The emergence of antimicrobial resistance during therapy is often monitored solely at the site of clinically apparent infection, potentially overlooking cryptic evolution in unexamined anatomical reservoirs. We report a case of acute cholecystitis caused by a virulent Salmonella enterica serovar Paratyphi B. While the bile isolate was susceptible to β-lactams, a fecal isolate obtained on day 7 of otherwise successful systemic therapy harbored an IncI1-ST1 plasmid encoding blaCTX-M-14, conferring extended-spectrum β-lactamase resistance. Whole-genome sequencing confirmed the isolates were clona

The typhoid toxin is a secreted virulence factor of typhoidal serovars of the bacterial pathogen Salmonella enterica implicated in typhoid fever and chronic infections. The toxin causes a DNA damage response in human cells, characterised by cell-cycle arrest and cellular distension, resulting in cellular senescence and increased bacterial burden. To better understand host responses to typhoid toxin, we performed a transcriptomic analysis of intoxicated host cells and found that the toxin induced expression of genes relating to the type-I interferon response, including the ubiquitin-like protei

Intrahepatic cholestasis of pregnancy is a gestational pathology with an unusual epidemiological and pathophysiological behavior that remains partially unexplained. Its current global incidence ranges from 0.1% to 2%. However, in Chile, the incidence reached up to 15% in the 1970s, with a marked decrease over subsequent decades, being nowadays about 1 to 2%. The reasons for this historical change are not fully understood. A literature-based analysis was conducted, focusing on clinical, microbiological, and epidemiological studies on intrahepatic cholestasis and other hepatobiliary diseases. Th

Fever in the returning international traveler presents a diagnostic challenge in the emergency department due to the wide range of potential infectious diseases that may be encountered abroad, many of which may manifest with nonspecific presentations. Malaria remains the most common and life-threatening tropical cause of fever and must be ruled out in all febrile patients returning from malaria-endemic regions. Other key infections to consider in returning travelers include dengue, enteric fever, and leptospirosis. Early recognition and treatment of these diseases can reduce morbidity and mort

This study assessed whether combining the aminoglycoside gentamicin with caffeine enhances antimicrobial efficacy in a murine typhoid model. We infected murine peritoneal macrophages in vitro with Salmonella enterica serovar Typhimurium and treated them for 24 h with non-toxic concentrations of caffeine (0.05–5 µg/ml) plus gentamicin (10 µg/ml). Subsequently, Swiss mice were orally challenged with S. Typhimurium and received daily intraperitoneal doses of caffeine (0.05-5 mg/kg) plus gentamicin (10 mg/kg) for five days post-infection. Macrophages treated with 5 µg/ml caffeine plus 10 µg/ml gen

Modern knowledge regarding 'healthy bacterial carriers' traces back to the legacy of typhoid Mary, who unknowingly infected >50 people with enteric fever. Yet, the intricacies of typhoidal Salmonella carriage, inclusive of Salmonella Typhi and Paratyphi A/B, remain largely unknown. Using data collected by the United Kingdom Health Security Agency's Salmonella reference laboratory and enhanced surveillance, we examined cases of enteric fever in England and Wales between 2004-2023, to track disease trends and identify carriers. In total, 8,297 cases of enteric fever were reported during the stud

Salmonella Paratyphi A is a major cause of paratyphoid fever. Despite growing global concern, knowledge of S. Paratyphi A immunity remains limited, and no licensed vaccine exists. In a controlled human infection model (CHIM) homologous S. Paratyphi A rechallenge showed a non-significant reduction in risk of acute disease. This study evaluates humoral immunity following experimental S. Paratyphi A exposure and vaccination with a bivalent S. Typhi-S. Paratyphi A conjugate vaccine (Sii-PTCV). Serum samples from 2 UK CHIM studies and a Phase I trial in India of Sii-PTCV were analyzed. S. Paratyphi

BACKGROUND: Febrile diseases such as Malaria, Typhoid fever, HIV/AIDS, Tuberculosis, respiratory tract infections, and urinary tract infections remain major public health concerns in low-to-middle-income countries (LMICs). Their prevalence is driven by interacting socioeconomic, environmental, behavioral, and biological factors such as vector exposure, poor sanitation, overcrowding, and high-risk behaviors including intravenous drug use and smoking. This study investigates the key risk factors influencing the prevalence and diagnosis of febrile diseases in Southern Nigeria. METHODS: A cross-se

INTRODUCTION: Typhoid intestinal perforation (TIP) remains a significant cause of pediatric morbidity in resource-limited settings, with prolonged hospitalization straining healthcare resources. Accurate prediction of hospital stay duration could optimize resource allocation and clinical decision-making, yet no TIP-specific prediction models exist. The objective of this research was to develop and validate a machine learning models for predicting prolonged hospital stay (>8 d) in pediatric TIP patients using readily available admission parameters, comparing logistic regression and random fores

In this work, a compartmental model of disease transmission due to typhoid fever, taking into account both the human-to-human transmission and the environmental route due to bacterial contamination of water resources, has been developed and analyzed. The human population under consideration are subdivided into four classes based on the epidemiological status of individuals with respect to typhoid fever transmission dynamics, these are, susceptible human, carrier humans, infectious humans, and recovered humans. There exist additional compartment which represent the concentration of bacteria in

The duration of fever defervescence in enteric fever has been observed to be increasing in recent times. An evaluation of the factors influencing fever defervescence in 150 children aged 1 month-18 years with confirmed (blood culture yielding Salmonella typhi or S. paratyphi) enteric fever was undertaken, and the minimum inhibitory concentrations (MICs) of ceftriaxone and azithromycin were determined. The median (q1, q3) time to fever defervescence was found to be 88 (58.75, 117.25) hours. Prior antibiotic therapy, splenomegaly and systemic complications were significantly associated with late

Salmonella enterica serovar Typhi (S. Typhi) is a leading cause of typhoid fever, significantly impacting morbidity and mortality in Ghana. However, genome-resolved data on circulating typhoidal strains remains scarce. We conducted this study to explore the genetic diversity, virulence, and antimicrobial resistance (AMR) profiles of S. enterica strains isolated from clinically diagnosed typhoid patients to inform targeted management and surveillance strategies. Twenty-eight S. enterica isolates recovered from stool and blood cultures were confirmed by PCR targeting 211 bp amplicon of the bcfD

INTRODUCTION: Typhoid fever remains a major Global public health concern, with treatment outcomes strongly influenced by antimicrobial resistance (AMR) and inter-patient variability. Determining the most appropriate antibiotic for an individual patient remains clinically challenging. Machine learning-based clinical decision support systems (CDSS) offer a promising avenue for improving diagnostic precision and guiding antibiotic selection using routinely collected clinical data.
METHODS: We developed a machine learning-based decision-support framework using XGBoost models to predict (i) treatm

BACKGROUND: Early diagnosis of typhoid is important for reducing mortality. In the absence of a blood culture, the quantitative tube Widal test is the standard serological test used for the diagnosis of Salmonella typhi , but rapid test kits are largely used in Northern Ghana. Variations in the performance of rapid test kits have been noted in other studies. We compared the diagnostic performance of two rapid test kits to the quantitative Widal method.
METHODS: This was a cross-sectional study conducted from April to May 2024 in the Northern Region of Ghana. Quantitative tube Widal and rapid

Typhoid toxin, a genotoxin secreted by typhoidal Salmonella serovars, has been implicated in diverse host responses ranging from DNA damage to cellular senescence. In this Research Highlight, we discuss how recent findings reveal an unexpected link between toxin-induced DNA damage and a noncanonical interferon-like response centered on ISG15. Using transcriptomic profiling and functional infection models, a recent report by Stark and colleagues shows that intoxicated cells mount a type-I interferon-like transcriptional program independent of interferons. Instead, ISG15 is upregulated via a STI

BACKGROUND: Typhoid fever incidence estimates are central to policy decisions on vaccine introduction and investments in non-vaccine prevention and control but are often unavailable. We explored whether prevalence metrics from sentinel studies of community-onset bloodstream infections could accurately predict local Salmonella enterica serovar Typhi (Salmonella Typhi) incidence.
METHODS: Using a previous systematic review (January 2018-December 2024), we identified studies reporting both typhoid incidence and prevalence of community-onset bloodstream infections from sentinel sites. From author
