Metabolomic Biomarkers for Non-Invasive Diagnosis of Coronary Artery Disease and Obstructive Sleep Apnea Co-Occurrence.

BACKGROUND AND AIMS: The comorbid state of obstructive sleep apnea (OSA) and coronary artery disease (CAD) has emerged as a significant public health challenge. However, the underlying metabolic regulation and molecular biomarkers linking to the CAD and OSA comorbidity (CADOSA) remain poorly understood. This study aimed to identify non-invasive biomarkers for the CADOSA comorbidity. METHODS: We randomly recruited a total of 143 participants from a hospital-based setting, comprising four groups: healthy controls (n=38), patients with OSA-only (n=37), CAD-only (n=36), and comorbid CADOSA (n=32)

Central venous pressure elevations are associated with intracranial pressure spikes in a model of acute obstructive sleep apnea.

Obstructive sleep apnea (OSA) is associated with increased cerebrovascular disease, but acute intracranial dynamics during obstructive events remain uncertain. We hypothesized that negative intrathoracic pressure elevates intracranial pressure (ICP) by increasing central venous pressure (CVP) and impairing cerebral venous outflow. In an anesthetized porcine model, we measured ICP during simulated OSA with intermittent negative airway pressure (INAP) or positive end-expiratory pressure (PEEP) under controlled blood gas conditions. INAP consistently produced post-apneic ICP surges (+10 mmHg) tha

Machine Learning-Based Sleep Electroencephalographic Brain Age Index and Dementia Risk: An Individual Participant Data Meta-Analysis.

IMPORTANCE: Microstructures of sleep electroencephalography (EEG) are closely related to cognition and undergo age-dependent changes. However, their multidimensional nature makes them challenging to interpret using conventional approaches. The machine learning-based EEG brain age index (BAI) measures the deviation between sleep EEG-based brain age and chronological age. OBJECTIVE: To determine the association between sleep BAI and incident dementia in community-dwelling populations. DATA SOURCES: For this individual participant data (IPD) meta-analysis, sleep study data from 5 community-base

Sleep Duration, Chronotype, and Mortality in Parkinson's Disease: A Prospective Cohort Study.

AIM: Sleep disturbances are a common non-motor symptoms in Parkinson's disease (PD) and may occur already in the prodromal phase. In the general population, shorter sleep duration is linked to higher mortality, while longer sleep duration has been associated with an increased risk of developing PD. We examined the relationships between sleep duration, chronotype, and mortality in patients with PD. METHODS: A total of 855 randomly selected patients with PD completed a structured questionnaire including self-reported sleep duration and chronotype. Mortality data were collected from the national

Multifactorial anticholinergic toxicity-like presentation and malignant parkinsonism in Perry syndrome.

Perry syndrome is a rare autosomal dominant neurodegenerative disorder characterized by parkinsonism, depression, weight loss, and central hypoventilation. Early diagnosis is challenging because presentations are heterogeneous. A 51-year-old man presented with hyperthermia, impaired consciousness, tachycardia, and gastrointestinal hypomotility suggestive of anticholinergic toxicity. Despite therapeutic fesoterodine levels, he developed hypercapnia requiring mechanical ventilation and later exhibited rigidity with marked creatine kinase elevation, consistent with malignant parkinsonism. Dopamin

Uncoupling of apnea-hypopnea index and oxygen desaturation in als: characterization and diagnostic implications of an "AHI-SpO₂ dissociation" phenotype.

BACKGROUND: In Amyotrophic Lateral Sclerosis (ALS), sleep-disordered breathing (SDB) is primarily driven by neuromuscular hypoventilation. Traditional metrics like the Apnea-Hypopnea Index (AHI) may therefore lack sensitivity. This study aimed to characterize an “AHI-SpO₂ Dissociation” phenotype—low event frequency despite significant desaturation—and explore its associations. METHODS: We retrospectively analyzed polysomnography and pulmonary function data from 43 ALS patients. The “AHI-SpO₂ Dissociation” phenotype was defined as AHI < 5 events/h with Minimum SpO₂ <90%. Spearman correlations a

Telomere dysfunction is associated with exacerbated intermittent hypoxia-induced cognitive deficits and nerve damage.

Cognitive impairment associated with obstructive sleep apnea (OSA) is more prevalent and severe in the elderly, possibly due to age-related increases in neuronal susceptibility to intermittent hypoxia (IH). As telomere dysfunction is a key driver of cellular aging, this study aimed to characterize the interaction between telomere dysfunction and IH, and to explore the associated molecular alterations. Using telomere-damaged PC12 cells and G3 Tert-/- progeria mice exposed to IH, we assessed cellular stress responses, apoptosis, cognitive function, and hippocampal structural changes. The effects

Case Report: Immune checkpoint inhibitor-triggered anti-Ma2 paraneoplastic encephalitis in sarcomatoid pleural mesothelioma: a fatal case.

BACKGROUND: Neurological immune-related adverse events (N-irAEs) represent rare but potentially fatal complications of immune checkpoint inhibitor (ICI) therapy. Among these, encephalitis associated with paraneoplatic neuronal antibodies poses a major diagnostic and therapeutic challenge, as it blurs the distinction between drug-induced toxicity and tumor-driven autoimmunity. CASE PRESENTATION: We report a 61-year-old male diagnosed with unresectable sarcomatoid pleural mesothelioma who was treated with first-line nivolumab plus ipilimumab. Following the third treatment cycle, the patient dev

Emergence of central sleep apnea at high altitude in patients with mild obstructive sleep apnea: a case report.

BACKGROUND: Patients with obstructive sleep apnea ascending to high altitude are known to experience worsening of sleep-disordered breathing, often with an increase in central events. However, the management of a full phenotypic shift from mild obstructive sleep apnea to severe central sleep apnea at altitude remains poorly characterized. CASE PRESENTATION: We report a case of a 44-year-old Han Chinese man with symptomatic mild obstructive sleep apnea (apnea-hypopnea index: 11.4 events/hour) well-controlled on continuous positive airway pressure at low altitude. Upon ascent to Lhasa (3650 m),

Application of the SF-BARI Score for Evaluating long-term Metabolic and Clinical Outcomes after Bariatric Surgery.

BACKGROUND: Bariatric surgery is an effective treatment for severe obesity, resulting in significant weight loss and remission of obesity-related diseases. However, weight gain remains a challenge, potentially impacting long-term outcomes. The Swiss-Finnish BARIatric metabolic outcome score (SF-BARI) is a composite tool that integrates percentage total weight loss (%TWL), obesity associated disease remission, and operative complications rates to evaluate surgical optimal clinical response. This study assessed the associations between SF-BARI scores, obesity associated diseases remission, and w

Social Vulnerability and Subclinical Cardiovascular Risk in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is an established risk factor for cardiovascular disease (CVD). Multiple clinical factors, including OSA severity, obesity, smoking, and hypertension, mediate this risk. However, whether the association between OSA and CVD is influenced by non-clinical factors, specifically social determinants of health, remains unclear. We examined the association between neighborhood-level social determinants of health, indicated by the Social Vulnerability Index, and subclinical CVD risk in adults with untreated severe OSA. Racial and ethnic minority status (REMS) and socioecon

GLP-1 Receptor Agonists at the Crossroads of Circadian Biology, Sleep, and Metabolic Disease.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have transformed the management of type 2 diabetes and obesity, yet their actions extend beyond glycemic control and weight loss. This narrative review synthesizes current preclinical and clinical evidence examining the bidirectional relationship between glucagon-like peptide-1 (GLP-1) receptor agonists and circadian biology. A structured literature search was conducted in PubMed using combinations of the terms 'GLP-1,' 'circadian,' 'chronobiology,' 'sleep,' 'obesity,' and 'type 2 diabetes' through January 2026. Accumulating evidence indicat

Oxygen desaturation index and fibrinogen: An independent association in sleep-disordered breathing - A retrospective cross-sectional study.

BACKGROUND AND AIMS: Obstructive sleep apnea (OSA) elevates cardiovascular risk partly through coagulation dysregulation, in which fibrinogen (FIB) plays a central role. However, the independent association between oxygen desaturation index (ODI), rather than apnea-hypopnea index (AHI), and FIB requires robust validation amid challenges of confounding. To determine whether ODI is an independent and robust predictor of plasma fibrinogen levels in a clinical sleep-disordered population. METHODS: We conducted a retrospective analysis of 690 OSA patients who underwent sleep monitoring and FIB mea

Systemic immune-inflammation index and systemic inflammation response index: novel hematological predictors of obstructive sleep apnea.

BACKGROUND: While the association between obstructive sleep apnea (OSA) and chronic inflammation is well-established, the role of novel hematological indices, such as the systemic immune-inflammation index (SII) and systemic inflammation response index (SIRI), remains unclear. Their specific relationship with OSA has not been fully elucidated. METHODS: A total of 590 consecutive patients with suspected OSA were enrolled between June 2023 and August 2025. OSA was defined as an apnea-hypopnea index (AHI) of ≥5 events per hour. Spearman's rank correlation analysis was used to assess the relation

Effect of CPAP or oxygen on high loop gain sleep apnea and associated blood pressure change- an analysis using respiratory self-similarity.

BACKGROUND: Respiratory high loop gain (HLG) is an important driver of obstructive apnea and may be modified by continuous positive airway pressure (CPAP) or supplemental oxygen. Respiratory self-similarity is a marker of expressed HLG. METHODS: The baseline and on-therapy (CPAP, oxygen, healthy lifestyle and sleep education, HLSE) follow-up home sleep apnea tests (HSAT) from the Heart Biomarker Evaluation in Apnea Treatment (HeartBEAT) study were analyzed using self-similarity based on the respiratory effort signal. Linear regression analysis assessed the role of treatment efficacy in improv

Causal effects of basal metabolic rate and adiposity on obstructive sleep apnea: univariable, multivariable, and bidirectional Mendelian randomization evidence.

BACKGROUND: Obstructive sleep apnea (OSA) is a common disorder strongly associated with obesity, yet the causal contribution of basal metabolic rate to OSA and the directionality of the OSA-metabolism relationship remain unclear. The objective of this study was to evaluate the independent and potential bidirectional causal relationships between basal metabolic rate and adiposity-related traits with obstructive sleep apnea using Mendelian randomization. METHODS: Two-sample Mendelian randomization (MR) was performed using genome-wide association study data from predominantly European population

Dual Implementation of Phrenic and Hypoglossal Nerve Stimulators in a Patient With Obstructive and Central Sleep Apnea and Associated Cardiac Arrhythmias.

When patients with obstructive sleep apnea (OSA) and central sleep apnea (CSA) fail to benefit from first-line therapies, combinations of new treatment modalities may be beneficial. The hypoglossal nerve stimulator (HNS) and phrenic nerve stimulator (PNS) are two exciting options in the management of OSA and CSA, respectively. A 72-year-old man with concurrent CSA and OSA underwent sequential implantation of a PNS followed by a unilateral HNS, and his response to each device was monitored with polysomnography (PSG). This patient also had a significant history of atrial flutter and premature ve
