Dense temporal sleep apnea profiling from single-channel PPG with downstream AHI estimation: a large-scale retrospective feasibility study.

Sleep apnea (SA) diagnosis remains challenging due to the cost and limited accessibility of polysomnography. Most automated screening approaches estimate the Apnea-Hypopnea Index (AHI) from oximetry-derived features and do not provide temporally resolved characterization of respiratory events. This study investigates the feasibility of dense temporal respiratory event profiling and clinically meaningful SA severity estimation from single-channel photoplethysmography (PPG).
Approach: A two-stage framework was developed using single-channel PPG. A fully convolutional neural network operating

Artificial intelligence for sleep-disordered breathing: a taxonomy of methods, data modalities, and clinical applications.

Sleep-disordered breathing (SDB), particularly obstructive sleep apnea (OSA), is a highly prevalent group of disorders associated with disrupted sleep, intermittent hypoxemia, excessive daytime sleepiness, neurocognitive impairment, and cardiometabolic morbidity. Although laboratory polysomnography remains central to diagnosis, conventional sleep testing can be costly, labor-intensive, and difficult to scale. Recent advances in data mining, machine learning, deep learning, wearable sensing, mobile health, multimodal analytics, and large language models are creating new opportunities for more a

Comprehensive analysis of the single-cell and transcriptomics uncovers the pathogenic network and potential biomarkers related to hypoxia, metabolic reprogramming and atherosclerosis in obstructive sleep apnoea.

This study aimed to identify key biomarkers for obstructive sleep apnoea (OSA) by analysing RNA-seq (GSE75097: 6 control and 42 OSA) and single-cell RNA-seq (GSE214865: 11 control and 11 OSA) datasets. Differential expression analysis revealed 575 DEGs between OSA and control groups. Using weighted gene co-expression network analysis (WGCNA) combined with machine learning approaches (Random Forest, SVM-RFE and LASSO), we identified four hub genes (C17ORF91, HBEGF, PGBD2 and ZNF616) as candidate biomarkers, showing exploratory discriminatory performance (AUC > 0.7). Gene set enrichment analysis

Unsupervised Clustering Analysis of Snoring Identifies Acoustic Biomarkers for the Severity of Obstructive Sleep Apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent public health concern linked to cardiovascular, metabolic, and neurocognitive consequences. Although polysomnography (PSG) is the diagnostic standard, the availability can be limited with long wait time. Snoring is a common symptom of OSA, but all individuals with snoring do not have OSA. In this study, we hypothesized the characteristics of snoring may be used to detect OSA. We applied an unsupervised clustering method to classify snoring patterns and examine their correlations with OSA parameters. METHODS: We enrolled 287 adults with

Vitamin C mitigates early auditory cortical hyperexcitability and antioxidant imbalance induced by acute intermittent hypoxia.

To test whether brief intermittent hypoxia (IH) induces early hyperexcitability and antioxidant imbalance in the primary auditory cortex (Au1) of rats, and whether high-dose vitamin C (VC) pretreatment mitigates these effects. Adult male Sprague-Dawley rats were assigned to four groups (n = 6/group): control, IH, IH plus intraperitoneal normal saline (IH+IPNS), and IH plus intraperitoneal VC (IH+IPVC). Rats underwent a single 3 h IH protocol; VC (500 mg/kg, intraperitoneal injection) or saline was administered 30 min before IH. Three hours after IH, in vivo Au1 multiunit recordings quantified

Redefining PHOX2B-Related Neurodevelopmental Disorders as Circuitopathies: A New Paradigm in Autonomic Network Medicine.

The paired-like homeobox transcription factor 2B (PHOX2B) is a master regulator whose mutations cause a spectrum of severe neurological and neurodevelopmental disorders, including Congenital central hypoventilation syndrome (CCHS), Hirschsprung disease, and neuroblastoma. For decades, a central paradox has challenged the field: how can PHOX2B's seemingly restricted expression in the brainstem account for the broad, multi-system clinical phenotypes observed in patients? Here, we elucidate how this paradox has been resolved by recent methodological advances. We synthesize evidence from multiple

Model-Based Analysis of Pulse Transit Time-Derived Features for the Classification of Obstructive and Central Apneas.

OBJECTIVE: Pulse transit time (PTT) is a promising tool for the non-invasive identification and characterization of obstructive and central apneas in sleep apnea syndrome (SAS). However, a lack of standardized and physiologically explainable features extracted from PTT limits its interpretability and the possibility to perform this classification automatically. METHODS: Features were extracted from PTT and its oscillations ($\Delta$PTT) to characterize their variation during obstructive and central apneas, on a database of 26 patients from the HYPNOS clinical study. They were used to train a

Hypoxemia and arousals modulate cardiac responses to respiratory events in obstructive sleep apnea.

STUDY OBJECTIVES: Respiratory events during sleep induce immediate cardiac alterations, including increased RR intervals during events and decreased RR intervals after events. However, the extent to which related desaturations and arousals modulate these responses remains underexplored. We hypothesized that desaturations and arousals are the main contributors to respiratory event-related cardiac response, with greatest cardiac alterations expected when both are present. METHODS: We analyzed RR, QT, and heart rate-corrected QT (QTc) intervals before, during, and after 4310 respiratory events f
