Methylxanthines for the prevention or treatment of intermittent hypoxemia or respiratory insufficiency in late preterm infants.

RATIONALE: Late preterm infants (34 0/7 to 36 6/7 weeks' gestation, i.e. 34 weeks and 0 days to 36 weeks and 6 days) are at increased risk of intermittent hypoxemia and respiratory insufficiency due to physiological immaturity, compared to full-term infants. Methylxanthines, most frequently caffeine, are used to improve respiratory function and long-term outcomes in very preterm infants. However, the use of methylxanthines in late preterm infants for preventing or treating intermittent hypoxemia and respiratory insufficiency needs to be assessed, also to avoid the risk of over-treatment. Hemodynamic Assessment in Newborn Infants With Sepsis: A Prospective Observational Study.

BACKGROUND AND OBJECTIVE: Sepsis is defined as SIRS (systemic inflammatory response syndrome) in the presence of confirmed or suspected infection. Neonatal sepsis remains a major cause of morbidity and mortality and is frequently complicated by septic shock, resulting in impaired tissue perfusion and organ dysfunction. Hemodynamic alterations in septic neonates are heterogeneous and often under-recognized. The primary objective of this study was to characterize the hemodynamic profile of neonates with sepsis for early detection of circulatory dysfunction using functional echocardiography. The

Stimulation of growth in Piper betle L. via induced systemic resistance (ISR) under combined pathogenic stress through priming of Jasmonic acid.

BACKGROUND: Choosing an orthopedic surgeon is a pivotal decision shaped by clinical competence and multiple non-clinical influences. While surgical expertise and outcomes are fundamental considerations, patients and their families also weigh communication quality, cultural alignment, institutional reputation, financial constraints, and increasingly, digital information sources. Understanding these determinants is crucial to delivering patient-centered orthopedic care. METHODS: A scoping review was conducted following Arksey and O'Malley's framework and reported in accordance with PRISMA-ScR g

A comprehensive review of caffeine population pharmacokinetics in preterm infants: Factors affecting clearance.

Caffeine is an FDA-approved drug for preventing and treating apnea in preterm infants. However, the pharmacokinetic (PK) characteristics of caffeine in preterm infants differ significantly from those in adults. Several population pharmacokinetic (PopPK) models have been developed to investigate potential covariates influencing PK parameters. This review aimed to summarize PopPK studies of caffeine in preterm infants and explore the identified influencing covariates. It has been observed that most caffeine pharmacokinetics followed a one-compartment model (1-CMT), although one study utilized a

Double-blind, non-inferiority, randomized controlled trial of dexamethasone 4, 5 and 6 mg for preventing adverse neonatal and maternal outcomes in very preterm to late preterm pregnancies between 29 0 and 36 6 weeks of gestation: study protocol.

BACKGROUND: Premature birth poses significant health challenges, including respiratory distress syndrome (RDS). Corticosteroids reduce the incidence of RDS, but higher dexamethasone doses may lead to adverse neonatal outcomes, such as growth restriction and neurodevelopmental issues. Determining the appropriate dose is crucial to balance efficacy and safety. Dexamethasone is inexpensive and widely available in most low- and middle-income countries. This study aims to compare the efficacy and safety of 4-mg, 5-mg and 6-mg dexamethasone in preventing RDS among preterm infants. This trial aims to

Safety and efficacy of proton pump inhibitors in preterm infants with gastroesophageal reflux disease.

BACKGROUND: Although physiological reflux is seen in nearly all newborns to varying degrees, symptoms can be severe and cause gastroesophageal reflux disease (GERD). In preterm infants, one symptom that is often attributed to GERD is apnea and associated cardiorespiratory events, such as bradycardia and oxygen desaturation. Although the relationship between GERD and apnea, bradycardia, and desaturation events remains a subject of ongoing investigation, trials of agents that reduce gastric acidity, such as proton pump inhibitors (PPI), have been conducted to assess the effect of these agents on

Effectiveness of nasal high-flow oxygen during apnoea on hypoxaemia and intubation success in paediatric emergency and ICU settings: a randomised, controlled, open-label trial.

BACKGROUND: The use of nasal high-flow (NHF) oxygen for apnoeic oxygenation during emergency paediatric intubation is not universally adopted. Although previous studies suggest potential benefits, it remains unclear whether NHF enhances the likelihood of achieving successful first-attempt intubation without oxygen desaturation in children. We aimed to investigate whether the provision of NHF oxygen during paediatric emergency intubation can improve intubation outcomes. METHODS: We conducted a randomised, controlled, open-label trial at ten hospitals in Australia, New Zealand, and Switzerland
