OBJECTIVE: Intravenous epinephrine administration is preferred during neonatal resuscitation, but may not always be rapidly administered due to lack of equipment or trained staff. We aimed to compare the time to return of spontaneous circulation (ROSC) and post-ROSC haemodynamics between intravenous, endotracheal (ET) and intranasal (IN) epinephrine in severely asphyxic, bradycardic newborn lambs.
METHODS: After instrumentation, severe asphyxia (heart rate <60 bpm, blood pressure ~10 mm Hg) was induced by clamping the cord in near-term lambs. Resuscitation was initiated with ventilation follo

Hypoplastic left heart syndrome (HLHS) is a severe congenital heart disease affecting 2-3 neonates every 10,000 live births. While prior research has highlighted associations of HLHS with specific chromosomal abnormalities and genetic mutations, the precise pathophysiology remains elusive. Despite early surgical intervention potentially allowing most HLHS patients to survive their critical heart disease with a single-ventricle physiology, patients frequently experience complications of arrhythmias and right ventricular heart failure, culminating in the need for an eventual heart transplant. Severe bradycardia due to advanced atrioventricular (AV) block in neonates is rare but can be life-threatening, requiring urgent pacing. In the smallest infants, transvenous pacing is not feasible, and epicardial systems remain the standard. However, the pacemaker size still remains a significant issue in these small patients. Herein, we report the case of a premature neonate with complete intrauterine atrioventricular block successfully treated with an epicardial system using a custom-designed Micra-based Pediatric implantable pulse generator (IPG) connected to a standard epicardial lead.

Despite improvements in survival following staged surgical palliation for single ventricle heart disease, morbidity and mortality remain substantial, particularly during the interstage period following the Norwood operation. Post-operative arrhythmias are common in this population, yet their risk factors and impact on outcomes are not well defined. This study aims to characterize the risk factors and clinical outcomes of arrhythmias following the Norwood operation. We conducted a retrospective cohort study of neonates who underwent the Norwood operation between January 2010 and December 2023 a

Cardiac rhabdomyomas are the most common primary cardiac tumors observed in neonates and children, which have been frequently associated with the tuberous sclerosis complex. These benign tumors usually have a natural history of regression but can sometimes cause significant clinical issues by obstructing blood flow and arrhythmias. We present an unusual case of a neonate who presented with a large cardiac rhabdomyoma with obstruction to the right ventricular inflow and outflow. The tumor necessitated an urgent surgery, which proved to be technically challenging.

Congenital heart block is a rare disorder of neonates. It is generally diagnosed in the antenatal period or at birth and is commonly associated with transplacental passage of anti-Ro and/or anti-La antibodies to the fetus. It has a high mortality rate if left untreated. A pacemaker is considered as the treatment of choice, but the expertise for pacemaker implantation is available at only a few centres in India. We present a case of a late preterm neonate with antenatally diagnosed heart block due to anti-Ro antibody in the mother. The baby required delivery room resuscitation and mechanical ve

Cardiac rhabdomyomas are the most common pediatric cardiac tumors and are frequently associated with tuberous sclerosis complex (TSC). Large fetal cardiac rhabdomyomas causing inflow or outflow obstruction can lead to hemodynamic compromise and arrhythmias, sometimes necessitating emergent perinatal interventions. We present the fetus of a 32-year-old with multiple intracardiac rhabdomyomas identified at 23 weeks gestation. The largest, located in the left atrium (LA), enlarged by 32 weeks, causing mitral inflow obstruction and moderate mitral regurgitation (MR). Maternal sirolimus therapy was

Pathogenic CACNA1C variants are associated with long QT syndrome, cardiac conduction disorders (CCD), short QT or Brugada syndromes, sudden cardiac death, and rarely hypertrophic cardiomyopathy. Neonatal complete AV block (CAVB) has not been previously reported in patients with CACNA1C-mutations. We present a neonate with CAVB, severe heart failure, and non-compaction cardiomyopathy, found to have a de novo, likely pathogenic, heterozygous CACNA1C variant and heterozygous variants of uncertain significance (VUS) in TNNI3K, GATA6, and PDHK1 genes. This case expands the CACNA1C-associated phenot

BACKGROUND: The International Liaison Committee on Resuscitation recommends initiating chest compressions (CC) in newborns when heart rate (HR) is <60 beats per minute (bpm) after 30 s of effective positive pressure ventilation (PPV).
METHODS: Near-term lambs with asphyxia induced bradycardia (HR < 60 bpm) were randomized to: (i) control (n = 6)- Resuscitation per current Neonatal Resuscitation Program (NRP) (ii) study (n = 6)-PPV continued until HR = 0, only then CC with PPV were administered in a ratio of 3:1 until return of spontaneous circulation (ROSC). Outcomes included timing, incidenc

Sudden arrhythmic death syndrome (SADS) is one of the most common post-mortem diagnoses in the young suffering sudden cardiac death (SCD). Phenotypic characterization may facilitate prevention. The aim of the present study was to characterize phenotypes of SADS in the young including comparison with up to 10 controls per case. All SCD cases among 0-35 years old in Sweden during 2000-2010, have previously been included in the sudden cardiac death in the young cohort (SUDDY), including five controls. Medical records, ECGs, death certificates, autopsy reports and national register data were analy

AIM: Left ventricular pressure overload (LVPO) in adults is associated with adverse electrical remodeling, characterized by reduced conduction velocity (CV). However, the progression of LVPO differs when imposed during the proliferative phase of cardiac development. It remains unknown how increased cardiomyocyte proliferation affects LVPO electrical remodeling.
METHODS: CV maturation from rat postnatal day (PD) 1 to PD90 and analyzed underlying connexin 43 (Cx43) profile. Pressure overload was induced by abdominal aortic constriction (AAC) in rats during the proliferative phase of cardiac gro

Amniotic fluid embolism (AFE) is a rare, life-threatening obstetric event associated with extremely high maternal and perinatal morbidity and mortality with no known definitive pathophysiologic pathway. Current recommended management strategies are mainly supportive and include high-quality cardiopulmonary resuscitation and prompt delivery of the potentially viable fetus to improve neonatal survival and maternal resuscitation. More recently, case reports have described success in the resuscitation of patients with presumed AFE with a protocol that includes intravenous administration of atropin

Brugada syndrome (BrS), a hereditary cardiac channelopathy linked to malignant arrhythmias and sudden cardiac death, exhibits significant gender disparity with higher prevalence in males. This case report details a rare occurrence of BrS in a 32-year-old primigravida who suffered an acute cardiopulmonary arrest 2 h postpartum. The patient, with no prior cardiac history or family history of sudden death, underwent an uncomplicated delivery but developed sudden pulselessness post-transfer to the ward. Immediate resuscitation and subsequent evaluations revealed a type I Brugada ECG pattern, while

BACKGROUND: The AAP recommends that infants born <37 weeks gestation or with risk factors for positional cardiorespiratory instability undergo a car seat tolerance screen (CSTS) before discharge. Understanding characteristics of CSTS failure may inform screening practices.
OBJECTIVE: Compare clinical, oxygen saturation, and heart rate features in infants who failed vs. passed CSTS.
METHODS: We retrospectively reviewed CSTS data from a single level IV NICU (2018-2024). Clinical characteristics of infants who failed vs. passed their first CSTS were compared. For a subset with SpO₂ and HR data,

BACKGROUND: Gorlin syndrome (GS) is a rare autosomal dominant disorder, associated with pathogenic PTCH1 or SUFU variants, predisposing to tumors such as basal cell carcinoma, medulloblastoma (MB), odontogenic keratocyst, and, rarely, cardiac fibroma (CF). MB occurs in ~5% of GS cases, typically in early childhood, while CF appears in 1-3%. Their coexistence in childhood is extremely rare. This report describes a pediatric GS case with synchronous MB and CF, focusing on the management priorities between oncologic and cardiac interventions.
METHODS: A 15-year follow-up is reported for a girl d

BACKGROUND: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, characterized by atrial fibrosis, a crucial substrate facilitating its initiation and persistence. CKAP4 (cytoskeleton-associated protein 4) has been associated with fibroblast activation; however, its involvement in atrial remodeling and AF susceptibility remains unclear.
METHODS: We measured serum CKAP4 by ELISA in 189 patients with drug-refractory AF and 79 controls and then correlated these levels with left atrial scar burden assessed by 3-dimensional electroanatomic mapping. CKAP4 cell-type specificity

OBJECTIVE: Cardiopulmonary resuscitation (CPR) during interfacility neonatal and pediatric transport is a rare phenomenon (0.33% and 0.18%, respectively). The ability to maintain high-quality manual CPR in a moving vehicle with limited personnel and equipment impacts its effectiveness. There are limited data about neonatal and pediatric transport teams training staff on resuscitation during transport. This study evaluated the feasibility of in situ, simulation-based CPR training for a tertiary hospital's pediatric-neonatal transport team to better replicate real-world constraints.
METHODS: Cr

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease presenting beyond the neonatal period. Since its first comprehensive description by Étienne-Louis Fallot in 1888, the management of TOF has evolved remarkably - from a universally fatal condition to one with high survival and long-term quality of life. The introduction of the Blalock-Taussig shunt in 1944 provided the first effective surgical palliation by increasing pulmonary blood flow. In the 1950s, the development of cardiopulmonary bypass enabled complete intracardiac repair, which soon became the definitive tr

Fetal therapy has advanced with novel pharmacological approaches to address congenital tumors and malformations, including cardiac rhabdomyomas and lymphatic malformations (LMs). Cardiac rhabdomyomas, often associated with tuberous sclerosis complex (TSC), can lead to significant morbidity due to arrhythmias, left or right ventricular outflow tract obstruction and heart failure, particularly in neonates. LMs, resulting from abnormal development of the lymphatic system, can cause considerable morbidity, including airway obstruction, disfigurement, recurrent infections, bleeding into cysts, and
