Simulated kangaroo care in very preterm infants does not reduce physiological instability: the COSYBABY randomised controlled cross-over trial.

INTRODUCTION: Infants who are born very preterm experience frequent episodes of physiological instability including apnoea, oxygen desaturation and bradycardia due to immaturity of the pulmonary and nervous systems. Parental contact, such as kangaroo care, may reduce physiological instability. However, there may be long periods when parents cannot be with their baby. The BABYBE SYSTEM® is a medical device designed to simulate kangaroo care. METHODS: We conducted a randomised cross-over trial to determine whether episodes of apnoea and other episodes of physiological instability were reduced w

Sleep quality measured by cardiopulmonary coupling spectrogram in pregnant women predicts neonatal outcomes.

STUDY OBJECTIVES: We assessed the impact of objective sleep quality during pregnancy, estimated using cardiopulmonary coupling sleep spectrograms, on neonatal outcomes. METHODS: We conducted a secondary analysis of the Nulliparous Pregnancy Outcomes Study: Monitoring Mothers-to-Be (nuMoM2b). Cardiopulmonary coupling analysis was done using the raw photoplethysmogram signal within the oximetry sensor (photoplethysmogram) from a home apnea sleep test. High- and low-frequency coupling reflect stable and unstable sleep, respectively. A multicomponent Sleep Quality Index integrates sleep stability

Caffeine and preterm infants: multiorgan effects and therapeutic creep: scope to optimise dose and timing.

Caffeine is a methylxanthine used for nearly 50 years in the treatment of apnoea of prematurity (AOP). Caffeine citrate is effective in the treatment of AOP using standard dosing (loading dose 20 mg/kg, maintenance 5-10 mg/kg/day) and is associated with long-term neurological benefits and other improved organ outcomes as well as immunomodulatory effects. Therapeutic creep has been noted in the use of caffeine in preterm infants differing from the criteria in randomised controlled trials. A Cochrane review showed insufficient evidence to support prophylactic use of caffeine citrate in preterm n

Unusual Presentation of Necrotizing Enterocolitis Presenting in a Term Infant.

Necrotizing enterocolitis (NEC) is a gastrointestinal emergency causing inflammation and necrosis of the intestine, most commonly in premature formula-fed infants. There exists limited conflicting data on the pathophysiology behind NEC development and the underlying intestinal bacteria behind the disease. We report a case of a two-week-old female patient who was born full term. During her birth hospitalization, she was transferred to the neonatal intensive care unit for late-onset tachypnea and apnea, where she received ampicillin and gentamicin intravenously through a peripheral line for a to

Visualizing natural sleep state during nasolaryngoscopy in infants: A feasibility study.

INTRODUCTION: Otolaryngology consultation for infant airway obstruction is common, with nasolaryngoscopy being the primary diagnostic tool. However, traditional awake nasolaryngoscopy is often limited by infant crying and brief examinations. A more detailed drug-induced sleep endoscopy requires anesthesia and operating room resources. This study explores Time-Induced Sleep Endoscopy (TISE) as a relatively brief, non-invasive alternative for evaluating sleep-related airway dynamics in infants. METHODS: This prospective study enrolled infants under three months of age at UPMC Children's Hospita

Management of Apnoea in Extremely Preterm Infants: A European Survey.

INTRODUCTION: Episodes of apnoea are common in extremely preterm infants (EPIs) and usually treated with caffeine and respiratory support. Understanding differences in apnoea definitions, monitoring practices, and use of respiratory stimulants is essential to improve future treatment. METHODS: Between March and July 2024, one lead consultant at European tertiary neonatal intensive care units (NICUs) was invited to complete a web-based survey on respiratory practices in EPIs. We sought information how they defined apnoea and monitored for it, and how they treated it with caffeine, doxapram, an
