The respiratory activity (EMGs) of the posterior cricoarytenoid (PCA), cricothyroid (CT) and diaphragm (D) were examined during slow wave sleep (SWS) and paradoxical sleep (PS).-Chronically implanted, free-to-move adult rats were used. In SWS, CT exhibited inspiratory or expiratory or inspiratory and expiratory bursts in each respiratory cycle. The latter was common during CO2 (4%) breathing. PCA manifested phasic inspiratory discharges along with tonic expiratory activity. The latter was augmented by breathing CO2. At onset of PS, inspiratory PCA and CT activity declined. In those PS epochs o

Seventeen patients with sleep apnea were studied with reference to the frequency and duration of apneic episodies, the mechanism of the apnea and the extent to which oxygen saturation was depressed by the apnea. In all patients, esophageal pressure was recorded simultaneously with respiratory magnetometer records of rib cage and abdominal motion. Thermistor records from the nose and mouth indicated the presence of apnea. Nine patients had purely obstructive apnea and eight a mixture of central and obstructive apnea. In all instances the magnetometer records alone permitted distinction between

Six human obese subjects with the sleep apnea hypersomnolence syndrome associated with upper airway occlusion (UAO) were studied during sleep to characterize respiration. Measurements included the timing components of ventilation, pulmonary resistance, flow, and esophageal and gastric pressures before and during UAOs. During the period between UAOs, the resistance progressively increased (9.4-18.1 cmH2O/l(-1) . s, P less than 0.05) as the ventilation decreased (1.82-0.77 l/s, P less than 0.05), but without changes in esophageal pressure swings. During this period, inspiratory time-to-total cyc

Day time somnolence or excessive snoring, or both, occurred in five out of 11 patients with acromegaly. All five had episodes of sleep apnoea, and three had the sleep apnoea syndrome. Growth hormone concentrations were higher (p less than 0.025) in these patients than in the six patients without these symptoms. One patient with daytime somnolence and one asymptomatic patient had flow loop evidence of upper airways obstruction. Two of the patients with the sleep apnoea syndrome had cardiomegaly. Sleep apnoea appears to be common and clinically important in acromegaly, and it may be central, obs

The purpose of this report is to present the findings in a neonate with tetralogy of Fallot and a history of prolonged sleep-related apnea requiring resuscitation. At a baseline arterial oxygen pressure of 46 mm Hg, his preoperative responses to carbon dioxide during non-REM sleep (26.1 and 26.6 ml/kg/min/mm Hg of alveolar carbon dioxide tension [PACO2]) were identical to those reported in near-miss sudden infant death syndrome. Following a systemic-pulmonary arterial shunt, the arterial saturation reached 95 percent, and no further apnea occurred. The prompt normalization of the response to c

Respiratory rates and breathing pauses of various durations were examined in 19 normal infants, 12 male and 7 female, during the first 6 months of life. Twelve-hour all-night polygraphic recordings were obtained during the first week of life and at 1, 2, 3, 4 and 6 months. A computer program and visual analysis were used to score sleep state and calculate respiratory rates and apnea densities. In the newborn period there were no gender differences. Males breathed faster and exhibited fewer breathing pauses between 2 and 5 sec from one month of age on. The close correlation between respiratory

Obstructive sleep apnea is becoming more widely recognized. Otolaryngologists will be called upon to evaluate affected patients and offer consultation as to the appropriateness of surgical intervention. The extent to which this syndrome is attributable to edema of the larynx and hypopharynx is unknown. It may, on occasion, explain chronic fatigue, weight loss, and daytime somnolence seen in some patients following irradiation of the head and neck. Sleep apnea associated with upper airway obstruction can be confirmed by polysomnography during sleep. Monitoring the EEG, electro-oculogram, electr

Fourteen children with adenoid and tonsillar hyperplasia were studied for one night in the sleep laboratory. Six of the children had at least 40 apneic episodes per night, mostly central or obstructive. The largest number of episodes was found in rapid eye movement sleep. Mixed episodes were the longest and central episodes the shortest. Adenotonsillectomy was performed on children with at least 40 episodes and on children with predominantly obstructive episodes. In two of these children, preoperative and postoperative recordings were taken, revealing a reduction in the total number of apneic

Subsequent siblings of infants who died of the Sudden Infant Death Syndrome are at a four- to six-times increased risk to die of this syndrome. This study compares the respiratory development during sleep state of this epidemiologic high risk group with that of normal infants during the first six months of life. Subsequent siblings exhibited higher respiratory rates in all states at 3 months of age. Quiet sleep and indeterminate respiratory rates were elevated at 1 week of age compared to control infants. Indeterminate respiratory rates remained higher at 6 months of age. These differences wer

Congenital central hypoventilation (Ondine's curse) is described in an infant with persistant symptoms throughout the first nine months of life. Respiratory control was most severely affected in quiet sleep, although abnormalities were present in rapid eye movement (REM) sleep and while awake. Failure of metabolic control in quiet sleep led to profound hypoventilation. Behavioral or "behavioral-like" inputs in the awake state and REM sleep increased ventilation, but not to expected normal levels. The ventilatory response to inhaled 4% CO2 was markedly depressed in all states.

A 4-week-old infant experienced prolonged central sleep apnea requiring resuscitation. At 6 months of age he developed episodic obstructive apnea (diagnosed as laryngospasm by direct laryngoscopy) with an abnormal EEG and a normal computed tomography (CT) scan, and at 14 months of age he developed hemiparesis due to a gemistocytic astrocytoma grade III-IV. Following removal of the mass, he has had occasional seizure activity, but no further episodes of obstructive apnea.

Five patients with severe kyphoscoliosis were studied during sleep. A spectrum of breathing abnormalities was found ranging from no abnormalities to severe episodes of prolonged central apnea. Rapid-eye-movement (REM) sleep was the period of greatest physiologic disturbance in all the patients, being the time of greatest oxygen desaturation. Within this small group, those with the most clinical evidence of chronic hypoxemia, polycythemia, and cor pulmonale, had the most severe derangements during sleep. Derangements in breathing pattern and arterial oxygen saturation (SaO2) had no apparent rel

We have found that skin surface electrodes for continuously measuring oxygen (PSO2) and carbon dioxide (PSCO2) tensions provide reliable means of determining whether infants with apnea due to airway obstruction during sleep require treatment. Nine patients referred for evaluation of aqnea during sleep were studied. In 6 patients with PSO2 less than 50 torr during sleep, the hypoxic episodes were clearly associated with partial or total airway occlusion as judged by increasing respiratory effort without concomitant airflow. PSCO2 was found to increase during the episodes of obstruction in all 6

Bilateral diaphragmatic pacing was successfully performed in an 18-month-old child with congenital central hypoventilation syndrome (Ondine's curse) as an alternative to long-term mechanical ventilation. Subsequent complications were related more to cor pulmonale and tracheostomy care than to the pacing itself.

Two infants, both born before term, were found apneic, cyanotic and limp at home when they were 9 weeks and 2 weeks old, respectively. Their respiration was monitored in the hospital and found to be remarkably periodic during sleep, and was in one case accompanied by pronounced bradycardia. The periodic breathing and apnea seemed to be caused by a decreased oxygen tension which can induce an instability of the central respiratory control mechanisms.

Apneic episodes in normal 10-, 20-, and 40-day-old kittens were assessed with polygraphic recordings. End expiratory apneas, usually preceded by somatic activity and/or augmented breaths, with durations less than 10 sec were observed in quiet sleep, active sleep, and transitions between states in all age groups. The highest apnea density was found at state transitions. Heart rate decelerations occurred before, during, and following apneas, but decelerations were not related to apnea duration. Combined central and obstructive components were associated with 9% of apneas in normal kittens.

A three year old with sleep apnea due to adenotonsillar hypertrophy is presented. The picture improved after treatment with antibiotics and corticosteroids, and disappeared after adenotonsillectomy. The symptomatology is studied in relation to primary alveolar hypoventilation and the "sudden death" syndrome in infants. The need of an early diagnosis is emphasized to avoid onset of an irreversible "cor pulmonale". It is suggested that aside from a probable predisposing "central" and constitutional factor in children with this syndrome, apparently there are heterogeneous electroencephalographic
