Successful extracardiac total cavo pulmonary connection (TCPC) after external tracheobronchial stenting for tracheobronchomalacia.

OBJECTIVES: A girl who was diagnosed with cyanotic congenital heart disease in a newborn, showed anoxic spell attacks which we thought were correlated with the congenital heart disease. Although she underwent the palliative operation at 8 months old and her SpO2 was increased, she experienced severe and life-threatening respiratory dysfunction many times after that. After careful examinations, the respiratory complaint was proved to be not only due to cyanotic congenital heart disease but also tracheobronchomalacia. METHOD: She had undergone the external stenting to the trachea and right bron

Acute respiratory distress syndrome due to cyanotic spell in an infant with tetralogy of Fallot.

Acute respiratory distress syndrome (ARDS) is characterized by fulminant respiratory failure due to noncardiac pulmonary edema. This can be triggered by a heterogeneous group of diseases. We report an unusual case of an infant who developed severe ARDS in association with a severe cyanotic attack due to tetralogy of Fallot.

[Laryngo-tracheal pathology in newborns and children].

In the newborn and in the children are frequent features like congenital laryngeal stridor, croup, weak or absent cry, cyanotic attacks, apneic attacks, and sometimes are due to disease like laryngomalacia, tracheomalacia, laryngeal web, post-intubation lesions and stenosis, tracheo-esophageal fistula. The Author present an experience of four years in the Vicenza's Hospital with rigid telescopes and flexible fiberoptic instruments in the endoscopic valuation and treatment of this pathology. Moreover present a new technics for the preoperative incannulation of the tracheoesophageal fistula usin

Role of aortopexy in the management of primary tracheomalacia and tracheobronchomalacia.

Between 1986 and 1988 aortopexy was carried out on 12 patients with primary tracheomalacia and five with tracheobronchomalacia; their median age was 5 months (1 month-7 years). All patients had stridor, 14 had had cyanotic and apnoeic episodes, and 10 had had 'death' attacks. Eight patients (47%) had associated gastro-oesophageal reflux. Six patients (35%) were helped by aortopexy alone, and a further five (29%) improved with a combination of aortopexy and antireflux treatment. Four patients required tracheostomy, one of whom died. Aortopexy failed if gastro-oesophageal reflux was present. We

Management of tracheomalacia in association with congenital tracheo-oesophageal fistula.

Tracheomalacia is commonly associated with oesophageal atresia and tracheo-oesophageal fistula. Severe cases may present with life threatening cyanotic and apnoeic attacks following surgical repair of the oesophageal atresia. The anaesthetic, and surgical management (by tracheopexy), of such a case are described.

Congenital vascular anomalies causing tracheoesophageal compression.

Congenital vascular anomalies of the great vessels causing marked tracheoesophageal compression are rare. Notable symptoms usually manifest in the neonatal period and include stridor, repeated cyanotic attacks, recurrent pneumonia, and dysphagia. Although the diagnosis can often be made from barium swallow studies, the preoperative examination should nevertheless include an endoscopy. This is essential if the degree of tracheomalacia is to be adequately assessed and other tracheobronchial abnormalities are to be identified. Possible airway problems can then be anticipated, thus minimizing post

[Indication for operation and results in cardia insufficiency with or without hiatic hernia in childhood (author's transl)].

After discussing the problems in cardia insufficiency in childhood it is suggested that conservative treatment is indicated in minor-oesophageal refluxes, most medium-range refluxes and hiatic hernias of minor degree. Surgical treatment is indicated if conservative therapy over 4 weeks is ineffective. Still and absolute indication for operation must be distinguished from a relative indication. Absolute indications are: oesophagitis, oesophageal stricture, Roviralta's-syndrome, major refluxes, some medium-range refluxes as well as all hiatic hernias except minor forms in young infants. Relative

Recurrent cyanotic attacks due to unilateral upper airway obstruction in infants.

Two infants presented with recurrent brief cyanotic attacks and had unilateral nasal discharge. Both were poor feeders. The triad--transient cyanotic attacks, poor feeding and unilateral nasal discharge--points to unilateral upper airway obstruction. When the obstruction was relieved, the symptoms disappeared.

[Neurological complications in children with congenital cyanotic heart disease (author's transl)].

Figures of incidence and risk factors of neurological complications (NC) endangering children with cyanotic congenital heart disease (CCHD) are very different. We examined 308 children with CCHD over an 8 years' period finding 39 cases (= 12,7) of NC. There were 5 hypoxic attacks and 3 brain abscesses. In 31 patients a cerebro-vascular accident was proven or probable. 56% of NC occured during the first year of life, most frequently in children with simple TGA. Mortality was not influenced by NC. Children without NC, surviving the first year of life represent the control group for comparison of

Endoscopy and biopsy in gastroesophageal reflux in infants and children.

The clinical features of abnormal gastroesophageal reflux in infants and children extend beyond repeated vomiting and include dysphagia, pain, bleeding, failure to thrive, esophageal stricture, and recurrent respiratory symptoms including aspiration pneumonitis and cyanotic attacks. The unreliability of the traditional barium swallow examination as a diagnostic test is well known. This study reports the results of endoscopic assessment and esophageal biopsy in 100 infants and children and relates them to the clinical findings and the changes in the contrast esophagogram. The results show that

[The risks involved in the heart catheter examination. A retrospective evaluation of the complications after 700 examination. III. Irregularities of heart (author's transl)].

Disturbances of heart rhythm, observed during 700 heart catheterizations in infants and children, are discussed. Paroxysmal supraventricular tachycardia has been observed in 25 investigations (3,6%), sinus bradycardia in 18 (2,6%), junctional rhythm in 10 (1,4%), second degree AV-block in 9 (1,3%), ventricular fibrillation in 8 (1,1%), sinus tachycardia in 7 (1%), complete block in 7 (1%), asystole and atrial flutter in 2 (0,3%) each, and ventricular tachycardia in 1 (0,15%). Supraventricular tachycardia occurred equally in all ages without preference of a special malformation. The two patient

Pulmonary function following feeding in low-birth-weight infants.

Determination of functional residual capacity, arterial gas tensions and pH, and arterial-alveolar differences was carried out in a group of nondistressed premature infants to determine if significant alterations in lung volumes or ventilation perfusion relationships sufficiently large to cause cyanosis could be detected after feeding. The only statistically significant changes observed in these parameters were immediately following feeding, a fall of .01 pH units and a fall of 6 mm Hg in arterial oxygen tension; and 15 minutes after feeding, a fall of .01 pH units and a rise in arterial PCO2
