Signal Recognition Granule Receptor Beta Subunit Promotes Arrhythmogenic Remodeling in the Heart Failure Mice.

BACKGROUND: Heart failure is a major cause of global morbidity and mortality, often complicated by ventricular arrhythmias (VAs) that worsen prognosis. The Srprb (signal recognition particle receptor beta subunit) is an endoplasmic reticulum membrane-anchored protein involved in protein processing. METHODS: Male C57BL/6 mice received tail vein injections of adeno-associated virus to cardiac-specifically overexpress or knock down Srprb. A pressure overload-induced heart failure model was established via aortic banding 3 weeks later. Cardiac function, structural/electrical remodeling, and VA su

Tachycardiomyopathy-like presentation in neonatal MCAD deficiency: A novel cardiac phenotype.

BACKGROUND: Medium-chain acyl-CoA dehydrogenase deficiency (MCADD) is the most common fatty acid oxidation disorder in Europe. Clinical onset typically occurs between 3 and 24 months of life with hypoketotic hypoglycemia, while neonatal presentations are less common. Although the disorder classically manifests with metabolic decompensation, atypical cardiac involvement has occasionally been reported but remains exceedingly rare. MCADD is included in many newborn screening programs, enabling early detection and timely management. CASE PRESENTATION: We report a full-term female neonate who, at

Macrolides for the prevention of bronchopulmonary dysplasia in preterm neonates.

RATIONALE: Bronchopulmonary dysplasia (BPD) continues to be the most frequent complication of prematurity despite advances in neonatal care. It occurs from interactions between antenatal exposures, postnatal oxygen, ventilation-mediated injury as well as other postnatal injuries that induce a proinflammatory state in an immature developing lung. Macrolides, particularly azithromycin, have anti-inflammatory actions that may play a role in preventing BPD. A current review is required to investigate whether macrolide use in the highest-risk patient population, intubated and ventilated very preter

Postoperative junctional ectopic tachycardia with variable QRS morphology: A case report.

Junctional ectopic tachycardia (JET) is a rare but clinically important arrhythmia most often encountered in infants and children, either congenitally or within 72 h after congenital heart surgery. We report a neonate who developed postoperative JET with variable QRS morphologies after an arterial switch operation, in whom QP interval and QRS duration correlated with the preceding RR interval. The arrhythmia caused hemodynamic instability requiring antiarrhythmic therapy and supportive measures. This case illustrates the diverse electrocardiographic manifestations of postoperative JET and high

Development and internal validation of a scoring model for early identification of ventricular arrhythmia risk in children with acute myocarditis.

BACKGROUND: Early identification of ventricular arrhythmia (VA) risk in children with acute myocarditis (AMC) is challenging, as existing tools lack pediatric targeting. AMC’s non-specific early symptoms in children lead to underdiagnosis. VA, a major complication of AMC, is linked to poor prognosis, but not all cases present at admission, highlighting the need for simple, accessible predictive indicators. METHODS: This retrospective single-center study included 312 children (1 month − 17 years) with AMC (2021–2024), divided into a training set (n = 208, 2021–2023) and validation set (n = 104,

Sotalol Dose Optimization for Fetal Tachycardia: A Pregnancy Physiologically Based Pharmacokinetic Model Study.

OBJECTIVE: To establish an optimized sotalol dosing strategy for fetal tachycardia using a pregnancy physiologically based pharmacokinetic (PBPK) model. METHOD: A verified PBPK model was developed incorporating pregnancy-induced physiological changes and placental transfer. Simulations were performed to evaluate current dosing recommendations and alternative dosing scenarios were explored. To avoid maternal dose-related toxicity (QT-prolongation), we aimed for maternal concentrations < 2.5 mg/L. Based on neonatal concentration-effect data, we aimed for fetal Ctrough concentrations of 0.4-1.0

Newborn With Abnormal ECG and Family History of Sudden Cardiac Arrest.

Congenital long QT syndrome (LQTS) is a cardiac channelopathy caused by mutations in cardiac ion channel genes, leading to delayed ventricular repolarization, QT interval prolongation, and risk of malignant arrhythmias and sudden cardiac death. The condition often presents during childhood or adolescence but can be identified in neonates, particularly when there is a positive family history. Early recognition is critical, as timely intervention with beta-blockers and avoidance of QT-prolonging medications can prevent life-threatening events. We report the case of a full-term neonate born via c

Short SCN5A Transcript Yields a NaV1.5 Fragment Influencing Cardiac Metabolism.

BACKGROUND: SCN5A encodes the cardiac NaV1.5 (voltage-gated Na+ channel), classically known for initiating action potentials and recently implicated in cardiomyocyte metabolism via mitochondrial Na+/Ca2+ exchange. SCN5A variants are linked to arrhythmias and heart failure, but mechanisms controlling SCN5A/NaV1.5 expression and its metabolic interface remain understudied. METHODS: We used bioinformatic approaches to identify novel SCN5A regulatory features and discovered an alternative polyadenylation (APA) signal downstream of exon 2, which is conserved in humans and several other species but

Fto-mediated m6A demethylation of Lox drives atrial fibrosis and promotes atrial fibrillation in a murine model of hyperthyroidism.

AIMS: Hyperthyroidism is a risk factor for atrial fibrillation (AF), and N6-methyladenosine (m6A) RNA methylation is crucial in cardiovascular regulation. However, the role of Fto-mediated m6A demethylation in hyperthyroidism-related AF remains unclear. METHODS AND RESULTS: We consecutively recruited 232 AF patients undergoing ablation, stratified into age-, gender-, and comorbidity-matched cohorts, 116 with manifest hyperthyroidism and 116 without manifest hyperthyroidism. Assessments included thyroid profiles, echocardiography, low-voltage area (LVA) mapping, and 1-year recurrence. T4-treat

Fetal Long QT Syndrome: Case Series and Literature Review With Focus on Multidisciplinary Care Coordination.

Congenital long QT syndrome (LQTS) is a group of heritable conditions that are associated with cardiac repolarization abnormality characterized by QT prolongation on ECG and risk of life-threatening arrhythmias. Prenatal detection of LQTS presents many challenges for clinicians and a multidisciplinary approach is needed for optimal prenatal and postnatal management. We describe five cases of fetal diagnosis of LQTS with variable initial presentation, diagnostic strategies, and management approaches. A multidisciplinary team approach including fetal cardiologist, adult and pediatric electrophys

Impact of β-Blockers on the Risk of Low-Birth-Weight Infants in Women with Long QT Syndrome or Marfan Syndrome: A Single-Center Retrospective Study from 2008 to 2022 in a Tertiary Care Center.

INTRODUCTION: β-Blockers are an essential treatment for cardiovascular disease, the incidence of which is rising among women of childbearing age. Several large cohort and registry studies have reported an association between β-blocker exposure in pregnancy and reduced birth weight, but most have included women with hypertension or structural heart disease. Our study's primary objective was to evaluate the impact of these medications on the birth weight of infants born to mothers with morphologically normal hearts, including women with long QT syndrome (LQTS) or Marfan syndrome (MFS). Secondary

Regulation of sodium/calcium homeostasis by BacNav gene therapy rescues cardiac dysfunction in chronic heart failure.

Despite continued progress, therapies to augment contractile function and prevent arrhythmias in patients with heart failure remain limited. Here, we present a two-pronged gene therapy approach that simultaneously augments peak Na+ current and Ca2+ transient amplitude in cardiomyocytes (CMs) to alleviate heart failure pathogenesis. Using in vitro engineered neonatal rat heart tissues, ex vivo adult mouse CMs, and in silico rabbit CMs, we show that expression of prokaryotic Na+ channels (BacNav) dose-dependently enhances Ca2+ transients and contractility of CMs by modulating Na+/Ca2+ exchanger

When the gatekeeper falls: developmental vulnerability of the thalamic reticular nucleus in neonatal and pediatric hypoxic-ischemic brain injury.

The thalamic reticular nucleus orchestrates thalamocortical oscillations and sensory gating. Its early development features a unique confluence of depolarizing GABA signaling, immature chloride regulation, and transient electrical coupling via connexin-36 gap junctions. These developmental specializations, essential for synchronizing cortical maturation, also render thalamocortical networks vulnerable to hypoxic-ischemic insults such as perinatal asphyxia or pediatric cardiac arrest. Following cellular ATP depletion, rapid chloride imbalance eliminates fast synaptic inhibition, permitting abno
