Prenatal diagnosis and postnatal outcomes of foetal situs abnormalities: a single tertiary centre experience.

BACKGROUND: Foetal situs abnormalities, including situs inversus totalis (SIT), right atrial isomerism (RAI) and left atrial isomerism (LAI), are associated with substantial variation in cardiac anatomy and postnatal outcomes. This study aimed to compare prenatal characteristics, associated abnormalities, perinatal outcomes, postnatal management and survival among foetuses with SIT, RAI and LAI. METHODS: This retrospective cohort study included 73 foetuses prenatally diagnosed with SIT (n = 12), RAI (n = 24) or LAI (n = 37) at a tertiary foetal cardiology and perinatology centre between 2021

A comprehensive review of usefulness of sodium butyrate for the management of inflammatory bowel disease: from molecular mechanisms to clinical application.

Sodium butyrate (SB), a salt of the short-chain fatty acid butyrate, has emerged as a promising adjunctive therapy in inflammatory bowel disease (IBD). IBD is commonly associated with intestinal dysbiosis characterized by depletion of butyrogenic bacteria. This leads to a functionally relevant butyrate deficiency that contributes to epithelial barrier dysfunction, immune dysregulation, and persistent mucosal inflammation. Experimental studies demonstrate that butyrate supports colonocyte energy metabolism, strengthens tight-junction integrity, enhances mucin and antimicrobial peptide productio

Ivabradine in refractory neonatal ectopic atrial tachycardia: expanding therapeutic options in early life.

Neonatal ectopic atrial tachycardia is a rare arrhythmia that may be resistant to conventional therapy. We report a neonate with persistent foetal tachycardia refractory to flecainide and digoxin. Ivabradine resulted in rapid and sustained heart rate reduction without haemodynamic compromise. This case supports ivabradine as a therapeutic option in neonatal automatic tachycardias when standard treatments fail.

Safety of caffeine citrate in neonates with Congenital Heart Disease receiving continuous infusions of alprostadil.

Neonates with ductal-dependent congenital heart disease (CHD) will receive a continuous infusion of a prostaglandin E1 analogue such as alprostadil prior to surgical correction. Alprostadil can cause apnoea, which may be mitigated in practice with the use of caffeine citrate. Caffeine citrate can lead to tachyarrhythmias, which could be detrimental in this patient population. The purpose of this study was to determine if the practice of utilising caffeine citrate in neonates with ductal-dependent CHD receiving continuous alprostadil infusions is safe. In this single-centre, retrospective, coho

Genome-wide variation in cell-free DNA end-motif entropy predicts immunotherapy response in head and neck cancer.

BACKGROUNDMinimally invasive biomarkers predicting the immunotherapy response in head and neck squamous cell carcinoma (HNSCC) remain an unmet clinical need.METHODSIn a prospective, multi-institutional phase II trial, we performed whole-genome sequencing of 185 longitudinal plasma cell-free DNA (cfDNA) samples from 68 patients with locally advanced, surgically resectable HNSCC who received neoadjuvant and adjuvant pembrolizumab. We developed the regional motif diversity score (rMDS), a fragmentomic metric that quantifies the entropy of cfDNA 5'-end motifs across genomic regions.RESULTSUnsuperv

Sex differences in out-of-hospital cardiac arrest across age groups.

BACKGROUND: Sex differences in out-of-hospital cardiac arrest (OHCA) have been studied in adults but remain poorly explored in younger populations. We aimed to assess sex differences in OHCA across age groups to provide a comprehensive overview. METHODS: All OHCA occurring between May 2011 and December 2018 in Paris and its suburbs were analysed. Primary outcomes included sex differences in OHCA characteristics and survival to hospital discharge across three age groups: paediatric patients (28 days-18years), young adults (18-35 years) and older adults (≥35 years). Secondary outcomes included

Innovative application of six-lead KardiaMobile device in children under three years of age.

The use of the six‑lead AliveCor KardiaMobile (6LACKM) has not been thoroughly evaluated in children under three years of age. This report presents our initial experience with modified 6LACKM ECG acquisition techniques tailored to this population, including hand-assisted chest placement and adhesive electrodes adaptation, in various patient states and positions. In a prospective study, we aim to assess the feasibility of 6LACKM in this population and compare its diagnostic quality to standard 12‑lead ECG. Our preliminary experience demonstrate that high-quality tracings can be obtained with th

Signal Recognition Granule Receptor Beta Subunit Promotes Arrhythmogenic Remodeling in the Heart Failure Mice.

BACKGROUND: Heart failure is a major cause of global morbidity and mortality, often complicated by ventricular arrhythmias (VAs) that worsen prognosis. The Srprb (signal recognition particle receptor beta subunit) is an endoplasmic reticulum membrane-anchored protein involved in protein processing. METHODS: Male C57BL/6 mice received tail vein injections of adeno-associated virus to cardiac-specifically overexpress or knock down Srprb. A pressure overload-induced heart failure model was established via aortic banding 3 weeks later. Cardiac function, structural/electrical remodeling, and VA su

Congenital left ventricular diverticulum revisited: a systematic review of clinical phenotypes, imaging features, and therapeutic strategies.

BACKGROUND: Congenital left ventricular diverticulum (LVCD) is a rare cardiac malformation characterized by a contractile outpouching that includes all three layers of the ventricular wall. Although most frequently diagnosed in infancy, LVCD may also be identified in adults, either incidentally or in association with embolic events, arrhythmias, or heart failure. OBJECTIVES: This systematic review synthesizes current clinical evidence on LVCD, encompassing demographic patterns, imaging characteristics, associated malformations, treatment strategies, and outcomes. METHODS: All published case re

Tachycardiomyopathy-like presentation in neonatal MCAD deficiency: A novel cardiac phenotype.

BACKGROUND: Medium-chain acyl-CoA dehydrogenase deficiency (MCADD) is the most common fatty acid oxidation disorder in Europe. Clinical onset typically occurs between 3 and 24 months of life with hypoketotic hypoglycemia, while neonatal presentations are less common. Although the disorder classically manifests with metabolic decompensation, atypical cardiac involvement has occasionally been reported but remains exceedingly rare. MCADD is included in many newborn screening programs, enabling early detection and timely management. CASE PRESENTATION: We report a full-term female neonate who, at

Macrolides for the prevention of bronchopulmonary dysplasia in preterm neonates.

RATIONALE: Bronchopulmonary dysplasia (BPD) continues to be the most frequent complication of prematurity despite advances in neonatal care. It occurs from interactions between antenatal exposures, postnatal oxygen, ventilation-mediated injury as well as other postnatal injuries that induce a proinflammatory state in an immature developing lung. Macrolides, particularly azithromycin, have anti-inflammatory actions that may play a role in preventing BPD. A current review is required to investigate whether macrolide use in the highest-risk patient population, intubated and ventilated very preter

Postoperative junctional ectopic tachycardia with variable QRS morphology: A case report.

Junctional ectopic tachycardia (JET) is a rare but clinically important arrhythmia most often encountered in infants and children, either congenitally or within 72 h after congenital heart surgery. We report a neonate who developed postoperative JET with variable QRS morphologies after an arterial switch operation, in whom QP interval and QRS duration correlated with the preceding RR interval. The arrhythmia caused hemodynamic instability requiring antiarrhythmic therapy and supportive measures. This case illustrates the diverse electrocardiographic manifestations of postoperative JET and high
