Artificial I ntelligence-Assisted Fetal Echocardiography: Improving Early Detection and Neonatal Outcomes of Congenital Heart Disease.

Congenital Heart Disease (CHD) encompasses a range of structural abnormalities of the heart and great vessels present at birth and is associated with significant lifelong morbidity, including heart failure and arrhythmias. Early diagnosis is critical for optimizing perinatal management and improving outcomes. This review focuses on early fetal echocardiography, a specialized ultrasound technique that enables detection of cardiac abnormalities in utero, as early as 11-14 weeks of gestation, compared to traditional imaging performed at approximately 20 weeks. Advancements in early imaging have i

Percutaneous Deployment of the Sinus-SuperFlex-DS Stent for Hybrid Stage I Palliation in Neonates Weighing ≤ 2.5 kg: A Multicenter Study.

BACKGROUND: Hybrid stage I palliation (HS1P) has developed as an alternative to the Norwood stage I palliation for neonates with hypoplastic left heart and related left-sided obstructive lesions. HS1P is currently used in various clinical settings, such as single ventricle palliation, bridge to decision, bridge to biventricular repair, or destination therapy. The procedure comprises bilateral pulmonary artery banding and duct stenting, thus avoiding the need for cardiopulmonary bypass surgery, which may be particularly beneficial in high-risk patients. AIMS: The aim of this multicenter retros

Prenatal Detection and Neonatal Management of Tuberous Sclerosis Complex Compounded by Symptomatic Cardiac Arrhythmia.

Tuberous sclerosis complex (TSC) is a rare autosomal dominant syndrome affecting multiple organ systems through the development of benign hamartomas. We present a newborn female patient suspected antenatally of having TSC based on fetal echocardiography, which suggested cardiac rhabdomyomas; these findings were confirmed postnatally. The patient was managed with regular multidisciplinary follow-up and treated according to her presenting manifestations, including neonatal cardiac arrhythmias. This case highlights the importance of early genetic confirmation and proactive management of neonatal

Postoperative arrhythmias and long-term cardiac function in pediatric congenital heart disease after cardiopulmonary bypass: a single-center retrospective cohort study.

Postoperative arrhythmias are common in pediatric patients undergoing cardiopulmonary bypass (CPB); however, their impact on long-term cardiac function remains unclear. To investigate the association between postoperative arrhythmias and long-term cardiac function, and to develop a prediction model for adverse outcomes. This retrospective cohort study included 712 children with congenital heart disease who underwent CPB. Postoperative arrhythmias were assessed using continuous electrocardiographic monitoring. The primary outcome was impaired long-term cardiac function at 6-12 months postoperat

Alumina nanoparticles-induced reduction in Ito, f contributes to the occurrence of ventricular arrhythmias.

BACKGROUND: Aluminum oxide nanoparticles (Al₂O₃-NPs) are currently widely applied, although they are known to induce electrocardiographic abnormalities, their crystal phase-dependent effects (including α- and γ-phases) and underlying electrophysiological mechanisms remain unclear. METHODS: Cardiac injury of α-, γ-, and mix-phase Al₂O₃-NPs was investigated via intraperitoneal injection (100 mg/kg) in rats and exposure (20-100 µg/mL) in neonatal rat ventricular myocytes (NRVMs). Multidimensional analyses included electrocardiography (ECG), monophasic action potential (MAP) recording, patch-clam

Neonatal right ventricular rhabdomyoma: Imaging findings and spontaneous regression.

Neonatal cardiac rhabdomyoma is a rare benign mesenchymal tumor that frequently serves as the sentinel clinical marker for tuberous sclerosis complex (TSC). We report a case of a term female neonate presenting with immediate respiratory distress and a Silverman score of 4/10. Initial transthoracic echocardiography (TTE) on day 3 of life revealed a voluminous, multilobulated, hyperechoic mass measuring 28 mm × 16 mm occupying the right ventricular cavity, involving both the inflow and outflow tracts. Despite the significant tumor burden, hemodynamic stability was maintained without significant

Neonatal outcomes following chronotropic rescue and delayed umbilical cord clamping in fetal complete atrioventricular block with low heart rate.

Fetal complete atrioventricular block (f-CAVB) with ventricular bradycardia of ≤ 55 bpm is associated with increased perinatal mortality. With the goal of increasing perinatal survival of f-CAVB cases, we initiated a delivery room protocol to increase fetal heart rate (HR) and cardiac output using intramuscular and intravenous epinephrine given immediately before Cesarean delivery and prior to umbilical cord clamping. We tested the safety and efficacy of this concept of 'chronotropic rescue' in the delivery management of seven fetuses with f-CAVB meeting fetal HR criteria of ≤ 55 bpm in the 12

Paediatric long QT syndrome: clinical outcomes and therapy in the Spanish National Registry.

BACKGROUND AND AIMS: Congenital long QT syndrome (LQTS) is a heterogeneous disorder in which genotype and QTc duration modulate the risk of major arrhythmic events (MAEs), but contemporary paediatric outcome data remain limited. This study aimed to characterize clinical features, management strategies, and predictors of MAEs in a nationwide paediatric LQTS cohort. METHODS: This retrospective multicentre study analysed children (<18 years) diagnosed with LQTS across 30 tertiary centres (2004-24) using 2022 European Society of Cardiology criteria. MAEs were defined as sudden cardiac death, abor

ECG challenge: a Kangaroo pitfall.

An apparently irregular rhythm was observed on bedside monitoring in a 32-week preterm infant receiving high-flow nasal cannula support during kangaroo care. The single‑lead ECG tracing demonstrated alternating QRS morphologies, initially raising concern for neonatal arrhythmia. Closer inspection revealed two independent rhythms with different cycle lengths, one representing incomplete monitor detection of the infant's underlying sinus rhythm and the other representing a second rhythm with distinct morphology. After repositioning the infant and adjusting the respiratory interface, the abnormal

Salidroside Alleviates Mobocertinib-Induced Cardiotoxicity by Improving Cardiac Electrophysiology.

Mobocertinib, a tyrosine kinase inhibitor approved for EGFR exon 20 insertion-mutated non-small cell lung cancer, induces cardiotoxicity manifesting as QT prolongation. Salidroside, a bioactive compound from Rhodiola rosea, exhibits cardioprotective properties. We investigated electrophysiological mechanisms underlying mobocertinib cardiotoxicity and salidroside's protective efficacy using Langendorff-perfused rat hearts and neonatal rat cardiomyocytes (NRCMs). Hearts were treated with escalating mobocertinib concentrations (214, 428, 856 nM) with or without salidroside (20 μM) pretreatment; c

How to evaluate, triage, and sedate a patient in the congenital cardiac catheterization laboratory.

The congenital cardiac catheterization laboratory (CCCL) has evolved into one of the most physiologically demanding environments in pediatric cardiovascular care for not only the interventional cardiologist, but also the anesthesiologist. Once primarily diagnostic, the CCCL now functions as a high-acuity interventional laboratory capable of treating both acquired and congenital heart disease (CHD), through the entire lifespan of patients with CHD. Technological innovation in device design, advanced imaging modalities, and hybrid surgical collaboration has expanded the therapeutic scope to incl

Weighted gene co-expression network reveals CCND1 as a promising biomarker and therapeutic target for myocardial fibrosis in hypertrophic cardiomyopathy.

BACKGROUND: Myocardial fibrosis represents a critical prognostic indicator of poor outcomes in hypertrophic cardiomyopathy (HCM), frequently predisposing patients to lethal ventricular arrhythmias and progressive heart failure. However, the upstream molecular mechanisms driving cardiac fibrogenesis in HCM remain largely unresolved. This study aimed to identify key fibrotic pathways and regulatory genes in HCM patients using weighted gene co-expression network analysis (WGCNA). METHODS AND RESULTS: We quantified the severity of myocardial fibrosis using late gadolinium enhancement (LGE) on car
