Invasive Candidiasis in Liver Transplant Recipients Undergoing Antifungal Prophylaxis.

INTRODUCTION: Invasive candidiasis (IC) is the most frequent invasive fungal infection (IFI) affecting liver transplant (LT) recipients. Antifungal prophylaxis (AP) effectively reduces the incidence IFI after LT. However, a considerable proportion of LT recipients undergoing AP are still affected by IC. OBJECTIVES: To assess the incidence and factors associated with IC in high-risk LT recipients undergoing AP. PATIENTS AND METHODS: Retrospective single-center study including high-risk LT recipients undergoing AP between 2015 and 2020. The primary outcome was the occurrence of IC within the f

Disseminated Cryptococcosis in a Non-HIV Patient: Diagnostic Value of Integrating mNGS, Culture, and Antigen Testing.

Cryptococcus neoformans is an opportunistic fungal pathogen most commonly observed in individuals with human immunodeficiency virus (HIV) infection. Disseminated infections involving multiple organs, such as lungs, bloodstream, urinary tract, pleural cavity and central nervous system, are uncommon in patients without HIV. This work describes the case of a 72-year-old man who developed high fever and increased inflammatory markers after coronary artery bypass surgery. C. neoformans (8 sequence reads) was initially detected by metagenomic next-generation sequencing (mNGS) of bronchoalveolar lava

Clinical Significance of Serum (1→3)-β-D-Glucan Positivity in Cryptococcosis: A Retrospective Cohort Study.

Cryptococcosis is associated with negative serum (1→3)-β-D-glucan (BDG) results, but the clinical significance of serum BDG (sBDG) positivity is unclear. We characterized the clinical features of sBDG-positive cryptococcosis in patients with cryptococcal disease. We measured the sBDG levels and classified patients as BDG-positive (>11 pg/mL) or BDG-negative. Clinical characteristics, diagnostic modalities, antifungal treatment, post-treatment BDG level changes, and 90-day cryptococcosis-related mortality were compared. BDG-positive patients showed disseminated cryptococcosis, higher inflammato

A soft tissue lesion on the scalp as a peculiar source of disseminated cryptococcosi.

Cryptococcosis is a potentially fatal invasive fungal infection with considerable health challenges. A seemingly immunocompetent middle-aged man with frequent exposure to pigeons and bird excreta presented with headaches, fever, gait instability and diplopia. Two years prior to his presentation, he sustained a blunt head trauma which resulted in sporadic headaches. On examination, a hard scalp nodule measuring approximately 3 x 3 cm was palpable on the topmost part of his head. Cryptococcus neoformans was cultured from his cerebrospinal fluid, blood and the biopsied scalp nodule. A diagnosis o

Early Disseminated Cryptococcosis Following Orthotopic Heart Transplantation Requiring VA-ECMO: A Case Report.

Disseminated cryptococcosis is a rare but serious opportunistic infection following solid organ transplantation and rarely occurs within the first month of transplantation. We report a case of disseminated Cryptococcus neoformans infection occurring 14 days after orthotopic heart transplantation which required mechanical circulatory support with venoarterial extracorporeal membrane oxygenation (VA-ECMO). Management was complicated by alterations in antifungal pharmacokinetics and the inability to perform serial lumbar punctures while anticoagulated. Antifungal therapy with nonliposomal amphote

Cryptococcal scleritis mimicking scleromalacia perforans.

Scleromalacia perforans is a rare, non-inflammatory type of anterior scleritis that usually signals an underlying autoimmune disorder. However, certain infectious pathogens can mimic this clinical presentation which often leads to diagnostic difficulty. We describe a rare case of primary cryptococcal scleritis in an elderly man with diabetes who showed no evidence of systemic cryptococcal disease. He initially presented with progressive scleral thinning and minimal pain, which was interpreted initially as scleromalacia perforans. But surgical intervention with scleral patch grafting revealed Cr

Aerosolized antifungal therapy in pulmonary fungal disease: pharmacology, drug delivery and emerging clinical applications.

Pulmonary fungal diseases remain associated with substantial morbidity and mortality, particularly in immunocompromised individuals, transplant recipients, and critically ill patients. Although systemic antifungal therapy remains the cornerstone of treatment, its effectiveness may be limited by suboptimal penetration into selected pulmonary compartments, while systemic toxicity, drug-drug interactions, and prolonged treatment requirements may restrict therapeutic options. Aerosolized antifungal therapy provides a pharmacologically attractive approach to increase local pulmonary drug exposure w

Pulmonary Cryptococcosis in an Immunocompetent Patient: Negative mNGS but Positive Antigen Test.

BACKGROUND: Pulmonary cryptococcosis is common in patients with immunodeficiency, application of immunosuppressive and cytotoxic drugs. Its incidence has increased in recent years, with a greater number of patients with no underlying disease or history of exposure, and a greater number of immunocompetent patients. Its clinical presentation lacks specificity and imaging features are atypical, making it easy to misdiagnose or miss the diagnosis. METHODS: Diagnosis was confirmed by serum cryptococcal antigen after negative bronchoalveolar lavage fluid mNGS in an immunocompetent patient with atyp

Serum cryptococcal antigen gradient detection for monitoring the efficacy and medical guidance of the treatment of pulmonary cryptococcosis: a case report.

BACKGROUND: The duration of drug maintenance therapy for pulmonary cryptococcosis (PC) has been controversial, and good monitoring methods for drug efficacy are lacking. CASE PRESENTATION: An immunocompetent patient diagnosed with PC was admitted in 2020. He had no history of immune deficiency but reported being extremely tired at the time and possibly exposed to pigeon dropping. The diagnosis was confirmed through serum cryptococcal antigen (CrAg) testing and pulmonary high-resolution computed tomography (HRCT) imaging, and the patient received oral fluconazole therapy immediately. During the

Rethinking Routine Lumbar Puncture and Induction Therapy in Non-HIV Pulmonary Cryptococcosis: A Call for Risk-Based Management.

Current guidelines recommend lumbar puncture (LP) in patients with pulmonary cryptococcosis (PC) to exclude central nervous system (CNS) involvement. However, recent multicenter data suggest that CNS dissemination is rare in immunocompetent, asymptomatic patients with low serum CrAg titers. And, although guidelines advocate induction therapy for severe PC, most patients in practice receive fluconazole monotherapy with favorable outcomes. This Perspective evaluates the gap between guideline-based universal intervention and real-world clinical practices, advocating for a risk-stratified manageme

Diagnostic value of combined detection of cryptococcal antigen in serum and lung puncture fluid using lateral flow assay for diagnosing pulmonary cryptococcosis: a single-center prospective study.

OBJECTIVE: To evaluate the diagnostic value of the combined detection of cryptococcal antigen (CrAg) in the serum and lung puncture fluid (LPF) using a lateral flow assay (LFA) in patients with pulmonary cryptococcosis (PC). METHODS: Patients with suspected PC were prospectively enrolled between June 2023 and July 2025 and underwent the IMMY CrAg LFA (Immuno-Mycologics, Norman, OK, United States) of serum and LPF specimens. Conventional fungal cultures were performed simultaneously and the diagnostic efficiencies of these methods for PC were compared. RESULTS: Samples were obtained from 167

Two cases of refractory Cryptococcus gatti (C gatti) infection and literature review: Case report.

RATIONALE: The incidence of Cryptococcus gatti (C gatti) infection has been increasing, posing significant challenges in clinical diagnosis and treatment due to its high virulence, refractoriness, and mortality. PATIENT CONCERNS: Both patients had no immune function defects. One of the cryptococcal drug sensitivity results suggested resistance to fluconazole, which ultimately required surgical intervention. Two patients had recurrent symptoms and worsened examination results during antifungal treatment, and the course of anti-infection was more than 6 months. DIAGNOSES: Pulmonary cryptococco

Limited genome evolution of Cryptococcus neoformans following an accidental infection in the research laboratory.

BACKGROUND: Cutaneous infections resulting from accidental exposure to fungal pathogen Cryptococcus neoformans in research laboratories are rarely reported in the literature. To fill a gap in published guidance for handling these situations promptly when they arise, we describe a case report plus three additional examples as a guide to effective resolution. CASE SUMMARY: An immunocompetent laboratory researcher developed primary cutaneous cryptococcosis following accidental infection with Cryptococcus neoformans H99 via a needle scrape that was initially assumed not to have penetrated the ski

Intralobar pulmonary sequestration coexisting with pulmonary cryptococcosis: A case report and literature review.

Pulmonary sequestration (PS) is a rare congenital foregut abnormality characterized by non-functioning lung tissue that receives an arterial blood supply from an anomalous systemic artery, typically the aorta, and is separated from the normal tracheobronchial tree. It often presents as a mass-like, cystic, cavitary or pneumonic lesion on imaging. Patients often present with symptoms of recurrent pulmonary infection; however, cryptococcal infection is rare. The present case study reports on a 52-year-old man with PS and cryptococcal infection. The mass in the lower lobe of the right lung was re

Progression of Primary Cutaneous Cryptococcosis to Disseminated Cryptococcosis in an Immunosuppressed Older Patient: A Case Report.

Primary cutaneous cryptococcosis (PCC) is acquired through direct inoculation of Cryptococcus neoformans into wounds or damaged skin, typically resulting in a localized infection confined to a single cutaneous site. Progression of PCC to disseminated cryptococcosis is extremely rare. The purpose of this case report is to describe an unusual instance of PCC progressing into disseminated disease in an immunosuppressed older patient and to highlight the importance of considering cryptococcosis as a differential diagnosis of cutaneous lesions unresponsive to antimicrobial therapy. We report the c

[Cutaneous Cryptococcus neoformans Infection Mimicking Necrotizing Fasciitis in a Myelofibrosis Patient Receiving Ruxolitinib: A Case Report].

Cryptococcus neoformans is an encapsulated opportunistic yeast widely distributed in the environment and classified as critical-priority fungal pathogen by the World Health Organisation due to its high mortality and limited access to timely diagnosis and effective treatment. Infection is typically acquired via inhalation, with the primary pulmonary focus often remaining asymptomatic. Particularly in individuals with impaired cell-mediated immunity, it may disseminate hematogenously to central nervous system (CNS), skin and other organs. Cutaneous cryptococcosis is a rare clinical manifestation
