Invasive fungal infections after Bruton tyrosine kinase inhibitor treatment: a systematic review and meta-analysis.

The epidemiology of invasive fungal infections (IFIs) among patients receiving Bruton tyrosine kinase inhibitors (BTKIs) remains incompletely characterized. We conducted a systematic review and meta-analysis of 88 studies including 23 737 patients to evaluate the prevalence and risk factors for IFIs in this population. Among 16 studies that applied the European Organization for Research and Treatment of Cancer and the Mycoses Study Group Education and Research Consortium consensus definitions of IFIs, the pooled prevalence of proven/probable IFIs was 2.83% (95% confidence interval [CI], 1.97-4

The dove of peace is not peaceful : the ghost of aortic arch pseudoaneurysm.

BACKGROUND: Infectious pseudoaneurysms are rare, and those caused by Cryptococcus, an organism commonly found in pigeon feces, are exceptionally uncommon. This case highlights this severe vascular complication as a sequel of cryptococcal infection. CASE PRESENTATION: A 71-year-old male with a history of pigeon exposure presented with a persistent fever and was diagnosed with cryptococcal pneumonia and bacteremia. After successful initial antifungal treatment, a follow-up chest CT scan three months later revealed a pseudoaneurysm at the aortic arch. The patient underwent successful endovascular

Oral Cryptococcosis due to Naganishia diffluens in a Patient With Thalassemia: A Case Report and a Literature Review.

BACKGROUND: Naganishia diffluens (formerly Cryptococcus diffluens) is a non-neoformans cryptococcal species rarely isolated as a human pathogen. We reported a case of oral cryptococcosis caused by N. diffluens for the first time and reviewed relevant literature. METHODS: Herein, we present the case of a 31-year-old Iranian male with a history of major β-thalassemia, who presented with non-hemorrhagic white spots on the buccal mucosa, the mouth, lips, and the tongue's surface. Abdominal ultrasound results of the pelvis and abdomen showed that the patient has significant splenomegaly. Two conse

Test performance of a commercial cryptococcal antigen lateral flow assay: a retrospective and prospective study at five Canadian sites.

Cryptococcal antigen (CrAg) lateral flow assay (LFA) enables the rapid and sensitive detection of cryptococcal infection. The FungiXpert CrAg LFA is a Health Canada-licensed product that currently lacks evaluation in the North American setting. This retrospective and prospective study across five Canadian laboratories aimed to evaluate the FungiXpert CrAg LFA compared to the IMMY CrAg LFA for serum and cerebrospinal fluid (CSF) samples. Positive percent agreement (PPA) and negative percent agreement (NPA), precision, and cross-reactivity were assessed. A retrospective data review was performed

Fungal infections in diabetes mellitus.

PURPOSE: Diabetes mellitus is recognised as a global health problem and has increasingly been associated with fungal infections, as an independent risk factor. Diabetic patients are predisposed to both superficial as well as invasive fungal disease, and account for substantial morbidity and mortality. Recent pandemic of COVID-19 underlined the global problem of mucormycosis, however, the burden of fungal infections among diabetics has a much broader horizon. This review aims to summarise the spectrum of fungal infections encountered among diabetic patients, along with elucidating immunopathoge

Microbiology and clinical characteristics of brain abscess in adults from 3 tertiary hospitals in Singapore - a 11-year multicenter retrospective study.

BACKGROUND: Brain abscess remains a serious infection with high morbidity and mortality despite advances in neuroimaging, molecular diagnostics and stereotactic neurosurgical techniques. Most large studies were based on European cohorts. Most Asian studies are single center, with smaller cohorts that consist almost entirely of patients with pyogenic brain abscess (PBA). Here, we present the results of our multi-center, retrospective study, representing the largest cohort of adults with brain abscess in Singapore. METHODS: Radiological reports of patients who underwent neuroimaging between 1 N

A SuperSelective primer-based real-time PCR Platform for hypersensitive detection of azole heteroresistance in Cryptococcus neoformans.

Cryptococcus neoformans is the leading cause of fungal meningitis with limited treatment options, making early and accurate diagnosis critical for improved patient outcome. Current diagnostic methods for cryptococcosis rely largely on capsule antigen detection and fungal culture, which are time-consuming and unable to identify mutation-driven antifungal heteroresistance. In this study, we have developed a SuperSelective primer-based PCR (SSP-PCR) platform for the rapid and specific detection of azole resistance-associated single-nucleotide polymorphisms (SNPs). We demonstrate that SSP-PCR reli

Cryptococcus neoformans adapts to host CO2 concentrations via metabolic and stress-response remodeling.

Cryptococcus neoformans is an environmental pathogen that remodels its cellular physiology to survive within mammals and, in susceptible hosts, cause life-threatening meningoencephalitis. Of the many distinctions between the external environment and mammalian tissues, CO2 concentration in the host is two orders of magnitude higher than in the environment and represents a critical stress for C. neoformans. C. neoformans strains that do not replicate at host CO2 concentrations are less virulent in mouse models of infection, further supporting CO2 tolerance as a virulence trait. To further unders

Cryptococcal infection masquerading as a posterior mediastinal mass on cross sectional imaging: A case report and review of literature.

Pulmonary fungal infections are typically opportunistic but can also develop in immunocompetent hosts. These infections frequently present with diverse imaging appearances, often mimicking more common pulmonary or mediastinal conditions. We report the case of a 64 year old male with cough, shortness of breath and chest heaviness whose imaging features (neural formainal extension; destruction of adjacent ribs and vertebral bodies and chest wall invasion) initially suggested a malignant peripheral nerve sheath tumor. However, biopsy performed twice confirmed the lesion to be a cryptococcal infec

Epidemiology and Outcomes of Cryptococcal Infections in Liver Transplant Recipients: A Retrospective Cohort Study at a US Academic Center.

BACKGROUND: Cryptococcal infections in solid organ transplant (SOT) are relatively uncommon posttransplant, but carry significant morbidity and mortality. SOT represents a significant proportion of cryptococcosis among immunocompromised patients. Data specific to liver transplant recipients remains limited. METHODS: We performed a retrospective cohort study of 346 liver transplant recipients at a US academic center to identify predictors and outcomes associated with cryptococcal infection. Associations between infection and clinical variables were analyzed. Univariate logistic regression was

Construction of a "three-stage and four-level" evaluation system for cerebrospinal fluid pathogens based on mNGS: insights from a case of co-detection of Cryptococcus and Epstein-barr virus.

BACKGROUND: Metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) has brought about a revolutionary change in the diagnosis of central nervous system (CNS) infections. Nevertheless, the interpretation of results with multiple detected pathogens still poses a substantial clinical challenge. AIM: A preliminary exploration of the application value of the developed "Three-Stage, Four-Level" assessment system in interpreting mNGS cerebrospinal fluid test reports. METHODS: Based on a systematic review of relevant domestic and international literature, combined with practical e

Isolated Hepatic Cryptococcosis in an Immunocompetent Child: A Rare Cause of Granulomatous Hepatitis with Cholestasis.

Cryptococcosis is an opportunistic fungal infection acquired through inhalation of Cryptococcus neoformans and is typically encountered in immunocompromised hosts. In children, reported cases most commonly involve the central nervous system or lungs, with hepatic disease usually occurring as part of disseminated infection. Isolated hepatic cryptococcosis in an immunocompetent child is exceptionally rare and provides insight into developmental immune responses in early childhood. We report a young immunocompetent child presenting with fever, hepatomegaly, and cholestatic liver dysfunction. Live

Cryptococcus detection technologies: An overview of conventional, immunological, and molecular methods, and future development directions.

Cryptococcus, notably Cryptococcus neoformans and Cryptococcus gattii, is a pathogenic fungus that poses a significant threat to global health. Early and rapid detection is pivotal for reducing mortality, yet conventional diagnostic techniques suffer from inherent limitations-including slow turnaround times, low sensitivity in early infection, and heavy reliance on specialized expertise-that delay clinical intervention. Advancements in detection technology have established immunological and molecular methods as essential tools for identifying Cryptococcus. This review moves beyond descriptive

Anticryptococcal Evaluation of the Allylimine 3H2: Modulation of Virulence Traits and Synergistic Action with Amphotericin B.

The development of new antifungal scaffolds is critical for expanding therapeutic options against cryptococcosis. Here, we evaluated the allylimine 3H2 as a potential lead compound against Cryptococcus gattii. The allylimine 3H2 exhibited consistent in vitro activity with a geometric mean MIC of 7.5 μg mL-1 across 12 strains and showed a synergistic interaction with amphotericin B, with FICI values reaching 0.5. The compound impaired major virulence determinants, reducing capsule size, melanin synthesis, and laccase activity while altering the negative surface charge. In a murine model, 3H2 po

Systematic Profiling of Essential Fungal Transcription Factors Uncovers Ezt1 as a Central Pathobiological and Morphogenic Regulator in Cryptococcus neoformans.

Cryptococcus neoformans is a global fungal pathogen that causes fatal cryptococcosis, underscoring the urgent need for novel therapeutics given the limitations of current antifungals. Here, we systematically investigate essential transcription factors (TFs) in C. neoformans, focusing on their roles in growth and their potential as drug targets. We developed experimental pipelines to assess growth requirement, essentiality, and function using conditional gene expression, constitutive overexpression, and meiotic spore analysis. Through these systematic analyses, we identify one quasi-essential (

Right parietal cryptococcoma in a young immunocompetent patient: illustrative case.

BACKGROUND: Cryptococcoma is a rare form of CNS cryptococcosis, typically associated with immunocompromised patients. However, cases reported in immunocompetent individuals have been increasing in recent years and are often initially misdiagnosed. OBSERVATIONS: A 39-year-old previously healthy male patient presented with progressive headache, left-sided hemiparesis, and tonic-clonic seizures. Imaging studies revealed a right parietal expansive lesion with perilesional edema, initially suggestive of a neoplasm. Because of the diagnostic uncertainty, resection of the lesion was performed, and h

Therapeutic potential and synergistic effects of antifungal and off target drugs against clinical isolates of Cryptococcus laurentii.

Cryptococcus laurentii is an emerging opportunistic yeast pathogen associated with unpredictable antifungal susceptibility and biofilm-mediated resistance. This study investigated the therapeutic potential and synergistic interactions of standard antifungals (fluconazole, amphotericin B) and off-target drugs against clinical isolates of C. laurentii. Three clinical isolates recovered from blood (n = 2) and cerebrospinal fluid (n = 1) were confirmed by phenotypic characteristics and PCR amplification of the 18 S rRNA gene (~ 480 bp). Biofilm formation was screened using Congo red agar and quant
