Invasive Aspergillus Tracheobronchitis Presenting as Subglottic Stenosis: A Case Report.

Invasive Aspergillus tracheobronchitis (IATB) is a rare but potentially fatal manifestation of invasive aspergillosis. Because of its high mortality, prompt recognition and initiation of antifungal therapy are crucial. We report the case of a 71-year-old woman with a history of diffuse large B-cell lymphoma (DLBCL) who presented with progressive dyspnea. Computed tomography revealed subglottic airway stenosis, necessitating an emergency tracheostomy to secure the airway. Histopathological and microbiological examination of bronchoscopic biopsy specimens demonstrated infiltration of Aspergillus

Treatment Monitoring and Outcome Prediction in Invasive Aspergillosis Using Immunologic Markers.

BACKGROUND: Invasive aspergillosis (IA) is a leading cause of mortality despite antifungal therapy. Current biomarkers for treatment monitoring and outcome prediction are limited, with host biomarkers showing potential in other fungal diseases but not in IA yet. METHODS: In this ECMM multicenter study, we prospectively evaluated a cohort of 51 patients with probable or proven IA with underlying hematological malignancies. Serial serum samples, collected over a 3-week period, were analyzed using the Olink Target 96 Inflammation panel. Differential expression analyses and survival analysis for

Oncostatin M receptor deficiency as a novel candidate genetic cause of autosomal recessive hyper-IgE syndrome.

Hyper-IgE syndrome (HIES) is characterized by elevated serum IgE levels, eczema, and recurrent skin and pulmonary infections, classically caused by autosomal dominant (AD) STAT3 loss-of-function variants. Here we describe a patient presenting with elevated IgE levels, atopic eczema, chronic pulmonary aspergillosis, and bone fractures, homozygous for a rare missense variant in the oncostatin M receptor (OSMR) gene. We demonstrate that the patient OSMR V436D variant is deleterious and leads to decreased OSMR surface expression and impaired OSM-induced STAT3 phosphorylation. Blood profiling showe

25 years of improvement in mortality in invasive aspergillosis in haematology patients: will it be sustained or is it under threat?

Invasive aspergillosis (IA) crude mortality has shown a sustained reduction over the past decades, demonstrated in randomized controlled clinical trials of new antifungal agents and across large population surveys. New diagnostic tools and integrated management approaches have driven faster, more targeted initiation of appropriate antifungal therapy. In parallel, improvements in the identification of periods at highest risk for IA and in practices for management of the underlying disease processes predisposing to immunosuppression, including immunomodulatory therapies, have progressed. Given t

Development and application of a rapid detection system for Aspergillus fumigatus based on ERA/CRISPR-Cas12a.

Aspergillus fumigatus (AF) is the predominant pathogen implicated in invasive aspergillosis (IA) in humans; therefore, prompt and accurate detection is critical for the effective prevention and management of IA. This study developed a rapid detection system targeting the AF-specific anxC4 gene by integrating enzymatic recombinase amplification (ERA) with CRISPR/Cas12a. The reaction proceeds at a stable temperature of 37 °C, with amplification and detection systems separately positioned in the tube lid and bottom, respectively, effectively minimizing aerosol contamination typically associated w

Protocol for a systematic literature review and network meta-analysis of the evidence for therapies in allergic bronchopulmonary aspergillosis (ABPA).

INTRODUCTION: Allergic bronchopulmonary aspergillosis (ABPA) affects millions worldwide, yet current treatment approaches remain suboptimal due to a one-size-fits-all model that fails to account for the significant heterogeneity across affected populations. While corticosteroids and itraconazole are commonly prescribed, there are associated risks and uncertainties regarding efficacy. Additionally, the efficacy of alternatives such as newer and higher activity triazole antifungals, or the place of "biologic" therapies like anti-IL5 antagonists, has not been thoroughly investigated. OBJECTIVES:

Stepwise, precision-based management of allergic Bronchopulmonary Aspergillosis/Mycosis: A real-life multicenter study.

BACKGROUND: Allergic bronchopulmonary aspergillosis (ABPA) and allergic bronchopulmonary mycosis (ABPM) are hypersensitivity disorders characterized by fungal colonization of the airways and exaggerated Th2-mediated immune responses. Despite corticosteroids being the cornerstone of treatment, adverse effects, relapses, and steroid dependence necessitate adjunctive antifungal and biologic therapies. This study aimed to evaluate clinical, functional, and immunologic responses to different treatment combinations in patients with ABPA or ABPM. METHODS: This retrospective, multicenter, cross-secti

Epidemiology of Aspergillosis Diagnoses in US Adults Using a National EHR Database, 2013-2023.

BACKGROUND: Aspergillosis is a fungal infection associated with rising hospitalizations and substantial morbidity and mortality. In the United States, data remain fragmented due to the absence of centralized surveillance. This study aimed to evaluate demographic, geographic, and temporal trends in aspergillosis diagnoses across the United States and evaluate changes in those patterns following the emergence of COVID-19. METHODS: We conducted a retrospective cohort study using electronic health record data from 142 US healthcare systems (Oracle Health), including adults aged ≥18 years who rece

Post-Lobectomy Pleural Aspergillosis with Bronchopleural Fistula in a Patient with Metastatic Synovial Sarcoma of the Lung: A Case Report.

In clinical practice, healthcare providers encounter a rising incidence of aspergillosis, which significantly affects morbidity and mortality in vulnerable patients. Over the past few decades, molds have increasingly affected patients with underlying pleuropulmonary, hematological, or oncological diseases undergoing cytotoxic treatment or immunosuppression, leading to impaired cell-mediated immunity and an increased risk of postoperative complications. Although the spectrum of Aspergillus infection is variable, ranging from allergic to chronic, invasive manifestation, pleural involvement is ra

Rapid antifungal susceptibility testing for Aspergillus fumigatus using a loop-mediated isothermal amplification method.

In patients with hematological diseases and immunodeficiency, aspergillosis is associated with high mortality, and the early selection of effective antifungal agents is critical. Considering the emergence of azole-resistant Aspergillus and the skill- and time-intensive nature of Clinical and Laboratory Standards Institute (CLSI)-compliant broth microdilution (BMD), there is an urgent need for a rapid and simple antifungal susceptibility testing method. Here, we adapted a loop-mediated isothermal amplification (LAMP)-based phenotypic approach to detect azole resistance in Aspergillus fumigatus,

Evidence that increased azole persistence and stress resistance precede the in vivo evolution of azole resistance in Aspergillus fumigatus.

UNLABELLED: Aspergillus fumigatus is the major causative agent of human aspergillosis. Azoles are the first-line therapy, but resistance is increasing. Here, we characterized voriconazole persistence by screening a global collection of 495 clinical and environmental A. fumigatus isolates. We demonstrate two persistence phenotypes: non-growth persisters (NGPs) and slow-growth persisters (SGPs). We focused on nine sequential clinical isolates from a single patient treated with voriconazole for 2 years. Genome sequencing and phylogenomic analyses show that these isolates form a single genetically

Accelerated mutator phenotype in a clinical Aspergillus fumigatus isolate contributes to adaptive evolution.

The opportunistic pathogen Aspergillus fumigatus represents a major threat to immunocompromised individuals and is increasingly resistant to antifungal therapies. Resistance selection primarily takes place through environmental selection to azole fungicides, but in-host resistance may develop in patients with chronic aspergillosis receiving azole therapy. In this study, we examine clinical A. fumigatus isolates that exhibit irregular growth and accumulated mutations rapidly during antifungal treatment. Whole-genome sequencing of serial isolates revealed an accelerated mutation rate as the like

Pseudomonas aeruginosa adaptation and persistence in the aspergilloma microbiome revealed by integrated multi-omics.

Chronic pulmonary aspergillosis involves the formation of a fungal ball (aspergilloma) in lung cavities. Pseudomonas aeruginosa commonly co-colonizes these lesions; however, the in vivo mechanisms underlying its persistence are unknown. Using a multi-omics approach on resected aspergillomas, we defined the genomic, transcriptional, and metabolic adaptations of P. aeruginosa within this polymicrobial niche. We reconstructed high-quality P. aeruginosa genomes and identified a conserved core genome, along with accessory genes for secondary metabolism, virulence, and antimicrobial resistance. Phyl

Evaluation of the Prognostic Value of PCR Cycle Threshold and Mortality in Pulmonary Aspergillosis: A Retrospective Cohort Study.

BACKGROUND: Pulmonary aspergillosis remains highly lethal despite modern therapy. Although polymerase chain reaction (PCR) has enhanced detection, the clinical significance of its cycle threshold (Ct) values is unclear. The key problem is the lack of validation of Ct values for assessing fungal burden or predicting outcomes, limiting their diagnostic and prognostic utility. METHODS: We conducted a retrospective cohort study of 154 adult patients diagnosed with pulmonary aspergillosis at a tertiary medical center in Taiwan (2021-2024). All patients had a positive Aspergillus PCR from a respira

Non-fumigatus Aspergillus-associated pulmonary events: a diagnostic challenge.

UNLABELLED: Diagnosing pulmonary diseases caused by non-fumigatus Aspergillus species remains challenging. We conducted a single-center, retrospective observational study in a French Respiratory Medicine Department. Patients with at least one respiratory sample positive for a non-fumigatus Aspergillus species, without concurrent A. fumigatus isolation over a 12-month period, were included. The primary objective was to determine the prevalence of pulmonary events (colonization or pulmonary diseases) associated with non-fumigatus Aspergillus species. Secondary objectives included species charact
