Persistent pulmonary hypertension of the newborn infant.

Persistent pulmonary hypertension of the newborn infant can be difficult to distinguish from other cardiopulmonary causes of cyanosis during the newborn period. Infants with PPHN have cyanosis, tachypnea, acidemia, normal pulmonary parenchymal markings on the chest radiography, and anatomically normal hearts. We have identified and treated 11 infants and have noted several signs and symptoms not previously emphasized. These are cineangiocardiographic evidence of atrioventricular valve insufficiency in association with systolic murmurs and slow ventricular emptying, apnea, hypocalcemia, only a

The magnesium load test: III. correlation of clinical and laboratory data in infants from one to six months of age.

A study of 64 infants from one to six months of age was made using the 32-hour parenteral magnesium load test. Most of the infants were studied for neuromuscular hyperirritability or other signs compatible with magnesium deficiency; some with unrelated problems served as controls. Magnesium retention below 40 per cent was found in 26 infants who presented with minor sign or signs that were otherwise explained, as by infection. Twelve who retained 72 per cent of the load were normal or small at birth, amply fed on demand, and grew at accelerated rates, increasing from the 50th to the 88th mean

Continuous positive airway pressure and mechanical ventilation by facemask in newborn infants.

During a nine-month period 24 newborn infants were treated with continuous positive airway pressure (CPAP) or mechanical ventilation delivered through a facemask. The mask was held in place in a way that minimised trauma and distortion of the head. The median birth weight of the infants was 1096 g and their median gestational age 29 weeks. The usual reason for treatment was hyaline membrane disease or recurrent apnoea due to inadequate control of breathing. Twenty-one of the infants survived. The technique was simple to apply and complications were minimal. We suggest that it may have advantag

Central and peripheral neural respiratory activity in the mature sheep foetus and newborn lamb.

Activity from respiratory neurones in the medulla, the phrenic and intercostal nerves was recorded in 25 foetal sheep, exteriorized a few days before term from ewes given a spinal anaesthetic, and from nine newborn lambs, anaesthetized with an allobarbitone-urethane mixture. In 12 foetuses, there was little or no sustained respiratory activity, central activity consisting of tonically discharging expiratory and other neurones and silent inspiratory neurones. In the remaining 13 foetuses, respiratory activity was periodic or continuous and it was possible to confirm that the motor component of

The effects of four different radiant warmer temperature set-points used for rewarming neonates.

Temperatures for rewarming neonates have been established for convection incubators. The purpose of this study was to evaluate the effects on neonates when the skin surface was rewarmed at one of four different levels with an infra-red warmer. A total of 42 normal term neonates were first divided into four groups according to deep rectal temperature on admission; infants in each of the respective groups were then allotted to one of the following skin surface temperature set-point groups: 35, 36, 37 or 38 degrees c for rewarming under a servo-controlled infra-red heat source. Although rewarming

Coagulation studies in preterm infants with respiratory distress and intracranial haemorrhage.

Coagulation studies were performed on 48 preterm infants with respiratory distress or recurrent apnoea who were considered to be at risk from intracranial haemorrhage. One or more coagulation abnormalities were detected in 16 cases though only one infant had a bleeding diathesis in life. Coagulation abnormalities were more frequent in infants subsequently found to have intracranial haemorrhage (intraventricular or subarachnoid haemorrhage) than in those with hyaline membrane disease only. Of 26 infants who died, 6 had hyaline membrane disease only, 17 had intracranial haemorrhage (usually intr

Apnoea after THAM administration in the newborn.

The use of THAM in the treatment of neonatal acidaemia is surveyed over a 3-year period. Of 100 babies to whom the drug was given while they were breathing spontaneously, 17% had respiratory depression during, or within two minutes of injection. If only potentially viable babies are considered, the incidence of respiratory depression increases to 20% and the incidence in babies with respiratory distress is 17%. No single group of babies was particularly susceptible to this action of THAM, except those who had had a previous spontaneous apnoeic attack. No babies became apnoeic after the injecti

THE NEUROLOGICAL AND DEVELOPMENTAL EFFECTS OF NEONATAL HYPOGLYCEMIA: A FOLLOW-UP OF 22 CASES.

Twenty-two infants in whom hypoglycemia (blood sugar less than 20 mg./100 ml.) was noted during the first few days of life were followed up when eight to 30 months of age. In eight such symptoms as muscular tremors, cyanosis, apneic spells and convulsions were associated with the hypoglycemia; five of these had abnormal central nervous system signs and retarded development. One other had possible impairment of development and another had a recurrence of hypoglycemia after having been well for four years. Fourteen of the 22 infants had no symptoms associated with the hypoglycemia, and on follow

Hypoglycemia associated with symptoms in the newborn period.

Hypoglycemia in the neonatal period is a well-recognized phenomenon, but many authors have commented upon the infrequent association of symptoms attributable to it. Six infants were seen who appeared normal at birth but who, between 24 and 72, hours of age, developed apnea, irritability, lethargy, muscular twitchings and convulsions. Blood sugar concentrations of 10 mg./100 ml. or less were found in each case. The mothers of four of the babies had toxemia of pregnancy. Three babies were premature. The hypoglycemia was self-limiting in all cases, and four of the babies recovered completely with
