Development and internal validation of a scoring model for early identification of ventricular arrhythmia risk in children with acute myocarditis.

BACKGROUND: Early identification of ventricular arrhythmia (VA) risk in children with acute myocarditis (AMC) is challenging, as existing tools lack pediatric targeting. AMC’s non-specific early symptoms in children lead to underdiagnosis. VA, a major complication of AMC, is linked to poor prognosis, but not all cases present at admission, highlighting the need for simple, accessible predictive indicators. METHODS: This retrospective single-center study included 312 children (1 month − 17 years) with AMC (2021–2024), divided into a training set (n = 208, 2021–2023) and validation set (n = 104,

Sotalol Dose Optimization for Fetal Tachycardia: A Pregnancy Physiologically Based Pharmacokinetic Model Study.

OBJECTIVE: To establish an optimized sotalol dosing strategy for fetal tachycardia using a pregnancy physiologically based pharmacokinetic (PBPK) model. METHOD: A verified PBPK model was developed incorporating pregnancy-induced physiological changes and placental transfer. Simulations were performed to evaluate current dosing recommendations and alternative dosing scenarios were explored. To avoid maternal dose-related toxicity (QT-prolongation), we aimed for maternal concentrations < 2.5 mg/L. Based on neonatal concentration-effect data, we aimed for fetal Ctrough concentrations of 0.4-1.0

Newborn With Abnormal ECG and Family History of Sudden Cardiac Arrest.

Congenital long QT syndrome (LQTS) is a cardiac channelopathy caused by mutations in cardiac ion channel genes, leading to delayed ventricular repolarization, QT interval prolongation, and risk of malignant arrhythmias and sudden cardiac death. The condition often presents during childhood or adolescence but can be identified in neonates, particularly when there is a positive family history. Early recognition is critical, as timely intervention with beta-blockers and avoidance of QT-prolonging medications can prevent life-threatening events. We report the case of a full-term neonate born via c

Short SCN5A Transcript Yields a NaV1.5 Fragment Influencing Cardiac Metabolism.

BACKGROUND: SCN5A encodes the cardiac NaV1.5 (voltage-gated Na+ channel), classically known for initiating action potentials and recently implicated in cardiomyocyte metabolism via mitochondrial Na+/Ca2+ exchange. SCN5A variants are linked to arrhythmias and heart failure, but mechanisms controlling SCN5A/NaV1.5 expression and its metabolic interface remain understudied. METHODS: We used bioinformatic approaches to identify novel SCN5A regulatory features and discovered an alternative polyadenylation (APA) signal downstream of exon 2, which is conserved in humans and several other species but

Fto-mediated m6A demethylation of Lox drives atrial fibrosis and promotes atrial fibrillation in a murine model of hyperthyroidism.

AIMS: Hyperthyroidism is a risk factor for atrial fibrillation (AF), and N6-methyladenosine (m6A) RNA methylation is crucial in cardiovascular regulation. However, the role of Fto-mediated m6A demethylation in hyperthyroidism-related AF remains unclear. METHODS AND RESULTS: We consecutively recruited 232 AF patients undergoing ablation, stratified into age-, gender-, and comorbidity-matched cohorts, 116 with manifest hyperthyroidism and 116 without manifest hyperthyroidism. Assessments included thyroid profiles, echocardiography, low-voltage area (LVA) mapping, and 1-year recurrence. T4-treat

Fetal Long QT Syndrome: Case Series and Literature Review With Focus on Multidisciplinary Care Coordination.

Congenital long QT syndrome (LQTS) is a group of heritable conditions that are associated with cardiac repolarization abnormality characterized by QT prolongation on ECG and risk of life-threatening arrhythmias. Prenatal detection of LQTS presents many challenges for clinicians and a multidisciplinary approach is needed for optimal prenatal and postnatal management. We describe five cases of fetal diagnosis of LQTS with variable initial presentation, diagnostic strategies, and management approaches. A multidisciplinary team approach including fetal cardiologist, adult and pediatric electrophys

Impact of β-Blockers on the Risk of Low-Birth-Weight Infants in Women with Long QT Syndrome or Marfan Syndrome: A Single-Center Retrospective Study from 2008 to 2022 in a Tertiary Care Center.

INTRODUCTION: β-Blockers are an essential treatment for cardiovascular disease, the incidence of which is rising among women of childbearing age. Several large cohort and registry studies have reported an association between β-blocker exposure in pregnancy and reduced birth weight, but most have included women with hypertension or structural heart disease. Our study's primary objective was to evaluate the impact of these medications on the birth weight of infants born to mothers with morphologically normal hearts, including women with long QT syndrome (LQTS) or Marfan syndrome (MFS). Secondary

Regulation of sodium/calcium homeostasis by BacNav gene therapy rescues cardiac dysfunction in chronic heart failure.

Despite continued progress, therapies to augment contractile function and prevent arrhythmias in patients with heart failure remain limited. Here, we present a two-pronged gene therapy approach that simultaneously augments peak Na+ current and Ca2+ transient amplitude in cardiomyocytes (CMs) to alleviate heart failure pathogenesis. Using in vitro engineered neonatal rat heart tissues, ex vivo adult mouse CMs, and in silico rabbit CMs, we show that expression of prokaryotic Na+ channels (BacNav) dose-dependently enhances Ca2+ transients and contractility of CMs by modulating Na+/Ca2+ exchanger

When the gatekeeper falls: developmental vulnerability of the thalamic reticular nucleus in neonatal and pediatric hypoxic-ischemic brain injury.

The thalamic reticular nucleus orchestrates thalamocortical oscillations and sensory gating. Its early development features a unique confluence of depolarizing GABA signaling, immature chloride regulation, and transient electrical coupling via connexin-36 gap junctions. These developmental specializations, essential for synchronizing cortical maturation, also render thalamocortical networks vulnerable to hypoxic-ischemic insults such as perinatal asphyxia or pediatric cardiac arrest. Following cellular ATP depletion, rapid chloride imbalance eliminates fast synaptic inhibition, permitting abno

Respiratory Stabilization for Successful Intubation in Neonates: A Randomized Controlled Trial.

OBJECTIVE: Maintenance of non-invasive respiratory support during endotracheal intubation (ETI) is a feasible intervention to prevent procedure-related physiological instability (PI) in neonates. However, the evidence for its use is conflicting. The present study evaluated the effects of continuing pre-existing respiratory support during ETI in neonates compared to standard care. METHODS: Neonates (≥ 26 weeks) requiring ETI in the neonatal intensive care unit (NICU) were randomized to undergo ETI either on respiratory support (intervention; n = 75) or in room air (control; n = 75) using a com

Ergonomic Considerations in Neonatal Cardiopulmonary Resuscitation: Implications for Practice.

BACKGROUND: Prolonged neonatal cardiopulmonary resuscitation (CPR) is critical for survival in cases of severe bradycardia due to asphyxia. High-performance CPR, emphasizing consistent compression depth and full recoil, improves survival rates. PURPOSE: This pilot study explored how environmental and provider-specific factors-including bed height, positioning, and anthropometry-affect the consistency of neonatal CPR using the 2-thumb technique. METHODS: Twenty-two neonatal intensive care unit providers performed 2-minute CPR trials on a neonatal simulator under 2 conditions: self-selected be

Transcatheter Interventions in Infants With Congenital Heart Disease: A Two-Year Retrospective Case Series From a Tertiary Centre in India.

BACKGROUND: Severe forms of congenital heart disease (CHD) remain a leading cause of neonatal morbidity and mortality. In recent decades, transcatheter interventions have emerged as a less invasive, repeatable, and effective alternative to surgical treatment, particularly for critically ill neonates. OBJECTIVE: This retrospective study evaluates the demographic characteristics, diagnoses, procedures, complications, and 30-day outcomes of infants undergoing transcatheter interventions for CHD at Geetanjali Medical College and Hospital, Udaipur, Rajasthan, between December 2021 and November 202

Neonatal Atrial Flutter as the First Manifestation of Duchenne Muscular Dystrophy: A Case Report.

Cardiac involvement is a well-recognized component of Duchenne muscular dystrophy (DMD), typically manifesting later in childhood as progressive cardiomyopathy and arrhythmias. Early cardiac presentations, particularly in the neonatal period, are exceedingly rare. We report a neonate who presented with atrial flutter in the absence of structural heart disease or ventricular dysfunction. The patient underwent successful cardioversion, and initial cardiac evaluation, including echocardiography, was unremarkable. However, markedly elevated creatine kinase levels prompted further investigation. A

Prenatal Detection of Interatrial Septal Aneurysm With Postnatal Follow-Up: A Rare Pediatric Case of Arrhythmia in a Two-Year-Old Male.

Interatrial septal aneurysm (IASA) is not uncommon in fetal and neonatal screening echocardiography. However, its persistence into childhood is rare. The presence of an IASA was associated with increased risk of arrhythmias and embolic events. Fetal tachycardia was suspected at 34 weeks of gestational age. Fetal echocardiography showed a large IASA with a normal fetal heart rate and no other congenital abnormalities. The case presented again at the age of two years with recurrent syncopal attacks, with confirmed atrioventricular (AV) nodal re-entry tachycardia correlated with the symptoms. Ech

Cardiorespiratory tolerance of continuous dexmedetomidine infusion in preterm and term newborn infants: a retrospective cohort study.

Dexmedetomidine (DEX) is increasingly used for neonatal sedation, but safety data remain limited. We conducted a single-center retrospective study including neonates receiving continuous DEX infusion. Cardiorespiratory events were extracted from bedside monitoring during the 8 h before and the 24 h after initiation. Hemodynamic and clinical parameters were analyzed, and autonomic activity was assessed using Newborn Infant Parasympathetic Evaluation (NIPE) monitoring in a subgroup. Thirty-seven infants (18 preterm, 19 term) were included; 86% received concomitant morphine. Bradycardia episodes

Dual-chamber epicardial pacing in preterm and low birth weight infants with congenital complete atrioventricular block: a staged approach.

BACKGROUND: Congenital complete atrioventricular block (CCAVB) in preterm low birth weight neonates poses significant technical challenges for permanent pacemaker implantation. Epicardial pacing is generally preferred in this population; however, definitive implantation in infants weighing < 2500 g may be technically demanding. We evaluated the feasibility and safety of dual-chamber epicardial pacing in preterm low birth weight neonates with autoimmune-associated CCAVB. METHODS: Eight preterm neonates weighing < 2500 g underwent epicardial pacing between April 2021 and May 2022. All patients

Cyanotic Congenital Heart Disease in Pregnancy: A Comprehensive Review of Pathophysiology, Maternal-Fetal Outcomes, and Contemporary Management.

Advances in pediatric cardiac surgery and medical management have enabled an increasing number of women with cyanotic congenital heart disease to reach reproductive age and contemplate pregnancy. Maternal complications occur in approximately 32% of pregnancies in women with uncorrected cyanotic heart disease, with heart failure, arrhythmias, and thromboembolism representing the primary adverse events. Fetal outcomes are critically dependent on maternal oxygen saturation, with live birth rates of 92% when saturation exceeds 90% but plummeting to 12% when saturation falls below 85%. This review

Tactile stimulation and newborn heart rate responses in the delivery room: an observational video study.

AIMS: Newborn resuscitation guidelines recommend stimulation as a first step in non-breathing newborns to facilitate spontaneous breathing. We aimed to evaluate the extent of stimulation in the delivery room, heart rate (HR) responses to stimulation, and association between bradycardia (HR <100 beats per minute (bpm) ≥10 s) in the first minute after birth and the need for positive pressure ventilation (PPV) versus stimulation alone.
