Theophylline reduces the activity of cerebroside-sulfotransferase, a key enzyme in myelination, in cell cultures from newborn mouse brain.

Theophylline, a drug used in neonatology for the treatment of apnea, affects cholesterol synthesis if administered in concentrations of 10(-4) M (a concentration found in serum of treated patients) for 24 hr to dissociated brain cell cultures. The rate-limiting enzyme of cholesterol synthesis, beta-hydroxy-beta-methylglutaryl-coenzyme A reductase (EC 1.1.1.34), is lowered to 45% 48 hr after removal of theophylline. At the same time, cholesterol content of the cells is lowered to 73%. Inasmuch as the phospholipid content of the cells remains stable, the treatment changes the cholesterol phospho

Tracheal compression as a cause of apnea following repair of tracheoesophageal fistula: treatment by aortopexy.

Tracheal compression is one of the many causes of apnea following repair of esophageal atresia with tracheoesophageal fistula. Since May, 1977 we have treated eight patients with "dying spells" usually occurring during and following a feeding. These episodes were characterized by cyanosis and bradycardia, which progressed to apnea. Tracheal compression was documented to be the etiology of the apnea spells. Other possible causes such as aspiration from gastroesophageal reflux, recurrent tracheoesophageal fistula and esophageal stricture were eliminated. Eight infants underwent aortopexy that wa

Recurrent cyanotic attacks due to unilateral upper airway obstruction in infants.

Two infants presented with recurrent brief cyanotic attacks and had unilateral nasal discharge. Both were poor feeders. The triad--transient cyanotic attacks, poor feeding and unilateral nasal discharge--points to unilateral upper airway obstruction. When the obstruction was relieved, the symptoms disappeared.

[Neurological complications in children with congenital cyanotic heart disease (author's transl)].

Figures of incidence and risk factors of neurological complications (NC) endangering children with cyanotic congenital heart disease (CCHD) are very different. We examined 308 children with CCHD over an 8 years' period finding 39 cases (= 12,7) of NC. There were 5 hypoxic attacks and 3 brain abscesses. In 31 patients a cerebro-vascular accident was proven or probable. 56% of NC occured during the first year of life, most frequently in children with simple TGA. Mortality was not influenced by NC. Children without NC, surviving the first year of life represent the control group for comparison of

Endoscopy and biopsy in gastroesophageal reflux in infants and children.

The clinical features of abnormal gastroesophageal reflux in infants and children extend beyond repeated vomiting and include dysphagia, pain, bleeding, failure to thrive, esophageal stricture, and recurrent respiratory symptoms including aspiration pneumonitis and cyanotic attacks. The unreliability of the traditional barium swallow examination as a diagnostic test is well known. This study reports the results of endoscopic assessment and esophageal biopsy in 100 infants and children and relates them to the clinical findings and the changes in the contrast esophagogram. The results show that

Sleep apnea in normal kittens.

Apneic episodes in normal 10-, 20-, and 40-day-old kittens were assessed with polygraphic recordings. End expiratory apneas, usually preceded by somatic activity and/or augmented breaths, with durations less than 10 sec were observed in quiet sleep, active sleep, and transitions between states in all age groups. The highest apnea density was found at state transitions. Heart rate decelerations occurred before, during, and following apneas, but decelerations were not related to apnea duration. Combined central and obstructive components were associated with 9% of apneas in normal kittens.

[Sleep apnea in a child with adenotonsillar hypertrophy (author's transl)].

A three year old with sleep apnea due to adenotonsillar hypertrophy is presented. The picture improved after treatment with antibiotics and corticosteroids, and disappeared after adenotonsillectomy. The symptomatology is studied in relation to primary alveolar hypoventilation and the "sudden death" syndrome in infants. The need of an early diagnosis is emphasized to avoid onset of an irreversible "cor pulmonale". It is suggested that aside from a probable predisposing "central" and constitutional factor in children with this syndrome, apparently there are heterogeneous electroencephalographic

Primary alveolar hypoventilation (Ondine's curse syndrome) in an infant without external arcuate nucleus. Case report.

A five month old boy suffered from primary alveolar hypoventilation, with attacks of apnea during sleep (Ondine's curse syndrome). The ventilation did not increase when PACO2 was increased. However, it decreased during hyperoxia. This indicates a strong peripheral chemoreceptor drive and a dysfunction of the central chemoreception. The infant died from circulatory failure following an aspiration pneumonia. Autopsy revealed the absence of the external arcuate nucleus, which may be involved in the central chemocreception.

Mixed and obstructive sleep apnea and near miss for sudden infant death syndrome: 2. Comparison of near miss and normal control infants by age.

Twenty-nine full-term near miss for sudden infant death syndrome (SIDS) and 30 normal control infants underwent 24-hour polygraphic monitoring. Several types of respiratory events during sleep (eg, central, mixed, and obstructive apnea, periodic breathing) were defined and tabulated. Analysis of these respiratory variables and comparison of groups of near miss and control infants indicated that between 3 weeks and 4 1/2 months of age only one variable was consistently different at a statistically significant level: the number of mixed and obstructive apnea greater than 3 seconds during total s

Preferential distribution of lobar emphysema and atelectasis in bronchopulmonary dysplasia.

The chest roentgenograms of 142 neonates who survived mechanical ventilation for respiratory distress syndrome (N = 99) and prolonged apnea (N = 43) were reviewed. Thirty-seven infants had bronchopulmonary dysplasia (BPD) and 17 of these developed lobar hyperinflation of the right lower lobe and collapse of the right upper lobe. Regional lung function was measured with a xenon 133 technique in three of these infants and in five other patients who either died or were lost to follow-up. All had BPD with right lower lobe overinflation. Ventilation was less in the lower regions than the upper regi

Fetal breathing: its physiologic and clinical implications.

The respiratory movements the human fetus makes in utero can be detected and used as a valuable aid in determining fetal well-being. Their presence or absence, amplitude, and duration can all provide important information. Their absence, however, does not carry a poor prognosis unless other indices of fetal status, such as the nonstress test and oxytocin-challenge contraction-stress test, are also abnormal at the same time.

[Sudden death in new-born babies (author's transl)].

This sudden death of a new-born baby, which had been thriving up to then, is a dramatic but not exceptional event. A systematic search for the etiology must be made, and this should include an autopsy in order that the diagnosis of "sudden and unexplained death" is confirmed only after eliminating all other possible causes. Knowledge of this syndrome has benefited from epidemiological surveys which have defined the previous clinical condition, but none of the many etiollotical hypotheses envisaged can take into account the total number of cases reported. One of the most recent cases developed

Obstructive sleep apnea and near miss for SIDS: I. Report of an infant with sudden death.

An infant girl followed up from birth to death at the University Medical Center had ""congenital stridor'' and a ""near miss for SIDS'' event at 3 months of age. As part of an ongoing SIDS research project, she underwent 24-hour polygraphic monitoring at 21 weeks of age. Sudden infant death occurred within 30 hours after the polygraphic study. Polygraphic data obtained from this infant are compared with those from control infants and other infants with near miss for SIDS who were of similar ages. The number of mixed and obstructive respiratory events during sleep was abnormally high on the inf

[Present status of retrolental fibroplasia in France].

A national survey showed that retrolental fibroplasia still exists in France, as it does in other countries of the world. Retrolental fibroplasia is a condition that principally affects low birth weight premature babies in whom oxygen therapy has not been adequately controlled. The premature babies who have apnaeic attacks are most risk. Strict control of all oxygen therapy, preferably using continuous monitoring techniques together with other changes in the methods of treatment should reduce the incidence of this condition.

16 premature babies (all needing artificial ventilation, 15 suffering from idiopathic respiratory distress syndrome and one from severe apneic spells) received a single or double dose of indomethacin once the symptoms of an open ductus arteriosus further complicated their disease. Within 24 hours four patients showed closure of their duct, three other patients a very distinct improvement of their hemodynamic situation. Cardiovascular and respiratory responses to right atrial injections of phenyl diguanide in pentobarbital-anesthetized newborn piglets.

The present series of experiments was designed to determine whether the piglet has mature cardiorespiratory responses to the administration of phenyl diguanide (PDG) similar to those reported in adult mammals. A total of 26 acutely instrumented piglets aged 2--23 days were lightly anesthetized with sodium pentobarbital. After control conditions were established, PDG was injected into the right atrium. Each animal was its own control for determination of presence and magnitude of aortic pressure, heart rate, respiratory rate and volume, and blood flow responses to test doses of PDG. A fall in b

[Respiratory distress in the newborn (author's transl)].

Diseases which manifest with the respiratory distress in the newborn include 1) respiratory diseases-IRDS, type II RDS, neonatal asphyxia, and MAS etc. 2) anemia, CHD 3) CNS and 4) metabolic diseases. Among these, IRDS has high mortality rate because of the lack of the pulmonary surfactant and immaturity of respiratory center, and has many difficult problems in terms of its prevention and respiratory management. The points of its respiratory management are as follows: 1) Estimation of the level of arterial oxygen ation-this is the most important point. It has become possible, these days, to mo
