Recovery from central apnea: effect of stimulus duration and end-tidal CO2 partial pressure.

The present study was designed to investigate the effect of stimulus duration and chemosensory input on the recovery of central respiratory activity from apnea induced by superior laryngeal nerve (SLN) electrical stimulation. Newborn piglets less than 8 days of age were anesthetized, paralyzed, and mechanically ventilated at differing levels of end-tidal CO2 partial pressure (PCO2). The vagi were cut bilaterally in the neck. Integrated phrenic nerve activity was used as the index of respiratory activity. SLN stimulation caused apnea that persisted after stimulus cessation. The length of apnea

Chemical specificity of a laryngeal apneic reflex in puppies.

In neonatal mammals the introduction of water and some other fluids into the larynx causes prolonged reflex apnea by stimulation of afferents in the superior laryngeal nerve (SLN). We have studied the chemical specificity of this reflex in 1- to 9-day-old anesthetized puppies. The laryngeal lumen was perfused with a variety of substances while ventilation through a tracheal cannula was recorded. Water consistently elicited apnea, which was terminated by 150 mM NaCl. Sucrose and urea solutions (100-500 mM) also elicited apnea, suggesting that osmolarity is not a critical factor. Phosphate buffe

Factors inducing periodic breathing in humans: a general model.

A general model is developed to account for all kinds of periodic breathing (PB) resulting from instability in respiratory control: in normals during sleep and on acute exposure to high altitude, in sleeping infants, and in patients with cardiovascular or neurologic lesions. It is found that in almost every case the ventilatory oscillation is mediated predominantly by the peripheral controller. System stability is decreased by hypoxia, hypercapnia, increased lung washout times, prolonged lung-chemoreceptor delays, and high controller sensitivity. Stability is enhanced by large lung CO2 and O2

[Neuropsychic follow-up of the premature infant: mortality, severe sequelae, early diagnosis and prognosis].

Neurological prospective follow-up study of premature newborns must be carried out to determine the usefulness and efficacy of modern perinatal intensive care. Over the last 30 years an improvement in perinatal care has led to a dramatic reduction from 70% to 20%-40% in mortality rate among VLBW infants (less than or equal to 1500 g); while among those weighing less than or equal to 1000 g the mortality rate has dropped below 60% in some intensive care units. The concentration of high-risk pregnancies and deliveries in the same hospital in which the premature is cared for has led to a further

Apneic seizures in the newborn.

Electroclinical features of convulsive apnea and its relation to the behavioral state were described on the basis of polygraphic recordings from 21 newborns with various underlying disorders, including perinatal anoxia, purulent meningitis, and intracranial bleeding. The most frequent ictal discharges were rhythmic alpha waves, but other types of discharges, such as repeated sharp waves, rhythmic theta waves, delta waves, and repeated paroxysmal wave complexes, were also frequently seen. The area where the ictal discharges initially occurred or were most prominent was the temporal area, sugges

The cardiovascular response to laryngeal water administration in young lambs.

The cardiovascular response to administration of water to the larynx was studied in unanesthetized newborn lambs. The cardiovascular components of this laryngeal reflex response consisted of a decrease in heart rate associated with a decrease in cardiac output, an increase in systemic and pulmonary vascular resistance associated with increased blood pressure and blood flow redistribution. Blood flow decreased in carcass, kidneys, intestines, liver and spleen and increased in head, brain and heart. The magnitude of the separate components of the cardiovascular response correlated significantly

Tissue distribution of caffeine in premature infants and in newborn and adult dogs.

Caffeine concentrations were measured in postmortem tissues of two premature infants who had received caffeine for the prevention of apnea. The distribution of caffeine was not uniform: Up to a 2.1-fold difference in caffeine concentrations existed among the various tissues in one infant and up tu a 4.3-fold difference in the other. This variability was only slightly reduced when caffeine concentrations were corrected for the water content of the respective tissues. Caffeine was also analyzed in tissues of newborn and adult dogs 3, 10, and 36 hr after single doses of caffeine (50 mg/kg IV). Th

Theophylline metabolism in premature infants.

The theophylline metabolite pattern in premature infants was studied with gas chromatography-mass spectrometry. The identities of metabolites were established by retention time indices and mass chromatograms. In the steady state of a multiple-dose regimen, the urinary metabolites of theophylline identified and quantified were caffeine (9.6 plus or minus 4.8%), theophylline (50.4 plus or minus 6.7%), 3-methylxanthine (1.3 plus or minus 0.7%), 1,3-dimethyluric acid (27.7% plus or minus 8.8%), and 1-methyluric acid (9.3 plus or minus 5.4%). Those in plasma were caffeine (21.4 plus or minus 6.1%),

Sleep-related breathing disorders.

Disorders of breathing related to sleep are relatively newly recognized and less than fully understood. This review presents the terminology used to describe them, and describes the physiology of sleep and the control of ventilation, the pathophysiology of breathing disorders during sleep, their various clinical manifestations, current diagnostic techniques, and the treatment modalities available at present. Among the diagnostic approaches discussed are airway fluoroscopy during sleep, pneumography, and polysomnography. Approaches to medical and surgical management of these disorders are revie

The efficacy of caffeine in the treatment of recurrent idiopathic apnea in premature infants.

This prospective controlled study was aimed at evaluating the efficacy of caffeine in treating recurrent idiopathic apnea in the premature infant. Eighteen preterm infants (29 to 35 weeks' gestation) were studied. Recordings during the first 24 hours and on the fifth day of caffeine treatment showed a significant decrease of severe apnea (P less than 0.01) and of mild apnea (P less than 0.001) in the treated group (group 1) as compared with the control group (group II). No treatment of apnea other than caffeine was required in group I, whereas six neonates in group II had such severe and frequ

Cervical tumors of infancy and childhood.

Primary spinal cord tumors of childhood and adolescence affect the cervical segment with the same frequency as other segments. Apart from epidermoids and dermoids, all oncotypes are represented. Astrocytomas and lipomas seem to prefer these metameres. Differential diagnosis has been habitually problematic for cord compression in children. Cervical tumors are too often mistaken for congenital torticollis, birth palsy, cerebral palsy and posterior fossa tumors. Of the various diagnostic aids, CT is of great importance and in selected cases so is vertebral angiography. An aggressive surgical stra

[Local manifestations of neonatal group B streptococcal disease (author's transl)].

In the case of neonatal group B streptococcal disease, two distinct types of illness have been described in the past few years. The early onset type occurs in the first hours of days of life and emerges as septicemia. The late onset type occurs after the first week and emerges as meningitis. Other frequent local manifestations are pneumonia and pleural effusions, but there are few reports on localized inflammation of other organs. This paper gives a brief survey of the local manifestations of the disease. In addition, a case of a 1120 g premature of 29 gestational week is reported. After ten u

Low-dose theophylline therapy in idiopathic apnea of prematurity.

The effects of theophylline at a dose of 2 mg/kg/day (with resultant serum concentrations of 2.8 to 3.9 micrograms/ml) were observed in seven premature infants with severe idiopathic apnea. The frequency of apneic episodes decreased 62 +/- 9% (SEM) and the frequency of episodes of bradycardia decreased 73 +/- 9% following 24-48 hours of this therapy. No changes in ventilatory response to CO2 resting respiratory minute volume, arterial blood gas values, resting heart rate, or serum electrolyte concentrations were observed. Theophylline at this dose appears to be a safe and effective treatment f

Pneumocystis carinii pneumonitis in young immunocompetent infants.

Of 67 infants enrolled in a prospective study of infant pneumonia ten (14%) had evidence of Pneumocystis carinii infection. Diagnosis was achieved by demonstrating circulating P carinii antigens by counterimmunoelectrophoresis in all ten cases and by histopathology in the only infant who underwent an open lung biopsy. Antigenemia did not occur in 64 control infants (P = .003), nor in 57 patients of similar age who were hospitalized with pneumonitis due to Chlamydia trachomatis, respiratory syncytial virus, cytomegalovirus, adenovirus, and influenza A and influenza B viruses. Respiratory patterns and risk of sudden unexpected death in infancy.

The overnight respiratory patterns of 25 infants at high risk and 42 infants at low risk of sudden unexpected death were studied during the first 6 months of life. 'Risk' was determined using the birth scoring criteria of Carpenter and Emery. Recordings were made at home using a twin-channel radar chest movement detector which was designed to avoid the need for contact with the infant. The recordings were analysed for respiratory frequency, total duration of apnoea, periodic breathing, duration of regular breathing, and amount of body movement. The high risk group of infants showed respiratory

Special considerations in oxygen therapy of infants and children.

There are several unique aspects of O2 therapy in infants. Inhalation of O2 by preterm infants decreases the frequency of apnea and cyanosis, and increases the ventilatory response to CO2, but the reasons for this are unclear. Immature infants receiving O2 therapy are subject to retinopathy, but we do not know the magnitude or duration of hyperoxia necessary to damage the developing retina. Newborns with persistent pulmonary hypertension, without radiographic signs of pulmonary disease, frequently remain hypoxemic despite breathing 100% O2. In these infants, the unresponsiveness of teh postnat

Theophylline reduces the activity of cerebroside-sulfotransferase, a key enzyme in myelination, in cell cultures from newborn mouse brain.

Theophylline, a drug used in neonatology for the treatment of apnea, affects cholesterol synthesis if administered in concentrations of 10(-4) M (a concentration found in serum of treated patients) for 24 hr to dissociated brain cell cultures. The rate-limiting enzyme of cholesterol synthesis, beta-hydroxy-beta-methylglutaryl-coenzyme A reductase (EC 1.1.1.34), is lowered to 45% 48 hr after removal of theophylline. At the same time, cholesterol content of the cells is lowered to 73%. Inasmuch as the phospholipid content of the cells remains stable, the treatment changes the cholesterol phospho

Tracheal compression as a cause of apnea following repair of tracheoesophageal fistula: treatment by aortopexy.

Tracheal compression is one of the many causes of apnea following repair of esophageal atresia with tracheoesophageal fistula. Since May, 1977 we have treated eight patients with "dying spells" usually occurring during and following a feeding. These episodes were characterized by cyanosis and bradycardia, which progressed to apnea. Tracheal compression was documented to be the etiology of the apnea spells. Other possible causes such as aspiration from gastroesophageal reflux, recurrent tracheoesophageal fistula and esophageal stricture were eliminated. Eight infants underwent aortopexy that wa

Preferential distribution of lobar emphysema and atelectasis in bronchopulmonary dysplasia.

The chest roentgenograms of 142 neonates who survived mechanical ventilation for respiratory distress syndrome (N = 99) and prolonged apnea (N = 43) were reviewed. Thirty-seven infants had bronchopulmonary dysplasia (BPD) and 17 of these developed lobar hyperinflation of the right lower lobe and collapse of the right upper lobe. Regional lung function was measured with a xenon 133 technique in three of these infants and in five other patients who either died or were lost to follow-up. All had BPD with right lower lobe overinflation. Ventilation was less in the lower regions than the upper regi

Fetal breathing: its physiologic and clinical implications.

The respiratory movements the human fetus makes in utero can be detected and used as a valuable aid in determining fetal well-being. Their presence or absence, amplitude, and duration can all provide important information. Their absence, however, does not carry a poor prognosis unless other indices of fetal status, such as the nonstress test and oxytocin-challenge contraction-stress test, are also abnormal at the same time.
