Prepregnancy GLP-1RA Exposure and Risk of Hypertensive Disorders of Pregnancy Among Women With Obesity and Diabetes.

OBJECTIVE: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are prescribed to women of reproductive age for obesity, type 2 diabetes, obstructive sleep apnea, and more. Data on pregnancy outcomes following prepregnancy exposure remain limited. We evaluated associations between prepregnancy GLP-1RA exposure and maternal and neonatal outcomes among women with preexisting obesity and diabetes. METHODS: We conducted a retrospective cohort study using electronic health records data. Prepregnancy GLP-1RA exposure was defined by prescription orders within 12 and 24 months prior to delivery. TGF‑β1 in obstructive sleep apnea: linking intermittent hypoxia, upper‑airway remodeling, and systemic comorbidities.

BACKGROUND: Obstructive sleep apnea (OSA) is a common but underdiagnosed disorder associated with cardiometabolic, neurocognitive, and malignant comorbidities. Transforming growth factor-β1 (TGF-β1) is a central profibrotic cytokine and may link OSA exposures to upper-airway remodeling and systemic injury. OBJECTIVE: To synthesize current evidence implicating TGF-β1 signaling in OSA pathogenesis and comorbidities, and to highlight translational implications and key knowledge gaps. METHODS: We conducted a structured literature search in PubMed, Embase, Web of Science, and the Cochrane Library

Wireless, skin-interfaced multimodal sensing system for continuous psychophysiological monitoring-A wearable polygraph device.

Accurate, continuous monitoring of psychophysiological states is central to understanding stress and autonomic dysfunction across diverse medical contexts. Current approaches such as polygraphy and polysomnography rely on cumbersome, wired sensors that limit real-world utility and burden patients, particularly vulnerable populations such as infants. Here, we introduce a wireless, skin-interfaced multimodal sensing system capable of simultaneously recording cardiac, respiratory, electrodermal, and thermal signals in a time-synchronized manner. Leveraging compact and soft designs, the technology

GLP-1/GIP dual agonist tirzepatide in obstructive sleep apnea syndrome: mechanisms, evidence, and clinical perspectives.

Obstructive sleep apnea syndrome (OSAS) is closely associated with obesity and metabolic dysfunction. Tirzepatide, a dual GLP-1 and GIP receptor agonist, has demonstrated superior efficacy for weight reduction and improved metabolic health compared with single GLP-1 agonists. Emerging evidence indicates that tirzepatide may alleviate OSAS through its multifaceted effects on adiposity, inflammation, and neuromuscular regulation. This review synthesizes the pharmacological mechanisms, clinical findings, and safety data of tirzepatide in OSAS management. Preliminary studies show promising reducti

Obesity and cardiovascular risk in down syndrome: Challenges and updated management.

Down syndrome (DS) or trisomy 21 is a genetic condition presenting with intellectual disabilities, distinctive phenotypic features, and multisystem involvement. As life expectancy in individuals with DS increases, obesity has emerged as a significant and growing clinical concern. Although obesity is highly prevalent among DS patients, its contribution in shaping cardiovascular risk has not been comprehensively synthesized in the existing literature. Studies published between January 2000 and March 2026 were identified through searches of PubMed and ScienceDirect, supplemented by manual screeni

Systemic inflammatory response index is associated with increased all-cause and cardiovascular mortality in obstructive sleep apnea: evidence from NHANES and validation with an external hospital-based dataset.

OBJECTIVE: Obstructive sleep apnea (OSA) is increasingly recognized for its association with significant morbidity and mortality. Despite advances in understanding OSA's impacts, the role of systemic inflammation in this context remains underexplored. The systemic inflammatory response index (SIRI) has emerged as a potential marker for evaluating inflammatory status and predicting adverse health outcomes. We aim to examine the association between the SIRI and the risks of all-cause and cardiovascular mortality in individuals exhibiting symptoms of obstructive sleep apnea (OSA), with further va

Mechanisms of upper airway muscle control in sleep reveal therapeutic targets for obstructive sleep apnea.

Obstructive sleep apnea (OSA) is the most prevalent sleep-related breathing disorder and is associated with cardiovascular, metabolic, neurocognitive, and mortality risk. OSA arises from recurrent upper airway collapse during sleep, producing intermittent hypoxia and sleep fragmentation. While anatomical vulnerability contributes to airway instability, a key determinant of OSA pathophysiology is the sleep-related reduction in upper airway neuromuscular activity that occurs at the wake-sleep transition. Despite this central role, no approved pharmacologic therapies have targeted the neuromuscul

The Somnolink-Hub: A Central Infrastructure That Unites Sleep Data at Point of Care.

Obstructive sleep apnea (OSA) is a sleeping disorder which can lead to severe health consequences. Information gaps in the patient's diagnostic path may result in duplicate examinations, while patients wait long for sleep diagnostics. We propose an architecture for standardized sleep data exchange along the OSA patient diagnostic path. Our system consists of extended and interconnected established components for biosignal storage and electronic patient data and is entirely based on open source software. Specifically, our system supports sharing of all necessary data in the diagnostic path, fro

A pilot study to assess a novel screening questionnaire for central sleep apnea in patients undergoing overnight diagnostic polysomnography.

PURPOSE: A novel central sleep apnea (CSA) screening questionnaire, SCOUTS-BAG-HARMS (SBH), was developed and assessed with polysomnography (PSG)-confirmed "any CSA", defined as a central apnea index (CAI) ≥ 5 per hour. Unlike the STOP-BANG questionnaire, which screens for obstructive sleep apnea (OSA), SBH incorporates CSA-specific questions in three sections (SCOUTS), including silent apneas, demographics (BAG), including body mass index, and comorbidities (HARMS), including heart failure. METHODS: In this pilot study, patients undergoing a PSG at our sleep lab between January and June 2023

Cheyne-Stokes respiration screening using cyclic variation in heart rate detected by Holter electrocardiogram.

BACKGROUND: Cheyne-Stokes respiration (CSR) is a major form of central sleep apnea in patients with heart failure and is associated with mortality in these patients. Although full polysomnography is required for CSR diagnosis, access may be limited. Cyclic variation of heart rate (CVHR) detected by Holter electrocardiogram reflects changes in cardiac autonomic activity associated with apnea-hypopnea events and is easier to perform than polysomnography. METHODS: We examined whether RR interval shortening time in the CVHR analysis could be useful for screening CSR. In this study, 41 patients we

Mapping the Evidence: Central Sleep Apnea Syndromes During Sleep and Stroke-A Scoping Review.

BACKGROUND: Central sleep apnea (CSA) syndromes and acute ischemic stroke are linked by a complex, bidirectional relationship. After ischemic stroke, CSA prevalence rises markedly compared with the general population, in which it is usually below 1%. In stroke cohorts, CSA frequencies of around 8%-12% have been reported, although estimates are heterogeneous owing to differing diagnostic criteria. In this scoping review we map the evidence on CSA syndromes during sleep in patients with stroke, summarizing prevalence, mechanisms, clinical correlates, and prognostic implications. METHODS: The Me

Sleep spindle density increases in patients with severe obstructive sleep apnea after 6 months of treatment: a longitudinal study.

Obstructive sleep apnea (OSA) disrupts sleep structure and alters NREM sleep microarchitecture, including reductions in sleep spindle activity. Given the central role of sleep spindles in sleep continuity and neurophysiological processing, their reduction in patients with OSA suggests a potentially reversible sleep microstructural disruption. We evaluated sleep spindles across four EEG derivations (F3, F4, C3, and C4) during NREM sleep in patients with suspected OSA, assessed before and after 6 months of routine clinical practice. The 6-month polysomnography was performed with participants usi
