 24 weeks of gestation and required oxygen therapy until three years of age. One month before presenting at

Bronchopulmonary dysplasia (BPD) is a multifactorial disease occurring as a consequence of premature birth, as well as antenatal and postnatal injury to the developing lung. BPD morbidity and severity depend on a complex interplay between prenatal and postnatal inflammation, mechanical ventilation, and oxygen therapy as well as associated prematurity-related complications. These initial hits result in ill-explored aberrant immune and reparative response, activation of pro-fibrotic and anti-angiogenic factors, which further perpetuate the injury. Histologically, the disease presents primarily b

Congenital diaphragmatic hernia (CDH) survivors are at risk of developing significant chronic health conditions and disabilities. The main purpose of this study was to compare the outcomes of CDH infants at 2 years of age (2y) according to whether the infants had undergone fetoscopic tracheal occlusion (FETO) during the prenatal period and characterize the relationship between morbidity at 2y and perinatal characteristics. Retrospective cohort single center study. Eleven years of clinical follow-up data (from 2006 to 2017) were collected. Prenatal and neonatal factors as well as growth, respir

The "developmental origins of health and disease" (DOHaD) hypothesis refers to the influence of early developmental exposures and fetal growth on the risk of chronic diseases in later periods. During fetal and early postnatal life, cell differentiation and tissue formation are influenced by several factors. The interaction between genes and environment in prenatal and early postnatal periods appears to be critical for the onset of multiple diseases in adulthood. Important factors influencing this interaction include genetic predisposition, regulation of gene expression, and changes in microbio

BACKGROUND: Children with medical complexity (CMC) have been defined (Cohen et al., Pediatrics 127: 529-538, 2011.) as an emerging population potentially eligible for PPC. The current study investigated the prevalence of children with medical complexities eligible for a local palliative care network, including a paediatric hospice.
METHODS: A retrospective cross-sectional survey has been conducted using children clinical charts from 14 local health authorities of our region (Emilia Romagna, Italy).
RESULTS: The total number of children with life-limiting conditions was 601, with a mean age o

BACKGROUND: The role of intrauterine infection in the development of neonatal pulmonary disease is unclear and the impact of histological chorioamnionitis (HCAM) on respiratory outcomes in preterm infants remains controversial. The aim of this study was to explore the association between HCAM and neonatal respiratory outcomes in very-low-birth-weight (VLBW) preterm infants and evaluate the stepwise difference in the stage and severity of HCAM among neonatal respiratory outcomes.
METHODS: We retrospectively enrolled 129 VLBW preterm infants with placenta histopathology examinations in this stu

Introduction: Rare diseases are characterized by their low prevalence, often of genetic origin, degenerative and life threatening. Objective: To describe mortality by orphan diseases and to analyze its trends in Colombia from 2008 to 2013. Materials and methods: We conducted a descriptive study to analyze mortality rate trends from the death certificates between 2008 and 2013. We calculated specific mortality rates and adjusted by age and sex. Results: Seven thousand one hundred and thirty five deaths were attributed to orphan diseases, and 51.4 % of them occurred among men of all ages. The me

Chorioamnionitis (CAM) is associated with abortion, premature labor and neonatal disorders. In this condition, precise pathological diagnosis of the placenta is very important. Besides histological examination, macroscopic examination of the placenta is indispensable. Diagnostic points and complications of CAM are emphasized in this paper. Blanc's classification (revised by Nakayama) is introduced for the accurate determination of CAM stage. Principles of classification based on the causes of fetal growth restriction (FGR) (mainly resulting from placental pathology) are also described. Childre

Wilson-Mikity syndrome (WMS) was first reported in 1960 by Wilson and Mikity. They described preterm infants who developed areas of cystic emphysema in the first month of life with subsequent progression to chronic lung disease (CLD) of infancy, although these infants did not exhibit early respiratory distress, such as respiratory distress syndrome (RDS). This condition was widely accepted over the next 20 years, but WMS is now rarely mentioned and is commonly considered an anachronism. In Japan, CLD is classified into six types according to the presence of RDS and/or intrauterine inflammation

BACKGROUND: Sivelestat sodium (sivelestat), a neutrophil elastase inhibitor, is used to treat acute respiratory distress syndrome (ARDS). We report two cases that developed elevated C5-acylcarnitine (C5-AC) levels following treatment with sivelestat. Case 1 was a 14-day-old female infant born at 25 weeks and 1 day of gestation who was treated with sivelestat for the prophylaxis of Wilson-Mikity syndrome soon after birth. Isovaleric acidemia (IVA) was suspected based on a newborn screening using tandem mass spectrometry (MS/MS). Her C5-AC level was elevated to 4.49 μM (cut-off, <1.0) after trea

Chronic lung disease of prematurity (CLD) is commonly considered to be a consequence of assisted ventilation. However, prior to the description in 1967 of bronchopulmonary dysplasia (BPD), following ventilator therapy for respiratory distress syndrome, Wilson-Mikity syndrome (WMS) had been described in very preterm infants on minimal oxygen supplementation. In the 1970s and 1980s, many infants treated with assisted ventilation required prolonged mechanical ventilation after developing radiographic features of coarse infiltrates, severe hyperinflation, and microcystic changes, associated with h

BACKGROUND: Funisitis is a manifestation of the fetal inflammatory response syndrome, and intrauterine inflammation is involved in the pathogenesis of lung injury in premature infants. The aim of the present paper was to examine the relationship between funisitis and lung injury in premature infants born at <28 weeks gestation. The present study focuses on the number of macrophages in tracheobronchial aspirate fluid (TAF).
METHODS: The numbers of CD68-positive cells in cell cytopreps in TAF collected at <24 h of age were determined on immunocytochemistry. The funisitis (+) group (n > 8) was c

BACKGROUND: Nearly 50 years ago, Wilson and Mikity described a syndrome (WMS) of chronic lung disease (CLD) in premature infants, characterized by early development of cystic interstitial emphysema (PIE), despite minimal ventilatory support. The validity of the diagnosis is currently unclear; now considered either an anachronism, part of BPD spectrum or included within various poorly defined diagnoses such as chronic pulmonary insufficiency of prematurity (CPIP).
OBJECTIVES: To define clinically useful diagnostic criteria for WMS so its position in the spectrum of CLD of infancy can be establ

OBJECTIVES: To analyse IGFs in respect to somatic growth and neonatal diseases during the first 6 months of life.
METHODS: IGF-I, IGF-II and IGF binding proteins (IGFBP-1, -2, -3) were determined by immunoassays in neonatal patients after birth (n = 67) and at 3 (3 mo; n = 75) and 6 months (6 mo; n = 47) of corrected postnatal age. Data on growth were tested for associations to the gestational age, birth weight (bw) SDS (/-2 SDS), neonatal morbidity and therapeutic strategies.
RESULTS: All IGFs and IGFBPs changed significantly between birth and 3 mo of corrected age (p < 0.05). Perinatal res
