Electric stimulation and transcranial magnetic stimulation for obstructive sleep apnea.

Obstructive sleep apnea syndrome (OSAS) is a prevalent sleep disorder that significantly compromises human health. While continuous positive airway pressure (CPAP) remains the gold standard for treatment, its clinical effectiveness is often limited by poor patient adherence, underscoring the need for alternative therapies. Emerging evidence suggests that physical stimulation therapies may offer a promising non-invasive approach by activating upper airway dilator muscles to maintain airway patency, thereby reducing the severity of OSAS and alleviating related symptoms. This review summarizes re

Comparative effectiveness of continuous positive airway pressure and glucagon-like peptide-1 receptor agonists in obstructive sleep apnea: A network meta-analysis of randomised trials.

To compare the effects of continuous positive airway pressure (CPAP), glucagon-like peptide-1 receptor agonists (GLP-1 RAs), their combination, and no active intervention on respiratory, sleepiness, and metabolic outcomes in adults with obstructive sleep apnea (OSA). We searched PubMed, Embase, and CENTRAL through August 2025 for randomised trials of CPAP, exenatide, liraglutide, tirzepatide, or their combinations. The primary endpoint was apnea-hypopnea index (AHI). Secondary endpoints were Epworth Sleepiness Scale (ESS), body mass index (BMI), systolic and diastolic blood pressure (SBP, DBP)

Chronic intermittent hypoxia reshapes circadian metabolic architecture in a model of sleep apnea.

Obstructive sleep apnea (OSA), characterized by chronic intermittent hypoxia (IH) during sleep, is increasingly recognized as a driver of metabolic dysfunction. However, its impact on circadian metabolic regulation remains poorly understood. Here, we investigated how chronic IH reshapes 24-hour hepatic and systemic metabolic programs in a mouse model mimicking OSA-associated chronic hypoxia. Through integrated circadian transcriptomic, metabolomic, and physiological 24-hour profiling, we show that 4 weeks of rest phase-restricted IH reprograms hepatic and systemic metabolism in a time-specific

In Vitro Investigation of the PneumoWave Biosensor for the Identification of Central Sleep Apnea in Pediatrics.

The interpretation and diagnosis of central sleep apnea in pediatrics by nocturnal polysomnography is challenging due to its technical complexity, which involves the simultaneous recording of multiple physiological parameters related to sleep and wakefulness. Furthermore, the unfamiliar environment of a sleep laboratory can hinder sleep evaluation, and diagnostic backlogs are common due to restricted capacity at specialist tertiary centers. The ability to undertake home sleep studies in a familiar environment using simple, robust, and low-cost technology is attractive. The potential to repurpo

Gut microbiota-cognitive function interactions in patients with comorbid insomnia and sleep apnea (COMISA): a study on brain dynamic specificity.

Cognitive impairments in comorbid insomnia and sleep apnea (COMISA) show an additive effect, but the neurophysiological mechanisms remain unclear. This study explored the links between brain dynamic functional activity, gut microbiota (GM), and cognitive changes in COMISA patients. Twenty-five COMISA patients and 22 healthy controls (HCs) underwent fMRI scanning and cognitive function assessments. Dynamic regional homogeneity (dReHo) differences between groups were compared, followed by dynamic functional connectivity (dFC) analysis using regions with abnormal dReHo as seed points. Cognitive f

Sleep-Disordered Breathing in Chung-Jansen Syndrome.

We report a thirty-six-year-old woman with intellectual disability who was referred for evaluation of suspected obstructive sleep apnea. The initial clinical impression suggested a syndromic case, so comprehensive genetic testing was undertaken. Overnight polysomnography revealed a severe rapid eye movement-predominant obstructive sleep apnea syndrome with an apnea-hypopnea index of 31.9 events per hour, rapid eye movement apnea-hypopnea index of 113.8 events per hour, and lowest oxygen saturation of 66%. Treatment with continuous positive airway pressure improved respiratory and sleep quality

TRPC Channels as Mediators of Hypoxia-Induced Pulmonary Hypertension in Obstructive Sleep Apnea.

Pulmonary hypertension (PH) is a progressive disorder characterized by elevated pulmonary arterial pressure and the extensive remodeling of pulmonary vasculature. Chronic intermittent hypoxia (CIH), a hallmark of obstructive sleep apnea (OSA), is a well-established contributor to the pathogenesis of PH. OSA is defined by repetitive episodes of upper airway obstruction during sleep, leading to cycles of hypoxia and reoxygenation that trigger a cascade of deleterious events including oxidative stress, inflammation, endothelial dysfunction, and vascular remodeling. Growing evidence underscores th

Impact of Sleep Apnea Treatment in Patients with Unexplained Syncope: The SINCOSAS Study.

Objectives: Unexplained syncope (US) persists despite extensive diagnostic evaluations, with autonomic dysfunction as a central mechanism. Sleep apnea (SA) may contribute through intermittent hypoxemia and autonomic imbalance. We evaluated the impact of SA treatment on syncope recurrence, nocturnal heart rate variability (HRV), and quality of life in patients with US. Methods: We conducted a prospective multicentre study in three hospitals in Galicia (Spain), including adults with US who underwent home respiratory polygraphy. SA was diagnosed according to guideline criteria, and treatment was

Feasibility and Form Factor Validation of Reflective Shoulder-Mounted Pulse Oximeter in Patients with Suspected Sleep Apnea.

The shoulder may be an effective central site for continuous oxygen saturation (SpO2) monitoring but studies of shoulder-mounted pulse oximetry technology are limited. We hypothesized that an alternative location would be similar in function and user acceptance to a standard FDA-cleared finger-based pulse oximeter. We conducted a quantitative and descriptive pilot study of two prototype biosensor designs in patients with clinical suspicion of hypoxic episodes at an outpatient sleep center. Participants wore two prototype biosensors-the primary a shoulder-mounted adhesive and the secondary a co

Effects of controlled carbon dioxide delivery by a novel device on sleep in healthy subjects.

Low-dose carbon dioxide (CO2) can stabilize ventilatory drive, reduce sleep-disordered breathing, and improve sleep quality. However, the existing delivery systems introduce dead space and resistance that limit tolerability. We developed a novel mask with low resistance and minimal dead space to deliver CO2 and evaluated its overnight effects on neural respiratory drive, sleep architecture, and potential CO2 accumulation in healthy individuals. Sixteen healthy volunteers [age 42 ± 15 yr; body mass index (BMI) 22.0 ± 2.3 kg/m2] first underwent polysomnography with diaphragmatic electromyography

Recognition of a Critical Functional Domain and Improved PHOX2B Missense Variant Interpretation by Utilization of In Silico Prediction Tools.

Pathogenic heterozygous variants in PHOX2B are associated with congenital central hypoventilation syndrome (CCHS), which is characterized by autonomic nervous system dysregulation severely affecting respiratory control. The interpretation of PHOX2B missense variation is challenging due to their rarity and the lack of available functional evidence. Consequently, most PHOX2B missense variants are classified as variants of uncertain significance (VUSs), complicating the timely diagnosis and clinical management of the condition. To generate an improved model for assessments of PHOX2B missense vari

Sex Differences in the Effects of Etonogestrel on Respiratory Recovery in an In Vivo Rat Model of Central Chemoreflex Impairment.

AIM: Rhythmic breathing movements driven by the brainstem rely on chemosensory inputs to homeostatically adjust motor output to the prevailing metabolic demand. The central CO2 chemoreflex is a critical component of this neural circuitry, as defects in these sensors cause hypoventilation syndromes, which are typically difficult to manage pharmacologically. Progesterone has long been known to stimulate breathing in both sexes, and remarkably the progestinic metabolite, etonogestrel (ETO), enhances CO2 chemosensitivity in animal models and female patients affected by congenital central hypoventi
