Prenatal Presentation of Congenital Alveolar Capillary Dysplasia With Misalignment of Pulmonary Veins: A Case Initially Suspected as Posterior Urethral Valves.

Alveolar capillary dysplasia with misalignment of the pulmonary veins (ACDMPV) is a rare, typically lethal pulmonary disorder characterized by underdeveloped pulmonary vascularization. It usually presents within the first 48 h of life with progressive respiratory distress and therapy-resistant persistent pulmonary hypertension of the newborn (PPHN). ACDMPV is associated with genomic alterations at the FOXF1 locus, with most cases arising de novo. We present a case in which suspected urethral valves, managed prenatally with fetal vesicoamniotic shunting, were the first indication of ACDMPV. In

How to evaluate, triage, and sedate a patient in the congenital cardiac catheterization laboratory.

The congenital cardiac catheterization laboratory (CCCL) has evolved into one of the most physiologically demanding environments in pediatric cardiovascular care for not only the interventional cardiologist, but also the anesthesiologist. Once primarily diagnostic, the CCCL now functions as a high-acuity interventional laboratory capable of treating both acquired and congenital heart disease (CHD), through the entire lifespan of patients with CHD. Technological innovation in device design, advanced imaging modalities, and hybrid surgical collaboration has expanded the therapeutic scope to incl

Endomyocardial calcification with pulmonary and systemic hypertension in a term neonate with multisystem inflammatory syndrome of newborn (MIS-N).

Multisystem inflammatory syndrome in neonates (MIS-N) is a rare condition linked to perinatal exposure to SARS-CoV-2. We report a case of a term male neonate presenting at birth with severe respiratory failure and persistent pulmonary hypertension of the newborn. Initial evaluations, including chest X-ray and ECG, were unremarkable, but echocardiography showed elevated pulmonary pressures without structural abnormalities. Over the next few days, the infant developed diffuse skin peeling, acute kidney injury and hypertension, creating a perplexing clinical picture. Only after retrospective mate

Weighted gene co-expression network reveals CCND1 as a promising biomarker and therapeutic target for myocardial fibrosis in hypertrophic cardiomyopathy.

BACKGROUND: Myocardial fibrosis represents a critical prognostic indicator of poor outcomes in hypertrophic cardiomyopathy (HCM), frequently predisposing patients to lethal ventricular arrhythmias and progressive heart failure. However, the upstream molecular mechanisms driving cardiac fibrogenesis in HCM remain largely unresolved. This study aimed to identify key fibrotic pathways and regulatory genes in HCM patients using weighted gene co-expression network analysis (WGCNA). METHODS AND RESULTS: We quantified the severity of myocardial fibrosis using late gadolinium enhancement (LGE) on car

High-Frequency Oscillatory Ventilation Combined With Inhaled Nitric Oxide for Persistent Pulmonary Hypertension of the Newborn: A Single-Center Randomized Clinical Trial.

BACKGROUND: Persistent pulmonary hypertension of the newborn (PPHN) may respond incompletely to inhaled nitric oxide (iNO) when poorly recruited lung limits delivery to ventilated alveoli. We evaluated whether high-frequency oscillatory ventilation (HFOV) combined with iNO improved clinical and physiologic outcomes compared with conventional mechanical ventilation (CMV) + iNO or HFOV alone. METHODS: In this prospective, single-center, 3-arm, open-label randomized clinical trial, 138 neonates born at 34 weeks of gestation or later with echocardiographically confirmed PPHN and an oxygenation in

Research on ultra-fast fabrication method of bidirectionally anisotropic Ion beam figuring for large-aperture nanoradian X-ray mirrors.

The grazing-incidence operating mode of large-aperture X-ray mirrors imposes bidirectional anisotropic requirements on surface figure accuracy: extremely stringent in the meridional direction, while relatively relaxed in the sagittal direction. The strict meridional accuracy requirement necessitates the use of small-beam deterministic figuring processes, making machining efficiency a critical bottleneck to be overcome. To address this issue, this paper proposes a spatially decoupled figuring strategy based on a bidirectionally anisotropic elliptical diaphragm. By exploiting the disparity in ac

Impact of hydroxychloroquine on pregnancy outcomes in systemic lupus erythematosus: A 25 years retrospective cohort study from Asia.

BackgroundSystemic lupus erythematosus (SLE) is an autoimmune disorder that affects various organs and follows a pattern of remission and relapse. Hydroxychloroquine (HCQ), an antimalarial drug, has recently become widely used in pregnant women with SLE due to its multi-level immune anti-inflammatory mechanisms and potential endothelial protective and thromboprophylaxis properties. While several studies have evaluated the impact of HCQ on SLE activity and the occurrence of neonatal lupus, its effects on improving early delivery, preeclampsia, and intrauterine growth restriction (IUGR) remain c

Role of Hydroxychloroquine in Prevention of Congenital Heart Block in Pregnant Women With Positive Anti-Ro/SSA and/or Anti-La/SSB.

Neonatal lupus, a condition primarily affecting infants born to mothers with anti-Ro/SSA and/or anti-La/SSB antibodies, is linked to congenital heart block (CHB), a potentially life-threatening complication. Hydroxychloroquine (HCQ), an antimalarial with immunomodulatory properties, has been shown to reduce CHB recurrence. This review examines current evidence on HCQ's role in preventing CHB in pregnancies affected by those autoantibodies, focusing on overall, primary and secondary prevention. Evidence supports HCQ in overall and secondary prevention, but data on its effectiveness for primary

Androgen-mediated DKK1 upregulation in bronchial epithelial cells suppresses Th2 response and ameliorates asthma.

BACKGROUND: Mutations in the human mitochondrial genome have been suspected to play a significant role in the etiological development of mitochondrial diabetes. Detection of the 3243A>G mutation in the mitochondrial transfer RNALeu(UUR) gene (MTTL1), especially at low heteroplasmy levels, is highly desirable since it facilitates the diagnosis and subsequent management of the disease. The proportions of mutant mitochondrial DNA (mtDNA) can vary between tissues and are usually significantly higher in muscle than in blood, but muscle biopsies from patients with diabetes are rarely available. Refractory Neonatal Apnea Revealing Congenital Central Hypoventilation Syndrome: Improved Outcome through Early Multidisciplinary Intervention.

Background: Congenital central hypoventilation syndrome (CCHS) is a rare genetic disorder that causes alveolar hypoventilation because of impaired chemoreceptor response to hypercapnia and hypoxia occurring due to a dysfunctional central autonomic drive. CCHS is a result of pathogenic variants of the PHOX2B gene, which affects neural crest cell development. Consequently, CCHS can be accompanied by dysregulation of the autonomic system, Hirschsprung's disease and other gastrointestinal disorders, cardiac arrhythmias, and neural cell-derived tumors. Disease severity correlates with mutation type

Survivors of Cyanotic Congenital Heart Disease: A Review.

IMPORTANCE: Cyanotic congenital heart diseases, which occur in approximately 0.2% of live births in the United States, have high mortality rates if untreated, but survival to adulthood is common with current surgical interventions. OBSERVATIONS: Approximately 80% of all cyanotic congenital heart diseases are caused by tetralogy of Fallot (TOF), D-loop transposition of the great arteries (TGA), and congenital heart disease with single-ventricle circulation. Survivors of cyanotic congenital heart diseases benefit from multidisciplinary care including pediatric and adult cardiologists, congenita

Spectrum of potentially lethal cardiac conditions presenting with fetal sinus bradycardia: a report on 34 cases.

OBJECTIVE: Fetal sinus bradycardia (FSB) is usually a benign antenatal finding but it can be a marker of an underlying serious condition that manifests in later life. The aim of this study was to report the antenatal course and postnatal outcome of FSB. METHOD: This study was a retrospective review of all cases of persistent FSB, defined as a fetal heart rate (FHR) < 3rd percentile for gestational age (GA) with 1:1 atrioventricular conduction on at least two consecutive fetal echocardiograms, diagnosed between January 2014 and December 2024 at the Brompton Centre for Fetal Cardiology, London,

pedQTNet: A Deep Learning Approach to Estimate Corrected QT Intervals from Multi-Lead Conventional ECG Waveforms in Pediatric Patients.

Long QT syndrome (LQTS) is a primary risk factor for ventricular arrhythmias and sudden cardiac death in children. Accurate corrected QT intervals (QTc) measurement is imperative but challenging for non-heart-rhythm specialists, especially in children. We developed and evaluated pedQTNet, a deep neural network model for estimating QTc and detecting LQTS in pediatric patients. We analyzed a cohort of 37,992 patients aged 0-18 years with 65,370 ECGs annotated by pediatric electrophysiologists (PEPs) between 2010 and 2020. Using PEP-annotated QTc measurements as ground truth, pedQTNet was trained
