Neonates with ductal-dependent congenital heart disease (CHD) will receive a continuous infusion of a prostaglandin E1 analogue such as alprostadil prior to surgical correction. Alprostadil can cause apnoea, which may be mitigated in practice with the use of caffeine citrate. Caffeine citrate can lead to tachyarrhythmias, which could be detrimental in this patient population. The purpose of this study was to determine if the practice of utilising caffeine citrate in neonates with ductal-dependent CHD receiving continuous alprostadil infusions is safe. In this single-centre, retrospective, coho

BACKGROUND: Nonketotic hyperglycinemia is a rare metabolic disorder caused by glycine accumulation due to defects in the glycine cleavage system. While severe metabolic disorders can theoretically affect fetal growth, nonketotic hyperglycinemia is not recognized as a common or typical cause of symmetric intrauterine growth retardation.
CASE PRESENTATION: This case report describes a Iranian 36-week male neonate with symmetrical intrauterine growth restriction, born to consanguineous parents with a history of preeclampsia and prior fetal demise. Despite initial stabilization, the infant develo

The criteria for Brief Resolved Unexplained Events (BRUE) are used to stratify the risk in infants presenting with apneic episodes. However, these criteria often classify a high proportion of infants as higher risk. This study aimed to investigate the etiology of apneic episodes, particularly those compatible with BRUE, and to assess whether age ≤60 days is associated with serious underlying conditions. We conducted a retrospective study of infants aged <1 year who presented with apnea as the chief complaint or had an ICD-10 code P28.4 (Other apnea of newborn) recorded between 2014 and 2018 at

BACKGROUND: Less invasive surfactant administration (LISA) is now a routinely practised method of administration of surfactant to neonates with respiratory distress. Many centres routinely use analgesia and/or sedation premedication prior to the LISA procedure; however, many different drugs are presently used, and the optimal premedication drug is unknown. We aimed to assess the efficacy and short-term safety of routine midazolam 50-100 µg/kg use as premedication for LISA.
METHODS: This was a prospective single-centre audit of routine midazolam use as premedication before the LISA procedure o

We report a case of a term male neonate, born via repeat cesarean section (CS), who developed transient tachypnea of the newborn (TTN) shortly after birth, followed by two episodes of bradycardia with desaturation and apnea/bradycardia/desaturation on days of life (DOL) three and four, respectively, temporally linked to maternal oxycodone use during breastfeeding. The infant was transitioned from maternal breast milk to donor milk until the mother's oxycodone dose was below 40 mg/day. This report highlights the importance of careful monitoring for respiratory depression and bradycardia in neon

Obstructive sleep apnea (OSA) affects approximately 5% of children and requires polysomnography (PSG) for diagnosis. As such, population-based longitudinal studies of OSA in children are scarce. We aimed to validate pediatric case definitions of OSA using provincial health administrative datasets and establish a methodology for future longitudinal studies in this population. We performed a multicenter validation study, linking Ontario health administrative data with clinical data for children aged 0-18 years who underwent PSG between 2009-2016 at two tertiary care children's hospitals. We used

UNLABELLED: Infants with Robin sequence (RS) are at high risk for obstructive sleep apnea (OSA), but screening with poly(somno)graphy (P(S)G) is challenging. The aim of the study was to assess the relationships between different pulse oximetry (SpO2) parameters and the mixed obstructive apnea-hypopnea index (MOAHI) in these infants. We also compared the findings to infants having other comorbidities (OSA-III). Different SpO2 parameters were correlated with MOAHI in infants with RS and with OSA-III having similar OSA severity. The SpO2 parameters were also compared between the 2 groups. Half of

Early institutions of enzyme replacement therapy (ERT) for infantile-onset Pompe disease (IOPD) greatly improved patients' outcome. However, the long-term prognosis of patients' muscular and respiratory systems has not been fully elucidated. We conducted a longitudinal study for a cohort of early-treated IOPD patients with continuous monitoring of motor and respiratory functions (spirometry and polysomnography). Statistical analyses were performed for the relationship between Gross Motor Function Measure (GMFM), forced vital capacity (FVC), apnea-hypopnea index (AHI), and oxygen desaturation i

BACKGROUND: Seizures and apneas are always concerning symptoms in near-term and term newborns, but finding a clear explanation is not always easy. In addition, it is not always clear which investigations should be promptly conducted. We aimed to evaluate the relationship (or lack thereof) between apneas and seizures and the role of MRI and EEG in their investigation. METHODS: We conducted a retrospective analysis including infants born at ≥ 36 + 0 weeks of gestational age who presented with seizures or apneas soon after birth and underwent brain magnetic resonance imaging (MRI) during their st

RATIONALE: Sucrose is, in general, safe and effective for analgesia during venepuncture in hospitalised neonates. However, there is a lack of evidence on its analgesic effects.
OBJECTIVES: To evaluate the benefits and harms of orally administered sucrose for pain relief from venepuncture in preterm and term neonates compared to no intervention, standard care, and other types of analgesic interventions.
SEARCH METHODS: We searched CENTRAL, MEDLINE, Ovid Embase, and trial registries in July 2025, and the China National Knowledge Infrastructure, VIP Chinese Science and Technology Periodicals, a

BACKGROUND: Mutations in the human mitochondrial genome have been suspected to play a significant role in the etiological development of mitochondrial diabetes. Detection of the 3243A>G mutation in the mitochondrial transfer RNALeu(UUR) gene (MTTL1), especially at low heteroplasmy levels, is highly desirable since it facilitates the diagnosis and subsequent management of the disease. The proportions of mutant mitochondrial DNA (mtDNA) can vary between tissues and are usually significantly higher in muscle than in blood, but muscle biopsies from patients with diabetes are rarely available.

OBJECTIVE: Neonatal developmental and epileptic encephalopathy with movement disorder and arthrogryposis (NDEEMA) represents the most severe end of the gain-of-function (GOF) SCN1A disorder spectrum. Sporadic cases of congenital arthrogryposis have also been reported in individuals with SCN2A-, SCN3A-, and SCN8A-related developmental and epileptic encephalopathy. Here, we investigated whether NDEEMA occurs in other brain-expressed sodium channelopathies and characterized its features.
METHODS: Individuals with the clinical phenotype of NDEEMA were identified through internal databases, an int

UNLABELLED: Laser photocoagulation for retinopathy of prematurity (ROP) in preterm infants causes significant procedural pain. Although general anaesthesia (GA) is the standard of care, many settings frequently rely on inadequate sedation-analgesia or topical anaesthesia alone, exposing neonates to considerable pain and clinical instability. This systematic review aimed to evaluate the efficacy and safety of procedural sedation-analgesia during laser photocoagulation for ROP. We searched MEDLINE, Embase, and Cochrane CENTRAL for trials investigating sedation-analgesic agents during laser photo

BACKGROUND: Electroencephalogram (EEG) interpretation is essential for neurological diagnosis, but expert interpretation is limited globally, and existing AI methods address narrow tasks. This study aimed to develop and externally validate a broadly applicable foundation model capable of expert-level performance across diverse EEG tasks and clinical settings.
METHODS: In this multicentre study we developed a multidomain omnibus for reading and generalising over thorough EEG interpretation (MORGOTH), a foundation model that supports broad clinical interpretation across all major settings. We d

Infants born prematurely often develop apnea of prematurity (AOP), characterized by periodic apneas with intermittent hypoxia (IH) and commonly treated with supplemental oxygen. On the other hand, infants born at high altitude experience sustained hypoxia from birth. Thus, during critical developmental period, many infants experience abnormal oxygen environments, including hypoxia or hyperoxia. Emerging evidence indicates that disrupted neonatal oxygen homeostasis can produce long-lasting effects on cardiorespiratory function. Preterm infants also exhibit systemic inflammation, and elevated in

RATIONALE: The vestibular system is a sensory system within the inner ear and the brain that controls balance and physical coordination. Vestibular stimulation may have an impact on various physiological processes, including breathing, reducing apnea episodes and the need for respiratory support. It has been suggested that targeted vestibular stimulation in premature infants may improve neurodevelopmental outcomes and neuromotor function.
OBJECTIVES: To evaluate the benefits and harms of vestibular stimulation for reducing morbidity and promoting development in hospitalized preterm infants.
