Adrenal Corticosteroids and Increased Intracranial Pressure in Nonimmunocompromised Patients With Cryptococcal Meningitis.

Elevated opening pressures on lumbar puncture were followed in 7 nonimmunocompromised patients being treated for a postinfectious inflammatory response syndrome that had begun following antifungal treatment of cryptococcal meningitis. During the initial period of high-dose corticosteroid therapy, opening pressures on lumbar puncture fell an average of 14 cm. Despite this, 5 of 7 patients later required a ventriculoperitoneal shunt for pressure control.

Unexpected Variability in Laboratory and Clinical Assays Used to Quantify Cryptococcus neoformans Polysaccharide.

The detection and quantification of cryptococcal capsular polysaccharide (PS) antigen in body fluids is useful for the diagnosis of cryptococcosis. We compared the sensitivity of 4 assays for measuring the amount of cryptococcal PS and 2 clinical assays (lateral flow assay [IMMY] and the cryptococcal latex agglutination assay [Remel]) using the PS of 4 single-motif-expressing Cryptococcus strains. These showed relatively consistent laboratory assay interstrain sensitivity with significant interstrain variation, with the more sensitive clinical assays showing variation consistent with antigenic

The catalase gene CAT2 and its role in the virulence of one sub-cluster of Cryptococcus gattii VGI clinical isolates.

Cryptococcus gattii causes cryptococcosis and life-threatening cryptococcal meningitis. Currently, the pathogenic virulence mechanisms of C. gattii remain a significant area of ongoing research with considerable unexplored aspects. On the basis of our established research, a sub-cluster of strains with independent evolutionary relationships from WM276 in the phylogenetic analysis of VGI-type strains is identified. In vivo infection experiments on this sub-branch of strains reveal that there are hypervirulent strains and hypovirulent strains among these strains, and the virulence differences ar

Analytical validation: Comparison of two laboratory-based methods for CSF quantitative cryptococcal cultures.

Quantitative cryptococcal culture of the cerebrospinal fluid (CSF) is commonly used in cryptococcal meningitis research. We assessed the 'marble method' to evenly spread CSF onto two Sabouraud dextrose agar plates compared with the 'droplet method' using drops of CSF evenly placed around two halves of one agar plate. We found high concordance between the two methods. The Bland-Altman plot showed that the droplet method differed from the marble method by an average of -0.044 log10 CFU/ml CSF, with 95% limits of agreement from -0.324 to 0.235, indicating a small bias in favor of the marble metho

A biomimetic nanovesicle derived from BCG-primed macrophages with mannose modification for targeted immunotherapy against cryptococcal meningitis.

Cryptococcal meningitis is a life-threatening fungal infection of the central nervous system (CNS), characterized by an immunosuppressive microenvironment driven by Cryptococcus neoformans-induced polarization of macrophages toward M2 phenotype, which undermines host immunity and limits the efficacy of conventional antifungal therapies. To address these challenges, we developed a biomimetic mannose-modified nanovesicle (MBNV) system derived from Bacillus Calmette-Guérin (BCG)-primed bone marrow-derived macrophages (BMDMs) for dual action CNS immunotherapy. In vitro and in vivo studies demonstr

Evaluating Desirability of Outcome Ranking (DOOR) and Win Ratio for Hierarchical Composite Endpoints in Cryptococcal Meningitis Trials.

BACKGROUND: Traditional antifungal trials have relied on all-cause mortality as the primary endpoint, which may overlook important treatment-related outcomes such as adverse events or tolerability. METHODS: We evaluated the use of hierarchical composite endpoints to capture clinically meaningful treatment differences in cryptococcal meningitis trials. A Delphi survey determined priorities in constructing the composite endpoint. We analyzed three randomized clinical trials on cryptococcal meningitis with win ratio and the Desirability of Outcome Ranking (DOOR). We estimated the Better DOOR Pro

Occult cryptococcal meningitis presenting as myelopathy from an arachnoid cyst.

An immunocompromised patient presented with acute-on-chronic back pain following minor trauma. The pain was accompanied by worsening right-sided weakness, paraesthesias, progressive gait impairment and hyperreflexia concerning for compressive myelopathy. The patient's spouse also reported months of increasing forgetfulness. Imaging revealed a thoracic arachnoid cyst, which was treated by fenestration and resection of the cyst via laminoplasty. At the time of surgery, the thickened cyst wall was sent off for pathological evaluation. Postoperatively, pathology identified yeast consistent with cr

Acetylation-mediated fluconazole inactivation: A novel antifungal resistance mechanism.

Antifungal resistance is considered a global health threat. However, enzymatic inactivation of antifungals, a common mechanism seen in antibacterial resistance, has not yet been described in fungi. From a One Health perspective, this study demonstrates that Cryptococcus deuterogattii and C. neoformans, the leading agents of cryptococcosis, enzymatically inactivate fluconazole. Agrochemicals induce the overexpression of genes that code for acetyltransferases, specifically GCN5 and NAT10. These enzymes catalyze the acetylation of fluconazole into O-acetyl-fluconazole. This metabolite is unable t

Use of a human immortalized microglia cell line to study recognition, phagocytosis, and intracellular survival of Cryptococcus neoformans.

UNLABELLED: Cryptococcus neoformans, the etiological agent of cryptococcal meningitis (CM) is a globally distributed environmental yeast that mainly causes infections in immunocompromised individuals. Particularly in low-resource countries, the mortality rate of CM can reach 81% and accounts for ~19% of HIV/AIDS-related deaths each year. In immunocompromised individuals, once inhaled, C. neoformans escapes from the lungs and disseminates with special predilection for the central nervous system (CNS). Once in the brain, C. neoformans interacts with microglia, the tissue-resident macrophages of

Organ-specific immune responses are strain-dependent in a mouse model of Cryptococcus neoformans brain infection.

Cryptococcal meningitis remains the top cause of death from an invasive fungal infection in humans, responsible for ~100,000 deaths annually of vulnerable patients with underlying immune deficiencies. Animal models of cryptococcal meningitis are important for understanding the immune parameters that correlate with protection. However, modeling this infection in mice is challenging. There is wide variability in infection routes, doses, and mouse background used in the field, which makes understanding phenotypes of mutants and immune interventions difficult to broadly apply. Our intention was to

Effects of human immunoglobulins on Cryptococcus neoformans morphology and proteome.

Cryptococcus neoformans (Cn) is the main cause of fungal meningitis in people living with HIV. Perturbations in normal immunoglobulin (Ig) levels are observed in these individuals, but their association with Cn pathogenesis is unclear. Here, we investigated the physical and biological effects of normal (not elicited by known cryptococcal infection) human immunoglobulins (Igs), IgM, IgG, and IgA on Cn (strain H99). Each isotype affected the growth, surface morphology, and proteome of Cn. However, IgA had the most prominent effect. It induced growth inhibition after 24 h of co-culture with Cn, a

Antifungal repurposing of clinical chelator DMSA against Cryptococcus neoformans via laccase and histidine biosynthesis inhibition.

As a globally distributed opportunistic fungus, Cryptococcus neoformans poses a significant threat to human health by causing fatal meningitis, which is associated with high mortality and morbidity rates. Current antifungals-polyenes, flucytosine, and azoles-are limited by toxicity and resistance, underscoring the need for new agents. This study evaluated dimercaptosuccinic acid (DMSA), a chelating agent used in the treatment of heavy metal poisoning, beginning with its effect on the virulence factor laccase. DMSA showed broad-spectrum activity against standard (H99, B3501) and clinical isolat

Inhibition of the α7 nicotinic acetylcholine receptor alleviates pyroptosis in blood-brain barrier induced by Cryptococcusneoformans.

Cryptococcus neoformans meningitis (CM), a severe AIDS-defining illness with high mortality, remains refractory to effective therapies. This study investigated the mechanisms underlying Cryptococcus neoformans (Cn)-induced disruption of the blood-brain barrier (BBB). Using in vitro models of cerebral microvascular endothelial cells, we demonstrated that Cn disrupted the BBB via adhesion and invasion, leading to pyroptosis in these cells. Furthermore, our findings revealed that Cn induced upregulation of the α7 nicotinic acetylcholine receptor (α7nAChR). Importantly, inhibition of α7nAChR allev

An integrative ultrastructural and transcriptomic analysis of host-pathogen interactions with human brain microvascular endothelial cells during cryptococcal infection: a preliminary study.

BACKGROUND: In humans, Cryptococcus neoformans and Cryptococcus gattii species complexes are the leading cause of fungal meningitis globally. To establish CNS infection, Cryptococcus must breach the BBB, primarily comprised of specialized brain microvascular endothelial cells (BMECs), which is the prerequisite for cryptococci to invade the brain. Despite its clinical impact, the mechanisms underlying host-pathogen interaction at the BBB particularly involving environmental isolates remain under-characterized. OBJECTIVE: This study aimed to investigate the cyto-morphological and transcriptomic

Potential therapeutic targets for Cryptococcus.

Cryptococcus is a deadly opportunistic pathogenic fungus that causes severe infections such as meningitis in immunocompromised patients. Current antifungal therapeutics face the problem of drug resistance and the limitation of high side effects, and there is an urgent need to develop new therapeutic strategies. The growth and pathogenicity of Cryptococcus depend on its complex metabolic network, and by interfering with the metabolic pathways of Cryptococcus, its virulence can be effectively reduced and the immune response of the host enhanced. This paper systematically summarizes the potential

MHC-I Upregulation and Increased Glucose Dependence Define Innate Antifungal Immune Responses to Cryptococcus neoformans.

BACKGROUND: Cryptococcus neoformans, the etiologic agent of cryptococcosis, can survive and replicate within host immune cells, contributing to development of cryptococcal meningitis. Within the lung, C neoformans interacts with innate immune subsets, including macrophages and dendritic cells (DCs). Our previous work demonstrated that Ly6c- monocyte-like macrophages restrict fungal proliferation, whereas CD11b+ DCs promote fungal growth. Transcriptomic profiling revealed differential expression of MHC-I gene H2-K1, calreticulin, and metabolic genes. Therefore, we hypothesized that H2-K1 and me
